Statistics and epidemiology of inflammatory bowel disease-associated colorectal neoplasia.

Inflammatory bowel diseases (IBD), including ulcerative colitis (UC) and Crohn's disease (CD), are associated with an increased risk of intestinal neoplasia, representing a major long-term complication of chronic inflammation. This review summarizes the recent epidemiological trends and clinicopathological features of IBD-associated colorectal cancer (CRC) and dysplasia in UC and CD. In UC, the cumulative risk of CRC has declined in recent decades, possibly reflecting improvements in medical management and surveillance strategies. However, long disease duration, extensive colitis, concomitant

Hepatitis B Vaccination Coverage and Vaccine-Associated Immunity Among US-Born Children and Adolescents: NHANES 2017-2020.

Since the initial recommendations for universal infant hepatitis B vaccination in the United States, vaccination coverage has increased significantly. However, a trend of declining antibody levels has been observed among early vaccinees. This study aims to investigate hepatitis B vaccination coverage and vaccine-associated immunity among US-born children and adolescents. We used data from the 2017-2020 National Health and Nutrition Examination Survey to include US-born participants aged 2 to 18 years who had complete hepatitis B serological data and vaccination history. We determined the preva

Dyspepsia Care as a Missed Gateway to Gastric Cancer Prevention in High-Risk U.S. Populations.

Dyspepsia is among the most common gastrointestinal complaints, affecting nearly one in five adults annually. Guidelines diverge in their recommendations: the American College of Gastroenterology and Canadian Association of Gastroenterology endorse Helicobacter pylori "test-and-treat" for patients under 60 years, with upper endoscopy for those aged 60 and above or with alarm features. By contrast, the American Society for Gastrointestinal Endoscopy advises endoscopy for patients with new-onset dyspepsia beginning at age ≥ 50, even without alarm features. The British Society of Gastroenterology

[Etiological work-up for chronic pancreatitis].

The first etiology of chronic pancreatitis is excessive alcohol consumption, which accounts for 46% of causes in the USA. Other include metabolic, genetic, obstructive or autoimmune mechanisms. In 2/3 of cases, several factors are interrelated, and it is essential to investigate the etiology to interrupt, if possible, the natural course of progression to chronic pancreatitis. The initial etiological approach should include detailed questioning, a physical examination, an initial laboratory work-up and cross-sectional imaging (CTscan and MRI). Genetic analysis is indicated if the cause remains

[A specific situation: the genetic pancreatitis].

Genetic pancreatitis is a rare cause of chronic pancreatitis (1% of the cases). A genetic origin was suspected as early as 1952 and confirmed in 1996 with the identification of the PRSS1 gene, which predisposes individuals to the disease. Since then, new genes have been identified as potential contributors to chronic pancreatitis, usually interacting with environmental factors (alcohol, tobacco consumption). Diagnosis relies on genetic analysis, particularly in cases of unexplained pancreatitis in young patients or those with a family history of pancreatitis. The management of genetic pancreat

High Detection Rates and Associated Factors of Colorectal Serrated Lesions: A Multicenter Prospective Study in the Asia-Pacific.

INTRODUCTION: Colorectal serrated lesions (SLs) are recognized as precursors of colorectal cancer (CRC); however, their detection rates and prevalence remain inadequately defined. We aimed to assess their detection rates and estimate their prevalence in the Asia-Pacific, as well as to examine their associated factors. METHODS: This was a multicenter prospective study in the Asia-Pacific. Asymptomatic individuals aged 40-74 years undergoing first-time colonoscopy for CRC screening were prospectively enrolled. To ensure precise prevalence estimates, colonoscopy procedures involved repeated prox

Risk factor for colorectal neoplasms: obstructive sleep apnea or nocturnal hypoxemia?

OBJECTIVE: The objective of this study is to determine whether nocturnal hypoxemia, as compared to obstructive sleep apnea (OSA), serves as a risk factor for colorectal neoplasia, encompassing colorectal cancer (CRC), adenomas, and advanced adenomas. METHODS: We conducted a study involving individuals who underwent both standard polysomnography (PSG) and colonoscopy. Analyses included the univariate and multivariate logistic regression analyses, receiver operating characteristic (ROC) curve analyses, subgroup analysis and sensitivity analysis. RESULTS: In this study, a total of 335 individua

Long-term and second recurrence following curative ESD for synchronous multiple early gastric neoplasia: a single-center cohort study.

BACKGROUND: Synchronous multiple early gastric neoplasia (SM-EGN) is increasingly detected with advanced endoscopic imaging. However, long-term recurrence dynamics after curative endoscopic submucosal dissection (ESD), particularly second recurrence, remain insufficiently defined. METHODS: We retrospectively analyzed 987 patients who underwent ESD for early gastric neoplasia between 2012 and 2024, including 91 SM-EGN and 896 solitary cases. Among 766 patients with curative resection and ≥ 1 year follow-up, clinical outcomes and recurrence patterns were compared. Logistic regression identified

Financial toxicity among gastric and pancreatic cancer survivors in India and its impact on quality of life.

BACKGROUND: Financial toxicity (FT) associated with cancer treatment has emerged as a critical issue impacting cancer patients globally. The absence of comprehensive research on FT among gastric and pancreatic cancer survivors in India leaves a gap in understanding the full spectrum of economic burdens and challenges faced by them. This study addresses this gap by examining the factors associated with FT and its impact on the quality of life for gastric and pancreatic cancer survivors. METHODS: Data for this study were collected from April to September 2024 at Tata Memorial Hospital (TMH), Mu

Prevalence and Size-Based Risk Categorization of Pancreatic Cysts Among Asymptomatic Individuals With Screening MRI.

IMPORTANCE: Subsets of pancreatic cystic lesions (PCLs) are the only radiographically evident precursors to pancreatic cancer. Guidelines prioritize cyst size and high-risk features to determine the need for resection. However, the true prevalence of PCLs and risk factors for their development in the general population remain unknown, as available data are often biased by imaging performed in symptomatic patients. OBJECTIVE: To determine prevalence of PCLs in an asymptomatic North American cohort using size-based risk categorization and identify associated demographic factors and exposures.

[Advanced colorectal cancer in a 19-year-old male patient with Lynch syndrome:a case report].

A 19-year-old male patient with abdominal pain and diarrhea was presented to our hospital. He was diagnosed with advanced colorectal cancer based on imaging. Lynch syndrome was suspected, considering his family history. After a laparoscopic subtotal colectomy, genetic testing confirmed a pathogenic variant in the MLH1 gene, leading to a definitive diagnosis of Lynch syndrome. The onset of colorectal cancer is extremely rare in the teenage years. Herein, we discuss the potential involvement of genetic anticipation, emphasize the importance of early surveillance in individuals at risk, and consi

Inverted colonic diverticulum of the cecum mimicking a submucosal tumor: a case report.

BACKGROUND: Inverted colonic diverticulum (ICD) is a rare endoscopic finding that often mimics a sessile polyp or submucosal tumor, increasing the risk of misdiagnosis and inappropriate intervention. Accurate identification is crucial to avoid potential complications such as perforation. This case is notable for its unusual location in the cecum and its initial misdiagnosis as a gastrointestinal stromal tumor (GIST). CASE PRESENTATION: A 66-year-old man with a 10-year history of chronic constipation underwent routine surveillance colonoscopy, which revealed a 30–40 mm sessile lesion in the cec

Molecular Profiling of Germline Variants in the DNA Mismatch Repair Genes in Chinese Colorectal Cancer Patients.

BACKGROUND: A multicenter study on the DNA mismatch repair (MMR) genes enabled us to study the profiling of germline variants in MMR genes of colorectal cancer (CRC) patients with MMR deficiency (dMMR). The clinicopathological differences between Lynch syndrome (LS) patients and sporadic dMMR CRC patients were compared by Student's t-test and χ2 test. The molecular profiling of germline variants in MMR genes in Chinese CRC patients with dMMR is clarified. METHODS: A total of 326 CRC patients with dMMR were enrolled. Next-generation sequencing (NGS) and Sanger sequencing were performed using t

Epigenetic memory of colitis promotes tumour growth.

Chronic inflammation is a well-established risk factor for cancer, but the underlying molecular mechanisms remain unclear1,2. Using a mouse model of colitis, we demonstrate that colonic stem cells retain an epigenetic memory of inflammation following disease resolution that persists for more than 100 days. Here we find that memory of colitis is characterized by a cumulative gain of activator protein 1 (AP-1) transcription factor activity, with durable changes to chromatin accessibility. Further, we develop SHARE-TRACE, a method that enables simultaneous profiling of gene expression, chromatin

A PMS2-deficient pediatric high-grade glioma with PI3K-pathway mutations and adjacent developmental venous anomaly suggestive of CMMRD.

PURPOSE: Constitutional mismatch repair deficiency (CMMRD) is a rare hereditary cancer predisposition syndrome that frequently manifests with pediatric high-grade gliomas. However, recognition remains challenging, particularly in the absence of a clear family history. We report a pediatric high-grade glioma with PMS2 deficiency and complex molecular alterations to highlight key diagnostic clues and the importance of routine mismatch repair assessment. METHODS: Clinical, radiological, histopathological, immunohistochemical, and molecular findings of an 8-year-old girl presenting with a high-gr

Association Between Index Adenomas and Advanced Findings at Third Colonoscopy: Data From the New Hampshire Colonoscopy Registry.

BACKGROUND & AIMS: The United States Multi-Society Task Force for Colorectal Cancer (CRC) provides recommendations for the second surveillance colonoscopy for patients with adenomas on index exam. However, evidence which informs risk at second surveillance is limited due to lack of data on quality measures and known risk factors. We evaluated the risk for an advanced outcome on second surveillance colonoscopy for patients with index adenomas and a first surveillance colonoscopy without polyps, while accounting for risk factors and adenoma detection rates. METHODS: Our primary outcome was the

Paediatric Non Alcoholic Fatty Liver Disease (NAFLD) on the Context of the Global Epidemic of Obesity.

Nonalcoholic fatty liver disease (NAFLD) is the most common chronic liver disease of adults and children worldwide. NAFLD is a spectrum of disease, from simple hepatic steatosis to nonalcoholic steatohepatitis (NASH) which may progress to fibrosis, ultimately leads to cirrhosis. Non-alcoholic fatty liver disease (NAFLD) means the presence of 5% or more hepatic steatosis as diagnosed by liver imaging or liver biopsy in the absence of secondary hepatic fat accumulation. In North American studies, NAFLD prevalence is 29.0%-38.0% in obese children (by screening of enzyme elevation and also by auto
