Management of childhood sexual abuse.

Childhood sexual abuse is increasingly recognized as a major problem in the United States. These children are frequently seen by the primary care provider, and physicians must be able to evaluate them. Common presentations for sexually abused children include direct allegations, suspicions by the caretaker, unexplained vaginal trauma or bleeding, sexually transmitted disease, and a wide spectrum of behavioral problems including sexual promiscuity and runaway behavior. The interview and physical examination must be performed in a quiet, private area and sufficient time allocated to allow a thor

Family therapy: its role in the prevention of criminality.

The family imprints its members with selfhood in all cultures. Absence of family imprints can result in the development of deviant childhood behavior and loss of identity. In two black racial groups in dissimilar world areas and sampling (Kenya, Africa and Newark, N.J.), five familial determinants interfered with the development of culturally accepted family imprints and led to deviant and criminal behavior. It is suggested that if these deterrents, namely poverty, lack of family authority figures, rejection of of the individual child, absence of family cohesiveness, and loss of individual ide

Childhood fear and anxiety states in relation to dental treatment.

Five years experience by the authors in organizing a dental psychology clinic has impressed on them that many excessively fearful child patients have a history of untoward dental and medical experiences which might have been prevented. A background to an understanding of fear and anxiety in children is presented, and two cases are described to illustrate straight-forward behavior modification techniques which can be used to help the fearful or excessively anxious child cope with dental treatment.

Adaptation of siblings to childhood malignancy.

To explore the impact of childhood cancer, school-aged patients and their healthy siblings from 71 families were given one or more of the following psychologic tests: the Piers-Harris Children's Self-Concept Scale, the Bene-Anthony Family Relations Test, and the Thematic Apperception Test. The siblings showed even more distress than the patients in the areas of perceived social isolation, perception of their parents as overindulgent and overprotective of the sick child, fear of confronting family members with negative feelings, and concern with failure (older siblings only). In other areas, su

Psychological reactions to chronic medical illness.

Psychological reactions to chronic medical illness can be categorized as follows: the chronically ill patient is vulnerable, first and foremost, to eight types of psychological stress, all of which have their roots in early childhood. Specifically, chronic illness evokes a threat to the patient's self esteem; fear of strangers; separation anxiety; fear of loss of love, and of the control of developmentally achieved functions; fear of loss of, or injury to body parts; guilt and fear of retaliation; and fear of pain, which cuts across all of these stresses. Other psychological reactions to chron

Stage fright.

Stage fright is a universal human experience that occurs with varying intensity in everyone who stands before an audience. The anxiety generated in this situation stems from the re-emergence of certain key developmental experiences. The dynamics involved are related both to genital and to pre-genital conflicts. Shame arises from conflicts around exhibitionism, from concerns over genital inadequacy, and from the fear of loss of control. Guilt is produced from the aggression inherent in self-display and from the fear of the destruction of one's rivals, along with the dread of retaliation. A majo

An hypothesis about heroin addiction, murder, prostitution, and suicide: acting out parenting conflicts.

From interviews with forty subjects, we have developed a preliminary hypothesis which can be refined and deepened and then tested by psychotherapeutic investigation. These murderers, prostitutes, heroin addicts and successful suicides all demonstrated various aspects of difficulty with their capacity for reciprocal tenderness. This arose from their experiences of exploitation and derprivation with their own parents. Their unique characteristics included primitive rage, insatiable demands and capacity for unconflicted manipulation. These characteristics colored the expression of their intrapsyc

[An exceptional human relationship (Michael P. Kierkegaard, Søren Kierkegaard)].

There were a father and a son, Michael and Sören Kierkegaard, both highly talented, imaginitive, witty, humorous, especially interested in shrewd dialectrical dialogues respectively debates, both profound melancholy ponderers. But according to very own individuality their congeniality wasn't absolute. The father, being under a questionable rigorous opinion of sin, suffered from undeserved divine grace since early childhood. And Michael Kierkegaard made Sören anxious with the religious experience of original sin. This anxiety moved (touched or affected) the sensitive son mighty. Nevertheless he

Excessive masturbation of childhood: a symptom of tactile deprivation?

Five cases of excessive masturbation are presented. All cases represent adequate parent-child relationships altered by parental change of behavior toward the child. Upon reinstitution of affectionate tactile contact by the parents, the excessive masturbation ceased. Theoretical considerations are discussed.

Parent Attitude Research Instrument (PARI): clinical vs. statistical inferences in understanding abusive mothers.

Attitudes toward child rearing and early experiences in childhood traditionally have been regarded as psychodynamic factors that influence adult behavior. The present study assessed the capacity of the Parent Attitude Research Instrument (PARI) to differentiate between court-identified abusive mothers and a control sample of nonabusive, well-baby clinic mothers of comparable socioeconomic status. While univariate t-tests showed statistical significance for 3 of the 23 PARI scales, more sophisticated and appropriate multivariate tests (Discriminant Function Analysis) demonstrated that the PARI

Sexual excitement.

Sexual excitement depends on a scenario the person to be aroused has been writing since childhood. The story is an adventure, an autobiography disguised as fiction, in which the hero/heroine hides crucial intrapsychic conflicts, mysteries, screen memories of actual traumatic events and the resolution of these elements into a happy ending, best celebrated by orgasm. The function of the fantasy is to take these painful experiences and convert them to pleasure-triumph. In order to sharpen excitement-the vibration between the fear of original traumas repeating and the hope of a pleasurable conclus

[Biographical situation of scoliotic children and their relationship to the social environment(author's transl)].

Studies were conducted to determine whenther or not scoliosis in childhood affects the behavior and experience of the affected children. Two groups of scoliotic and nonscoliotic children were examined with selected psychological tests. No differences could be found in regard to neuroticism, extraversion, manifest fear or the children's self-image. Differences were found in regard to the social environment which scoliotic children experienced as more demanding and supportive. As a result they were more concerned with achievement in a performance situation and more confident of success. The type

[Desire and self-worth].

Correlation between the experience of pleasure and selfesteem of neurotic patients is examined, based on practical knowledge in the therapeutic area. The embittering of essential parts of later self- and identityfeeling by cultural discrimination of primary pleasure of oneself during the early childhood is shown by a clinical case. Primary selffeeling is degraded into guilt and fear and cannot be replaced by secundary satisfactions. Finally the ambiguous aspects of pleasure in psychoanalytic theory are deomonstated which recently have found a more obvious contour in the polarisation between na

Life experiences and the decision to become a mental patient.

This research focuses upon life events influencing the decision to adopt or reject the mental patient role. Studied were 30 mental health center patients who defined themselves as mentally ill and aggressively sought treatment. These were contrasted with 21 persons who consistently denied that they were mentally disordered, and persistently resisted the treatment forced upon them. Patients in both groups had been exposed to the mental health system on the average of 7 years each. Findings showed that those identifying with the mental patient role, in contrast to those resisting it, were more l

[The reasons for the resistance to treatment in childhood. Presentation of the problem based on a 2 1/2-year patient study with 65 treatment-resisting children, aged 3 to 14 years].

On the basis of the experience gained with 65 children trying to resist dental treatment, the problems causing and influencing this behavior of children having an aversion against the dentist are presented. Apart from factors resulting from specific situations or specific ages, factors were found that are due to parental education, social environment, and serious psychical disturbances of the child.

Deprivation in the childhood of depressed women.

With recent developments in community psychiatric services, concern with prevention has become an urgent social, as well as medical challenge. Comprehensive investigation into causation must therefore be given systematic emphasis. This paper is an effort toward clarification of etiology, specifically of the depressive disorder, in terms of early childhood experiences. The work is based on social and psychiatric history data collected by experienced psychiatric social workers on two groups of subjects from the National Institute of Mental Health (NIMH) Collaborative Depression Studies under the

BLOOD LOSS DURING PEDIATRIC OPERATIONS.

The gravimetric method of measuring blood loss was used during all types of pediatric operations at The Montreal Children's Hospital. Results of 1787 such measurements indicated that the method is of value in the management of transfusion. Grading blood loss as minor, moderate or severe, in terms of its relation to total blood volume or body weight is a procedure of practical value. Operations were grouped into those that usually led to a loss of less than 10% of the blood volume (7.5 ml./kg.), those with a loss usually between 10% and 14% and those with blood loss usually over 14% (10.5 ml./k
