Giant anterior mediastinal thymolipoma: A rare case of thoracoscopic resection and surgical management challenges.

INTRODUCTION: Thymolipoma is a rare benign anterior mediastinal tumor composed of thymic and adipose tissues. Early diagnosis and accurate surgical management are crucial to prevent misdiagnosis and optimize patient outcomes. CASE PRESENTATION: A 17-year-old male with no significant medical history presented to our thoracic surgery department with exertional dyspnea and a sensation of chest tightness. Given the worsening symptoms, a chest computed tomography (CT) scan was performed, revealing a well-circumscribed anterior mediastinal mass in the right paracardiac region, predominantly compose

Dedifferentiated liposarcoma of the anterior mediastinum: An uncommon case presentation and complex diagnosis.

Mediastinal liposarcoma are rare occurrences of liposarcomas, arising from mesenchymal fat cells. They have several histological subtypes including well-differentiated, dedifferentiated, myxoid and pleomorphic. They often present with nonspecific symptoms of mass effect on nearby structures. Surgical resection is the mainstay of treatment, but high recurrence rate remains a challenge. We present the case of a 70-year-old male with a large dedifferentiated anterior mediastinal liposarcoma presenting with compressive symptoms. Coordinated efforts from radiology, histopathology and surgical teams

Mediastinal lymphangioma and intestinal lymphangiomatosis presenting with chylothorax: a systematic review of therapeutic modalities.

INTRODUCTION: Chylothorax is defined by the accumulation of chyle in the pleural space, characterized by triglyceride levels exceeding 110 milligrams per deciliter. The clinical presentation of chylothorax varies depending on its acuity and underlying etiology. Mediastinal lymphangiomas are extremely rare and benign lesions that can affect both infants and adults. They may occur independently or in association with other lymphatic disorders and can lead to complications such as chylothorax. CASE REPORT: A 60-year-old male patient presented with shortness of breath and was diagnosed with left-

Enteric thymic adenocarcinoma: Understanding a unique pathology in the mediastinum.

Thymic malignancies are rare neoplasms, with thymoma and thymic carcinoma-collectively known as thymic epithelial tumors (TETs)-being uncommon tumors that originate from the thymus. Despite their rarity, TETs are the most prevalent tumors found in the anterior mediastinum of adults. The major histological types include squamous cell carcinoma, lymphoid epithelioid carcinoma, and basal-like carcinoma. Thymic enteric type adenocarcinoma, an exceptionally rare variant characterized by the expression of cytokeratin (CK) 20 and/or CDX-2, has been recently proposed as a distinct pathological entity,

Programmed death-ligand 1 expression in surgically resected thymomas.

Thymomas are one of the most common neoplasms of the anterior mediastinum with limited therapeutic options, particularly in advanced stages. The molecular profiles of these tumors remain poorly investigated. This study aims to evaluate the expression of programmed death ligand 1 (PD-L1) in a selected cohort of intentionally curative resected thymomas and evaluate a possible relationship with the risk of recurrence. This retrospective bicentric study analyzed a group of patients who underwent complete thymectomy with curative intent. PD-L1 expression was assessed through immunohistochemistry us

Multimodal Imaging of Mediastinal Epithelioid Hemangioendothelioma: Two Case Reports.

INTRODUCTION: Epithelioid Hemangioendothelioma (EHE) is a rare vascular neoplasm that typically occurs in the bone, soft tissue, liver, and lung but rarely in the mediastinum. Multimodal imaging of EHE is poorly understood, often leading to misdiagnosis as other mediastinal tumors. CASE PRESENTATION: Two female cases with incidental mediastinal masses were retrospectively analysed, focusing on multimodal presentations. For both cases, CT studies showed well-defined, low-density oval masses in the right anterior superior mediastinum with the Superior Vena Cava (SVC) invasion. Intralesional pun

Computed tomography findings of extralobar pulmonary sequestration in children: a retrospective study of 58 patients.

BACKGROUND: Identifying the blood supply from a systemic artery is crucial for diagnosing extralobar pulmonary sequestration. Contrast-enhanced computed tomography (CT) can accurately delineate the origin and course of anomalous systemic arteries and venous drainage. However, it remains challenging to make an accurate diagnosis in cases with atypical CT manifestations. OBJECTIVE: To improve our understanding of the diversity of extralobar pulmonary sequestration and facilitate a more accurate diagnosis, especially for cases with atypical CT manifestations. MATERIALS AND METHODS: A retrospect

Recent Clinical and Molecular Advances in the Management of Thymic Carcinoids: A Comprehensive Review.

Background: Thymic carcinoids are rare neuroendocrine tumors arising in the anterior mediastinum, often diagnosed at an advanced stage due to nonspecific clinical manifestations. Their management remains challenging because of the paucity of data, rarity of occurrence, and aggressive biological behavior compared to other well-differentiated neuroendocrine neoplasms. Methods: We conducted a comprehensive review of the current literature focusing on the classification, clinical presentation, diagnostics, treatment options, prognostic factors, and emerging experimental therapies for thymic carcin

[Cholesterol Granuloma Arising in the Thymus:Report of a Case].

The patient was a 64-year-old man who was found to have an abnormal shadow in the anterior mediastinum on a chest computed tomography( CT) scan during a health check. He was admitted for diagnosis and treatment, and underwent thoracoscopic thymic and thymic tumor removal. The postoperative course was uneventful, and the patient was discharged on the 4th postoperative day. Histopathological examination revealed numerous foreign body giant cells and cholesterol crystals in the thymus tissue, and the patient was diagnosed with cholesterol granuloma. One cause of cholesterol granuloma is trauma. T

Recurrent secondary hyperparathyroidism after parathyroidectomy due to anterior mediastinum ectopic parathyroid glands in a peritoneal dialysis patient-a case report and literature review.

BACKGROUND: Secondary hyperparathyroidism (SHPT) is a common complication of end-stage kidney disease (ESKD). Parathyroidectomy (PTX) is a reasonable option for patients with ESRD complicated by refractory SHPT, although only a few cases of ectopic parathyroid glands have been reported in patients undergoing peritoneal dialysis (PD). CASE PRESENTATION: We present a case of recurrent SHPT after parathyroidectomy due to ectopic parathyroid glands in a patient undergoing long-term PD for approximately 10 years. The ectopic parathyroid glands were identified when performing the first operation, b

A Rare Case of Mediastinal Seminoma with Granulomatous Features: Diagnostic and Therapeutic Considerations.

INTRODUCTION: Although seminomas typically arise in the testes, primary mediastinal seminomas are classified as extragonadal germ cell tumors. Diagnosis is often challenging and requires not only blood tests and imaging but also a tumor biopsy. However, diagnosis may be particularly difficult when the tumor shows nonspecific pathological features or is accompanied by granulomatous changes. CASE PRESENTATION: The patient was a 25-year-old man who had been experiencing labored breathing when leaning forward for the past month. Physical examination revealed distended jugular veins and neck edema

Risk classification of thymoma based on multi-feature fusion in dynamic enhanced CT.

BACKGROUND: Accurate classification of high-risk and low-risk thymomas is critical for guiding treatment strategies and assessing prognosis. Thymoma is the most common primary tumor of the anterior mediastinum. However, previous studies have limitations in comprehensively utilizing imaging data, particularly in combining radiomics and deep learning (DL) features for preoperative classification. PURPOSE: This study aimed to develop and validate a comprehensive model based on computed tomography (CT) imaging data (CSRT, Clinical Semantic, Radiomics, and Vision Transformer) to enhance the accura

Factors influencing the difficulty of video-assisted thoracoscopic anterior mediastinal tumor resection via the subxiphoid approach.

BACKGROUND: Video-assisted thoracoscopic mediastinal tumor resection via the subxiphoid approach (SVATS) is the mainstream surgical method for treating anterior mediastinal tumors. The impact of thoracic and mediastinal size on surgery has long been described only as a subjective feeling, and no related research has been reported. In this study, we retrospectively investigated the preoperative predictive factors affecting SVATS, focusing on the depth of the anterior mediastinum and thoracic morphology (width and depth). METHODS: We studied 133 patients with mediastinal tumors. All had video t

Comparative Evaluation of 68 Ga-FAPI-04 PET/CT and 18 F-FDG-PET/CT in Primary Staging of Treatment-naïve Anterior Mediastinal Seminoma.

A comparative evaluation using both 18 F-FDG-PET/CT and 68 Ga-FAPI-04 PET/CT in primary staging of a treatment-naïve anterior mediastinal seminoma is illustrated in this report. A 30-year-old man presented with elevated human chorionic gonadotropin levels and biopsy-proven seminoma of the anterior mediastinum. The investigation was performed for primary staging, considering that both tracers may show uptake in the germ cell tumors in the anterior mediastinum, and the potential role of 68 Ga-FAPI-04 PET/CT in treatment-naïve mediastinal seminoma would be examined. The germ cell tumors comprise

Primary Sternal Osteomyelitis with Acute Mediastinitis, Successfully Treated with Minimally Invasive Surgical Drainage.

INTRODUCTION: Primary sternal osteomyelitis (PSO) is a rare disease that occurs without any contiguous focus of infection, and there are few reports of acute mediastinitis due to PSO. In this report, we describe a case of PSO with acute mediastinitis successfully treated with a minimally invasive approach. CASE PRESENTATION: A 71-year-old man visited his local doctor for anterior chest pain. He had no history of trauma or chest surgery. He was treated conservatively because of a few abnormalities on CT. However, his symptoms worsened, and a CT was re-taken 13 days later. The CT showed an absc

The Secret Life of a Lost Guidewire.

BACKGROUND: Lost guidewires during central venous catheterization are rare but serious, requiring prompt removal to reduce morbidity and mortality. CASE SUMMARY: A 58-year-old woman presented with a pulsating back mass and a spontaneous bruise under her left clavicle. More than 10 years ago, a 0.035-inch guidewire had been lost in her circulation, and retrieval attempts had failed. Computed tomography scan revealed a fragmented guidewire that had crossed into the arterial circulation through a patent foramen ovale and extended through the anterior mediastinum. After a discussion among the hea

[Large intrathoracic thymolipoma].

Thymolipoma is a rare, benign tumour in the anterior mediastinum, representing about 2-9% of thymus neoplasms. It consists of fat tissue mixed with thymus tissue and is often discovered incidentally. Symptoms may occur due to compression of nearby structures. In this case, a 41-year-old woman with upper back pain and shortness of breath was diagnosed after an MRI showed a large lipomatous tumour in the mediastinum. The tumour was surgically removed, and pathology confirmed thymolipoma.

Metaplastic thymoma: A rare and indolent subtype.

Though thymomas are the commonest neoplasms of the anterior mediastinum, metaplastic thymoma is an extremely rare subtype, with only a few reported cases. It was included in the 2004 WHO classification of thymic epithelial tumors and exhibits a biphasic morphology distinct from the conventional thymomas. It has a benign clinical course with extremely rare cases showing recurrence and malignant transformation. However, metaplastic thymoma needs to be histologically distinguished from other more aggressive mediastinal neoplasms displaying biphasic patterns. Here we report a case of an incidental

Median Sternotomy for Mediastinal Mass Removal: Thymoma of Unknown Cause.

Thymoma is a rare mediastinal malignancy. Although it may often be associated with paraneoplastic syndromes such as hypogammaglobulinemia, pure red cell aplasia, or myasthenia gravis (MG), some thymomas may also be idiopathic in origin. Although thymoma is the most common mass found in the anterior mediastinum, its rarity still proves difficult in swift diagnosis and subsequent treatment. This case presents a patient with an unknown anterior mediastinal mass incidentally found on imaging, with the final diagnosis of thymoma made from a pathologic investigation that was performed post-sternotom
