Falls during walking are a major cause of injury and death in the elderly. From a falls perspective, a critical gait event is minimum toe clearance (MTC), as failure to achieve foot-ground clearance results in a trip and potentially a fall. MTC exhibits stride-to-stride variability, which must be minimised to avoid the occurrence of toe-ground contact events. Gait kinematics were acquired from 10 young and nine elderly men for 1000 sequential strides during treadmill walking. The swing toe was modelled as the endpoint of a 21 degree-of-freedom kinematic chain. Central tendency and dispersion o

The non-invasive estimation of hip axial rotation is prone to error. Most of this is likely to originate from soft tissue artefact (STA) at the thigh. The purpose of this study was to evaluate the relative performance of four different thigh cluster configurations. Three were novel configurations whilst one represented the Helen Hayes convention. Twenty able-bodied adults performed two alternative motor tasks involving hip axial rotation: gait and an isolated longitudinal rotation task. Kinematic data were acquired using a three-dimensional motion analysis system (VICON 512, Oxford Metrics, Ox

OBJECTIVE: We sought to investigate the efficacy of treatment strategies used to manage motor impairments following acquired brain injury (ABI) in order to provide guidance for clinical practice based on the best available evidence.
METHODS AND MAIN OUTCOMES: A systematic review of the literature from 1980-2005 was conducted focusing on pharmacological, non-pharmacological, and exercise interventions available for motor impairments post ABI. The efficacy of a given intervention was classified as strong (supported by two or more randomized controlled trials (RCTs)), moderate (supported by a si

PURPOSE: To evaluate a novel time-resolved contrast-enhanced (CE) projection reconstruction (PR) magnetic resonance angiography (MRA) method for identifying potential bypass graft target vessels in patients with Class II-IV peripheral vascular disease.
MATERIALS AND METHODS: Twenty patients (M:F = 15:5, mean age = 58 years, range = 48-83 years), were recruited from routine MRA referrals. All imaging was performed on a 1.5 T MRI system with fast gradients (Signa LX; GE Healthcare, Waukesha, WI). Images were acquired with a novel technique that combined undersampled PR with a time-resolved acqu

The problem of positioning mobile C-arms, e.g. for down the beam techniques, as well as repositioning during surgical procedures currently requires time, skill and additional radiation. This paper uses a Camera-Augmented Mobile C-arm (CAMC) to speed up the procedure, simplify its execution and reduce the necessary radiation. For positioning the C-arm in down-the-beam position, the pre-operative diagnostic CT is used for defining the axis. Additional CT visible markers on patient's skin allow the CAMC's optical camera to compute the C-arm's pose and its required displacement for positioning. In

Continuous muscle fibre activity in a patient with demyelinating neuropathy is rare. We report an 18 year old boy who presented with walking difficulty and continuous rippling in muscles of upper and lower limbs. He had dysarthric speech, hypertrophied arm and calf muscles with normal power, tone, reflexes and sensations. Myokymic discharges were seen in deltoid, biceps, quadriceps and calf muscles. His blood counts, chemistry, thyroid profile, DNA, Rh factor were normal and CPK was raised. CSF showed protein 50 mg/dl and 4 lymphocyte/mm3. Nerve conduction study revealed conduction block and a

Surgical correction of the footdrop deformity caused by various neuromuscular diseases is a time-honored technique, with most surgical procedures intended to correct the absence of active dorsiflexion and a concomitant cavovarus foot deformity. We describe suspension tenodesis of the tibialis anterior tendon to the distal tibia to correct the footdrop deformity. This technique is simpler to perform and more secure than transferring the distal attachment of a tendon to a new location in the foot itself or using tendon weaving techniques. In addition, compared with an ankle fusion, our technique

It is well established that body weight influences bone mass, but there is considerable controversy in the literature as to whether the principal determinant is lean mass, fat mass, or combinations of both, with variable degrees of relative influence as a function of age and sex. Total body and regional tissue composition studies were performed with DXA in a cohort of 102 HIV+ males who were on highly active antiretroviral therapy. These medications may be associated with a unique peripheral lipoatrophy without commensurate loss of lean tissue mass, and thereby provide an opportunity to assess

BACKGROUND: The late sequelae of infantile hip infection include absence of the femoral head and neck, proximal migration of the femur, lower-extremity length discrepancy, abnormal gait, and pain. The Ilizarov hip reconstruction includes an acute valgus and extension osteotomy at the proximal part of the femur combined with gradual distraction for realignment and lengthening at a second, more distal, femoral osteotomy. The purpose of this study was to determine whether this technique can successfully treat the sequelae of infantile hip infection.
METHODS: We performed a retrospective review o

INTRODUCTION: Medial rotation deformity of the hip is a problem to patients who are handicapped by cerebral palsy but able to walk, because the knees point inward during gait ("kissing patellae") and cause falls and frequent injuries. Knees and ankles are subject to stress and, therefore, they assume compensating positions. Lower legs assume position of valgus and external rotation, whereas feet rotate either inwards or outwards. Secondary deformities make gait more difficult and cause rapid tear of footwear.
AIM: The purpose of the paper was to retrospectively analyze the effects of transpos

Although amyotrophic lateral sclerosis and progressive spinal muscular atrophy have been recognized to occur in association with human immunodeficiency virus infection, to our knowledge, brachial amyotrophic diplegia, a form of segmental motor neuron disease, has not been previously reported. Brachial amyotrophic diplegia results in severe lower motor neuron weakness and atrophy of the upper extremities in the absence of bulbar or lower extremity involvement, pyramidal features, bowel and bladder incontinence, and sensory loss. We describe a human immunodeficiency virus-seropositive man withou

INTRODUCTION: Sensory motor multifocal acquired demyelinating neuropathy (SMMADN) is a variant of the chronic multifocal neuropathies. Several reports have been published about the clinical and electrophysiological progression in motor multifocal neuropathy (MMN) prior to and following immunoglobulin (Ig) therapy, but we have found no reports that deal with SMMADN.
AIMS: We describe a case of SMMADN in which we conducted a clinical and electrophysiological evaluation before and after therapy with Ig UNC Hemoderivados.
CASE REPORT: Female, 40 years of age, who presented asymmetrical weakness

BACKGROUND: Several genetic defects are associated with increased risk of venous thrombosis. The factor V Leiden (FVL) and prothrombin G20210A mutations are the most frequent causes of inherited thrombophilia.
OBJECTIVES: To evaluate combined genotyping for these 2 mutations in patients presenting with thromboembolic episodes and to correlate genotypic findings with clinical characteristics.
RESULTS: Blood specimens were collected from 401 patients presenting with thromboembolic disease between January 1998 and September 1998, and genotyping for both FVL and prothrombin mutations was perform

This is a prospective comparative study of magnetic resonance imaging (MRI) of the deep veins versus contrast venography in consecutive patients treated for various injuries to their lower extremities, showing no clinical symptoms of deep vein thrombosis. The majority of examinations referred to in this study were performed according to the following methodology: First, the patient was subjected to MRI. Subsequently, within a 24-h interval, he/she was subjected to contrast venography. The acquired results were compared in a blinded manner. The diagnostic indices for MRI were calculated on the

Fat redistribution in the setting of protease inhibitor use is increasingly common and is associated with insulin resistance in human immunodeficiency virus (HIV)-infected patients. However, little is known regarding the factors that may contribute to abnormal insulin regulation in this population. We assessed fasting insulin levels in HIV-infected men and determined the relationship among insulin, body composition, endogenous gonadal steroid concentrations, and antiviral therapy in this population. We also determined the effects of exogenous testosterone administration using the homeostatic m

PURPOSE: The purpose of this work was to compare rapidly acquired MR images with routinely employed SE and turbo SE (TSE) images in screening for hip avascular necrosis (AVN).
METHOD: Twelve patients with findings suspicious for radiographically occult AVN of one or both hips were studied with our routine screening protocol (imaging time >7 min) and similarly weighted, rapidly acquired MR sequences (imaging time <1 min).
RESULTS: The rapidly acquired MR images were judged to be similar to the routine protocol in demonstrating marrow edema, irregular lines within the femoral head characterist

Deep venous thrombosis is initiated primarily in the pockets of the valves of the veins of the lower limbs and the main pelvic veins. In addition to a genetic predisposition there are several acquired conditions associated with deep venous thrombosis such as major surgery, trauma, cancer, pregnancy, and immobilization. While major diseases as well as hormonal changes have been shown to cause changes in blood coagulation and fibrinolytic factors, the impact of immobilization per se is much less clear. It has been shown that in the absence of intermittent pulsatile flow the blood within the valv

A cross-sectional study was conducted to examine the relationship between bone mineral density (BMD) and life-style related factors including exercise and dietary habits in 1016 pre-menopausal women and 856 post-menopausal women in Ishikawa Prefecture, Japan. The achilles BMD in 1,872 women ages between 19 and 85 years were measured from 1995 to 1996 by an ultrasound system. The stiffness index calculated by the Lunar Achilles ultrasound machine was used as the BMD in this analysis. Self-administered questionnaires were used to obtain the following information: medical history, pregnancy, deli
