Presence and type of bystanders on delay time in acute myocardial infarction: a transcultural cross-sectional clinical study with 907 patients in Shanghai and Munich.

BACKGROUND: Bystanders may critically influence prehospital delay during acute myocardial infarction (AMI), but existing evidence is limited and largely outdated. This study aimed to examine the association between bystander presence and type on prehospital delay in AMI across two culturally distinct cities with attention to gender differences. METHODS: This cross-sectional observational study included 907 patients with confirmed AMI from the Munich Examination of Delay in Patients Experiencing AMI (MEDEA) study (n=614) and the multicentre MEDEA FAR-EAST study in Shanghai (n=293). The primary

Epigenetic Aging and Cognitive Performance in General and Psychiatric Populations: A Systematic Review and Narrative Synthesis.

Epigenetic aging and age-related cognitive decline both show substantial variability across individuals. While a potential link between the two is increasingly being studied in both general and psychiatric populations, a systematic review has not yet been performed. This review summarizes the state of the current literature as well as critical evaluation and suggestions for future research. A literature search (October 31, 2024) identified original peer-reviewed studies examining epigenetic aging and cognitive function in general and/or psychiatric populations following a preregistered protoco

Behaviors Associated With Improved Relationship Satisfaction: Examining Change in the Relationship Checkup.

Relationship distress and dissolution are reported by about one-third of couples in the United States. Despite evidence that couple therapy is an effective treatment for relationship distress, many couples encounter barriers (e.g., cost, time, transportation, childcare) to accessing this kind of relationship help. The Relationship Checkup is a two-session, individualized intervention where each couple receives evidence-based suggestions to address specific relationship concerns. The Relationship Checkup was delivered as a home visitation intervention in the Southeastern region of the United St

Stress and coping experiences of Black family cancer caregivers: a qualitative investigation.

PURPOSE: High costs and care needs impact not only individuals living with cancer but also their family caregivers. As cancer burdens disproportionately affect Black/African American populations, Black caregivers may provide more intensive care and face greater financial impacts than White caregivers, yet experiences of Black cancer caregivers are understudied. This research explored the lived experiences of stress and coping among Black caregivers of adult care recipients (CRs) with cancer. METHODS: In-depth qualitative interviews were conducted with twelve Black cancer caregivers (seven mal

Prediction of cognitive performance by demographics, sleep, and brain morphometry: machine learning findings from ENIGMA-Sleep Working Group.

Group-level studies have highlighted the roles of aging, poor sleep, and brain atrophy in cognitive performance (CP) but have overlooked inter-individual variability. We predict CP from feature sets (demographic, subjective/objective sleep parameters, and regional brain morphometry) using multisite ENIGMA-Sleep data (n = 2,372). Linear and non-linear machine learning models were trained on the largest cohort (n = 845), and the best-performing models were validated on independent cohorts. Subsequently, based on the best-performing model on the largest cohort, we characterized feature importance

From case-control averages to validated subgroups: Interpreting inflammatory and neuroaxonal-injury biomarkers in schizophrenia and major depression.

Psychiatric biomarker research is characterized by subtle but potentially informative biological signals that often lie close to methodological noise. Compared with neurodegenerative or infectious diseases, candidate blood and cerebrospinal fluid (CSF) markers in major psychiatric disorders often show small-to-moderate group differences, substantial patient-control overlap, and a more demanding path to clinical use. Nevertheless, such signals may help identify biologically-meaningful subgroups and contribute to a shift from symptom-based diagnoses toward neuro-immune-endocrine stratification f

Hearing loss and executive functions - results from a population-based cohort study.

INTRODUCTION: Hearing loss is linked to cognitive decline, yet its relation to specific executive functions remains unclear. This study examines associations between planning ability, pure-tone audiometry and speech-in-noise performance. METHODS: Data were drawn from the Gutenberg Health Study (GHS), a large population-based cohort at the University Medical Center Mainz. Participants with complete pure-tone audiometry and planning ability data were included (N = 4,458; 2,314 males (51.9%), 2,144 (48.1%) females; mean age 58.0 ± 10.1 years, range 40-80). Audiologic testing included air- and bo

Functional Outcome Prediction in Young Adults With Mental Health Symptoms Using Machine Learning and Large Language Models: Longitudinal Observational Study.

BACKGROUND: Functional impairments associated with mental health conditions are on the rise. Predicting functional outcomes may improve the targeting of preventive interventions. While prognostic models have primarily focused on psychosis, early recognition services require a transdiagnostic approach. OBJECTIVE: This study aimed to predict global functioning within a 2-year follow-up using baseline clinical and structural magnetic resonance imaging (MRI) data in a population-based sample of young, help-seeking individuals presenting with affective and anxiety symptoms as well as attention-def

Feasibility and safety of transcranial direct current stimulation in the treatment of adolescent depression in a naturalistic inpatient setting: a double-blind randomized controlled trial.

Transcranial direct current stimulation (tDCS) has potential as a treatment for adult depression, but its effectiveness in adolescents remains unexplored. This study evaluated the feasibility, safety, and efficacy of tDCS in young in-patients with depression. In a randomized, double-blind, sham-controlled trial, 34 adolescent in-patients (mean age 15.48 years) received ten sessions of either tDCS or sham stimulation over two weeks, alongside standard treatment. Depression severity, quality of life, emotional and behavioral issues, and executive functions were assessed before, immediately after

Global and regional DNA methylation patterns in heart failure: a case-control analysis.

BACKGROUND: Heart failure (HF) is a syndrome of heterogeneous aetiology and multiple exposomal causes. The role of DNA methylation (DNAm) changes in symptomatic HF and its phenotypes is unclear. This work addresses global and regional DNAm patterns and epigenetic ageing in HF. METHODS: Patients with HF were included from the MyoVasc Study, an investigator-initiated prospective cohort study. Individuals without HF were included from a population-based Gutenberg Health Study. DNAm was measured using the Illumina Infinium MethylationEPIC assay. Epigenetic ageing was calculated using GrimAge and

Measures used to evaluate psychosocial interventions in dementia care: A narrative review and synthesis.

Psychosocial interventions are widely used in dementia care, yet standardized outcome measurement remains highly variable, and recent frameworks emphasize outcomes prioritized by people living with dementia and their care partners. This narrative, measurement-focused review does not appraise or synthesize treatment effects. Instead, it aims to map outcome measures to the International Consortium for Health Outcomes Measurement (ICHOM) dementia set plus an additional carer-wellbeing domain, to organize them into a taxonomy of wellbeing domains that highlights patterns and gaps in measurement pr

Multidimensional psychosocial screening in routine cardiac care: preliminary findings from a cross-sectional survey.

BACKGROUND: Psychological distress is common among patients with cardiovascular disease. While European Society of Cardiology (ESC) position papers highlight the relevance of mental health in cardiology care, psychosocial assessment remains inconsistently implemented in routine clinical practice. METHODS: In this preliminary cross-sectional survey, patients attending a tertiary cardiology department completed an anonymous multidimensional psychosocial screening during routine care. Standardized instruments assessed depressive symptoms (PHQ-9), generalized anxiety (GAD-7), heart-focused anxiet

Creating community through Walking the Talk for dementia.

OBJECTIVES: Creating community is not only therapeutic-it has been described as essential. This study explores a process of community-building among people with lived and professional experience in the dementia field, during Walking the Talk for Dementia (WTD), a week-long, immersive event on the Camino de Santiago in Spain. Absence of community can lead to fragmented support for both people living with dementia and their care partners and for those working in the field. Achieving a sense of community, however, can be complex. METHODS: A qualitative evaluation was conducted with 79 participan

Two decades of progress in schizophrenia: Evolution, current insights, and future directions.

Schizophrenia research has undergone substantial conceptual and clinical evolution over the past two decades, driven by advances in neuroscience, genetics, neuroimaging, cognition, psychopathology, public health and therapeutics. Despite these developments, major questions regarding the nature, heterogeneity, trajectory, and treatment of schizophrenia remain unresolved. In this viewpoint article, the Spanish Journal of Psychiatry and Mental Health (SJPMH) invited leading experts from different fields within schizophrenia research who have authored influential work and contributed significantly

Dialectical behavior therapy for justice-involved veterans: randomized clinical trial protocol comparing treatment approaches for veterans with criminal-legal involvement.

Criminal-legal involvement is a commonly overlooked risk factor for suicide, preceding up to 30% of suicide deaths in the United States (U.S.). Interventions to reduce risk for suicide and/or criminal-legal involvement, however, rarely address these outcomes concurrently. This paper describes a recently developed intervention, Dialectical Behavior Therapy for Justice-Involved Veterans (DBT-J), that was specifically designed to address concurrent risk of criminal-legal involvement and suicide among at-risk U.S. military Veterans. A protocol for a multisite randomized clinical trial to evaluate

Post-Acute Care Pathways After Sexual Violence and Intimate Partner Violence: An International Health-Services Scoping Review with Implications for Italy.

Background/Objectives: Survivors of sexual violence and domestic violence/intimate partner violence (IPV) often require support beyond the immediate emergency encounter; however, post-acute care remains inconsistently defined, unevenly organised or conceptualised, and fragmented across service systems. This scoping review mapped international post-acute follow-up, care, assistance, and support pathways, with particular attention to organisational models, continuity mechanisms, loss to follow-up after first access, and implications for the Italian context. Methods: We conducted an international

The role of alcohol control policy on the level of alcohol consumption in member states of the Association of Southeast Asian Nations 2000-2022: identifying trends and country clusters for further analyses.

BACKGROUND: In his historical studies of current high-income countries, Angus Deaton demonstrated a correlation between a country's prosperity and the health of its population. His conceptual framework has recently been expanded to include the role of alcohol. In this ecological study, we test the key assumptions of the expanded framework and classify countries with respect to both their levels of alcohol consumption and alcohol control policies to economic development. METHODS: We explored linear trends from 2000 to 2022 in gross domestic product per capita at purchasing power parity (GDP-PP

Electroconvulsive therapy in Germany: a nationwide multilevel analysis of determinants, regional patterns, and institutional variation.

Electroconvulsive therapy (ECT) is an effective treatment for severe psychiatric disorders, yet its use may be shaped not only by clinical need but also by structural and institutional factors. Using nationwide 2023 administrative data from the German psychiatric prospective payment system (PEPP), we examined whether regional ECT utilization corresponded to regional diagnostic burden and assessed patient-, institutional-, and regional-level factors associated with receiving ECT. The analysis included 837,762 inpatient psychiatric discharges; in addition, descriptive information on day hospital

Baseline inflammatory profiles in moderate-to-severe depression and differential response to intermittent theta-burst stimulation.

Major depressive disorder (MDD) is a biologically heterogeneous condition, with a subset of patients exhibiting elevated inflammatory markers associated with greater symptom burden and lower responsiveness to antidepressants. Treatment outcomes with repetitive transcranial magnetic stimulation (rTMS) are also highly variable, and inflammation may impair neuroplasticity, thereby modulating treatment efficacy. This exploratory study investigated whether baseline inflammatory protein signatures could influence the response to intermittent theta burst stimulation (iTBS). Using the Olink® Target 96
