Are All Exertional Heat Strokes Equal?

PURPOSE: Exertional heat stroke (EHS) is commonly studied as a single clinical entity. While most EHS cases are associated with situational factors, few (complex) cases recur and/or lack clear contextual explanations. This study aimed to investigate thermoeffector responses by comparing individuals with complex EHS to those with simpler, context-related cases. METHODS: Fifty-six participants were divided into three groups: 18 controls with no history of heat illness (CONT), 38 EHS (ALL-EHS), subdivided into simple EHS (SIMP-EHS; n = 21) and complex EHS (COMP-EHS; n = 17). Following anthropome

Mild Traumatic Brain Injury and Psychiatric Disorders in Year 1 of the Adolescent Brain Cognitive Development (ABCD) Study.

OBJECTIVE: Pediatric mild traumatic brain injury (mTBI) is a major public health concern. New psychiatric disorders (NPDs) can arise after mTBI, yet postinjury NPDs in nonclinical samples remain poorly understood. METHODS: NPDs were prospectively characterized for youths ages 9-11 years who experienced an mTBI in their first year of participation in the Adolescent Brain Cognitive Development study, compared with control groups of orthopedically injured and noninjured individuals; a broad set of preinjury factors was accounted for in analyses. RESULTS: Between baseline (ages 9-10) and year 1

Executive Summary of the Clinical Practice Guideline on Tinnitus Update.

OBJECTIVE: Tinnitus is the perception of sound without an external source. More than 50 million people in the United States have reported experiencing tinnitus, resulting in an estimated prevalence of 10% to 15% in adults. The focus of this guideline is on tinnitus that is both bothersome and persistent (lasting 6 months or longer), which often negatively affects the patient's quality of life. The target audience for the guideline is any clinician, including allied health professionals, involved in managing patients with tinnitus. The target patient population is limited to adults (18 years an

Composite graft survival in allen zone II fingertip amputations: Independent predictors of graft failure.

BACKGROUND: Composite grafting is a commonly used treatment option for Allen Zone II fingertip amputations; however, reported survival rates vary considerably across studies. Although several clinical and injury-related factors have been suggested to influence graft survival, independent predictors of failure and clinically meaningful ischemia duration thresholds remain insufficiently defined. This study aimed to identify independent clinical predictors of composite graft survival and to explore, secondarily, whether ischemia duration is associated with graft failure. METHODS: Forty-two patie

A sound approach: scoping review of adult lower urinary tract foreign body recognition and management.

INTRODUCTION: Insertion of foreign bodies into the lower urinary tract by adults is an uncommon but clinically important presentation encountered by urologists and emergency providers across a range of motivations. Retained or migrated objects can lead to serious complications including infection, calculus formation, and urethral trauma, often necessitating careful and complex management. METHODS: We conducted a scoping review following the PRISMA extension for Scoping Reviews. Because only published, de-identified case material was used, institutional review board approval was not required.

Pyogenic Granuloma in the Sinonasal Cavity: A Potential 38-Year Latency Following Foreign Body Entrapment.

BACKGROUND: Lobular capillary hemangioma (LCH), also known as pyogenic granuloma (PG), is a benign vascular lesion typically associated with minor trauma or irritation, predominantly occurring in the oral cavity and in females. Its sinonasal occurrence, particularly in males, is rare, and an association with remote childhood trauma remains a speculative hypothesis. Clinically, it must be differentiated from other vascular sinonasal lesions such as angiofibroma, hemangioma variants, and malignant mimics. CASE PRESENTATION: This rare case report descripts a highly unusual case of an inflammator

Influence of Child Abuse and Neglect on Psychopathy.

Psychopathy is the confluence of antagonism toward others (meanness), fearless social dominance (boldness), and difficulty controlling impulses (disinhibition) and is at the root of many social woes. One factor associated with high levels of psychopathy in adulthood is a history of abuse and neglect in childhood. Although there are several studies on this topic, there is little research on the relative association of child abuse and neglect with psychopathy as measured using contemporary assessment instruments (i.e., incorporating indicators of meanness, boldness, and disinhibition). In this s

[Management of "Iron Swords" War Casualties at Shaare Zedek Medical Center: The Operational and Clinical Impact of Establishing a Dedicated Helicopter Landing Pad].

INTRODUCTION: The events of October 7, 2023, and the ensuing war presented unprecedented challenges to Israel's healthcare system. In response to the large-scale influx of casualties, a helicopter landing pad was emergently established at Shaare Zedek Medical Center (SZMC), facilitating direct aeromedical evacuation to the hospital for the first time in its history and enabling redistribution of the trauma care burden among Israel's supra-regional trauma centers. METHODS: A retrospective review was conducted using trauma registry data from SZMC and the Israel Defense Forces Medical Corps. All

A Retrospective Career-Long and Seasonal Study of Injury Patterns in 196 Elite Swimmers: The Role of Primary Discipline and Competitive Distance.

Elite swimming is characterized by high training volumes, which increases the risk of overuse injuries. Detailed information on whether specific disciplines or competitive distance matters for the occurrence of injuries is sparse. This study aimed to describe the injury epidemiology among elite swimmers and to identify potential explanatory factors. We employed a retrospective, self-reported design, with the known limitations of recall bias. In June 2024, a questionnaire was sent to elite swimming clubs across Denmark, Norway, and Sweden. A total of 220 swimmers completed the questionnaire des

Fear of falling risk after hip surgery in older adults: an updated sex-specific systematic review with GRADE and PAF assessment.

BACKGROUND: Fear of falling (FOF) is a common clinical issue in elderly individuals, especially after hip fractures. Studies show wide variation in FOF prevalence after fractures, with rates ranging from 38 to 72% and notable differences among study groups. Differences in study design and assessment methods make it necessary to conduct a rigorous systematic review. This will consolidate evidence on both epidemiological patterns and modifiable risk factors for FOF. METHODS: A systematic literature search was conducted across nine electronic databases (PubMed, Web of Science, Cochrane Library, E

Disability Risk for Male Professional Soccer Players: A Question of Age? A New Method Based on Transfermarkt®'s Collaborative Data.

The soccer industry generates billions in revenue, largely from matchday sales, commercial activities, and television rights. A significant portion of club revenues are spent on player wages making their availability for games and training economically vital. Any period of injury-related absence functions as a form of temporary disability, creating significant financial exposure for teams, insurers, and reinsurers. Preventing injuries is crucial for soccer clubs to mitigate financial and sports risks associated with player unavailability. Transfermarkt® database has been utilized to develop an

Impact of a standardized documentation template on hospital case mix index and reimbursement for patients with traumatic brain and spine injuries.

OBJECTIVE: The case mix index (CMI) measures the complexity and severity of hospitalized patients and is used to determine hospital reimbursement rates. Better documentation is associated with higher complexity assigned to cases as measured by CMI but remains understudied in the trauma population. The authors hypothesized that standardized templates for neurotrauma history and physical (H&P) notes are associated with increased CMI and reimbursement. METHODS: A multidisciplinary team consisting of trauma program professionals, trauma data specialists, neurosurgeons, revenue integrity specialis
