Utilization and Outcomes of Permanent Pacemakers After Bicaval Heart Transplantation: A Large Institutional Experience.

BACKGROUND: Permanent pacemaker (PPM) implantation after heart transplantation (HT) is common, but contemporary data regarding utilization, outcomes, and emerging pacing technologies are limited. METHODS: We studied PPM placement among 1225 bicaval HT recipients (28% women) treated at our institution from 2010 to 2025 and compared PPM recipients with HT patients without PPMs matched on sex, age, and transplant year. RESULTS: Median age at HT was 58 years (interquartile range, 48-65 years). Sixty-six (5%) patients received a PPM, with stable implantation rates over time. Median HT-to-PPM time

Single-nucleus atlas of cell-type specific genetic regulation in the human brain.

Genetic risk variants for common diseases are predominantly located in non-coding regulatory regions and modulate gene expression. Although bulk tissue studies have elucidated shared mechanisms of regulatory and disease-associated genetics, the cellular specificity of these mechanisms remains largely unexplored. Here we present a comprehensive, single-nucleus multi-ancestry atlas of genetic regulation of gene expression in the human prefrontal cortex, comprising 5.6 million nuclei from 1,384 donors of diverse ancestries. Through multi-resolution analyses spanning eight major cell classes and 2

Development and Validation of an Ultra-low-cost, Open-source Normothermic Ex Vivo Organ Perfusion Platform.

BACKGROUND: Normothermic machine perfusion (NMP) promises to catalyze organ preservation, therapeutic discovery, and disease modeling. Commercial organ perfusion devices remain inaccessible for research because of restricted access, high costs, and steep technical learning curves. METHODS: To overcome these barriers, an NMP circuit was developed using recycled, repurposed, and low-cost components. Porcine kidneys and autologous blood were used for iterative optimization. A porcine kidney autotransplantation protocol was adapted to evaluate in vivo kidney function after NMP. Nontransplantable

Fatal Pulmonary Mucormycosis in a Renal Transplant Recipient: A Missed Opportunistic Infection.

Pulmonary mucormycosis has a high mortality rate if not diagnosed promptly. Early diagnosis and aggressive antifungal treatment, along with surgical debridement, can improve outcomes. We present a case of a 55-year-old African American man with end-stage renal disease who developed fatal pulmonary mucor seven months following his second deceased donor kidney transplant. This case highlights the pitfalls of empiric approaches to infections in immunocompromised hosts, assumptions based on the presence of concomitant organisms that may be present and the importance of accurate interpretation of s

Tissue resident memory T cells populate the human uveal tract.

The current concept is that the eye is an immune privileged site endowed with innate immune regulatory networks to maintain organ function. We now have evidence that resident T cells occupy intraocular tissues. In immune-mediated inflammatory diseases, such as psoriasis and rheumatoid arthritis, tissue resident T cells trigger disease flares in the skin and joints. This suggests resident T cells in the uvea may have similar functions in non-infectious immune-mediated uveitis, a collective term for autoinflammatory and autoimmune diseases of the uveal tract causing intraocular inflammation. Her

First Experience with Hypothermic Oxygenated Perfusion in Human Uteri: Feasibility and Metabolic Characterization.

Background: Uterus transplantation (UTx) is an emerging treatment for absolute uterine factor infertility. However, the use of deceased donors is limited, and donation after circulatory death (DCD) has not yet been utilized. Ischemic injury remains a major barrier, particularly compared with living donor procedures. Hypothermic oxygenated perfusion (HOPE), which has shown protective effects in heart, liver, and kidney transplantation, may offer similar benefits for uterine grafts. Methods: We report the first series applying HOPE to human uteri to improve preservation and enable metabolic inju

Towards Benchmark-level Outcomes in Donation After Circulatory Death Liver Transplantation: A Comparative Analysis of Machine Preservation Strategies.

INTRODUCTION: Donation after circulatory death (DCD) graft use has become a standard practice globally to increase access to liver transplantation (LT). Machine preservation strategies have been widely adopted to improve graft quality, mitigate ischemia-reperfusion injury, expand organ utilization and enable organ viability assessment. However, the various approaches to MP have never been directly compared, particularly in the DCD cohort which might experience the greatest benefit. METHODS: All cases of adult DCD-LT 2014-2024 preserved MP were collected from centers in Italy, The Netherlands,

Organ-specific differences in the epidemiology of cytomegalovirus infections in high-risk (cytomegalovirus donor seropositive, recipient seronegative) solid organ transplant recipients with prolonged follow-up: A 10-year experience.

Cytomegalovirus (CMV) causes significant morbidity in CMV donor seropositive/recipient seronegative (D+R-) solid organ transplant recipients, but organ-specific differences in CMV epidemiology and the utility of laboratory surveillance after prophylaxis (SAP) are not well characterized. We performed a single-center retrospective study of 540 CMV D+R- heart (n = 145), kidney (n = 216), or liver recipients (n = 179) who received 6 months of CMV prophylaxis followed by SAP for 12 weeks and were followed for at least 24 months after transplant. The cumulative incidence of clinically significant CM

Transmission of Cytomegalovirus (CMV) from CMV-Seropositive Organ Donors to CMV-Seronegative Pediatric Solid Organ Transplant (SOT) Recipients.

BACKGROUND: The risk factors for donor-acquired (DA) CMV transmission in CMV-seronegative pediatric solid organ transplant (SOT) recipients (R-) receiving organs from CMV-seropositive donors (D+) remain unknown in the era of routine antiviral prophylaxis. METHODS: DA-CMV transmission was defined as CMV antigenemia/DNAemia and/or seroconversion in a D+/R- recipient by 13 months post-transplant. DA-CMV transmission, along with donor and recipient demographics, was studied in a retrospective cohort of CMV D+/R- children who had a SOT at the Stollery Children's Hospital from 2000 to 2018. Cytotoxic Vδ2+ T cell subsets expand in response to malaria in human tonsil and spleen organoids.

Vaccine effectiveness against malaria is dramatically reduced in malaria-exposed compared to malaria-naïve populations, potentially due to altered immune responses in secondary lymphoid organs following repeated infection. Newly developed human tonsil and spleen organoids, which replicate key features of B and T cell immunity, provide an exciting opportunity to overcome challenges of other models and to improve our understanding of innate-adaptive interactions in lymphoid tissue. The objectives of this study were to use these organoids to investigate the impact of malaria parasites on 1) cells

Parasitic infections in solid organ transplant.

Parasitic infections in solid organ transplant recipients are uncommon but potentially devastating. Transmission may occur through donor-derived infection, reactivation of latent disease, or post-transplant environmental exposure, often with nonspecific presentations that delay diagnosis. Clinically important pathogens include Toxoplasma gondii, Plasmodium spp., Babesia spp., Trypanosoma cruzi, Strongyloides stercoralis, Schistosoma spp., and selected free-living amoebae and intestinal apicomplexans. Diagnosis requires integration of epidemiologic risk, microscopy, serology, molecular testing,

Mapping structural aging across human tissues reveals tissue-specific trajectories and coordinated deterioration.

Organ structure, including the organization of cells, vasculature and extracellular matrix, underpins its function, yet how structure changes with age remains mostly unknown. Here we developed PathStAR, a framework that quantifies tissue structural aging from routine histopathology images, without being trained to predict chronological age. Applying PathStAR to 25,306 post-mortem biopsies from 40 tissues in 970 donors aged 21-70 years revealed that organ structural aging progresses via distinct, nonlinear temporal trajectories: vascular tissue structural aging accelerates early, uterus and vag

Impact of preformed donor-specific anti-HLA antibodies on pancreatic islet transplantation outcomes: do they matter as they do in solid organ transplantation?

BACKGROUND: Preformed donor-specific anti-HLA antibodies (DSA) are a major risk factor for antibody-mediated rejection and graft failure in kidney, heart, lung, and other vascularized solid organ transplants. Their clinical significance in pancreatic islet transplantation, however, remains uncertain. METHODS: We conducted a SANRA-guided narrative review using a focused PubMed search (January 1, 2000, to June 9, 2026) supplemented by citation chasing. Of 104 records identified, 11 directly evaluating islet transplantation formed the core evidence base, complemented by 38 references providing m

Porcine kidney xenotransplantation as a bridge to allotransplantation: a first-in-human study.

BACKGROUND: Kidney xenotransplantation offers a potential solution to the organ shortage, but questions remain regarding durability, zoonotic infection risk, and whether the immunological response to the xenograft elicits sensitisation that could complicate subsequent allotransplantation. We report outcomes from a porcine kidney xenograft in a living recipient followed by human allotransplantation. METHODS: A patient with end-stage kidney disease, a prolonged anticipated waiting time for deceased donor transplantation, and with no suitable living donor underwent transplantation at Massachuset

PLAIN ENGLISH SUMMARY: Outcome domains are aspects of a condition that matter to patients and clinicians and can be measured to assess treatment effects. For tinnitus, examples include 'tinnitus loudness' and 'ability to concentrate'. This study focuses on the first stage of agreeing which outcome domains should be measured in all clinical trials of tinnitus. Crucially, it involves identifying outcome domains, prior to a voting process. This article describes how we effectively involved patients in that study design process, and reflects on the impact of their input. The study first compiled a

Structured missingness in biomedical research.

Antipathogen defense; b-caryophyllene; vector-virus mutualism Plants respond to insect herbivory by producing and releasing distinctive blends of volatile compounds. These herbivore-induced plant volatiles (HIPVs) serve several roles, including direct defense through toxic or repellent effects on herbivores and indirect defense by attracting the herbivores' natural enemies (Bleeker et al. 2009; Veyrat et al. 2016). HIPVs also act as warning signals, informing undamaged tissues within the same plant and nearby plants of a pending insect attack (Heil and Bueno 2007). HIPVs are mostly composed of

Characteristics of patients hospitalized for diabetic foot in a national hospital in Peru, 2024.

INTRODUCTION: Expansive processes around the third ventricle have always been a major neurosurgical challenge. Despite all the technological and scientific progress recorded over the last few years, third ventricle tumors are still a very difficult pathology to approach due to their formidable complexity.Treating such a tumor demands a good knowledge of local anatomy, pathophysiology, pathology and a good capacity to integrate all the data gathered from the patient prior to and during surgery. Last but not least, the correct means to approach such a tumor involves using modern neuronavigation

Desmoplakin Mutations in Cardiac Fibroblasts Cause TGFβ1-Mediated Pathological Fibrogenesis in Desmoplakin Cardiomyopathy Via Beclin-1 Regulation.

BACKGROUND: Pathological fibrosis is a major finding in cardiovascular diseases and can result in arrhythmia and heart failure. Desmosome gene mutations can lead to arrhythmogenic cardiomyopathy. Among arrhythmogenic cardiomyopathies, pathogenic DSP (desmoplakin) variants cause a distinctive cardiomyopathy with excessive cardiac fibrosis that could precede ventricular dysfunction. DSP variants are also linked to other fibrotic diseases. Whether DSP plays any role in pathological fibrosis remains unknown. METHODS: Mesenchymal stromal cells (MSCs) are resident fibroblast-like cells that are res

Education and Ethical Guidance for Organ Donation at the End-of-Life: An Ethical Guidance Statement from the Neurocritical Care Society.

The neurocritical care treatment team frequently interacts with patient surrogates and Organ Procurement Organizations (OPOs) at the end-of-life. A 2025 survey of Neurocritical Care Society (NCS) members assessing experiences and attitudes related to organ donation identified the need to enhance education for the treatment team about organ donation and opportunities for improvement in palliative interventions after withdrawal of life-sustaining therapies for potential donation after cardiac death/death by circulatory-respiratory criteria. Additionally, members noted concerns regarding blurred

Donation Care Work: Family Members' Contributions to Deceased Organ Donation Following Determination of Death Using Circulatory Criteria.

Family members of potential deceased organ donors are often portrayed as gatekeepers to the individual experience of organ donation. We argue the success of deceased donation as an institution depends on often-unrecognised family labour that occurs after consent. Building on feminist notions of care work, we propose the concept of 'donation care work' to conceptualise the combination of emotion work, end-of-life care, and death work that family members perform to support efforts for donation while facing imminent loss. We interviewed 38 family members of potential deceased organ donors in Cana
