Using the connecting process to meet family caregiver needs.

Individuals who engage in caregiving responsibilities for dependent family members in the home setting face challenges that are little understood by others outside the circle of care. Because a large portion of energy is devoted to caregiving tasks, caregivers potentially fail to devote attention to meeting their own needs. The purpose of this study was to develop a beginning substantive theory describing the needs perceived by caregivers and the processes by which these caregivers are able to get their individual needs met. Grounded theory methodology was used to facilitate the identification

Psychosocial concerns of the ventilator-dependent child in the pediatric intensive care unit.

An increasing number of infants and children suffer from respiratory and neuromuscular diseases that render them ventilator-dependent. These patients spend months in pediatric intensive care units (PICUs) until they are either extubated or are stable enough to tolerate home ventilation. The critical nature of the PICU and its emphasis on meeting short-term goals is not readily compatible with the long-term and dynamic needs of developing children. In order to prevent the delays in growth and development that often occur during prolonged hospitalizations, this article addresses the impediments

Local coalitions for coordinating services to children dependent on technology and their families.

The emerging population of children who are technology dependent has made a significant impact on the health care service system in the last decade. The unique characteristics and numerous unmet needs of this group of children engender grassroots planning to improve the responsiveness and coordination of the service system. A description of three local planning initiatives is presented within the framework of a collaborative statewide planning effort surrounding the needs of children who are technology dependent and their families.

Technology-dependent children in New York State.

A survey in New York State revealed 210 medically stable technology-dependent children retained inappropriately in acute-care hospitals. They remained in hospitals primarily because of a shortage of skilled home nursing services in the communities where they lived, family problems that prevented their care at home, and a shortage of beds in appropriate skilled nursing facilities. Most of the children were younger than one year of age, most had disorders of the central nervous system or respiratory system, and most needed respiratory support. The average child needed more than 13 hours of skill

Community care for adolescents with developmental retardation: problems and proposals.

The quality of community care provision for adolescents with developmental retardation was examined in a health district with a well-developed community service. This paper describes a representative sample of such adolescents living at home, and is based on data from a wider study of the dependency needs of this group. Although many professionals and agencies were involved with each family during the six months prior to the study, their input was poorly co-ordinated and did not focus on the group's special needs. Psychiatric disorder in the adolescent, and relationship difficulties with the p

Social and demographic characteristics of patients admitted to a palliative care unit.

In this retrospective study we reviewed the charts of 169 consecutive admissions to the Palliative Care Unit, Edmonton General Hospital. Demographic and social characteristics of patients were assessed in order to determine the likelihood that the patients could be managed at home according to currently available services. The mean age of the population was 65 +/- 12 years, 97 (57%) were women, they had a variety of cancers with major prevalence of the most frequent adult tumors, and 72% of patients were coming from acute care hospitals. Each patient had an average of 2.7 +/- 1.8 children (med

Parental perceptions of newborn pharmacologic paralysis.

This is a descriptive exploratory study of 9 parent-dyads who experienced pharmacologic paralysis of their newborn within a prior 6-month period. Perceptual responses included feelings, values, and needs related to the pharmacologic paralysis therapy. Newborns receiving pharmacologic paralysis had direct dependence to the caretaker for mechanical ventilation, precise monitoring of physiological parameters, and extensive life support due to altered sensory-motor function and breathing. The appearance of the immobile newborn and the protocols for the administration of paralyzing agents, which el

[Care for dementia patients by their families and home care].

Present problems concerning home care for the aged including the following three points: 1. The extent of the period during which care is needed has been prolonged as the aged live longer than in previous times. 2. The range of need for home care has increased. 3. As a result of the increase of the individual-dependent "life support infrastructure" due to the decreased role of the family-dependent "life support infrastructure", the aged, who were more self-supporting in past, have become more dependent on care both in quality and quantity. In particular, dementia patients gradually worsen, the

Simple counts of the number of basic ADL dependencies for long-term care research and practice.

General acceptance of a patterned progression of dependency in activities of daily living has led to the widespread practice of simply counting the individual's basic ADL dependencies to reflect his or her self-care needs and level of impairment. This method is convenient, and it is practical to the extent that individuals do fit a scaled pattern of dependency that allows some meaningful comparison among individuals and between groups to be made. This research, based on 3,611 Medicaid cases in Virginia, reports that 36 percent of those individuals screened for nursing home admission do not mat

Transitioning the child fed by gastrostomy into school.

Young children dependent on gastrostomy feeding are examples of special-needs children now eligible for expanded nutrition services in schools through Public Law 99-457. With the implementation of this law, nutritionists have an opportunity to expand their roles as consultants to special education teachers. A team approach involving nutrition, nursing, and special education was used to prepare a gastrostomy-fed preschool child and his family for school. The family of the multihandicapped child had been isolated by the child's feeding problems and viewed the problems as barriers to his entering

A reappraisal of the Malaise Inventory.

In recent years Rutter's Malaise Inventory has been used increasingly to measure levels of stress among those who care for people with dependency needs. Although there has been an element of dispute in the literature about the unidimensional nature of the scale, later work (Bebbington and Quine 1987) tends to suggest that Malaise Inventory scores can be used as a single internally consistent measure of stress. In this study consideration is given to the results of a national sample survey of 527 members of the Association of Carers (now Carers: National Association) which suggests the existenc

Vying for time. Work and family.

Time--there is never enough of it. There are never enough hours in the day to do everything at work and at home. Today's employees, forced to choose between work obligations and family responsibilities, often feel frazzled and guilty. Some employers recognize these conflicts and are finding ways to lighten the work/family load. They are offering a sophisticated combination of dependent-care options, flexible work hours and family-related benefits tailored to meet the needs of the employees within their specific industries. HRMagazine looks at leaders in six major industries--manufacturing, hea

The dependency at discharge instrument as a measure of resource use in home care.

The dependency at discharge instrument, developed to evaluate the needs for care of hospitalized patients on the day of discharge, was tested for its usefulness in measuring resource consumption in home health care. The instrument evaluates dependency in relation to bathing/hygiene, activity, technical procedures, and monitoring signs and symptoms. The sample used in this study consisted of 150 subjects admitted to one of three types of home health care agencies. Interrater reliability and internal consistency of the instrument were high. Factor analysis produced results similar to those of th

Dependency and interdependency: the incomes of informal carers and the impact of social security.

Consideration of the income and social security needs of informal carers has remained conspicuously absent from discussions about 'community care'. Similarly, carers have been more or less invisible in the development of social security policies. This paper reports on a study of the financial circumstances of a sample of working age carers, who were living with and providing substantial amounts of help and support to a disabled person in the same household. The study highlights first, the substantial work-related costs incurred by carers with full time employment; and second the financial depe

[A study of social needs and human relations of residents of a nursing home for the physically handicapped].

Eighty-three physically disabled persons living in a nursing home for the physically handicapped were interviewed regarding family, daily living, and hope for the future. The results were analyzed from the point of view of their social needs and human relations. Concerning relations with the family, 76.8% were quite satisfied with their families. Satisfaction appeared to be dependent on their being able to communicate with their mother. In the nursing home, one bedroom was shared with 3 or 4 persons, and residents received a variety of care from female staff daily. While 50.6% of the residents

A case-comparison analysis of elder abuse and neglect.

This study examines factors which contribute to elderly abuse and neglect by caregivers in a domestic setting. Methodological and conceptual variations and problems in previous studies have led to considerable confusion as to the determinants of this important social problem. A more rigorous research design was used in this study than has been previously employed. Fifty-nine abused elders from a model project site for the study of elderly abuse were compared with forty-nine non-abused clients from a home care program in the same agency. Using a research instrument designed by the authors, data

[Nursing the patient with AIDS and the AIDS carrier].

The nursing care of the patient with AIDS consists of a challenge for the improvement of Nursing in general not only because of the gravity of the disease and the very bad prognosis but also because of the great socioeconomic and psychological effects that accompany it. Nursing care in the hospital consists of facing the symptomatology of the disease and meeting the many physical, psychological and social needs of the patient. Nursing care at home is influenced by many factors and is dependent on the good information given to the members of the family through a well organized service of the co

"A normality for us without confinement for them": notes on the associations of families of the mentally ill.

Associations of families of the mentally ill arose in Italy in the early 1980s mainly in the country's northern and central regions. They denounced the inadequacies of an extramural service conceived of as an out-patient clinic filling the void left by the shutting down of asylums. The positions elaborated by these associations focus on four main themes: the distribution of the "burden of care" for the ill between services and families; the ill-conceived interpretation of the families' needs owing to the professional's alleged tendency to "point the finger of guilt" at the families themselves;
