Prevalence and Cure of Primary Liver Cancer in Italy by Histologic Type.

BACKGROUND/OBJECTIVES: In Italy, one- and five-year survival of primary liver cancer patients have increased in recent decades and were higher after hepatocellular carcinoma (HCC) than after intrahepatic cholangiocarcinoma (ICC). This study aimed to estimate the complete prevalence and cure indicators (long-term survival) after liver cancer diagnosis by histologic type. METHODS: Data from 31 Italian cancer registries, covering 28,057,000 residents (47% of the population), were included. The complete prevalence was calculated in 2018. The mixture-cure model was used to estimate the cure fracti

Baseline Dementia Prevalence and Subtypes in Older Adults Hospitalized With Acute Coronary Syndrome: A Multicenter Study From a Large Integrated Health Care Delivery System.

BACKGROUND: Dementia is increasingly common among older adults (≥ 65 years) and is associated with poor outcomes. However, little is known about its baseline prevalence in patients hospitalized with acute coronary syndrome (ACS), particularly across revascularization strategies. METHODS: The authors conducted a cross-sectional analysis from a current longitudinal cohort study of Kaiser Permanente Northern California members hospitalized with ACS between January 2010 and December 2020. Dementia diagnoses and subtypes were identified using International Classification of Diseases 9/10 codes. Ba

Vitamin D for preventing acute respiratory infections in children up to five years of age.

RATIONALE: Acute respiratory infections (ARIs) are a leading cause of morbidity and mortality in children under five years of age and contribute to healthcare visits and hospitalisations globally. Vitamin D deficiency is common amongst pregnant women and young children. Given that vitamin D supplementation is safe, affordable, and easy to administer, its potential to reduce ARI-related healthcare visits holds substantial public health relevance. This review evaluates the benefit and harm of vitamin D supplementation in preventing ARI-related healthcare visits in young children, aiming to infor

Smoking Status, Including Non-Combustible Nicotine or Tobacco Products Use, and Site-Specific Colorectal Cancer Risk: A Nationwide Cohort Study.

PURPOSE: Cigarette smoking is an established risk factor for colorectal cancer (CRC), yet the association between smoking status, including non-combustible nicotine or tobacco product (NNTP) use, and site-specific CRC risk remains unclear. METHODS: This population-based retrospective cohort study used data from 4,489,451 participants in the Korean National Health Insurance System database. Smoking status was classified according to combustible cigarette (CC) and NNTP use. Multivariable Cox regression models were used to estimate adjusted hazard ratios (aHRs) for CRC risk. RESULTS: During a m

Downstream Treatment Burden and Health-Care Utilization Following Initiation of GLP-1 Receptor Agonists or SGLT2 Inhibitors in Type 2 Diabetes.

AIMS: Glucagon-like peptide-1 (GLP-1) receptor agonists and sodium-glucose cotransporter 2 (SGLT2) inhibitors are widely used for the treatment of type 2 diabetes, yet their downstream consequences in routine clinical practice remain incompletely characterized. We compared downstream treatment burden following initiation of GLP-1 receptor agonists versus SGLT2 inhibitors among adults with type 2 diabetes. MATERIALS AND METHODS: We conducted a retrospective, propensity score-matched cohort study using a multi-centre electronic health record network. Adults with type 2 diabetes who newly initia

Masked divergence in cancer mortality following COVID-19 disruption.

BACKGROUND: While overall cancer mortality in the United States has declined steadily for decades, systemic healthcare disruptions during the COVID-19 pandemic-including delays in screening, diagnosis, and treatment-may have created heterogeneous impacts across different malignancy types that are not visible in aggregate data. OBJECTIVE: To determine whether site-specific cancer mortality during the COVID-19 pandemic (2020-2023) deviated from expected trajectories based on established pre-pandemic trends. METHODS: This population-based analysis utilized U.S. National Vital Statistics System

Common recorded diagnoses in purebred vs. mixed-breed dogs: a multivariable analysis of primary-care electronic medical records in South Korea.

INTRODUCTION: In South Korea, the companion-dog population is strongly skewed toward a small number of small-breed purebred dogs, and distinguishing breed-type differences in recorded diagnoses from confounding by age, neuter status, and healthcare utilization is challenging in primary-care data. METHODS: This retrospective study used electronic medical records (EMRs) from primary-care veterinary clinics in South Korea, covering January 2016-December 2024, to compare the adjusted odds of the 20 most frequently recorded diagnoses between purebred and mixed-breed dogs. Records were standardized

The Infection Timeline in Solid Organ Transplantation: Lessons from the Swiss Transplant Cohort.

BACKGROUND: Dupuytren's disease (DD) is a common fibrotic disorder of the hand, characterized by progressive thickening and contracture of the palmar and digital fascia. Surgical excision remains the primary treatment; however, there are currently no therapies to prevent disease progression or recurrence. This study aims to develop a 3D in vitro model to test novel antifibrotic therapies. The model is based on decellularized pathological DD tissue seeded with patient-derived fibroblasts, capturing the role of both cellular and extracellular matrix components in disease progression. METHODS: F

Midline and central line catheters usage and blood stream infection: Trends and insights from 2016 to 2024.

BACKGROUND: Central line-associated bloodstream infections (CLABSIs) remain a major source of hospital-related morbidity and mortality. Replacing central venous catheters (CVCs) with midline catheters (MLCs), when clinically appropriate, has been proposed as a strategy to reduce the incidence of CLABSI cases. We hypothesized that increased MLC utilization would correlate with decreased CLABSI incidence. RESEARCH QUESTION: Is increased use of midline catheters, in place of central venous catheters, associated with reduced CLABSI incidence over time? STUDY DESIGN AND METHODS: We conducted a re

Serum vitamin B12 levels and 90-day outcomes in hospitalized patients with dementia: A cohort study.

Elevated serum vitamin B12 levels have emerged as a paradoxical prognostic marker associated with increased mortality across various clinical settings, yet their significance in hospitalized dementia patients remains unexplored. Given the high vulnerability of this population, we aimed to investigate the association between elevated serum vitamin B12 levels and all-cause mortality. This retrospective cohort study evaluated 90-day outcomes in hospitalized adults with dementia who had serum vitamin B12 levels measured within 7 days of admission (2010-2024). Patients were categorized into exposur

An unusual cause of chronic recurrent vomiting and life-threatening hyperkalemia: primary adrenal insufficiency due to probable bilateral adrenal tuberculosis.

BACKGROUND: Adrenal tuberculosis remains a leading cause of primary adrenal insufficiency in tuberculosis-endemic regions. Its clinical presentation is often insidious and non-specific, leading to significant diagnostic delays. Gastrointestinal symptoms, though common in adrenal insufficiency, may dominate the clinical picture and direct attention away from the underlying endocrine disorder. CASE SUMMARY: A 40-year-old Ethiopian woman presented with a five-month history of intermittent abdominal pain and recurrent non-bloody vomiting. Associated symptoms included myalgia, arthralgia, intermit

CBC-derived inflammatory indices predict adverse outcomes in geriatric emergency patients with acute infectious diarrhea: a MIMIC-IV analysis.

Acute infectious diarrhea remains a leading cause of morbidity and mortality worldwide. Similar to pediatric populations, geriatric patients represent a highly vulnerable group with increased hospitalization rates, prolonged length of stay, heightened dehydration susceptibility, diminished physiological reserve, and elevated mortality risk. While pediatric populations benefit from well-established clinical severity scores such as Vesikari and Clark scales, analogous prognostic tools specifically validated for geriatric patients with acute infectious diarrhea remain lacking. Specific inflammato
