Genetic heterogeneity in osteogenesis imperfecta.

An epidemiological and genetical study of osteogenesis imperfecta (OI) in Victoria, Australia confirmed that there are at least four distinct syndromes at present called OI. The largest group of patients showed autosomal dominant inheritance of osteoporosis leading to fractures and distinctly blue sclerae. A large proportion of adults had presenile deafness or a family history of presenile conductive hearing loss. A second group, who comprised the majority of newborns with neonatal fractures, all died before or soon after birth. These had characteristic broad, crumpled femora and beaded ribs i

Congenital rubella deafness: a preventable disease.

Over a 4-year period (1972-75) an unselected sample of 568 children aged under 4 years attending the Nuffield Hearing and Speech Centre were tested for rubella antibody. Sensorineural deafness was subsequently diagnosed in 349 of these children, and 83 (24%) of this group had rubella antibody. In contrast, only 19 (9%) of the remaining 219 children in whom sensorineural deafness was excluded had rubella antibody. The seropositive and seronegative children with sensorineural deafness showed striking differences in family history, history of maternal rubella, adverse perinatal events, and presen

The early detection of the acoustic neurinomas. Otoneurological indications of the radiological investigation.

The diagnostic possibilities concerning the early diagnosis of an acoustic neurinoma are discussed. As it is very important to detect these tumors in the otological stage, we scrutinize the different symptoms and signs which can arise this suspicion and try to delineate the criteria for further investigation, which signifies the indications for the radiological investigation: tomography and cisternography. The survey of the results obtained during 1976 confirms the value of the rules we adopted. Every patient complaining of hearing loss or tinnitus has to be examined by audiometry. Every unila

Identification of hearing loss in infants and young children.

An infant's hearing loss is detectable at any age. In spite of the importance of identifying deafness early, we do a poor job largely because the medical community is neither aware nor convinced that finding hearing loss is worth the trouble. There are two public health strategies for early detection, the high risk register (family history, hyperbilirubinemia, prematurity, rubella, and maxillofacial anomalies) and behavioral screening. The advantages and disadvantages are discussed. The incidence of handicapping hearing loss in newborns is 1 per 380 births and 1 per 61 in babies admitted to an

Abnormal neutrophil chemotaxis in a syndrome of unusual facies, proportionate small stature and sensorineural deafness-mutism.

Two of 5 children in one family presented with unique facies, proportionate small stature and sensorineural deafness-mutism. One of the children who had a history of recurrent infections, was shown to have a defect of leukocyte chemotaxis. Although impairment of chemotaxis could not be demonstrated in the other affected sibling, it is unlikely that the association of a previously undescribed syndrome and a rare disorder of chemotaxis is a chance occurrence.

Morquio's disease and the hearing organ.

Histopathologic evidence offered by a case of Morquio's disease and observed in the hearing organ showed widespread collagen damage, possibly initiated by the general hemorrhageous tendency, while the family history bore evidence of a genetic source. The clinical manifestations comprise striking dwarfism, beaking of the lumbar spine, sternal bulging, flaring of the rib cage, falt vertebrae, marked osteoporosis, prominent joints, semi-crouching stance, short nose. Roentgenograms showed retarded ossification.

Ocular myopathy (progressive external ophthalmoplegia) with neuropathic complications.

The condition known as Ocular Myopathy or Progressive External Ophthalmoplegia is reviewed. Three hundred and thirty-five published case reports have been analyzed and 13 personal cases are described. Histological and electron microscopical studies on one of our cases are reported. It is clear from these cases that, although in its simplest form the condition may consist merely of myopathic changes confined to the upper eyelids and external ocular muscles, myopathic changes may also occur in the muscles of the face, neck and proximal parts of the limbs. In addition, however, a variety of neuro

Medical interpreting for hearing-impaired patients.

Medical interpreting for deaf patients is a new dimension of the medical field. Medical interpreting allows for the fast, accurate, and complete exchange of medical information between medical professionals and their deaf patients. The exchange of accurate medical information is critical to the early diagnosis and treatment of medical problems.

Hearing loss. The physician's responsibility.

Hearing loss is a very important and common problem in the United States, affecting 13 to 14 million people. It is primarily a medical problem, requiring diagnosis and, when indicated, treatment by a physician, preferably in collaboration with an audiologist. History taking is the most important part of the diagnostic process, followed by a physical examination with special attention to the head, neck, and ears. The primary object of the workup is to exclude all possible extrinsic causes of hearing loss. Intrinsic or genetic deafness is a diagnosis of exclusion. After the hearing loss has been

Ichthyosiform dermatosis, keratitis, and deafness.

Two patients had an unusual ichthyosiform dermatosis, neurosensory deafness, and vascularizing keratitis. There was no family history of the disorder and no consanguinity. A squamous cell carcinoma of the tongue developed in one child. The association of such an eruption and deafness in previously reported cases is reviewed.

Hirschsprung's disease and congenital deafness. Familial assocation.

A family is described showing deafness in three consecutive generations. Hirshchsprung's disease was present in at least two of the affected patients and a history of bowel dysfunction was present in the third. The assocation of the two disorders in this family may be due to a single autosomal dominant gene and in this regard differs from previously reported isolated patients with Hirschsprung's disease and deafness.

Atypical ichthyosiform erythrodernam deafness and keratitis. A report of two cases.

Two patients with ichthyosiform erythroderma of the same unusual but characteristic distribution are described. Both patients were born with perceptive deafness and developed severe vascularizing keratitis in early childhood. There is no family history of the disorder in either case. This syndrome is discussed in relation to previous reports of atypical ichthyosiform erythroderma associated with deafness.

[Vesicoureterorenal reflux: an inherited disease? (author's transl)].

A family case history is presented with an increased incidence of vesicorenal reflux: four sons had a reflux while a daughter and the parents were healthy in this respect. From the literature and on own studies the possibility of reflux as an inherited disease is discussed. A new finding is the coincidence of reflux and the histocompatibility-antigen HLA-A2.

Identification of normal hearing carriers of genes for deafness.

The dominant and sex-linked forms of hereditary hearing loss, which have long been recognized, are readily identified on the basis of the family history and routine hearing tests. The mode of inheritance of the recessive forms of hereditary deafness, on the other hand, has been extremely difficult to determine. The research of the last few years, however, has disclosed that carriers of genes for recessive deafness can be identified by audiometric recording of certain peculiarities in the hearing function. This is an important advance, not only as regards diagnositc work, but also in the resear

The differentiation of paranoid from affective psychoses by patients' premorbid characteristics.

Various premorbid characteristics of a group of patients aged 50 or over suffering from paranoid psychoses (N = 54) were compared with those of patients of similar age suffering from affective psychoses (N = 57). (The diagnoses were those of the consultants.) The presence of deafness was investigated in the manner described by Cooper et al(1974). Using the discriminant function method, the diagnostic groups were highly significantly differentiated by six independent variables, which in combination predicted 40 per cent of the variance. These were: a 'schizoid personality' factor, the number of

[Nephropathy and deafness (Alport syndrome) without positive family history (author's transl)].

This is a report of a 22-year-old man affected with hereditary nephropathy and deafness (Alport syndrome) without positive family history. The diagnosis is based on the typical clinical history, presence of symmetrical neural deafness, and terminal stage of renal insufficiency, that could not be classified otherwise following renal biopsy. The differential diagnosis and genetics are discussed.

A new look at osteogenesis imperfecta. A clinical, radiological and biochemical study of forty-two patients.

In a clinical, radiological and biochemical study of forty-two patients from Oxford with osteogenesis imperfecta, it was found that patients could be divided simply into mild, moderate and severe groups according to deformity of long bones. In the severe group (seventeen patients) a family history of affected members was uncommon and fractures began earlier and were more frequent than in the mild group (twenty-two patients); sixteen patients in the severe group had scoliosis and eleven had white sclerae; no patients in the mild group had white sclerae or scoliosis. Hereditary thrombocytopenia, deafness, and renal disease.

The syndrome of hereditary thrombocytopenia, deafness, and renal disease was manifest in at least eight members in three generations of a family. They had a lifelong history of bleeding, usually as epistaxis, bilateral sensorineural deafness starting in late childhood or the teenage years, and persistent proteinuria with varying degrees of renal dysfunction. Two members died at a young age, one from central nervous system hemorrhage, the other from chronic renal failure. Splenectomy and steroid therapy have been of transient benefit. There was dominant inheritance of the syndrome. Hematologic

[Observations on the Prune-Belly syndrome].

5 Male children exhibiting the typical features of the Prune-Belly syndrome (malformations of the urogenital tract, aplasia of abdominal muscles) are described. Two of these are deaf. This syndrome has also been observed in a few females. Final interpretation of the cause and pathogenesis cannot yet be given. In two siblings with defects of the abdominal muscles an contractures of the joints, but without renal anomalies, a new and hitherto undescribed hereditary syndrome seems to be present.
