Predicting New-onset Diabetes Following Acute Pancreatitis: The CAPS Score.

OBJECTIVES: Postpancreatitis diabetes mellitus (PPDM-A) is a significant endocrine complication of acute pancreatitis, often with a severe disease course. Using the Los Angeles General Hospital acute pancreatitis (AP) cohort, we aimed to identify risk factors for PPDM-A and create a model to predict its occurrence. MATERIALS AND METHODS: This prospectively ascertained cohort included consecutive adults admitted with AP between 2015 and 2022 with no prior history of diabetes mellitus, chronic pancreatitis, or pancreas resection. The primary outcome was PPDM-A, defined as a hemoglobin A1c value

Clinical Characteristics of Chronic Pancreatitis Patients Presenting With Acute Pancreatitis, Abdominal Pain, or Primary Painless Disease.

BACKGROUND: Acute pancreatitis (AP) or abdominal pain does not always precede chronic pancreatitis (CP), but prevalence estimates from prior studies utilized variable definitions of CP. We aimed to examine the clinical characteristics of CP patients presenting with and without AP or abdominal pain. METHODS: The records of consecutive patients with definite CP per the M-ANNHEIM criteria between 2010 and 2023 were reviewed. AP was defined as per the revised Atlanta classification. AP preceding or concurrent with CP was defined as a diagnosis of CP >1 or ≤1 year after an index episode of AP, res

Expandable Constrained Transjugular Intrahepatic Portosystemic Shunt: An Individualised Approach for High-Risk Patients?

BACKGROUND AND AIMS: A transjugular intrahepatic portosystemic shunt (TIPS) can effectively overcome portal hypertension, but is prevented by contraindications in a considerable proportion of patients. In this study, we investigated the option of a constrained TIPS (cTIPS) with reduced diameter for high-risk patients. METHODS: Elective cTIPS were placed in 60 high-risk patients at Hannover Medical School (10/2020-04/2023). In the cTIPS procedure, a 6 mm stent(-graft) was inserted first into the intrahepatic tract to constrain the TIPS endoprosthesis. Procedural data, complications and 12 mont

Metastatic colorectal adenocarcinoma with APC and EGFR L858R co-mutations: a case report.

Mutations in the adenomatous polyposis coli (APC) gene are frequent in colorectal cancer (CRC), whereas epidermal growth factor receptor (EGFR) mutations, particularly the L858R variant, are exceedingly rare. Their coexistence in CRC is rare and poorly described in the literature and poses diagnostic and therapeutic challenges. We describe a 69-year-old man with a history of resected CRC who presented with a lung mass and adrenal lesions, initially suspected as metastatic non-small cell lung cancer (NSCLC). Biopsy revealed poorly differentiated carcinoma with immunohistochemistry suggestive of

Surgical management of small bowel adenocarcinoma in Crohn disease: a multicenter retrospective analysis.

BACKGROUND: Patients with Crohn disease (CD) face an elevated risk of developing small bowel adenocarcinoma (SBA), a malignancy characterized by late-stage diagnosis and poor prognosis. In this study we aimed to characterize surgical management and oncologic outcomes for CD-associated SBA. METHODS: A retrospective review was conducted across 3 tertiary IBD centers, analyzing 99 patients with CD who underwent surgery for SBA between 1992 and 2025. Data included patient demographics, CD history, surgical details, and oncologic outcomes. RESULTS: The median time from CD diagnosis to SBA was 25

Evaluation of Regorafenib-induced Pancreatic Enzyme Elevation in Metastatic Colorectal Cancer Treatment.

BACKGROUND/AIM: Regorafenib, a multikinase inhibitor widely used to treat metastatic colorectal cancer (mCRC), is associated with elevations in pancreatic enzyme levels. However, clinical predictors of this adverse event have yet to be adequately defined. This study aimed to identify risk factors for regorafenib-induced enzyme elevation in routine clinical practice. PATIENTS AND METHODS: We retrospectively evaluated 47 patients with mCRC who received regorafenib between May 2013 and October 2024. Pancreatic enzyme elevation was defined as a Common Terminology Criteria for Adverse Events (CTCA

Evaluating Focal Hepatic Lesions with Triple Phase Contrast Computed Tomography: A Prospective Observational Study.

BACKGROUND: Detection and characterisation of focal liver lesions (FLLs) remain challenging. Computed tomography (CT) provides crucial information on lesion number, location, nature and growth dynamics of FLLs over time. AIM: The study evaluated the use of contrast-enhanced triple-phase CT (TPCT) amongst patients with suspected FLLs in a tertiary hospital setting. METHODS: This was a prospective, observational study. Patients with clinically suspected FLL or prior history of FLLs who underwent contrast-enhanced TPCT scans at the tertiary care centre between June 2022 and November 2023 were e

Developing and Calibrating a Colorectal Cancer Microsimulation Model for Northern Ireland.

BACKGROUND: Individual-level microsimulation models are essential for evaluating colorectal cancer (CRC) screening programmes to capture the heterogeneity in disease progression. To ensure regional relevance, such models require detailed natural history structures and robust calibration to population-specific data. This study presents the development of the first CRC natural history microsimulation model tailored to Northern Ireland (NI) for evaluating the NI Bowel Cancer Screening Programme (NI BCSP). METHOD: The model simulates individual trajectories from adenoma onset to CRC diagnosis. Ei

Leveraging SARS-CoV-2-induced pre-existing humoral and cellular immunity for tumor elimination.

Tumor immunotherapy has demonstrated clinical success in some patients, but its efficacy in most solid tumors is hampered by poor infiltration of tumor-specific cytotoxic T lymphocytes (CTLs). Activating virus-specific bystander T cells via loading viral epitopes onto tumor-associated MHC class I molecules can induce antitumor immunity, but this approach is restricted by MHC polymorphism and individual infection histories. Here, we developed a hepatocellular carcinoma-targeted adeno-associated virus system that directs full-length membrane-anchored expression of the SARS-CoV-2 receptor-binding

Diabetic Ketoacidosis Unveiling Severe Hypertriglyceridemia and Acute Mild Pancreatitis: A Rare Initial Manifestation of Type 2 Diabetes Mellitus.

BACKGROUND Diabetic ketoacidosis (DKA) in combination with hypertriglyceridemia-induced acute mild pancreatitis is a rare but life-threatening clinical triad that occurs in a small subset of patients with DKA. It is an exceptionally uncommon initial presentation of type 2 diabetes mellitus in adults. This case highlights the importance of early recognition and multidisciplinary intervention in such complex clinical scenarios. CASE REPORT A 35-year-old man with no known history of diabetes mellitus presented with 3 days of epigastric abdominal pain. Initial laboratory tests confirmed DKA and se

Incidence of hepatitis C virus antibody seroconversion among people who inject drugs in Tbilisi, Georgia, 2017-2019.

BACKGROUND: Georgia launched a hepatitis C elimination program in 2015, and prioritized reducing hepatitis C virus (HCV) infections among people who inject drugs (PWID). METHODS: To assess the effectiveness of the program, we established a prospective observational cohort study to estimate incidence of HCV antibody (anti-HCV) seroconversion among PWID in Tbilisi during 2017-2019. Participants were recruited using incentivized chain-referral sampling. Incidence of anti-HCV seroconversion was calculated as the number of seroconversions divided by total person-years of follow-up (PYFU). Crude es

Utility of colon capsule endoscopy in colorectal cancer and post-polypectomy surveillance.

OBJECTIVES: Surveillance colonoscopy is recommended for follow up of colorectal cancer and polyps. It has, however, a low yield in this setting and presents a significant strain on endoscopy capacity. Colon capsule endoscopy (CCE) is an alternative diagnostic. We aimed to determine the utility of this test in surveillance patients. METHODS: A retrospective analysis of a prospectively collated service evaluation data was conducted looking at all surveillance CCE patients in NHS Highland. The primary outcomes were the need for follow up endoscopy and capacity saved. Secondary outcomes included

Gastric surgery as an acquired cause of pancreatic alpha-cell hyperplasia.

INTRODUCTION: Alpha-cell hyperplasia (ACH) is a rare condition, with only eighteen cases reported in the literature. Its primary cause is Mahvash disease, a hereditary disorder associated with pancreatic neuroendocrine tumours. Based on the experience of our site, previous gastric surgery (GS) is suspected to represent a potential acquired cause of ACH - the first such case described to date. MATERIAL AND METHODS: A case-control study was designed using pancreatic tissue samples, selecting six patients with a history of gastric surgery (GS) more than six months before, and twelve patients wit
