Evaluation of intolerance to analgesics, preservatives and food colorants with challenge tests.

In 504 patients with asthma or rhinitis 1,868 oral challenge tests with analgesics, preservatives and food colorants were performed. Sodium benzoate and other benzoic compounds, as well as tartrazine and other azo dyes, were found to give similar but milder adverse reactions than acetylsalicylic acid. No patients reacted to potassium sorbate, another preservative, or to Patent Blue, which is not an azo dye. Intolerance was demonstrated most frequently in patients with a history of adverse reactions to analgesics or food, but also in 6% of patients investigated for other reasons. There are obvi

Aspirin allergy.

This paper represents a 17-year prospective study of the clinical characteristics of patients with a history of having had allergic reactions to aspirin. The following points are especially important. (1) The most common manifestation is urticarica/angioedema; the second most common, asthma. (2) Women of child-bearing age are especially prone to develop aspirin sensitivity, otherwise the age of onset is approximately equal in the two sexes, varying from one year to 60 years. (3) Although an immunologic basis of aspirin sensitivity has not been demonstrated, 90% of its victims are also sensitiv

Asthma with aspirin intolerance. Clinical entity or coincidence of nonspecific bronchial hyperreactivity and aspirin intolerance.

The study was performed in 52 aspirin - sensitive asthmatics. It was possible to establish aspirin intolerance from the history in 46 persons and after aspirin challenge in 6 persons. All patients underwent allergological and laryngological examination and 41 aspirin oral challenge. A history of intolerance in the family was reported by 3 persons. In 6 persons, i.e. 11.5%, aspirin intolerance perceded the development of asthma. 36 persons, i.e. 70% exhibited at least 2 features of atopy, namely positive history and positive skin tests. Nasal and paranasal polyps were found in 77% of the group

Aspirin intolerance and recurrent urticaria in normal adults and children. Epidemiology and review.

The frequency of aspirin intolerance by history in 2,592 normal individuals was 0.3%. Although the frequency of aspirin intolerance was similar in adults and children, the bronchospastic type predominated in adults, and only the urticarial type was found in normal children. The frequency of recurrent urticaria, regardless of etiology, was significantly greater in adults than in children (3.8% vs. 0.3, P less than 0.001). In adults the frequency of aspirin intolerance was over 20 times greater in individuals with recurrent urticaria than in normal individuals (6.5% vs. 0.3%, P less than 0.001).

Cholinergic nervous system and immediate hypersensitivity. II. An analysis of pupillary responses.

We studied the pupillary responses of various subjects to carbamylcholine chloride (CCC) in order to assess cholinergic responsiveness. Using the concentration of topical CCC required to induce greater than or equal to 1 mm miosis in the dark as the end point, we compared the responses of five groups of subjects: normal controls; patients with allergic asthma, allergic rhinitis, or intrinsic asthma; and a group who had reproducibly positive skin tests in the absence of symptoms. Subjects with allergic rhinitis or allergic asthma were significantly more sensitive than were normal controls. The

Dental drugs and anaphylactic reactions. Report of a case.

The patient described here experienced an acute anaphylactic reaction after receiving parenteral lidocaine, oral aspirin, codeine, and penicillin following routine dental treatment. The patient had taken all of the drugs previously with no untoward effects. The incidence and mechanisms of drug-induced hypersensitivity reactions are discussed. The importance of monitoring the patient following drug administration, even in the case of a negative medical and drug history, is discussed.

Oral acetylsalicylic acid (aspirin) challenge in asthmatic children.

Thirty-two asthmatic children, mean age 9.6 years (range: 6-14 years), were studied by oral challenge with acetylsalicylic acid (Aspirin), and their PEFR was recorded at 30 min intervals for 3 hr. They had been asthmatic for a mean of 7.1 years. Other allergic symptoms (urticaria, rhinitis or atopic dermatitis), were present in 81% of the patients, and a family history of atopy in 94%; the mean blood eosinophilia was 590 cells per mm3. In three children aspirin induced a fall in PEFR values less than 8% which was non-significant. In the group as a whole there was an increase in the PEFR values

Acute adverse reactions to ibuprofen in systemic lupus erythematosus.

Three young women with systemic lupus erythematosus who were given ibuprofen for arthritis voluntarily discontinued taking the drug. On reinstitution of therapy, profound hypotension, fever, and headaches developed within hours, without other identifiable cause. A possible relationship to salicylate intolerance was considered. Because of these apparent side effects, we stress caution in the use of ibuprofen in systemic lupus erythematosus, particularly if there is any history of salicylate intolerance.

Incidence of bronchoconstriction due to aspirin, azo dyes, non-azo dyes, and preservatives in a population of perennial asthmatics.

Forty-five patients with moderately severe perennial bronchial asthma were challenged by ingestion of: acetylsalicyclic acid (ASA); 4 azo dyes (tartrazine, sunset yellow, amaranth, and ponceau); 3 non-azo dyes (erythrosine, brilliant blue, and indigotin); sodium benzoate (NaB); parahydroxybenzoic acid (OHBA); butylated hydroxyanisole (BHA); and butylated hydroxytoluene (BHT). A fall in forced expiratory volume is one second (FEV1) greater than 25% from baseline was considered positive. Seven patients who gave an unequivocal history of aspirin intolerance were not challenged with ASA; an additi

Nasal polypectomy in patients with asthma and sensitivity to aspirin.

Nasal polypectomy is safe in patients with asthma and intolerance to aspirin. Asthma should be controlled before polypectomy. Patency of the nasal airway after the operation and the effect of polypectomy on asthma are independent of severity of the asthma, history of atopy, and administration of steroids. We reviewed retrospectively the courses of 101 patients with the asthma triad who had nasal polypectomies between 1970 and 1974. Before polypectomy, 39 patients were hospitalized to control active asthma: during the postoperative hospitalization, 19 had wheezing, mostly mild. Of the 62 patien

Aspirin and concomitant idiosyncrasies in adult asthmatic patients.

The nasal and respiratory symptoms observed after oral challenge to aspirin (ASA), tartrazine, and other nonsteroidal anti-inflammatory substances are best described as idiosyncratic reactions. A positive response to oral challenge, defined as a 20% fall in forced expiratory volume in 1 sec (FEV1) from baseline for up to 4 hr, occurred in 44 of 230 patients with ASA, 11 of 277 with tartrazine, 2 of 93 with sodium salicylate, and 2 of 69 with acetaminophen. No one had a positive response to tartrazine, sodium salicylate, or acetaminophen who was not also positive to ASA. The dose of ASA causing

Dental patients were surveyed to determine the medications they were using and to compile information on their medical histories. Complete survey forms were obtained from 4,365 patients who were seen for emergency treatment or for diagnosis and treatment planning between August 1975 and July 1976. Of the patients surveyed, 1,833 (42%) were taking medications. The frequency of intake of different drug groups was: analgesics, 14.2%; oral contraceptives, 12.1%; antihypertensives, 9.4%; miscellaneous (vitamins, antacids, anorectics, nonprescription cold remedies), 9.4%; psychotherapeutic agents, 7

Health status of the New York City prison population.

During a two-week period in June 1975, all 1,420 prisoners admitted to New York City Correctional Facilities were studied to determine the prevalence of current and past health conditions. Information was obtained from a standard admission health examination. Almost three-quarters of the male and female prisoners were 30 years or less. Of the study population, 57 per cent were black, 24 per cent were Hispanic and 17 per cent were white. Forty-one per cent reported a history of illicit drug use. More than one-quarter of the prisoners reported a present illness and three-fifths received at least

Aspirin idiosyncrasy in patients admitted for nasal polypectomy.

Investigation of 100 consecutive patients admitted for nasal polypectomy revealed only 3 patients with aspirin idiosyncrasy and asthma. It is important that this combination be recognized but fortunately this can be done by the history. Incidental findings were that nasal polypi are much commoner in men and that the incidence of atopy was the same as that in the general population.

Hypersensitivity to acetylsalicylic acid (ASA) and tartrazine in patients with asthma.

One-hundred and forty asthmatics were tested perorally with acetylsalicylic acid (ASA), and/or with the azo-colour tartrazine; a fall in PEF of more than 20% was accepted as a positive result. About one quarter of the patients displayed a positive reaction to one of the two tested agents. No significant correlation was found between the reactions of these, and the presence of atopy, nasal polyposis, sinusitis, rhinitis, sensitivity to cold air, the age at onset, duration of asthma, or history of sensitivity to alcoholic drinks. The history suggested sensitivity to ingested, possibly coloured,

The diagnosis of aspirin idiosyncrasy by analgesic challenge.

Analgesic challenge was employed to identify fifty patients with aspirin idiosyncrasy. No serious reactions were observed using the method described. Patients with a history of the condition were highly sensitive and reacted to small doses of aspirin or to paracetamol. Patients with a positive challenge test, but previously unaware of aspirin idiosyncrasy, were less sensitive: they required larger challenge doses of aspirin and did not respond to paracetamol challenge. In the absence of an in vitro test, analgesic challenge is the only means of confirming the presence of aspirin idiosyncrasy.

Significance of tartrazine sensitivity in chronic urticaria of unknown etiology.

Of 38 patients with chronic urticaria of unknown etiology who were evaluated for food and drug additive sensitivity, 53% (20/38) had urticaria for 1 yr or more. Total eosinophil counts were not elevated in most patients, and the frequency of atopy was found to be similar to that in a general population. Of these 38 patients, 10 (26%) had a personal history of aspirin intolerance, but elimination of aspirin did not relieve the urticaria. In a double-blind crossover challenge with 0.22 mg of tartrazine and a control, tartrazine sensitivity was found in 8% (3/38) of patients with chronic urticari
