Efficacy and Safety of Drug-Coated Balloon Versus Percutaneous Transluminal Angioplasty in Below-Knee Arterial Occlusive Disease: A Meta-Analysis and Systematic Review.

BACKGROUND: Drug-coated balloon (DCB) have gained prominence as an innovative drug delivery approach in intravascular interventions. Despite its growing clinical application, the safety and efficacy of DCB, especially in the treatment of below-the-knee arterial occlusive disease, require further investigation and validation. OBJECTIVE: This study aimed to systematically evaluate the comparative clinical outcomes of DCB versus percutaneous transluminal angioplasty (PTA) in patients with below-the-knee arterial disease. METHODS: This systematic review and meta-analysis included studies from Pu

Additively manufactured reinforced polymeric stents for peripheral artery disease: design, manufacture and performance evaluation.

Stent failure in the peripheral arterial system remains a major clinical challenge due to high mechanical demands caused by limb movement. Unlike coronary arteries, below-the-knee vessels are exposed to continuous flexion, torsion and axial movement, increasing the risk of structural failure, stent collapse, fracture or migration. Due to their tuneable mechanical properties, polymeric stents enabled by advanced additive manufacturing emerge as a potential candidate to overcome the current limitations of metallic stents. This study examines whether polymeric stents made with the ST3DT process a

Preoperative BUN-to-Albumin Ratio Is Independently Associated with Major Reamputation After Distal Amputation in Diabetic Foot: A Retrospective Cohort Study.

Background: Major level escalation following distal amputation for diabetic foot-defined as subsequent below-knee amputation (BKA)-represents a clinically meaningful endpoint with substantial functional implications. Identifying reliable and readily available preoperative biomarkers capable of predicting major level escalation remains a clinical challenge. This study aimed to evaluate the independent prognostic value of the C-reactive protein-to-albumin ratio (CAR) and blood urea nitrogen-to-albumin ratio (BAR) in predicting postoperative major level escalation. Methods: We retrospectively ana

Editorial Commentary: Arthroscopic Surgery Is Not Silent; Repetitive Peak Noise Exposure During Surgery May Represent an Underrecognized Occupational Hazard.

Orthopaedic surgeons are routinely exposed to a variety of occupational hazards in the operating room; however, intraoperative noise exposure remains an underrecognized risk that may contribute to the development of occupational noise-induced hearing loss. Although traditional metrics such as time-weighted average noise levels (average decibels [dBA] over an 8-hour period) below 85 dBA may be considered low risk, repeated intraoperative exposure to maximum decibel levels ≥85 dBA may represent a clinically relevant cumulative hazard. These concerns are further supported by recent high-level evi

Fillet Flaps for Lower Extremity Salvage.

This article explores the role of fillet flaps in lower extremity salvage, highlighting their ability to provide durable coverage without additional donor site morbidity. When standard flaps are limited or not recommended, fillet flaps, which are taken from nonviable parts, offer a useful reconstructive alternative. The fillet of foot flap receives particular attention, as demonstrated by a case study involving below-the-knee amputation. This case demonstrates how the method can enhance prosthesis optimization and maintain limb length and sensation in difficult salvage situations.

Liver Dysfunction Assessed by Albumin - Bilirubin (ALBI) Score is Associated with Disturbed Cholesterol Profiles and Reduced Survival in Patients with Chronic Limb-Threatening Ischemia.

AIMS: Liver dysfunction may affect chronic limb-threatening ischemia (CLTI) outcomes, but its impact on survival and lipid profiles is unclear. To assess the association of liver dysfunction using the Albumin-Bilirubin (ALBI) score, with survival, liver fibrosis, and cholesterol in CLTI patients. METHODS: We retrospectively analyzed single-center data of CLTI patients with below-knee lesions between 2008-2018. ALBI scores were calculated from serum albumin and total bilirubin. Optimal cutoff for 3-year overall survival (OS) was determined by receiver operating characteristic (ROC) analysis, d

Pattern-Matched Powered Gait Orthosis Training in Patients with Neurological Gait Impairment: A Multicenter Prospective Pilot Study of Hip and Knee-Ankle-Foot Orthoses.

Background: Wearable powered gait orthoses offer a clinically flexible alternative to treadmill-based robotic systems, yet evidence on different device configurations matched to the site of neuromuscular impairment remains limited. Methods: In this multicenter prospective pilot study, 75 participants with neurological gait impairment were allocated to a hip orthosis (HO; n = 39) or a knee-ankle-foot orthosis (KAFO; n = 36) group based on clinical assessment of predominant gait pattern. Both groups completed six overground gait-training sessions over three weeks. Primary outcomes were the Six-M

Incidence of Clinically Documented Phantom Limb Pain During Hospitalization and Preoperative Risk Factors in Patients Who Underwent Nontraumatic Major Lower Limb Amputation: A Single-Center, Retrospective Study.

Background and Objectives: Phantom limb pain (PLP) frequently occurs after lower limb amputation (LLA). However, a consensus has not been reached regarding its incidence, particularly in nontraumatic amputations, because reported estimates vary according to case mix, follow-up duration, and outcome definitions. Data from Japan remain limited. Methods: After approval by the Institutional Review Board, the electronic medical records of patients who underwent above-knee amputation (AKA) or below-knee amputation (BKA) at Saitama Medical University Hospital between 1 January 2012 and 31 December 20

MedNet-FS: a few-shot learning framework for 3D MRI-based knee injury classification.

The development of deep learning models for 3D knee MRI analysis is critically constrained by the scarcity of large, annotated datasets. Few-shot learning (FSL) offers a promising pathway to leverage small data, but its effective application to volumetric medical imaging remains underexplored. This study introduces MedNet-FS, a 3D FSL framework that strategically integrates domain-specific pre-training on knee MRI data with a Generalized End-to-End (GE2E) loss to create a highly effective solution for data-scarce environments. Our central finding is that this targeted combination is paramount;

Repeated Knee Flexion Maneuvers after Wound Closure Improve Early but not Long-Term Knee Flexion Following Simultaneous Bilateral Total Knee Arthroplasty: A Double-Blinded Randomized Controlled Trial.

Restoring knee joint range of motion is a key objective after total knee arthroplasty (TKA), as it strongly influences functional outcomes. This study evaluated the efficacy of repeated knee flexion maneuvers performed after wound closure on clinical outcomes following one-stage simultaneous bilateral TKA. In this prospective, double-blinded, randomized controlled trial, 46 patients (92 knees) were randomized to determine the intervention side, which involved 15 repetitions of deep knee flexion and full extension after wound closure. The contralateral knee served as the control. Outcomes inclu

Automatic registration of generic bone models for X-ray-based biomechanics: Application to 3D rat hindlimb kinematics and sex-specific gait patterns.

Biplanar X-ray fluoroscopy offers sub-millimeter insight into skeletal motion, yet prevailing pipelines rely on subject-specific computed tomography (CT), implanted markers, digitally reconstructed radiograph (DRR)-based optimization, or deep networks trained on thousands of segmented images, which slow preparation and analysis, reducing throughput in rodent studies. We present a fully automatic, markerless, scan free framework that reconstructs six-degree-of-freedom poses of seven rat hindlimb bones from 100frames s-1 stereo radiographs. Seventeen landmarks per view are detected by DeepLabCut

Chronotropic index mediates the relationship between lower-limb loss level and 6-minute walk test performance.

INTRODUCTION: Lower-limb loss (LLL) significantly impacts exercise capacity. Low heart rate, i.e., chronotropic, response to exercise is known to contribute to reduced exercise capacity in other clinical populations; associations between population-specific variables (e.g., LLL level), chronotropic response to activity, and exercise capacity among adults with LLL remain unknown. METHODS: In this cross-sectional study, 130 community-dwelling adults with unilateral below-knee (n = 95; 58.5 ± 14.9 years-old, 74.7% male) or above-knee (n = 35; 54.0 ± 13.7 years-old; 68.6% male) LLL provided demog

Transtibial amputation increases the metabolic energy needed for stabilizing walking in the sagittal plane.

Walking energy consumption is higher in people with versus without a transtibial amputation, but the underlying reasons are poorly understood. Active prostheses that restore ankle power do not necessarily decrease walking energy consumption, suggesting other reasons than a lack of ankle power for the increased energy consumption. Transtibial amputation impacts walking stability, as evidenced by the increased fall risk, and there is an energetic cost associated with stabilizing walking. It is, however, unclear how transtibial amputation affects the energetic cost of stabilizing walking. We asse

Technical advances and clinical impact of photon-counting computed tomography angiography for peripheral artery disease.

OBJECTIVE: Accurate cross-sectional imaging is essential for diagnosis, procedural planning, and postintervention surveillance in patients with peripheral artery disease (PAD). Conventional computed tomography angiography (CTA) is limited by calcium blooming, beam hardening, and reduced diagnostic accuracy in small and heavily calcified vessels. Photon-counting computed tomography angiography (PCCTA) represents a recent technological advancement with the potential to overcome these limitations. This review summarizes the technical principles of PCCTA and evaluates current evidence regarding it

A hybrid in-line aspiration technique using a coronary aspiration catheter directly connected to a mechanical thrombectomy pump for acute limb ischemia.

Acute limb ischemia requires rapid thrombus removal to restore perfusion and prevent limb loss. Although mechanical aspiration thrombectomy systems enable effective clot debulking, residual thrombus may persist in below-the-knee vessels that are less accessible to large-bore catheters. We describe a reproducible hybrid in-line aspiration technique in which a coronary aspiration catheter is connected directly to a mechanical thrombectomy pump, allowing continuous aspiration in the distal arteries. In an illustrative case of popliteal and tibial thromboembolism, this configuration facilitated ef

The Utility of Intravascular Ultrasound Evaluation in Addition to the Conventional Risk Factors for Predicting Restenosis After Endovascular Therapy Using Drug-Coated Balloon in Femoropopliteal Artery Lesions.

PURPOSE: Baseline lesion characteristics have been reported to be associated with vessel patency after drug-coated balloon (DCB) treatment for femoropopliteal artery lesions. Intravascular ultrasound (IVUS) evaluation was also associated with vessel patency. The present study aimed to investigate whether the IVUS endpoints could predict the incidence of restenosis after DCB treatment, independent of classical risk factors. METHODS: We retrospectively reviewed 111 patients with 147 consecutive femoropopliteal (FP) lesions treated with IVUS-guided endovascular therapy (EVT) using DCB between 20

Pilot Validation of a Novel Inline Device for Real-Time Monitoring of Abdominal Mechanics During Pneumoperitoneum.

The abdominal pressure-volume (P-V) relationship during laparoscopic insufflation is curvilinear and subject to substantial inter-individual variability, yet clinical practice relies on universal pressure targets derived from population-level guidelines. The Smart Inline Compliance Module (SICM) is a novel inline retrofit device that acquires intra-abdominal pressure and insufflation gas flow through physically separated sensing circuits, reconstructs insufflated volume by numerical integration of the flow signal, and derives the abdominal P-V curve and its biomechanical parameters in real tim
