A Case of Hemoptysis due to Rupture of Bronchial Varices Requiring Multidisciplinary Treatment.

BACKGROUND: This paper describes a rare case of hemoptysis caused by the rupture of a bronchial varix. CASE PRESENTATION: A 68-year-old man was admitted to the hospital due to recurrent episodes of hemoptysis. Bronchoscopy showed a cord-like lesion in the left main bronchus, and the findings of the subsequent endobronchial ultrasound and bronchial venography on the third hospital day led to a diagnosis of hemoptysis due to a bronchial varix rupture. Through multidisciplinary collaboration, hemostasis was achieved by performing a combination of treatments, including bronchial artery embolizati

Primary Pulmonary Leiomyosarcoma Successfully Treated With Radiotherapy Followed by Chemotherapy After Recanalization of Central Airway Obstruction by Bronchoscopic Cryotherapy: A Case Report.

Primary pulmonary leiomyosarcoma (PPL) is the most common histological subtype of primary pulmonary sarcomas, although it is relatively rare. Herein, we report a case of a 76-year-old man with a mass arising in the hilar region of the right upper lobe of the lung and progressing to the right main bronchus, obstructing the bronchial lumen. He was admitted due to a five-day history of worsening respiratory distress and had a history of left upper lobectomy for primary lung adenocarcinoma. Bronchoscopic cryotherapy enabled the diagnosis of leiomyosarcoma of the lung and was effective in the recan

Hilar fat invasion is a poorly defined and inconsistently interpreted lung carcinoma staging parameter.

OBJECTIVES: Hilar fat invasion raises the T category of otherwise pT1 lung carcinomas to pT2a. Pathologists were surveyed to better understand their awareness and interpretation of hilar fat invasion. METHODS: Pathologists interested in pulmonary pathology were invited to participate in an online survey of hilar fat invasion assessment. RESULTS: Of 80 respondents, 58% considered hilar fat any fat that is outside the lung but inside the pleural reflection, 32% considered it to be fat inside the pleural reflection near hilar lymph nodes around the main bronchus, and 10% were uncertain or had a

Cross-field airway management for distal postintubation stenosis: a case of successful reconstruction without ECMO support.

BACKGROUND: Tracheal stenosis following intubation is a relatively rare but difficult condition. Some degree of airway injury is common following intubation, no matter whether it is prolonged or of short duration. CASE REPORT: Patient is a 54-year-old woman with a past medical history of severe traumatic brain injury (TBI) and intracerebral hemorrhage, for which she was admitted to the intensive care unit (ICU) for neuroprotection. She remained in the ICU for over two months with prolonged mechanical ventilation and tracheostomy. After full recovery, she developed wheezing and dyspnea. A post

A rare incidence of primary pulmonary undifferentiated pleomorphic sarcoma detected as an endobronchial mass and treated by bronchoscopic resection: a case report and literature review.

RATIONALE: Primary pulmonary undifferentiated pleomorphic sarcoma (PPUPS) is a highly malignant tumor with extremely rare occurrence and is known for its distinct cellular pleomorphism. This tumor lacks specific differentiation markers. Consequently, PPUPS diagnosis requires thorough exclusion of other tumors through a comprehensive assessment of clinical, histopathological, and radiological outcomes. PATIENT CONCERNS: A male patient of age 78 years presented to the hospital showing symptoms of continuous cough and blood-stained sputum for 1 month. Chest computed tomography detected a massive

Robot-Assisted Reinforcement with a Free Pericardial Fat Pad for Double-Lumen Tube-Related Injury of the Left Main Bronchus Incidentally Detected during Lobectomy: A Case Report.

INTRODUCTION: Tracheobronchial injury associated with double-lumen tube (DLT) placement is rare and typically diagnosed postoperatively. Intraoperative detection of subtle airway lesions is extremely uncommon. We report a case of DLT-related injury of the left main bronchus that was incidentally detected during robot-assisted thoracic surgery (RATS) and successfully reinforced with a free pericardial fat pad. CASE PRESENTATION: An 82-year-old man underwent robot-assisted left lower lobectomy and mediastinal lymph node dissection for suspected primary or metastatic lung cancer. During subcarin

Re-Evaluating Anatomical Determinants of Foreign Body Aspiration in Adults.

Objectives: Tracheobronchial foreign-body aspiration is a clinically significant condition in adults. Although classical teaching attributes the right-sided predominance to the steeper, shorter, and wider morphology of the right main bronchus, this explanation has not been empirically validated in patients with confirmed foreign-body aspiration. Methods: This retrospective study included adult patients (≥18 years) who underwent bronchoscopic removal of foreign bodies from the right or left main bronchus between 2012 and 2025. Only individuals with available preoperative chest radiographs were

ABPM secondary to Saccharomyces cerevisiae mimicking lung cancer in an immunocompetent patient.

SummaryWe present a case of allergic bronchopulmonary mycosis due to Saccharomyces cerevisiae mimicking lung cancer in an immunocompetent woman in her early 70s. She had a 6-week history of dry cough and chest X-ray changes concerning for malignancy. CT initially suggested primary lung cancer. Blood tests showed eosinophilia (4.2×10⁹/L) and moderately elevated total IgE (296 IU/mL). Aspergillus precipitins were mildly raised (140 mgA/L), but no Aspergillus-specific growth or serological sensitisation was found. She developed acute respiratory distress requiring emergency cardiothoracic interve

When allergy mimics catastrophe: allergic bronchopulmonary aspergillosis presenting as unilateral lung collapse with respiratory failure.

Allergic bronchopulmonary aspergillosis (ABPA) is a T helper 2 cell-mediated hypersensitive lung disorder in response to Aspergillus that usually affects asthmatic and cystic fibrosis patients. However, it rarely presents as lung collapse. We report a case of an elderly male who presented in the emergency department with respiratory distress, with oxygen saturation 80% with room air (RA). On evaluation with examination and radiology, the case was revealed as left lung collapse. Fibreoptic bronchoscopy revealed a huge mucus plug (MP) obliterating the left main bronchus, causing collapse of the e

Veno-venous extracorporeal membrane oxygenation-supported rigid bronchoscopy for airway obstruction in a post-pneumonectomy patient with recurrent neuroendocrine lung cancer: a case report.

BACKGROUND: Life-threatening airway obstructions caused by tumors require immediate and effective intervention. However, the management of severe airway obstruction during therapeutic bronchoscopy is challenging, particularly when there is a potential inability to ventilate the lungs using conventional techniques, due to extensive tracheobronchial lesions or the risk of major intraoperative bleeding related to disease characteristics. For these reasons, the multidisciplinary team opted for an endoscopic debulking with the aid of a procedural veno-venous extracorporeal membrane oxygenation (VV-

Robot-assisted tracheal carina resection and reconstruction for adenoid cystic carcinoma: a case report.

BACKGROUND: Carinal reconstruction is the primary surgical intervention for tracheal tumors involving the tracheal carina. However, the complexity of airway management and the challenges associated with invasive carina reconstruction significantly increase its operative difficulty. CASE PRESENTATION: A 48-year-old male patient presented with a 6-month history of persistent cough. Cervical and thoracic computed tomography (CT) imaging, along with bronchoscopic biopsy, confirmed a diagnosis of tracheal adenoid cystic carcinoma (TACC), with the tumor extending to the tracheal carina and left mai

Purulent Pericarditis After Esophageal Erosion With Streptococcus intermedius and Lactobacillus spp.

BACKGROUND: Purulent pericarditis is rare but rapidly fatal without urgent drainage and targeted antibiotics; malignancy-related effusions should not be presumed malignant when an anatomic breach is possible. CASE PRESENTATION: A 67-year-old male with esophageal squamous cell carcinoma presented with pleuritic chest pain. Electrocardiogram showed diffuse ST elevation; transthoracic echocardiography revealed a large effusion with fibrinous strands and mild right-ventricle diastolic collapse. Computed tomography angiography demonstrated tumor erosion into the left main bronchus. The patient rec

Case Report: Prolonged extracorporeal membrane oxygenation as a deliberate operative adjunct for tracheobronchial reconstruction in a child with invasive aspergillosis.

We report the case of a 6-year-old boy with newly diagnosed type 1 diabetes mellitus presenting in diabetic ketoacidosis, who developed a massive acquired tracheo-esophageal fistula (TEF) with extensive destruction of the tracheobronchial anatomy, including near-complete loss of the left main bronchus, secondary to invasive mediastinal aspergillosis. The clinical course was complicated by refractory respiratory failure and subsequent right ventricular failure, necessitating prolonged extracorporeal membrane oxygenation (ECMO) support totaling 96 days. The patient required veno-venous ECMO, fol

Surgical strategy after neoadjuvant immunochemotherapy for central-type NSCLC: restaging-guided versus pre-treatment-guided decision-making.

BACKGROUND: Effective neoadjuvant immunochemotherapy (nICT) can downstage central-type non-small cell lung cancer (NSCLC) and reopen previously obstructed bronchi, but surgical decisions are frequently still anchored to pre-treatment staging. This study aimed to evaluate whether a standardized post-nICT restaging-guided surgical strategy could safely support surgical de-escalation while maintaining oncologic safety in selected patients with central-type NSCLC. METHODS: We conducted a single-center retrospective cohort study of central-type NSCLC involving or abutting the lobar/main bronchus,

Post-pneumonectomy-like syndrome as a result of unilateral upper-lung field pulmonary fibrosis after lung cancer surgery: a case report.

Post-pneumonectomy syndrome is a rare complication following pneumonectomy, characterized as significant mediastinal shift and rotation toward the operated side, resulting in stenosis of trachea and bronchi. We report the case of a 60-year-old woman who underwent right lower lobectomy for lung adenocarcinoma in 2017. She subsequently developed unilateral upper-lung field pulmonary fibrosis (upper-PF) in the operated side, radiologically consistent with pleuroparenchymal fibroelastosis, as a late surgical complication. By 2025, the resulting mediastinal right shift due to upper-PF caused the de

Avoiding pneumonectomy in chronic complete lung atelectasis: sleeve right upper lobectomy for right main bronchus obstruction-case report.

BACKGROUND: Complete lung atelectasis resulting from main bronchial obstruction may arise from both benign and malignant etiologies. Diagnosis is often delayed, particularly in younger patients, leading to prolonged lung collapse and compensatory hemidiaphragmatic elevation. Pneumonectomy is traditionally considered in such cases, lung-sparing resections should be preferred, especially in young individuals. The indication for sleeve resection in the setting of long-standing complete atelectasis remains controversial. CASE DESCRIPTION: We report the clinical presentation, surgical management,

Rapid progression from croup to complete lung collapse: a case report of central airway obstruction by mucopurulent cast in influenza A (H3N2) and Pseudomonas aeruginosa coinfection in a toddler.

Croup is a common presentation in pediatric emergency settings, most often caused by viral infections. However, when influenza A virus co-occurs with Pseudomonas aeruginosa infection, it can lead to rare and lethal complications. We report a case of a 13-month-old boy who presented with fever and croup. The patient developed suffocation after 4.5 h of hoarseness and respiratory distress, with poor response to glucocorticoids therapy. Despite mechanical ventilation, respiratory failure persisted. Serial chest radiographs over a 7-h period revealed rapid progression to complete left lung atelect

Tracheobronchial stenosis as a rare extraintestinal manifestation of ulcerativecolitis: a case series with different therapeutic approaches.

BACKGROUND: Ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by extraintestinal manifestations in nearly half of patients. Although pulmonary involvement has traditionally been considered rare, with a reported prevalence of less than 1%, recent studies indicate that subclinical airway abnormalities may occur in 40-60% of cases. Tracheobronchial stenosis is an uncommon but clinically significant extraintestinal manifestation that causes considerable diagnostic and therapeutic challenges. CASE SERIES: This report presents three female patients with UC who developed

Fogarty balloon-assisted extraction of an impacted friable foreign body causing complete left main bronchus obstruction: A case report.

Foreign body aspiration (FBA) in adults is uncommon but can lead to significant morbidity due to delayed or atypical presentation. Flexible bronchoscopy is the preferred diagnostic and therapeutic modality, although extraction may be challenging depending on the foreign body's characteristics. We report a case of severe airway obstruction caused by a soft organic foreign body successfully managed using a Fogarty balloon catheter technique. A 73-year-old man with schizophrenia presented with severe hypoxemia and complete left lung atelectasis. Imaging demonstrated complete obstruction of the le
