Thermal Symmetry Between Residual and Intact Limbs in Individuals with Lower Limb Amputation: Resting and Post-Activity Conditions.

Background and Objectives: Individuals with lower limb amputation are at increased risk of developing post-amputation complications that may affect rehabilitation outcomes. Assessing thermal symmetry between the residual and intact limbs, as well as activity-related temperature changes, represents a non-invasive and cost-effective approach for preliminary clinical evaluation. This study aimed to investigate regional skin temperature symmetry between residual and intact limbs at rest and following activity, and to examine whether the presence of phantom limb sensation or pain influences thermal

Beyond Nerve Hyperexcitability: Potential Implications of Cerebrospinal Fluid CASPR2 Antibodies for Central Pain Processing in Isaacs Syndrome: A Case-Based Systematic Review.

INTRODUCTION: Isaacs syndrome (IS), or acquired neuromyotonia, is a rare immune-mediated neuromuscular disorder characterized by increased peripheral nerve excitability. Clinically, it manifests with fasciculations, cramps, excessive sweating, and the presence of CASPR2 autoantibodies in the serum. IS can occur as a paraneoplastic condition or may remain idiopathic. METHODS: We report an adult case of IS that prompted a case-based literature review investigating the association between cerebrospinal fluid (CSF) CASPR2 antibodies and IS. A systematic literature search of PubMed and Scopus was

Verticalization with Erigo® in early rehabilitation in patients with severe acquired brain injury: evidence from a pilot study.

INTRODUCTION: Following severe acquired brain injury (sABI), the inability to maintain orthostatism and prolonged bed rest triggers neurovegetative, haemodynamic, respiratory and osteo-myo-articular adaptations that may significantly affect rehabilitation. Restoring orthostatic tolerance is a key rehabilitation goal to prevent secondary complications. Although the safety of early verticalization with Erigo device has been established, evidence on its effects on muscle hyperactivity, trophism, and consciousness level in the subacute sABI remains limited. This pilot study aimed to monitor lower-

Light chain deposition disease masquerading as hypophosphatemic osteomalacia: a case report.

Light chain deposition disease (LCDD) is characterized by the deposition of monotypic immunoglobulin light chains in multiple organs. The kidneys are usually affected in cases of LCCD, and they typically present with mesangial nodule formation, extensive monoclonal light chain deposition, proteinuria, and renal insufficiency. We report a rare case of LCDD that presented with bone pain and proximal amyosthenia secondary to renal tubular phosphate loss. A 42-year-old woman presented with progressive pain in the waist and bilateral hips, along with increasing amyosthenia in both lower limbs that

Life-threatening spinal cord injury and occult intercostal artery rupture following low-energy trauma in a patient with ankylosing spondylitis: a case report.

BACKGROUND: Ankylosing spondylitis (AS) significantly increases vulnerability to severe spinal injuries from minor trauma due to rigid "bamboo spine" morphology and secondary osteoporosis. Isolated intercostal artery rupture without rib fractures is extremely rare in general trauma populations. The concurrent occurrence of multilevel spinal fractures with complete spinal cord injury and occult intercostal artery hemorrhage following low-energy blunt trauma in AS patients has rarely been documented, and the underlying pathophysiological mechanisms remain unclear. CASE PRESENTATION: A 43-year-o

When A Fall Isn't Just a Fall: Delayed Diagnosis of Acquired Hemophilia A in a Nonagenarian Patient.

BACKGROUND Acquired hemophilia A (AHA) is a rare but potentially life-threatening condition caused by autoantibodies against factor VIII. It often presents in older adults and its presentation can range from isolated lab abnormality of prolonged activated partial thromboplastin time (aPTT) without bleeding to mild bleeding or spontaneous vs disproportionate life-threatening bleeding. Delayed diagnosis is frequent due to the rarity of the condition and propensity to attribute bleeding to other common causes, such as falls. CASE REPORT A 92-year-old woman with recurrent ground-level falls presen

Yellow Nail Syndrome in Childhood: A Case Report Highlighting Diagnostic Challenges and the Possible Role of Nail Development-Related Genetic Variants.

INTRODUCTION: Yellow nail syndrome (YNS) is a rare disorder characterized by the triad of yellow nail discoloration, lymphedema, and respiratory manifestations. Pediatric cases are uncommon and often present incompletely, making diagnosis challenging. Although generally considered acquired, rare familial cases suggest possible genetic susceptibility. CASE PRESENTATION: An 8-year-old girl presented with diffuse yellow nail dystrophy involving all 20 nails and recurrent unilateral swelling of the left lower extremity. Examination revealed yellow discoloration, dystrophy, and cuticle loss affect

Analysis of risk factors and prognostic differences in hospital-acquired thrombosis between very preterm infants and moderate to late preterm infants.

OBJECTIVE: Neonates have the highest incidence of thrombosis among pediatric populations, and preterm infants are at a high risk of venous thrombosis, with its risk factors remaining inconclusive and treatment criteria remaining ununified. This study aimed to explore the high-risk factors for venous thrombosis in preterm infants with varying gestational ages and catheterisation types, and to clarify the clinical characteristics, treatment strategies, and short-term prognosis of neonatal venous thrombosis, to provide evidence for clinical decision-making. METHODS: A retrospective cohort study

A Rare Lesion of Soft Tissue Malakoplakia Mimicking Neoplasm in the Thigh.

Malakoplakia, a rare acquired histiocytic inflammatory disease. It is an incidental finding on histopathology examination, as it does not have definite clinical manifestations. It typically affects the genitourinary tract but can also involve various anatomical sites. We present malakoplakia involving the left thigh, a rare extragenital manifestation. A 42-year-old male patient presented with insidious left thigh swelling, which gradually increased in size over a year, without associated pain or systemic symptoms. Imaging and fine needle aspiration cytology prompted a suspicion of malignancy.

The role of Heme oxygenase-1 (HO-1) in sepsis-associated organ damage: A systematic review.

Sepsis-induced myopathy is a significant cause of intensive care unit-acquired weakness (ICUAW) and a leading cause of mortality in ICU patients. ICUAW primarily affects the respiratory muscles and limb skeletal muscles, leading to prolonged mechanical ventilation and bedridden periods. This condition increases the risk of complications such as pneumonia, atelectasis, and lower extremity venous thrombosis, thereby impacting patients' quality of life and, in severe cases, jeopardizing their lives. The pathogenesis of sepsis-induced myopathy is complex, and currently, there are no effective prev

A case and review of fibroblast growth factor-23-mediated hypophosphatemic osteomalacia in the absence of pathogenic PHEX variants.

We report an atypical case of fibroblast growth factor-23 (FGF23)-mediated hypophosphatemic osteomalacia without a pathogenic variant (PV) of PHEX, who improved biochemically, clinically and radiologically post burosumab treatment. We present a narrative review of FGF23-mediated hypophosphatemic osteomalacia and propose a roadmap for investigating the etiology of hypophosphatemia to guide therapy. A 29-yr-old female with FGF23-mediated hypophosphatemic osteomalacia experienced multiple insufficiency fractures, including bilateral femoral diaphyseal fractures and delayed healing. This occurred

Brazilian expert consensus on the diagnosis, classification, screening for complications and treatment of familial partial lipodystrophy.

BACKGROUND: Partial lipodystrophies are a rare and heterogeneous group of diseases characterized by variable loss of adipose tissue. Around the world, the high heterogeneity in phenotypic expression, limited awareness, and absence of standardized diagnostic criteria for familial partial lipodystrophies (FPLD) may contribute to the underdiagnosis of genetic forms. The estimated high prevalence of FPLD in Brazil, combined with resource limitations in the healthcare system and a lack of specialized medical centers, presents significant challenges in the diagnosis and treatment of lipodystrophy-re

A rare presentation of acute-onset chronic inflammatory demyelinating polyneuropathy with the detection of anti-GM3 and anti-sulfatides antibodies: a case report.

OBJECTIVES: Chronic inflammatory demyelinating polyneuropathy (CIDP) is an acquired immune-mediated neuropathy defined by clinical progression for more than 2 months. 16-20% of CIDP patients may present with rapidly progressive weakness that resembles GBS, known as acute-onset CIDP (A-CIDP). However, it is challenging to distinguish from GBS-TRF because of their similar clinical symptom and features. In this case review, we report a patient with A-CIDP with the detection of anti-GM3 and anti-sulfatides antibodies, which rarely have been in A-CIDP and may account for her progressive and recurre

Clinical and imaging features of women with polygenic partial lipodystrophy: a case series.

BACKGROUND: Familial partial lipodystrophy (FPLD) is an inherited disorder of white adipose tissue that causes premature cardiometabolic disease. There is no clear diagnostic criteria for FPLD, and this may explain the under-detection of this condition. AIM: This pilot study aimed to describe the clinical features of women with FPLD and to explore the value of adipose tissue measurements that could be useful in diagnosis. METHODS: In 8 women with FPLD and 4 controls, skinfold measurements, DXA and whole-body MRI were undertaken. RESULTS: Whole genome sequencing was negative for monogenic me
