Comparative study regarding the stability of a proximal ulnar stump with or without distal oblique bundle reconstruction during the Sauvé‒Kapandji procedure: a finite-element analysis.

BACKGROUND: The most common postoperative complication of the Sauvé‒Kapandji (S-K) procedure is proximal ulnar stump instability. The distal oblique bundle (DOB) is a stable tissue used to stabilize the distal radioulnar joint. This study created finite-element models of the distal oblique bundle (DOB) to determine its effect on the proximal ulnar stump instability encountered during the Sauvé‒Kapandji procedure. PURPOSE: We hypothesized that a proximal ulnar stump with distal oblique bundle reconstruction would provide greater stability than a proximal ulnar stump without distal oblique bund

Influence of electrode placement on the recognition of different gesture categories using high-density sEMG.

High-density surface electromyography (HD-sEMG)-based gesture recognition serves as a critical interface for human-computer interaction (HCI). However, recognition accuracy exhibits a significant dependency on gesture complexity and electrode positioning. To address this, we systematically investigated the relationship between gesture types and sEMG electrode placement locations through intra-subject, inter-day, and inter-subject validation protocols. Two distinct gesture categories were analyzed, i.e., single-degree-of-freedom (single-DoF) gestures and daily-used multi-finger synergistic gest

Unsupervised Domain Adaptation for Inter-Session Re-Calibration of Ultrasound-Based HMIs.

Human-Machine Interfaces (HMIs) have gained popularity as they allow for an effortless and natural interaction between the user and the machine by processing information gathered from a single or multiple sensing modalities and transcribing user intentions to the desired actions. Their operability depends on frequent periodic re-calibration using newly acquired data due to their adaptation needs in dynamic environments, where test-time data continuously change in unforeseen ways, a cause that significantly contributes to their abandonment and remains unexplored by the Ultrasound-based (US-base

Hyper-Adaptation in the Human Brain: Functional and Structural Changes in the Foot Section of the Primary Motor Cortex in a Top Wheelchair Racing Paralympian.

The human brain has the capacity to drastically alter its somatotopic representations in response to congenital or acquired limb deficiencies and dysfunctions. The main purpose of the present study was to elucidate such extreme adaptability in the brain of an active top wheelchair racing Paralympian (participant P1) who has congenital paraplegia (dysfunction of bilateral lower limbs). Participant P1 has undergone long-term wheelchair racing training using bilateral upper limbs and has won a total of 19 medals in six consecutive summer Paralympic games as of 2021. We examined the functional and

Deep learning and session-specific rapid recalibration for dynamic hand gesture recognition from EMG.

We anticipate wide adoption of wrist and forearm electomyographic (EMG) interface devices worn daily by the same user. This presents unique challenges that are not yet well addressed in the EMG literature, such as adapting for session-specific differences while learning a longer-term model of the specific user. In this manuscript we present two contributions toward this goal. First, we present the MiSDIREKt (Multi-Session Dynamic Interaction Recordings of EMG and Kinematics) dataset acquired using a novel hardware design. A single participant performed four kinds of hand interaction tasks in v

Multi-Joint Angles Estimation of Forearm Motion Using a Regression Model.

To improve the life quality of forearm amputees, prosthetic hands with high accuracy, and robustness are necessary. The application of surface electromyography (sEMG) signals to control a prosthetic hand is challenging. In this study, we proposed a time-domain CNN model for the regression prediction of joint angles in three degrees of freedom (3-DOFs, include two wrist joint motion and one finger joint motion), and five-fold cross validation was used to evaluate the correlation coefficient (CC). The CC value results of wrist flexion/extension motion obtained from 10 participants was 0.87-0.92,

Toward More Robust Hand Gesture Recognition on EIT Data.

Striving for more robust and natural control of multi-fingered hand prostheses, we are studying electrical impedance tomography (EIT) as a method to monitor residual muscle activations. Previous work has shown promising results for hand gesture recognition, but also lacks generalization across multiple sessions and users. Thus, the present paper aims for a detailed analysis of an existing EIT dataset acquired with a 16-electrode wrist band as a prerequisite for further improvements of machine learning results on this type of signal. The performed t-SNE analysis confirms a much stronger inter-s

Correlation and Correspondence between Skeletal Maturation Indicators in Hand-Wrist and Cervical Vertebra Analyses and Skeletal Maturity Score in Korean Adolescents.

This retrospective observational study aimed to examine the correlation and correspondence between skeletal maturation indicators (SMI), cervical vertebral maturation indicators (CVMI), and radius-ulna-short bones (RUS) skeletal maturity scores in Korean adolescents, and to determine whether easily obtainable SMI or CVMI can replace the RUS skeletal maturity score. A total of 1017 participants were included with both hand-wrist radiograph and lateral cephalogram acquired concurrently. From the lateral cephalogram, CVMI was determined; through the hand-wrist radiograph, SMI was categorized, and

Upper-Limb Electromyogram Classification of Reaching-to-Grasping Tasks Based on Convolutional Neural Networks for Control of a Prosthetic Hand.

In recent years, myoelectric interfaces using surface electromyogram (EMG) signals have been developed for assisting people with physical disabilities. Especially, in the myoelectric interfaces for robotic hands or arms, decoding the user's upper-limb movement intentions is cardinal to properly control the prosthesis. However, because previous experiments were implemented with only healthy subjects, the possibility of classifying reaching-to-grasping based on the EMG signals from the residual limb without the below-elbow muscles was not investigated yet. Therefore, we aimed to investigate the

End-to-End Estimation of Hand- and Wrist Forces From Raw Intramuscular EMG Signals Using LSTM Networks.

Processing myoelectrical activity in the forearm has for long been considered a promising framework to allow transradial amputees to control motorized prostheses. In spite of expectations, contemporary muscle-computer interfaces built for this purpose typically fail to satisfy one or more important desiderata, such as accuracy, robustness, and/or naturalness of control, in part due to difficulties in acquiring high-quality signals continuously outside laboratory conditions. In light of such problems, surgically implanted electrodes have been made a viable option that allows for long-term acqui

Should Hands Be Restricted When Measuring Able-Bodied Participants to Evaluate Machine Learning Controlled Prosthetic Hands?

OBJECTIVE: When evaluating methods for machine-learning controlled prosthetic hands, able-bodied participants are often recruited, for practical reasons, instead of participants with upper limb absence (ULA). However, able-bodied participants have been shown to often perform myoelectric control tasks better than participants with ULA. It has been suggested that this performance difference can be reduced by restricting the wrist and hand movements of able-bodied participants. However, the effect of such restrictions on the consistency and separability of the electromyogram's (EMG) features rema

Analysis of Compensatory Movements Using a Supernumerary Robotic Hand for Upper Limb Assistance.

Recently, extratheses, aka Supernumerary Robotic Limbs (SRLs), are emerging as a new trend in the field of assistive and rehabilitation devices. We proposed the SoftHand X, a system composed of an anthropomorphic soft hand extrathesis, with a gravity support boom and a control interface for the patient. In preliminary tests, the system exhibited a positive outlook toward assisting impaired people during daily life activities and fighting learned-non-use of the impaired arm. However, similar to many robot-aided therapies, the use of the system may induce side effects that can be detrimental and

Virtual reality-based treatment for regaining upper extremity function induces cortex grey matter changes in persons with acquired brain injury.

BACKGROUND: Individuals with acquired brain injuries (ABI) are in need of neurorehabilitation and neurorepair. Virtual anatomical interactivity (VAI) presents a digital game-like format in which ABI survivors with upper limb paresis use an unaffected limb to control a standard input device and a commonplace computer mouse to control virtual limb movements and tasks in a virtual world. METHODS: In a prospective cohort study, 35 ambulatory survivors of ABI (25/71% stroke, 10/29% traumatic brain injury) were enrolled. The subjects were divided into three groups: group A received VAI therapy only

[Secretan's syndrome: myth or pathomimia?].

UNLABELLED: Secretan's syndrome is a rare condition involving generally trauma-induced hard edema of the dorsal aspect of the hand. The cause is poorly understood but factitious trauma is often suspected. CASE REPORT: A 42-year-old woman presented with a fortuitous edema on the back of the right hand. The minimally depressible edema was associated with moderately intense mechanical pain. Routine laboratory tests were normal. An extensive imaging work-up (bone x-ray of the hand and wrist, bone scintigraphy, computed tomography phlebography, lymphoscintigraphy, magnetic resonance imaging) was e

Correlation of malrotation deformity in distal radius fractures with radiographic analysis: cadaveric study.

PURPOSE: The radiographic parameters commonly used for evaluating distal radius fractures are radial length, palmar tilt, radial inclination, and articular congruity. Rotation of the distal fragment is not routinely evaluated after distal radius fractures. The purpose of this study was to define the appearance of distal fragment malrotation on conventional radiographs and to correlate varying degrees of malrotation with the corresponding radiographic findings. METHODS: Six distal radiuses from embalmed cadavers were cut and stabilized in 10 degrees, 20 degrees, and 30 degrees of pronated malr

Thrombocytopenia absent radius syndrome: presence of brachiocarpalis muscle and its importance.

PURPOSE: Common clinical manifestations associated with thrombocytopenia absent radius (TAR) syndrome have been reported in the literature since its initial description in 1959. The purpose of this study was to report a clinical observation and describe the anatomic location of this brachiocarpalis muscle. METHODS: The radiographs of 20 patients (39 extremities) with TAR syndrome were reviewed and are the focus of this report. The presence or absence of an abnormal muscle crossing both the humero-ulnar and ulno-carpal joint, and its anatomic origin and insertion, were documented by radiograph

Surgical correction of Madelung's deformity by combined corrective radioulnar osteotomy: 14 cases with four-year minimum follow-up.

Fourteen wrists in 11 girls, mean age 13.3 years (range 9-16) at surgery, were treated for Madelung's deformity. The presenting complaint was incapacitating pain. All were treated by radial closing wedge osteotomy and ulnar shortening osteotomy. The dorsal retinaculum was also surgically repaired in six cases. At a mean follow-up of 5.1 years (range 4-8.75), we observed improved range of motion in both flexion/extension and pronation/supination and absence of pain during daily activity. Radiographically, positioning of the distal radial articular surface and lunate subsidence were improved. Un

Madelung's deformity: quantitative assessment of x-ray deformity.

PURPOSE: To evaluate 5 defined measurement techniques that are applicable to x-rays of Madelung's deformity: ulnar tilt, lunate subsidence, lunate fossa angle, palmar tilt, and palmar carpal displacement. The measurements rely on the longitudinal axis of the ulna and the carpal bones to determine drawing lines and avoid the distorted distal radius and its deformed lunate fossa. The reliability and reproducibility of the measurements is determined. METHODS: Forty-eight sets of posteroanterior and lateral x-ray views of the wrist of subjects with the clinical diagnosis of Madelung's deformity w

Enduring representational plasticity after somatosensory stimulation.

Somatosensory stimulation (SS), leading to increases in motor cortical excitability, influences motor performance in patients with brain lesions like stroke. The mechanisms by which SS modulates motor function are incompletely understood. Here, we used functional magnetic resonance imaging (fMRI, blood-oxygenation-level-dependent (BOLD), and perfusion imagings simultaneously acquired in a 3 T magnet) to assess the effects of SS on thumb-movement-related activation in three regions of interest (ROI) in the motor network: primary motor cortex (M1), primary somatosensory cortex (S1), and dorsal p
