OBJECTIVES: The modified endoscopic Lothrop (Draf III) procedure requires reliable mucosal coverage of the exposed bone within the frontal neo-ostium to prevent post-operative restenosis. Free mucosal grafts are effective but difficult to secure due to limited access and absent anchoring sites. We describe a novel use of bioabsorbable steroid-eluting implants as a pragmatic approach to achieve mucosal graft fixation.
METHODS: Following completion of a Draf III frontal sinusotomy, a PROPEL™ implant is deployed over a free mucosal graft within the frontal neo-ostium. Endoscopic assessment is pe

Frontal sinus setback is a procedure used for addressing severe protrusion of the frontal area. Various surgical techniques have been developed over time to address frontal sinus abnormalities with varying etiologies and severity. The present article introduces the piano technique as a novel method for frontal sinus setback with less aggressiveness and complexity compared to other available methods, while being efficient in providing substantial long-term improvements in aesthetic outcomes. In this method, the anterior wall of the frontal sinus is divided into numerous rectangular bony segment

Dyke-Davidoff-Masson syndrome (DDMS) is a rare disorder characterized by cerebral hemiatrophy and compensatory calvarial changes. We report a 22-year-old male with lifelong left-sided spastic hemiparesis and developmental delay, with a history of prematurity, neonatal meningitis, and childhood head trauma. Brain MRI demonstrated right cerebral hemiatrophy, exvacuo dilatation of the lateral ventricle, ipsilateral sulcal prominence, calvarial thickening, frontal sinus enlargement, and Wallerian degeneration of the corticospinal tract. This case is notable for the coexistence of multiple early-li

BACKGROUND: Respiratory cysts can be either congenital or acquired. Acquired cysts are thought to arise from the displacement of respiratory epithelium into adjacent tissues due to the disruption of normal anatomical barriers, possibly stemming from trauma.
CASE DESCRIPTION: A 14-year-old girl presented with a history of right orbital trauma, developed swelling in the right periorbital region 4 years after the injury. Magnetic resonance imaging demonstrated a cystic lesion in the lateral side of the intra-orbital space, showing communication with the frontal sinus and bony destructive erosion

Primary intraosseous meningiomas (PIOMs) are rare extradural meningiomas, originating within the skull, and accounting for approximately 2% of all meningiomas. These tumors are exceptionally uncommon in children, since the majority occurs in adults between 20 and 60 years old. Few cases have been reported in the literature, focusing on clinical findings, surgery, and outcomes in younger people. We report a case of a 9-year-old female with progressive painful frontal skull bossing, without neurological symptoms. Magnetic resonance imaging (MRI) diagnosed an osteolytic lesion in the right parame

This article presents a case involving close collaboration between ear nose and throat (ENT), maxillofacial surgeons and anaesthetics in the management of a young patient with a frontal sinus defect. The repair technique incorporated a bicoronal approach, pericranial flap and a 3D-printed Stryker PEEK implant, resulting in effective sinus obliteration and restored frontal aesthetics. This technique can be considered a viable 'scar-less' option for the correction of frontal sinus defects.

Human specimens from Border Cave thought to derive from its Middle Stone Age (MSA) deposits have been widely discussed in relation to Late Pleistocene evolution in southern Africa. Among the most prominent is the adult cranium BC 1. However, there are questions about the context and geochronological age of a number of these remains, including BC 1, which was recovered ex situ from a sediment dump created by miners digging for guano. The cranium has not been directly dated. Previous morphological analyses have clearly demonstrated its complete anatomical modernity, but its possible affinities t

INTRODUCTION: Frontal sinus osteomas are rare benign tumors that may cause functional or cosmetic concerns. Virtual surgical planning (VSP) offers promising adjuncts to classic craniofacial techniques to improve surgical precision and minimize long-term morbidity.
PATIENT: A previously healthy 15-year-old male presented with a two-year history of headaches and a slowly enlarging mid-forehead mass. Imaging revealed a 3 × 2 cm frontal sinus osteoma with mucocele formation and erosion of both the anterior and posterior tables, with early mass effect on the frontal lobes. VSP was used to design b

Synovial sarcoma is a high-grade soft-tissue sarcoma that commonly affects adolescents and young adults. Involvement of the head and neck region is rare, and tumors arising from the sinonasal tract are particularly uncommon and diagnostically challenging. We report the case of a 38-year-old male who presented with head trauma and decreased consciousness following a transient neurological episode while driving. Brain computed tomography revealed a lesion involving the left anterior ethmoidal air cells with associated old cerebral infarctions. Further magnetic resonance imaging demonstrated an a

BACKGROUND: Accurate identification of frontal sinus boundaries is a critical step in anterior skull base surgery, particularly in transsinusal approaches, as it allows optimization of the surgical bony window while minimizing unnecessary bone removal.
OBJECTIVE: To describe a simple and reproducible technique of external frontal bone transillumination for intraoperative frontal sinus mapping.
METHODS: After bicoronal exposure of the frontal bone, a standard fiber-optic light cable connected to the operating room light source is applied in direct contact with the frontal squama under low amb

Frontal sinus morphometry shows forensic promise for gender determination, yet paranasal sinus radiographs remain underexplored compared to CT-based assessments despite their accessibility. Therefore, it is of interest to evaluate frontal sinus dimensions (area, width, height) in 300 healthy adults (>20 years) using standardized paranasal sinus views with digital measurement techniques. Right frontal sinus height emerged as the most reliable predictor, achieving 69% accuracy for gender discrimination across all measured parameters. Paranasal radiographs demonstrated clinically viable predictiv

Pneumocephalus is a common complication of craniofacial trauma; however, delayed progressive enlargement resulting in intracerebral pneumatocele represents an uncommon and potentially life-threatening condition. We report a case of a 27-year-old male with severe traumatic brain injury following a road traffic accident with extensive skull base fractures. Initial imaging demonstrated hemorrhagic contusions with small intracranial air foci that resolved on early follow-up. Subsequently, delayed reappearance and progressive enlargement of air in the basifrontal region, manifesting as an intracere

Facial feminization surgery (FFS) reduces gender dysphoria and improves psychosocial well-being in transfeminine patients, but its multiprocedural nature leads to long operative times, increasing anesthetic risk, and limiting daily surgical output. This study analyzes operative times across FFS procedures and describes a comprehensive approach that maximizes efficiency during multiprocedural FFS. 55 patients who underwent FFS from August 2024 through October 2025 were included. A four-person operating team was utilized in all cases. Fourteen patients (25.5%) underwent full FFS, 22 (40.0%) unde

BACKGROUND: Traumatic anterior cranial fossa fractures complicated by cerebrospinal fluid (CSF) fistulas carry the risk of ascending meningitis.
METHOD: We describe our standardised technique for anterior cranial fossa repair via a bifrontal craniotomy, including preparation of a pericranial flap, closure of the frontal sinus, bilateral olfactory nerve dissection, and intradural pericranial reconstruction.
CONCLUSION: A systematic, anatomically informed approach enables durable watertight repair with acceptable morbidity. If surgery is properly timed after cerebral swelling has resolved, and

The supraorbital keyhole approach (SKA) is a minimally invasive corridor to the anterior skull base. However, inadvertent frontal sinus (FS) entry remains a primary complication. This study aims to quantify the relationship between surgical landmarks and FS morphometry to establish safety thresholds. Analysis was performed on 120 patients (53 males, 67 females; 48.5 ± 14.5 years old) using high-resolution computed tomography (CT) scans. Distances from the supraorbital notch (SON) and superior temporal line (STL) to the lateral FS border were measured. Statistical analysis was performed accordi

BACKGROUND: Maxillofacial fractures and ocular trauma are significant injuries in soccer, with potential functional, aesthetic, and visual consequences if unrecognized. Few reviews have specifically examined both injury patterns in this sport. The purpose of this narrative review is to synthesize the current literature to identify injury patterns associated with maxillofacial fractures and ocular trauma in soccer, thereby aiding clinicians in timely diagnosis and management. Given the heterogeneity and limited quality of the available evidence, this review also highlights areas where further r

To describe and assess the feasibility of a simple, low-cost retrograde transillumination technique for identifying and confirming the frontal sinus opening during endoscopic frontal sinus surgery in a small series of patients. This single-center descriptive surgical series, conducted between November 2024 and January 2025, included five patients who underwent endoscopic frontal sinus surgery. The technique's five steps were standardized: angled endoscopic view (45°) of the frontal recess, probing the probable opening with angled instruments, darkening the room, placing the light source of the

PURPOSE: Traditional craniofacial phenotyping of obstructive sleep apnea (OSA) relies on predefined anatomical hypotheses, often yielding incomplete assessments. This study aimed to apply multiple explainable deep learning (DL) approaches to explore novel craniofacial phenotypes.
PATIENTS AND METHODS: A multimodal DL model was trained using frontal and lateral facial images, lateral cephalograms, and demographic data from 130 participants (65 OSA patients diagnosed by overnight polysomnography, and 65 age- and sex-matched controls). Explainability analysis of the model's prediction was conduc
