Global, regional, and national burden of exposure to chewing tobacco, 1990-2023: a systematic analysis of the Global Burden of Disease Study 2023.

BACKGROUND: Exposure to chewing tobacco is a known health risk factor but has received less attention than smoking in global tobacco control efforts. As the evidence base linking chewing tobacco use to poor health grows, it is essential to closely track the changing use of chewing tobacco distinct from other forms of smokeless tobacco, as well as the estimated burden associated with its use. These data are essential to inform policy makers who seek to identify and avert the health burden associated with these dangerous products. METHODS: We estimated the prevalence of chewing tobacco use and

State of the global tuberculosis epidemic: burden, progress towards WHO End TB Strategy targets and systemic challenges.

Mycobacterium tuberculosis remains the leading cause of death from a single infectious pathogen globally despite decades of effective chemotherapy. In 2024, an estimated 10·7 million people developed tuberculosis, including approximately 620 000 people living with HIV (PLHIV), and tuberculosis caused an estimated 1·23 million deaths overall, including approximately 150 000 deaths among PLHIV. Men accounted for more than half of the cases, and children represented a substantial burden, reflecting ongoing transmission and diagnostic gaps. Approximately a quarter of the world's population has bee

The digital physician illusion: individual innovativeness, unlike artificial intelligence attitudes, drives rational drug use through health literacy.

BACKGROUND: The rapid integration of artificial intelligence into healthcare has transformed how individuals access medical information. However, the impact of artificial intelligence attitudes and individual innovativeness on clinically disciplined behaviors, such as rational drug use, and the mediating role of health literacy remains underexplored. OBJECTIVE: This study aims to empirically investigate the structural relationships between artificial intelligence attitude, individual innovativeness, health literacy, and rational drug use within the digital health ecosystem of Northern Cyprus.

Stress-induced reinstatement of nicotine conditioned place preference in a rodent model of dopamine dysregulation: genetic, behavioral and molecular correlates of drug abuse vulnerability.

RATIONALE: Schizophrenia and related psychoses are accompanied by poor stress adaptation and a high prevalence of cigarette smoking, contributing to reduced treatment adherence and poor quality of life. The mechanistic contributions to stress-induced drug-seeking in psychosis remain poorly understood. OBJECTIVES: The present study evaluated nicotine conditioned place preference (CPP) stress reinstatement in a heritable rodent model of dopamine dysregulation relevant to psychosis, alongside molecular and epigenetic alterations associated with drug use vulnerability. METHODS: Adolescent offspr

Assessment of Risk of Bias in Randomized Trials Using RoB 2: Reflections From a Systematic Review of Complex Interventions.

The Cochrane risk-of-bias tool (RoB 2) is used to assess the risk of bias in randomized trials in systematic reviews. However, studies have shown that the tool is often incorrectly used and published reviews rarely include detail regarding how it was applied or offer reflections on using it. This paper is a transparent applied case example with lessons learned and limitations of using RoB 2 in a (non-Cochrane) systematic review evaluating the effectiveness of remote therapies for alcohol and drug use disorders. We share our reflections on aspects to plan in advance (pre-selecting the outcome(s

Examining the Relationships Between Awareness of Rational Drug Use, Medication Adherence, and Compliance with Treatment Among the Older Adults: The Role of Perceived Social Support.

This study examined the relationships between awareness of rational drug use and two outcomes-medication adherence and compliance with treatment-and assessed whether perceived social support moderated these relationships. The sample consisted of 314 older adult patients who attended the geriatrics department of a university hospital. The study revealed that higher awareness of rational drug use was associated with higher levels of medication adherence and compliance with treatment. Moreover, the positive association between awareness of rational drug use and medication adherence was stronger a

Real-world substance use in body dysmorphic disorder: An ecological momentary assessment study of motives, contexts, and suicidal ideation.

Body dysmorphic disorder (BDD) frequently co-occurs with substance use, yet little is known about real-world patterns, motives, and contexts of use. This study characterized baseline substance use and examined day-to-day alcohol and cannabis use across 28 days of ecological momentary assessment (EMA) in adults with moderate to severe BDD with recent substance use or suicidal ideation. Eighty-seven participants (73.6% women; mean age 29.4 years, SD = 9.0, range 18-55) completed diagnostic interviews, self-report measures, and EMA surveys. At baseline, more than half (51.7%) met criteria for a c

Astrocyte molecular rhythm disruption in nucleus accumbens promotes increased binge-like drinking in mice.

Alcohol misuse is a leading cause of preventable death worldwide. Chronic alcohol is associated with disrupted circadian rhythms, yet molecular mechanisms linking circadian rhythm dysregulation and alcohol consumption are poorly understood. Current FDA-approved treatments for alcohol use disorder (AUD) do not target molecular rhythms or sleep-wake cycles. Mammalian circadian rhythms are regulated by transcription-translation feedback loops that regulate expression of 'clock genes' (e.g., Arntl encoding for BMAL1). Both human and rodent studies demonstrate associations between clock gene varian

COMT rs4680 polymorphism modulates openness to experience in women with alcohol use disorder.

PURPOSE: The objective of the study was to examine the association between COMT rs4680 genotypes, alcohol use disorder diagnosis, and differences in Five-Factor personality traits. METHODS: We examined 213 adult women in total, including 101 with a diagnosis of alcohol use disorder (AUD) and 112 non-addicted controls. All participants completed the NEO Five-Factor Inventory (NEO-FFI) and were genotyped for the COMT rs4680 polymorphism using real-time PCR. Group comparisons and genotype × diagnosis interactions were analyzed using ANOVA and post hoc tests. RESULTS: Women with AUD scored signi

Migrant women, alcohol and drinking experiences: a narrative review.

Migrant women's alcohol use has been understudied despite evidence that gender, culture, and migration shape drinking experiences. This review synthesizes qualitative research on how migrant women experience host-country drinking cultures, their gendered drinking experiences and factors influencing their alcohol use. The review was conducted across five databases (ProQuest, Dimensions, Scopus, PsycINFO and PubMed). Inclusion criteria were peer-reviewed qualitative studies published since 2000 examining migrant women's experiences with alcohol and drinking cultures. Twenty-two articles were inc

Subjective alcohol response across drinking contexts: a systematic review with bayesian meta-analysis.

RATIONALE: Alcohol subjective response (SR) predicts drinking behaviour and risk for alcohol use disorder, but there is limited consensus on how SR differs across contexts, research settings, and alcohol problem severity. OBJECTIVES: This systematic review and meta-analysis examined how alcohol SR differs across (1) commercial (e.g., bar/club/restaurant) versus non-commercial (e.g., home) settings and (2) social versus solitary drinking contexts, and if these effects are moderated by (3) laboratory versus naturalistic study setting and (4) non-problem versus problem alcohol use. METHODS: Stu

Survivorship Care Before Completion of Definitive Radiotherapy and Oncologic Outcomes in Head and Neck Cancer.

IMPORTANCE: Across cancer populations, a growing and aging survivor population combined with increasing use of multimodal therapy has expanded the need for survivorship care, particularly in head and neck cancer (HNC). Although survivorship begins at diagnosis, survivorship care is often emphasized after treatment completion, and whether earlier integration during treatment is associated with adherence to treatment and survival remains unclear. OBJECTIVE: To evaluate whether early multidisciplinary survivorship care before definitive radiotherapy completion is associated with adherence and su

Social jetlag and alcohol use in adolescents: A random-intercept cross-lagged panel analysis.

OBJECTIVE: Alcohol use and social jetlag (i.e., the misalignment between weekday and weekend sleep timing) frequently co-occur during adolescence, yet their temporal dynamics across different timescales remain underexamined. This study assessed bidirectional relationships between alcohol consumption and social jetlag at the person, 4-month, and weekly levels. METHOD: U.S. high school students (N = 302, analytic n = 297; Mage = 16.21 years) completed three 21-day assessment bursts, each comprising three consecutive weekly surveys and timeline follow-back assessments. Bursts were separated by 4

Risky sexual behaviors and associated factors among adolescent girls and young women in the context of HIV prevention pre-exposure prophylaxis in Tanzania: Analysis of the Tanzania HIV Impact Survey 2022-2023.

Risky sexual behavior contributes to HIV acquisition among adolescent girls and young women (AGYW) in Sub-Saharan Africa (SSA), including Tanzania. However, nationally representative evidence on its prevalence and associated factors remains limited. This study examines the prevalence and associated factors of risky sexual behavior among AGYW aged 15-24 years in Tanzania, in the context of the expanding Pre-Exposure Prophylaxis (PrEP) provision. A secondary analysis of nationally representative data from the 2022-2023 Tanzania HIV Impact Survey (THIS) was conducted among 3,669 sexually active A

Smokeless Tobacco Pouch Keratosis: A Report of Two Cases.

Tobacco pouch keratosis (TPK) is a white lesion caused by chronic contact with smokeless tobacco (SLT) on the oral mucosa. The lesion develops as a result of chronic tobacco chewing or snuff dipping. The lesion exhibits precancerous characteristics; thus, dentists must educate patients about the dangers of tobacco use and advise them to quit. This article discusses the clinical findings of a 17-year-old adolescent boy and a 48-year-old man who were SLE users diagnosed with TPK and nicotine replacement therapy to averse from the SLT chewing habit due to nicotine dependence. The novelty of this

Racial/Ethnic and Sexual Identity Differences in the Associations Between School Bullying and Cyberbullying Victimization and Types of Substance Use.

Bullying victimization is found to be associated with substance use, and this association is relevant to racial/ethnic minority and sexual minority (lesbian, gay, and bisexual) adolescents. However, few studies have focused on the role of intersectionality in the linkage between bullying victimization and substance use. The current research explores the relationship between school- and cyberbullying victimization and various types of substance use among a racially/ethnically diverse sample of heterosexual, lesbian, gay, and bisexual adolescents. The 2023 Youth Risk Behavior Surveillance System
