Acoustic properties of utterance-initial glottal stop in Hawaiian.

The Hawaiian phoneme inventory includes a consonant ('okina) typically described as glottal stop. Recent studies have found that 'okina is rarely produced with full glottal closure utterance-medially, and is instead produced as a period of creaky phonation. This study investigated the acoustic correlates of utterance-initial 'okina following a pause. Although visible creaky phonation did not occur for utterance-initial 'okina, several reliable acoustic correlates were found during the first vowels of utterances that started with 'okina. Vowels preceded by 'okina had higher fundamental frequenc

Diagnostic value of the percentage of glottic opening score for classifying videolaryngoscopy in children: a prospective validation study.

INTRODUCTION: The percentage of glottic opening (POGO) score quantifies the laryngeal view obtained during laryngoscopy. This secondary analysis of the prospective observational PeDiAC study aimed to compare the POGO score with subjective glottic view ratings on a visual analogue scale (VAS) for classifying difficult videolaryngoscopic tracheal intubation. METHODS: Videolaryngoscopy was used as the first-line approach for tracheal intubation in children over a study period of 16 months. Immediately after tracheal intubation, the airway operators documented a VAS rating of 0-100, with higher v

Clinical Applications of Deep Learning for Glottal Area Segmentation and Glottal Area Waveform Feature Computation.

Goal: Analysis of the glottal area during vocal fold vibration has gained increasing attention. However, traditional analysis requires manual, frame-by-frame glottal area annotation to compute the glottal area waveform, a time-consuming, and error-prone process. Methods: This study proposes an automated system for glottal area segmentation and glottal area waveform feature extraction from 36 videostroboscopy recordings of 23 patients with vocal fold nodules. The system integrates YOLO and U-Net architecture for glottis detection and segmentation. Subject-independent 5-fold cross-validation was

Dynamic 3D MRI of vocal fold oscillations: In vivo assessment of vocal fold thickness, contact area, and glottal area waveform across phonation types in comparison with high-speed imaging.

Conventional endoscopic imaging of vocal fold (VF) vibration provides only a two-dimensional superior view, missing essential vertical dynamics such as VF thickness and vertical displacement. Simulation studies have assigned these parameters important regulatory functions, underscoring the need for in vivo methods capturing the three-dimensional (3D) VF geometry and vibratory motion. In this in vivo case study, dynamic 3D VF MRI was applied in a professionally trained singer, achieving sub-millimeter spatial and sub-millisecond temporal resolution. Six phonation types were produced according t

Massive Retrosternal Thyroid Swelling: A Supraglottic Solution.

Massive thyroid swellings with retrosternal extension pose significant airway challenges due to tracheal compression, deviation, and distortion of normal anatomy. Awake fiberoptic bronchoscopy is the gold standard for anticipated difficult airways; however, failure of this technique, particularly when front-of-neck access (FONA) is not feasible, can rapidly lead to a life-threatening airway emergency. We report the management of a 68-year-old American Society of Anesthesiologists (ASA) physical status III female patient with a large retrosternal thyroid swelling causing positional respiratory

The Reliability of Intraoperative Frozen Section for Glottic Lesions Suspected of Early Malignancy.

OBJECTIVE: Glottic lesions suspected of malignancy may be treated by laser cordectomy. Although obviating the preoperative biopsy can expedite treatment and preserve the surgical planes, it poses a risk of mistreatment. This study sought to determine if intraoperative frozen section (FS) prior to cordectomy can reliably guide lesion management. STUDY DESIGN: Retrospective chart review, 2017 to 2024. SETTING: Two tertiary university-affiliated referral centers. METHODS: Patients who underwent intraoperative FS sampling of glottic lesions suspected of early malignancy were identified. In all

Personalized automatic management of tracheal cuff pressure and subglottic secretions drainage to prevent pneumonia in critically ill intubated patients. The MICROINHALO multicenter randomized controlled trial.

PURPOSE: The MICROINHALO trial investigated whether personalized management of endotracheal tube cuff pressure (Pcuff) based on exhaled CO2 measurement combined with automatic subglottic space drainage (SSD) may prevent tracheal colonization in critically ill intubated patients. METHODS: This cluster-randomized, international, open-label trial (NCT05403320) enrolled adult patients at 10 ICUs. They were randomly assigned to receive either an endotracheal tube equipped with automatic Pcuff management and SSD, or a conventional one with manual Pcuff management and manual SSD. The primary endpoin

Hybrid external-endoscopic wendler glottoplasty: achieving scarless anterior synechia in the challenging larynx.

BACKGROUND: Anterior commissure exposure is the primary challenge in Wendler glottoplasty, often hindering successful suturing in narrow or prominent larynges. METHOD: We describe a "scarless" hybrid technique for difficult cases. Following fiber-laser de-epithelialization, two 18-gauge trocars are inserted percutaneously through the thyroid cartilage. A suture is passed through the trocars, retrieved endoscopically, and tied externally over a Silastic sheet, bypassing endolaryngeal space constraints. Hyaluronic acid is injected to reduce suture tension. CONCLUSION: This combined approach sa

Near-Complete Airway Obstruction Caused by a Retropharyngeal Hematoma Following Anterior Cervical Discectomy and Fusion Managed With Nasotracheal Intubation.

Postoperative retropharyngeal hematoma after anterior cervical spine surgery is a rare but potentially life-threatening complication because it can rapidly cause airway obstruction. In addition, it may result in an unanticipated difficult airway due to anatomical distortion. A patient underwent an anterior cervical discectomy and fusion (ACDF) at the C3-C5 level for dropped head syndrome. Approximately one hour after surgery, the patient developed sudden dyspnea and agitation. Emergency reintubation was attempted; however, direct oral intubation was extremely difficult because the glottis had

[Microvascular Reconstruction of the Laryngotracheal Junction].

BACKGROUND: The laryngotracheal junction (LTJ) is a functionally critical zone, due to its narrow anatomy, complex airflow dynamics, and proximity to the glottis and cricoid cartilage. Extensive stenoses, cricoid defects, and stomal anterior wall loss frequently exceed the possibilities of conventional resection and reconstruction. OBJECTIVE: To outline the current role of microvascular reconstruction at the LTJ, with a focus on indications, techniques, and limitations from the perspective of otolaryngology and thoracic surgery. METHODS: Narrative literature review of clinically established

High-fidelity simulation in airway management training: results from the AIR-MASTER-SIM randomised study.

INTRODUCTION: Airway management is a key competency in emergency medicine and critical care, and effective training in endotracheal intubation is essential during undergraduate medical education. Direct laryngoscopy has traditionally been considered the standard technique for airway access, although it can present technical difficulties for inexperienced operators. Videolaryngoscopy provides indirect visualisation of the glottis and may facilitate airway management during the early stages of training. This study compared the acquisition of airway management skills among medical students traine

Tramadol-Induced Persistent Singultus: A Case Report on Diagnosis, Mechanism, and Management.

Hiccups, or singultus, are involuntary diaphragmatic contractions followed by the sudden closure of the glottis, resulting in a characteristic sound. These episodes are generally transient but can be classified as acute (lasting < 48 h), persistent (lasting > 48 h), or intractable (lasting > 1 month). While most cases are self-limiting, persistent or intractable hiccups often suggest an underlying organic, psychogenic, or idiopathic cause. Among the potential causes, drug-induced singultus is a significant yet frequently underrecognized etiology. Although generally benign, persistent hiccups c

Case Report: External fixation of airway stents for management of tracheal stenosis secondary to thyroid cancer.

BACKGROUND: Thyroid cancer, in the advanced stage, often invades the airway and leads to tracheal stenosis. Airway stents can be used as a palliative treatment to restore airway and relieve dyspnea. However, stent migration remains a frequent complication, especially for lesions near the glottis. Simple fixing technology suitable for environments with limited resources is rarely reported. METHODS: A 79-year-old woman with advanced thyroid cancer developed upper tracheal stenosis due to tumor compression. To restore airway patency, the patient accepted placement of a self-expanding metallic st

Impact of Infraglottic Capture in Anterior Glottoplasty Pitch Elevation in an Ex Vivo Porcine Model.

OBJECTIVE: Endoscopic vocal fold shortening (EVFS) often focuses on the anterior-posterior (AP) web dimension as a proportion of vocal fold (VF) length. We investigated the craniocaudal dimension to determine whether thick infraglottic web geometry produces greater fundamental frequency (F0) elevation than thin webs of equivalent AP dimension. METHODS: Supraglottic structures were removed from eight porcine larynges. Compressed air passed through the glottis achieved phonation under three conditions: baseline, thin web, and thick web. Thin web sutures placed at a mean of 37% of VF length capt
