How Are Support Surfaces Used in Pressure Ulcer Care Within Nursing Home Settings Assessed? A Scoping Review.

The aim of this research was to synthesise evaluative research on support surfaces in pressure ulcer care in nursing homes, by examining how studies address effectiveness, efficiency, user satisfaction and context of use. Following the Joanna Briggs Institute methodology and PRISMA-ScR guidelines, six databases were used. Studies assessing support surfaces in nursing homes were included. Data extraction covered studies' characteristics, support surfaces evaluated and methodologies employed. Among the 68 included studies, 40 focused primarily on support surface evaluation, mainly assessing effe

Assessment methods of pathological complete response in neoadjuvant head and neck cancer clinical trials: a scoping review.

Head and neck cancer (HNC) trials have demonstrated pathological complete response (pCR) in patients receiving neoadjuvant treatment prior to surgery; however, systematic reviews have reported marked variation in pCR rates. This scoping review was designed to determine the definition, criteria, and assessment methodologies for pCR in interventional neoadjuvant HNC clinical trials. The review was conducted in accordance with the JBI Manual for Evidence Synthesis and the protocol prospectively registered. Medical databases, trial registries, and grey literature were systematically searched, and

Beyond the radiology report: a multi-criteria decision analysis to define essential CT parameters for abdominal wall reconstruction : STAMP-C framework for preoperative hernia imaging.

BACKGROUND: Comprehensive preoperative CT assessment is essential for ventral hernia repair, yet no standardized reporting framework exists. This study evaluated the completeness of preoperative abdominal CT reports and developed an evidence-based protocol to guide standardized reporting for abdominal wall reconstruction (AWR). METHODS: We conducted a systematic evaluation of CT reporting completeness in 834 patients who underwent elective transversus abdominis release (TAR) at the Cleveland Clinic Center for Abdominal Core Health between January 2020 and December 2024. A panel of AWR experts

Nudging implementation of low tidal volume ventilation: a stepped wedge, cluster randomized trial.

BACKGROUND: "Nudges" embedded in the electronic health record (EHR) facilitate desired decisions while preserving autonomy and may provide a scalable strategy to overcome the common implementation barrier of lack of knowledge about a best practice. We sought to test whether EHR-based nudges targeting two intensive care unit (ICU) clinician groups would safely increase evidence-based use of low tidal volume ventilation. METHODS: We performed a stepped-wedge, cluster randomized, hybrid type 3 effectiveness-implementation trial in 12 ICUs from February 2021 to May 2023 to test three nudges targe

Positive end-expiratory pressure levels during resuscitation of preterm infants at birth (POLAR): study protocol for a randomised controlled trial.

BACKGROUND: The most effective and safe level of positive end-expiratory pressure (PEEP) during stabilisation at birth of extremely preterm infants is unknown. In observational studies, an initial high PEEP level, then titrated to response (dynamic PEEP strategy), improved clinically relevant outcomes compared with current practice using a static, and lower, PEEP level. The Positive End-Expiratory Pressure Levels during Resuscitation of Preterm Infants at Birth (POLAR) trial will evaluate a novel dynamic PEEP strategy at birth. METHODS: In an international multicentre prospective open-label r

[Tailoring care to the mind: The emerging role of artificial intelligence in precision psychiatry. Part I: Understanding, measuring and modelling clinical singularity].

OBJECTIVES: Precision psychiatry seeks to narrow the gap between knowledge generated at the group level and decisions made for an individual patient. This ambition responds to the limitations of current diagnostic categories, which bring together patients with markedly different clinical, biological, cognitive, and longitudinal profiles. Artificial intelligence may support this shift by combining multiple sources of data, identifying complex patterns, and tracking individual trajectories over time. This first part of the article, Tailoring Care to the Mind: The Emerging Role of Artificial Inte

Quercetin parallels ascorbic acid in neuroprotection against manganese-induced toxicity in Drosophila melanogaster.

Quercetin (QCT) is a dietary flavonoid with reported antioxidant and neuroprotective properties, but direct comparisons with established antioxidants, such as ascorbic acid (AA), under chemically induced neurotoxic stress are limited. We compared Quercetin and AA in adult D. melanogaster using a manganese (Mn)-sensitized model to capture Parkinson-like motor and biochemical vulnerability. Treatment groups comprised: Control; Mn (15 mM); AA (10 mM); Quercetin 0.5 mM/3.0 mM; AA + Mn; Quercetin 0.5 mM/3.0 mM + Mn. Each vial represented an independent biological replicate (mixed sex); biochemical

Barriers and facilitators for the implementation of medication safety recommendations: focus groups with stakeholders.

INTRODUCTION: Multiple medication safety recommendations to reduce preventable medication-related hospitalisations have been introduced in the Netherlands. Still, these hospitalisations remain prevalent. This might be due to suboptimal implementation in clinical practice. Therefore, more insight is needed into barriers and facilitators for the implementation of these medication safety recommendations across healthcare sectors. AIM: This study aimed to identify barriers and facilitators for implementing medication safety recommendations across healthcare sectors from the perspective of various

Evaluation of the appropriateness of pediatric seizure Management in Primary Care Clinics.

BACKGROUND: Effective management of any pediatric seizure event - not only febrile seizures - is a core requirement of pediatric primary care. In Japan, where the background prevalence of febrile seizures (FSs) is high (8-10% of children), pediatric primary care clinics may encounter status epilepticus (SE) requiring immediate antiseizure medication. Buccal midazolam was approved in Japan in December 2020. The pediatric rescue medication landscape in Japan differs substantially from many other countries: rectal diazepam gel is not approved, and intranasal diazepam was not yet on the market dur

The CARE study protocol: game-based cognitive assessment via recreational engagement in patients with mild cognitive impairment in India.

BACKGROUND: Cognitive Assessment Games (CAGs) represent an emerging area of research. To date, most evidence regarding the efficacy of these games comes from studies conducted in Western, high-income countries. However, evidence from low- and middle-income countries (LMICs), including India, remains limited. Given the ongoing epidemiological and demographic transition in India, and the projected growth in the population aged 60 years and above, it is vital to assess the efficacy of such games in the Indian context. A previous study evaluated such games in healthy young adults; however, it is n

Accuracy and Effectiveness of Point-of-Care Ultrasound for Peripheral Intravenous Access in Emergency and Trauma Patients: A Systematic Review.

Peripheral intravenous (PIV) access is essential in emergency and trauma care, but challenging in patients with difficult intravenous access (DIVA). This systematic review evaluates the accuracy and effectiveness of point-of-care ultrasound (POCUS)-guided PIV access compared to conventional landmark techniques in emergency and trauma patients. A Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA)-guided systematic search of PubMed/MEDLINE, Scopus, Web of Science, and Embase was conducted for studies published between January 2021 and December 2025 to focus on contempora

Physician-Led Teams: A Model Curriculum for Emergency Medicine Residency.

BACKGROUND: Physician-led teams in emergency medicine (EM) are associated with higher quality and value than independent non-physician practice, yet most residents receive little formal preparation in leading inter-professional teams that include nurse practitioners (NPs) and physician assistants (PAs). Clear, scalable training guidance for residency programs is lacking. METHODS: We conducted a narrative synthesis of national policy positions, scope-of-practice trends, and published literature on team leadership training in EM and related disciplines, including other specialties, nursing, and

Designing an Indicator‑Driven, Value‑Based Architecture for Pneumonia Prevention in Japan: A Formative Policy Viewpoint on Adult Vaccination and Oral Care.

Japan's aging society concentrates pneumonia burden across communities, long‑term care, perioperative pathways, and hospitals. Adult pneumococcal and influenza vaccination are supported by trials and meta‑analyses and are often cost‑effective; yet, realized value depends on targeting, measurement, financing, and pairing with bedside prevention such as oral care and hospital‑acquired pneumonia bundles. This viewpoint proposes a formative, indicator‑driven architecture linking cost‑effectiveness to operations by aligning vaccination with complementary oral‑care prevention and value‑based payment

Military Standards for Anaphylaxis: Aligning Allergy Science, Operational Risk, and Waiver Documentation.

PURPOSE OF THE REVIEW: Allergic disease is common in the United States, and a growing number of otherwise qualified applicants and service members present with histories of food allergy, venom allergy, medication reactions, physical urticarias, or prior anaphylaxis. The challenge for clinicians is to translate a nuanced allergy-immunology evaluation into strict military accession and retention standards designed for readiness, austere care, and deployability, rather than for diagnosis alone. RECENT FINDINGS: The current accession standard disqualifies a history of anaphylaxis other than to a

Safety in the Age of Artificial Intelligence: Evaluating Large Language Model Adherence to Antithrombotic Medication and Regional Anesthesia Guidelines.

The release of the 2025 American Society of Regional Anesthesia (ASRA) 5th edition guidelines for regional and neuraxial anesthesia procedures for patients receiving antithrombotic medications introduced complex, patient-specific hold times and resumption protocols. As clinicians increasingly utilize large language models (LLMs) as clinical decision support tools, the reliability of these models remains largely unvalidated. This study evaluates the accuracy of two of the foremost LLMs, ChatGPT (OpenAI, San Francisco, CA) and Google Gemini (Google DeepMind, London, UK), in adhering to these new

Short- Versus Standard-Duration Dual Antiplatelet Therapy After Percutaneous Coronary Intervention in Acute Coronary Syndrome: A Systematic Review and Meta-Analysis.

Dual antiplatelet therapy (DAPT) for 12 months has historically been the standard of care following percutaneous coronary intervention (PCI) for acute coronary syndrome (ACS); however, prolonged DAPT carries a cumulative, time-dependent risk of major bleeding. We aimed to determine whether a short DAPT duration (≤6 months) followed by single antiplatelet therapy reduces bleeding events without compromising ischemic safety compared with standard 12-month DAPT in ACS patients. We conducted a systematic review and meta-analysis of randomized controlled trials (RCTs) by searching Medical Literatur

Ultra-early versus early adjunctive vasopressin initiation after norepinephrine escalation in septic shock: a target trial emulation.

PURPOSE: The optimal timing of adjunctive vasopressin after norepinephrine escalation in septic shock remains uncertain. We emulated a target trial comparing ultra-early vasopressin initiation within 0-3 h with early initiation within > 3-6 h after the norepinephrine infusion rate reached 0.25 μg/kg/min or higher. METHODS: We emulated a target trial using electronic health record databases (MIMIC-IV 2008-2022 and eICU-CRD 2014-2015). Adults with septic shock who reached the norepinephrine threshold before vasopressin use were eligible. We used the clone-censor-weight method to estimate cumula
