The genetic landscape of breast cancer in young women from Morocco: implications for diagnosis and treatment.

Mortality rates from breast cancer in young women have fallen considerably in high-income countries but continue to rise in low-income countries such as Morocco. Young women with breast cancer face unique challenges due to the aggressive nature of the disease, the presence of aggressive subtypes (e.g., triple-negative, HER2-positive, and luminal B), and limited access to timely diagnosis and treatment. In addition, genetic factors play crucial roles in patient prognosis and response to treatment. The molecular landscape of breast cancer in young women has important implications for personalize

Association of high and moderate penetrance monogenic variants, polygenic risk, and family history with breast cancer in an ancestrally diverse population.

Limited data are available on genomic risk factors for breast cancer in diverse populations. We evaluated breast cancer risk conferred by high and moderate penetrance genes, polygenic risk scores (PRS), and family history (FH) in an ancestrally diverse biobank. We identified expected pathogenic variants (EPVs) in 14 genes among 30,223 sequenced individuals, including 15,919 unrelated women. Using electronic health records (EHRs), we evaluated associations of EPVs and PRS with breast cancer, stratified by FH and relative to individuals without EPVs and without FH. EPVs in high penetrance genes

Associations of reproductive history with serum and intratumoral sex steroid hormone levels among postmenopausal women with breast cancer: Analysis of paired serum and tumor tissue samples.

BACKGROUND: Intratumoral estrogens may contribute to the growth of breast cancer. Risk factors for breast cancer, including reproductive factors, may influence intratumoral hormone levels. METHODS: This cross-sectional study of 146 postmenopausal women with breast cancer investigated associations of reproductive factors, including parity and breastfeeding history, with serum and intratumoral (tissue) hormone levels, as well as aromatase activity, by hormone receptor subtype classified as estrogen receptor-positive or estrogen receptor-negative and progesterone receptor-negative, using analysi

Border irregularities in fibroadenomas: an under-recognised phenomenon of diagnostic impact.

AIMS: To objectively evaluate the prevalence of histological border irregularities in fibroadenomas (FAs) and characterise their association with specimen type, histological subtypes, prior procedures and associated pathologies. METHODS: A retrospective review of 912 FA specimens (549 core needle biopsies (CNBs), 45 vacuum-assisted biopsies (VABs), 242 excisions and 76 mastectomies) from 816 lesions was conducted. An irregular border was defined as an interface between the FA and adjacent parenchyma that was not smooth or circumscribed. Statistical analyses were performed to identify factors

Telehealth utilization during the COVID-19 pandemic: comparing breast cancer survivors to non-cancer patients and implications for current practice.

BACKGROUND: During the 2019 coronavirus (COVID-19) pandemic, in-person medical visits changed due to social distancing guidelines. Breast cancer (BC) patients, needing ongoing treatment or surveillance, faced increased challenges in accessing care. Telehealth became essential for providing convenient and cost-effective care while minimizing COVID-19 transmission. BC survivors, however, often value in-person visits for clinical exams. This study aimed to compare telehealth participation between patients with a history of BC and women without cancer history. METHODS: Adults aged 18 and older, i

A lasso-based model combining miRNA and clinical variables predicts future risk of breast and ovarian cancer.

Hereditary breast and ovarian cancer syndrome (HBOC) is principally caused by germline mutations in BRCA1 and BRCA2. However, most women with HBOC are undiagnosed, and some patients meeting clinical criteria for HBOC will have no identifiable mutation after genetic testing. Here, we deploy a lasso-based model to combine serum miRNA profiles with clinical data to identify women at elevated risk for ovarian cancer among a population of 1831 individuals enrolled in an institutional biobank. The miRNA and metadata variables are mapped to two-dimensional space using lasso, after which a linear clas

Localized Breast Amyloidosis in a Previously Treated Breast: A Case Report and Literature Review.

Breast amyloidosis is a rare disorder that can mimic malignancy, particularly in patients with a history of breast cancer. We describe a 61-year-old woman with a history of left breast invasive ductal carcinoma who developed an enlarging mass at the mastectomy site eight years post-treatment. Imaging suggested recurrence; however, biopsies revealed Congo red-positive amyloid deposits with admixed necrosis, fibrosis, and inflammation, with no evidence of recurrent carcinoma or lymphoproliferative disease. Mass spectrometry identified only amyloid signature proteins, with no specific amyloid fib

Re-evaluating hereditary breast and ovarian cancer risk: clinical impact of updated multigene panel sequencing and genetic counseling.

An increased lifetime risk of ovarian cancer is observed among women with a family history of ovarian cancer, with or without breast cancer, as well as among carriers of pathogenic germline variants in ovarian cancer (OC) predisposition genes. Over the past two decades, additional OC–associated genes beyond BRCA1/2 have been identified, highlighting the need for updated genetic evaluation in women previously classified as high-risk without a known PV to ensure correct diagnosis and prevent overtreatment. This retrospective, quality assurance cohort study aimed to evaluate whether updated multi

Psychologically informed reminder messages for promoting BRCA1/2 carrier screening: evidence from a large-scale population-based study.

Psychologically Informed Reminder Messages (PIRMs) may promote health-behavior change, but their impact on preventive genetic screening is unclear. Due to low adherence to a screening program, Clalit Health Services initiated an intervention to encourage BRCA1/2 carrier screening in July 2022. This observational study assesses the effectiveness of the intervention in encouraging screening and its spillover effect on related healthcare utilization, focusing on Ashkenazi Jewish females aged 25-50 with no relevant cancer history. Patients were randomly assigned to receive one of five PIRMs. Four

Time-Varying Hormonal Treatment and Metastasis-Free Survival Among ER+ Breast Cancer Patients: A Natural History Modelling Approach.

Breast cancer treatment depends on tumour subtypes. In particular, patients with oestrogen receptor-positive (ER+) tumours are treated with hormonal therapy. In Sweden, the recommended treatment duration is five years, with current guidelines advising an additional five years for women at high risk of disease recurrence. However, the impact of extended therapy on metastatic progression has not been thoroughly quantified at the population level. In this article, we use a modelling approach to estimate the time-varying effect of hormonal treatment on time to metastasis. The model is then used to

Fear of diagnosis as a barrier to the screening and diagnosis of breast cancer.

INTRODUCTION: Breast cancer diagnosis primarily occurs through mammography and biopsy. Treatment options span from surgery to chemotherapy, contingent on the cancer stage. While preventative measures such as lifestyle adjustments like weight management and reduced alcohol intake can reduce cases of breast cancer, challenges persist in accessing timely screening and treatment, especially in low- and middle-income areas of the country. METHODS: This survey engaged 59 US mothers 16 years and older with breast cancer. Administered to ensure distinct responses, it included questions covering family

Incidental discovery of second primary lung tumors in breast cancer patients: an institutional case series.

BACKGROUND: Guidelines recommend biopsy of first breast cancer recurrence. Our group recently showed that only half of patients in Washington diagnosed with metastatic breast cancer (MBC) between 2008 and 2017 underwent biopsy. METHODS: Oncologists in our group identified patients in which a lung malignancy was diagnosed in patients with a breast cancer history. RESULTS: We identified eight illustrative cases. Lung findings were typically identified on a CT obtained for staging or radiation planning. In five cases, patients were treated curatively for two early-stage malignancies. In two cas
