Levels of IL-1β and IL-1 receptor antagonist (IL-1Ra) in Moroccan children admitted to the pediatric emergency department for febrile seizures.

BACKGROUND: Febrile seizures (FS) are events in infancy or childhood, usually between 6 months and 5 years of age, associated with fever but without signs of intracranial infection or a defined seizure cause. Studies suggest that both the inflammatory response and genetic predisposition play crucial roles in FS development. Pro-inflammatory cytokines such as IL-1β and their natural antagonist IL-1Ra are key mediators in this complex interaction. OBJECTIVE: This study aimed to evaluate IL-1β, IL-1Ra levels, and the IL-1Ra/IL-1β ratio in Moroccan children with FS and to explore their association

Relationships between PRRT2 variants and clinical outcomes in infants with suspected self-limited infantile epilepsy.

OBJECTIVE: To compare the seizure and developmental outcomes according to the presence/absence of pathogenic PRRT2 variants in patients with clinically suspected self-limiting infantile epilepsy (SeLIE), to thereby clarify the prognostic value of PRRT2 testing. METHODS: Forty-two patients with clinically suspected SeLIE were assessed. Clinical information, including patient and seizure characteristics, final diagnosis, and developmental outcomes, were retrospectively obtained. Genetic testing for PRRT2 was conducted using Sanger sequencing, and patients were classified as PRRT2-positive or PR

Sex-specific elevated incidence of glaucoma associated with topiramate versus valproate or lamotrigine in epilepsy, not migraine: A population-based cohort study.

OBJECTIVE: Topiramate has been linked to increased glaucoma risk, potentially through mechanisms involving ocular fluid shifts. However, comparative risks vs other antiseizure medications (ASMs) and variation by sex or indication remain uncertain. This study evaluates glaucoma incidence in topiramate initiators compared to valproate or lamotrigine users among patients with epilepsy or migraine. METHODS: We conducted a retrospective active-comparator, new-user cohort study using electronic health records from the IQVIA Medical Research Data among patients with epilepsy or migraine initiating t

Natural History of YWHAG-Associated Neurodevelopmental Disorder.

BACKGROUND: The YWHAG gene encodes a member of the 14-3-3 protein family important for cellular signaling and neuronal migration. Recent studies suggest its role in early-onset epilepsy, epileptic encephalopathy, and seizures in children. However, the natural history of this disorder has not been described. Here we report results of the first prospective natural history study to date in children with YWHAG-associated Neurodevelopmental Disorders (YWHAG-NDDs). METHODS: The YWHAG Natural History Study is a prospective, single site, nonrandomized, longitudinal study of individuals with neurodeve

Application of microdeletion and microduplication screening in preimplantation genetic testing: a case report.

BACKGROUND: Microdeletions and microduplications are chromosomal variants ranging up to 3 Mb in size. These abnormalities often arise spontaneously and have significant clinical implications, including developmental delay and congenital anomalies. Given the resolution of conventional preimplantation genetic testing for aneuploidy is > 5-10 Mb, these clinically significant abnormalities are missed. This study presents two cases where preimplantation genetic testing-whole genome sequencing successfully identified small, clinically significant abnormalities. CASE PRESENTATION: In the first case,

Changing the Child: The Impact of Novel Treatments on the Identity of Children With Monogenic Neurodevelopmental Disorders.

Scientific advancements offer new possibilities for developing novel treatments for children with monogenic neurodevelopmental disorders (mNDDs), targeting the underlying effects of their condition. Although, in qualitative studies, parents of children with mNDDs have predominantly positive attitudes towards the development of novel treatments, a few parents express concerns about whether treatments could change who their child is. Existing literature on personal identity, including literature on how personal identity is affected by medical (neuro-)interventions, can assist in clarifying these

Family history of stroke and cardiovascular diseases in early-onset cryptogenic ischaemic stroke.

BACKGROUND: Familial aggregation of stroke is well-documented, yet few studies have examined associations between stroke subtypes-particularly early-onset cryptogenic ischaemic stroke (eCIS)-and broader family history (FH) of cardiovascular disease. Such associations may provide insights into underlying etiologic mechanisms. METHODS: In this multicentre case-control study, we included eCIS patients aged 18-49 years and matched stroke-free controls. We analysed the association between FH of stroke, venous thromboembolism (VTE), coronary artery disease (CAD), aneurysms and eCIS using multivaria

Knowledge and Awareness of Glaucoma: A Comparative Study Among Rural and Urban Patients in Bangladesh.

BACKGROUND: Glaucoma is a leading cause of irreversible blindness worldwide and often progresses silently until significant vision loss occurs. Early detection and timely treatment are crucial to preventing visual impairment; however, these efforts largely depend on public awareness. Assessing glaucoma-related knowledge, particularly among underserved populations, is therefore essential for developing effective prevention and control strategies. This study aimed to assess and compare the level of knowledge and awareness of glaucoma among rural and urban patients. METHODOLOGY: This comparative

Nonarteritic Anterior Ischemic Optic Neuropathy: Review and Update.

Nonarteritic anterior ischemic optic neuropathy (NA-AION) is a major cause of blindness in patients above the age of 50 years that is believed to occur due to insufficient blood supply to the optic nerve head. Patients typically present with sudden-onset, unilateral, painless vision loss accompanied by decreased visual acuity and field. Diagnostic work-up includes taking a thorough history and clinical examination including relative afferent pupillary defect, visual fields, and optical coherence tomography. Despite the abundance of NA-AION, there are currently no definitive treatment options,

Congenital Isolated Bilateral Anophthalmia: A Case Report.

Congenital anophthalmia is the complete absence of the eye resulting from a failure of optic vesicle formation during early embryogenesis. It may present as a unilateral or bilateral condition and can occur as an isolated anomaly or as part of a syndromic disorder. We report the case of a full-term newborn admitted on the second day of life for the management of bilateral congenital anophthalmia. Family history was notable for anophthalmia in three cousins belonging to the shared parental lineage, suggesting a possible genetic predisposition. The diagnosis is primarily clinical and is confirme
