Immune-related adverse events in patients with preexisting myasthenia gravis and thymoma following immune checkpoint inhibitor treatment: a retrospective, observational study.

OBJECTIVE: Immune checkpoint inhibitors (ICIs) can induce immune system activation and cause immune-related adverse events (irAEs). This study aimed to assess the incidence and management of irAEs in thymoma patients with preexisting MG who received ICI therapy. METHODS: This was a retrospective observational cohort study. From September 2018 to May 2024, 12,916 patients received ICI therapy at our hospital. Among them, six patients with preexisting MG and thymoma (MGT) received ICI treatment, and ten thymoma patients without MG (TOMA) served as controls. irAEs, MG flares, and treatment outco

Clinical characteristics and peripheral immune profile analysis of thymoma-associated myasthenia gravis with anti-titin antibodies: a multicenter, retrospective study.

BACKGROUND AND PURPOSE: Thymoma-associated myasthenia gravis (TAMG) patients with anti-titin antibodies exhibit uncertain clinical and immunological relevance. METHODS: We analyzed 91 TAMG patients (40 Titin + group, 51 Titin- group) treated between 2019 and 2025. Clinical features, severity scores, and peripheral immune markers were compared, and finally tested the correlation between the latter and severity. RESULTS: Titin + group had higher rates of MGFA ≥ III (55.00% vs. 25.49%, p = 0.004) and elevated QMG bulbar scores [3.00 (0.75-3.00) vs. 0.00 (0.00-0.00), p = 0.002]. They also showed

Multiple lung metastases from typical type A thymoma after long-term follow-up: a case report.

BACKGROUND: Typical type A thymomas are slow-growing tumors generally associated with a favorable clinical course, and distant metastasis is exceedingly rare. CASE PRESENTATION: We report a case of multiple lung metastases from a typical type A thymoma in a 74-year-old man, identified 9 years after the initial detection of an anterior mediastinal nodule. The patient underwent simultaneous thymectomy and wedge resection of the pulmonary nodule. Histopathological examination confirmed a diagnosis of typical type A thymoma with pulmonary metastases. Postoperatively, the patient was also diagnose

Understanding the uncommon: Insights into thymoma associated immunodeficiency.

BACKGROUND: Thymoma-associated immunodeficiency (TAI) is a rare, acquired adult-onset immunodeficiency. It includes the classic form of Good syndrome (GS), characterized by thymoma and hypogammaglobulinemia, as well as a non-classic form of GS. This condition leads to specific or combined deficiencies in both B- and T-cells, causing considerable morbidity and mortality, although the underlying immunopathology is still not well understood. OBJECTIVE: In this study, we examine the clinical features, laboratory investigations, immunological analysis and treatment outcomes of 21 patients with TAI

Germinal centers are associated with poor prognosis in patients with thymoma-associated myasthenia gravis.

BACKGROUND: Germinal centers (GCs) can be observed in the thymic tissues of individuals with thymoma-associated myasthenia gravis (TAMG). This study aimed to investigate the association between the presence of GCs in the thymus and the clinical outcomes of patients with TAMG following thymoma resection. METHODS: A retrospective review was conducted on patients diagnosed with TAMG who underwent surgical removal of the thymoma. Upon reviewing thymic tissue slides to determine the presence of GCs, patients were divided into GC-positive and GC-negative groups. The association between the presence

Robot-Assisted Versus Video-Assisted Thymectomy: A Narrative Review of Current Evidence.

OBJECTIVES: Minimally invasive thymectomy has transformed the surgical management of thymic epithelial tumours and myasthenia gravis (MG). Video-assisted thoracoscopic surgery (VATS) is well established, whereas robot-assisted thoracoscopic surgery (RATS) provides technical and ergonomic advantages but at a higher cost. We reviewed current evidence to clarify the indications, oncological validity, perioperative performance, and economic implications of RATS compared with VATS and open thymectomy. METHODS: A structured narrative review was conducted, drawing on multi-institutional cohorts, reg

Thymoma with suspected secondary myocardial infarction in a red panda.

A 17-y-old, female red panda (Ailurus fulgens) with unusual restlessness and a poor prognosis was euthanized and submitted for autopsy. Postmortem examination revealed a large cystic mass within the cranial mediastinum that severely compressed the lung. Histologic analysis classified the mass as a thymoma with abundant small lymphocytes and recognizable type B2 epithelial cells. Additionally, the heart had several chronic myocardial infarcts within the left ventricular free wall and septum. Granulomatous lymphadenitis and prominent anthracosilicosis were noted in the tracheobronchial lymph nod

The Safety and Feasibility of Laryngeal Mask Airway Management in Nonintubated Subxiphoid-Subcostal Thoracoscopic Thymectomy: Preliminary Results.

INTRODUCTION: While intubated general anesthesia with single-lung ventilation is regarded as mandatory for conventional thoracoscopic surgery, and various minimally invasive approaches to thymectomy have been described, nonintubated thoracoscopic thymectomy remains technically challenging. This study reports the initial results of nonintubated subxiphoid-subcostal thoracoscopic thymectomy (NI-STT) under laryngeal mask airway management for patients with thymic tumor or myasthenia gravis (MG), and evaluates the feasibility and safety of the procedure. MATERIALS AND METHODS: A retrospective ana

Thymoma-associated anti-AMPAR encephalitis with myasthenia gravis: a case report.

BACKGROUND: Thymoma can trigger a spectrum of paraneoplastic neurological disorders, including autoimmune encephalitis (AE) and myasthenia gravis (MG). However, the sequential development of anti-α-amino-3-hydroxy-5-methyl-4-isoxazole-propionic acid receptor (AMPAR) autoimmune encephalitis followed by MG in the same patient is rarely reported. OBJECTIVE: To describe the clinical presentation, diagnostic work-up, treatment response, and outcome of a patient who developed thymoma-associated anti-AMPAR encephalitis and later presented with MG. METHODS: We retrospectively reviewed the medical re

Left brachiocephalic vein replacement and superior vena cava reconstruction with aortic homograft for thymic carcinoma.

Advanced intrathoracic tumours pose a significant challenge for thoracic surgeons because of the possible involvement of adjacent vital organs and major mediastinal vessels. The most effective therapeutic option for resectable thymic carcinomas is currently represented by complete surgical asportation en bloc with adjacent infiltrated structures to achieve more effective oncological outcomes. Vascular reconstruction, when required, is traditionally performed using prosthetic conduits; however, vascular homografts represent a potential alternative with several surgical and clinical advantages.

Rebound Thymic Hyperplasia after Recovery from Ectopic Cushing Syndrome: A Case Report and Literature Review.

INTRODUCTION: Rebound thymic hyperplasia (RTH) is a rare benign condition resulting from compensatory proliferation of thymocytes after remission from stressful conditions, including hypercortisolism. Its appearance can sometimes be misleading, making differential diagnosis with thymic epithelial or neuroendocrine tumor challenging. CASE PRESENTATION: We describe the case of a 31-year-old patient who was referred to our outpatient clinic for worsening hirsutism, acne, and fatigue. Biochemical and dynamic tests showed an ACTH-dependent hypercortisolism of ectopic origin (Ectopic Cushing syndro

Perioperative Management of a Patient with Good Syndrome and Pancytopenia Undergoing Sternotomy and Thymectomy.

Good Syndrome is a rare form of immunodeficiency occurring in conjunction with thymoma. It is known to be associated with haematological manifestations, primarily pure red cell aplasia but more rarely aplastic anaemia and thrombocytopenia. We report a 61-year-old man with type AB thymoma, Good syndrome and severe pancytopenia undergoing sternotomy and thymectomy. Due to its rarity and often delayed presentation or diagnosis, case reports detailing peri-operative management are limited. This case highlights the complexity of perioperative management in Good syndrome and we detail the patient's

Quality of Life in a Swedish Cohort of Patients with Mycosis Fungoides.

Mycosis fungoides (MF) is a disease that evolves slowly over years with few options for curative treatment. Improving health-related quality of life (HRQoL) is therefore an important treatment goal. This study aims to contribute to a better understanding of HRQoL, using a dermatology-adapted HRQoL questionnaire, Dermatology Life Quality Index (DLQI), in patients with MF. Baseline data from a cohort study (BIO-MUSE) of patients with MF wasere used to investigate which aspect of DLQI is the most affected and how DLQI relates to different clinical and physiological parameters. Data from a period

Significant decreased CXCR3+ phenotype Tfh1 cells in three Good's syndrome patients.

BACKGROUND: Good's syndrome (GS) is a rare acquired immunodeficiency defined by the co-occurrence of thymoma and hypogammaglobulinemia. Follicular helper T (Tfh) cells, a specialized CD4+ T cell subset, play a critical role in supporting B-cell antibody production. The status and characteristics of Tfh cells in GS, however, remain largely unexplored. METHODS: This study utilized flow cytometry to analyze CD4+CXCR5+ T cells, along with Tfh1 (CXCR3+CCR6-), Tfh2 (CXCR3-CCR6-), and Tfh17 (CXCR3-CCR6+) subsets, in three GS patients compared to twenty healthy controls (HCs). Serum levels of Th1/Th2

Good's syndrome and infections in a large retrospective cohort of thymoma.

INTRODUCTION: Good's syndrome (GS) is an immunodeficiency associated with thymoma characterized by hypogammaglobulinemia. The co-existence of recurrent infections may be associated with quality of life and overall survival. Despite its relevance, the impact of GS on patients with thymoma has rarely been studied in large cohorts. METHODS: This is a retrospective cohort study of patients with pathologically confirmed thymoma recruited between May 2010 and November 2024. Data on peripheral immune profile, infections, and paraneoplastic syndrome (PNS) were collected and analyzed. RESULTS: Of 100

Oligoclonal expansion of atypical Vδ2- γδ T cells in Good's Syndrome.

Good's syndrome is a rare adult-onset immunodeficiency characterized by thymoma, hypogammaglobulinemia, B-cell lymphopenia, and T-cell dysfunction. Despite well-characterized defects in conventional immune subsets, the impact of this disorder on unconventional T cells, including γδ T cells, remains largely unexplored. In this study, we analyse γδ T cells in 10 patients with Good's syndrome using immunophenotyping, functional assays, and T-cell receptor (TCR)δ repertoire profiling of peripheral blood and thymoma tissue. Our analyses reveal a pronounced expansion of the Vδ2⁻ γδ T-cell compartmen

Comparative Study of Sporadic and Immune Checkpoint Inhibitor-Related Forms of Myasthenia Gravis-Myositis Overlap Syndrome.

BACKGROUND AND OBJECTIVES: Myasthenia gravis-inflammatory myopathy (MG-IM) overlap syndrome can occur as an immune-related adverse event (irAE) after immune checkpoint inhibitor (ICI) therapy. Before the advent of ICIs, a sporadic form of MG-IM (s-MG-IM), often thymoma-associated, was already described. This study compared clinical and serologic features, treatment strategies, and outcomes of s-MG-IM and ICI-related MG-IM (ir-MG-IM). METHODS: We conducted a multicenter, retrospective cohort study across 8 Italian centers, including consecutive patients fulfilling established criteria for both
