Retrograde cricopharyngeus dysfunction: what the gastroenterologist should know.

PURPOSE OF REVIEW: To review current literature about retrograde cricopharyngeus dysfunction (R-CPD) etiology, pathophysiology, clinical presentation and management. RECENT FINDINGS: R-CPD is a clinical condition characterized by inability to burp that is often associated with a variety of other gastrointestinal symptoms. Limited data suggest that R-CPD may affect up to 22% of the general population, with 13% experiencing severely troublesome symptoms. Symptoms usually begin in childhood, though diagnosis typically occurs many years later after prolonged symptom burden. Gastroesophageal reflu

Permanent enterostomy in Hirschsprung disease: can we predict which patients are at risk?

BACKGROUND: Permanent enterostomy in patients with Hirschsprung disease (HD) is usually never on the mind of a pediatric surgeon. It is uncommon but represents an important long-term outcome. Although disease segment length is a recognized risk factor, the factors influencing permanent diversion beyond disease extent are not well defined. The aim of this study was to characterize patients requiring permanent enterostomy and to identify clinical, surgical, and patient-centered factors contributing to this outcome. METHODS: We conducted a retrospective cohort study of patients with HD treated a

Prevalence of irritable bowel syndrome and its association with lifestyle factors among medical students in Jordan: a cross-sectional survey.

Irritable Bowel Syndrome (IBS) is a common functional gastrointestinal disorder marked by chronic abdominal discomfort and altered bowel habits, including constipation, diarrhea, or a combination of both. It significantly impacts quality of life and is often linked to psychological stress and dietary factors. Given the scarcity of multi-university data from Jordan, this study aimed to determine the prevalence of IBS among medical students across three Jordanian universities and to identify associated lifestyle, psychological, and demographic risk factors using validated instruments. A cross-se

Bayesian Inferential Analysis of Genetic Risks Based on Family History.

Classical statistical analysis is a frequently employed methodology in numerous domains of genetic research. In recent times, however, there has been a notable increase in the interest accorded to the deployment of Bayesian statistics in the field of genetics, as it incorporates a priori hypotheses about genetic knowledge into the problem. The potential risk of developing a genetic disease is influenced by the patient's genetics, ethnicity, gender, age, and family history. The objective of this study is there[Formula: see text]fore to provide molecular pathologists working with genetic testing

Multiplex Real Time PCR Assay Targeting the eqbE for Rapid Detection and Differentiation of Streptococcus equi from Other Streptococcus Species in Indian Equines.

INTRODUCTION: The 'Commission on the Future of Health Care in Canada' recognized that people living in rural and remote areas of Canada are at a disadvantage in health status, access to care and health professionals, and it considers the fight against these problems as a national priority. Although some attention has been paid to the prevalence of chronic diseases, very few studies have studied specifically the management and health issues in populations with chronic diseases in relation to rurality. The objective of this study was to describe systematic gaps across rural and urban populations

Association of repeated endoscopic inflammation with dysplasia and colorectal cancer in ulcerative colitis.

BACKGROUND AND AIM: ulcerative colitis (UC) confers an increased risk of dysplasia and colorectal cancer (CRC), largely driven by sustained mucosal inflammation. While inflammatory burden is a known risk factor, the specific impact of repeated endoscopic inflammation and contemporary therapies exposure warrant further investigation. We aimed to investigate the impact of cumulative inflammation in the risk of dysplasia and CRC development in UC. METHODS: a retrospective cohort study of adult UC patients diagnosed between 1975 and 2023 was performed at a tertiary IBD center. Repeated colonoscop

Cancer risks for ATM variant heterozygotes.

PURPOSE: Cancer risks of individuals heterozygous for an ATM pathogenic or predicted pathogenic variant (PV/PPV) remain imprecise to guide optimal clinical management. Therefore, we aimed to estimate these risks in different family settings. METHODS: Data were collected on 141 ataxia-telangiectasia families, 398 hereditary breast and ovarian cancer families, and 96 families with a history of pancreatic cancer enrolled in French nation-wide epidemiological studies CoF-AT2, TUMOSPEC, or GENESIS. Hazard ratios (HR) and cumulative risks were estimated using a modified segregation analysis method.

The LGR5/TFF2 ratio as a stem cell biomarker predicts high-risk status in chronic atrophic gastritis: a retrospective cohort study.

BACKGROUND: Chronic atrophic gastritis (CAG) severity risk assessment remains challenging due to limitations of current static histologic/endoscopic tools. We evaluated the leucine-rich repeat-containing G-protein coupled receptor 5 (LGR5)/trefoil factor 2 (TFF2) ratio - quantifying gastric stem cell niche imbalance- as a dynamic biomarker for high-risk CAG (OLGA/OLGIM III-IV). METHODS: In a retrospective cohort (n = 60 CAG patients), we constructed four prediction models (Baseline: demographics; Clinical: +H. pylori/family history; Endoscopic; Full: +LGR5/TFF2 ratio) using Firth penalized re

Hereditary haemorrhagic telangiectasia presenting with refractory gastrointestinal bleeding.

A man in his 20s presented with progressive fatigue and melena, with a history of recurrent epistaxis since adolescence and a positive family history. Evaluation revealed severe iron-deficiency anaemia (haemoglobin 27 g/L) with persistently positive faecal occult blood. Upper gastrointestinal endoscopy demonstrated multiple bleeding telangiectasias in the stomach and duodenum. Cross-sectional imaging of abdomen, thorax and brain identified multisystem arteriovenous malformations involving the liver, lung and brain. Despite repeated argon plasma coagulation and thalidomide therapy, bleeding per

ACG Clinical Guideline: Diagnosis and Management of Adenomatous Colorectal Polyposis Syndromes.

The hereditary adenomatous colorectal polyposis syndromes are incurable, systemic, cancer risk predisposition syndromes with substantial morbidity and mortality. They differ in their mode of inheritance, age at disease onset, phenotypic expression, and cancer risk. The most common cancer risks involve the gastrointestinal tract including the colon, rectum, duodenum, ampulla, and stomach. Identifying these syndromes through personal and family history risk assessment and germline genetic testing, along with timely surgical and endoscopic management, can reduce cancer incidence and mortality. Jejunocecal fistula combined with gastro-ascending colonic fistula in a child with chronic diarrhea and weight loss: a case report.

BACKGROUND: Gastrointestinal fistulas in children are rare, and the coexistence of a jejunocecal and gastric ascending colon fistula is exceptional. To our knowledge, no instance of this fistula combination has been reported previously. These patients present with nonspecific symptoms, which may lead to delayed or missed diagnoses. CASE PRESENTATION: We evaluated a 12-year-old boy with a 3-year history of intermittent nausea and vomiting, and a weight loss of 30 kg. Initially, he was managed as for gastroenteritis; however, his response was minimal. Further investigation included endoscopy and

Synchronous gastric cardia and antrum signet ring cell carcinomas: A rare case report highlighting diagnostic challenges and multidisciplinary management.

RATIONALE: Gastric cancer is one of the most common malignant tumors in China. Gastric signet ring cell carcinoma (SRCC) belongs to highly malignant undifferentiated gastric cancer, originating from the epithelial cells of the gastric mucosa. Research has shown that the incidence of gastric cancer is gradually decreasing, but the incidence of gastric SRCC is gradually increasing. We report an extremely rare case of dual primary SRCC involving the gastric cardia and antrum. This case highlights the diagnostic challenges of synchronous SRCC, where a high level of suspicion, comprehensive endosco
