Neutrophil Subset Immunophenotyping.

Neutrophils, the most abundant circulating leukocyte population, are principally involved in pathogen clearance through three major mechanisms: phagocytosis, degranulation, and release of neutrophil extracellular traps. Along with these roles, neutrophils can promote or suppress tumor growth and metastatic processes. These different immunological functions reflect a great heterogeneity of maturation and activation status, as at least two main circulating populations can be identified: low-density granulocytes and normal-density neutrophils. In this chapter, we provide a detailed protocol for m

Global epidemiology of myocarditis: disparities in age, sex and geography.

Myocarditis is a clinically and aetiologically heterogeneous disease that is associated with substantial morbidity and mortality, with approximately a million new cases and 17,000 deaths annually worldwide. However, the incidence of myocarditis varies widely by geographical region, sex and age. The aetiology of myocarditis is multifactorial and shaped by regional, demographic and socioeconomic contexts. In high-income countries, parvovirus B19 and human herpesvirus 6 are the predominant cardiotropic viruses that induce lymphocytic myocarditis, particularly in children. By contrast, in low-to-m

Unintended Shifts in High-Priority Adult Heart Transplant Listings after the OPTN's September 2025 Policy Change.

The Organ Procurement and Transplantation Network amended Status 2 heart allocation criteria in September 2025, requiring documented failure of high-dose inotropes before candidates could qualify via intra-aortic balloon pump (IABP) or percutaneous endovascular ventricular assist device (PEVAD) support. Using OPTN data, we compared seasonally matched pre-policy (September 2024-June 2025, n=4,206) and post-policy (September 2025-June 2026, n=4,231) adult candidates. While standard Status 3 high-dose inotrope listings increased 2.0% to 4.1% and standard Status 2 IABP/PEVAD listings decreased 18.

Menstrual Health in Children, Adolescents, and Young Adults with Cardiac Disease: An Underrecognized Component of Heart Failure, Mechanical Circulatory Support, and Transplant Care.

BACKGROUND: Menstrual abnormalities, including heavy menstrual bleeding, amenorrhea, and dysmenorrhea, are frequently encountered among children, adolescents, and young adults with congenital and acquired heart disease, yet remain underrecognized in routine cardiovascular care. Anticoagulation, thrombosis risk, heart failure physiology, and limited high-quality evidence complicate management and contribute to substantial variation in practice. METHODS: The Advanced Cardiac Therapies Improving Outcomes Network convened a multidisciplinary working group of cardiologists, hematologists, gynecolo

US-based Delphi Consensus Study on Adjuvant Radiation Therapy (ART) in Node-Negative Cutaneous Squamous Cell Carcinoma (CSCC).

PURPOSE: There is a paucity of high-level evidence for adjuvant radiation therapy (ART) in cutaneous squamous cell carcinoma (CSCC) creating practice variations. This study provides expert consensus recommendations on patient and tumor selection, timing, duration, dosage, and additional considerations for ART in node-negative CSCC. METHODS: The Delphi panel included 35 experts in radiation oncology (15), dermatology/Mohs micrographic surgery (9), medical oncology (7), head and neck surgery (3), and surgical oncology (1). Consensus was defined as ≥80% concordance; near-consensus 70-79% concord

Fecal microbiota transplantation for the treatment of neurogenic rosacea: protocol of a randomized, blinded, placebo-controlled explorer study.

INTRODUCTION: Neurogenic rosacea is a distinct phenotype of rosacea characterized by persistent facial erythema, flushing, burning, and stinging, and it often responds poorly to conventional therapies. Increasing evidence suggests that gut microbiota dysbiosis may be involved in the pathogenesis of NR. Fecal microbiota transplantation may provide a novel therapeutic strategy by restoring microbial homeostasis. OBJECTIVE: This study aims to evaluate the efficacy and safety of FMT in patients with NR and to explore its potential mechanisms through modulation of the gut microbiota. PATIENTS AND

Organ Preservation and Quality of Life After Total Neoadjuvant Treatment in Rectal Cancer.

IMPORTANCE: Data on quality of life (QOL) after total neoadjuvant therapy (TNT) are important for counseling patients on the benefits of watch-and-wait (WW) vs total mesorectal excision (TME) for rectal cancer. A thorough understanding of both options and their long-term impact on bowel, urinary, sexual function, and other QOL domains remains limited. OBJECTIVE: To examine the association of organ preservation with QOL and functional outcomes among patients with rectal cancer treated in 2 phase 2 clinical trials. DESIGN, SETTING, AND PARTICIPANTS: This unplanned, post hoc, cohort study used

Case report: Maternal engraftment of T-cells in a patient with Severe Combined Immunodeficiency due to DCLRE1C pathogenic variant - diagnostic and treatment challenges.

BACKGROUND: Severe combined immunodeficiency (SCID) is a rare but serious congenital condition caused by genetic defects that severely impair the development and function of T cells, B cells, and natural killer (NK) cells. Without early diagnosis and immune system reconstitution, SCID is usually fatal in infancy. The condition requires prompt recognition and hematopoietic stem cell transplantation (HSCT) to restore immune function. This report describes the first genetically confirmed case of SCID in Latvia - caused by a DCLRE1C gene pathogenic variant and diagnosed prior to the implementation

Prioritization and Severe Primary Graft Dysfunction after Lung Transplantation.

BACKGROUND: Primary graft dysfunction (PGD) is the leading cause of early morbidity and mortality after lung transplantation (LTx). Critically ill candidates may receive priority on the transplant waiting list; however, the relationship between prioritization and the development of severe PGD remains unclear. METHODS: We performed a retrospective analysis of a prospectively maintained cohort including all consecutive LTx performed at a tertiary center between January 2016 and June 2025. Recipients were categorized according to prioritization status. Severe PGD was defined as grade 3 PGD accor

The Prognostic Value of Entropy from T1 Mapping in Pulmonary Arterial Hypertension.

BACKGROUND: The prognostic value of left ventricular (LV) entropy from T1 mapping in pulmonary arterial hypertension (PAH) remains unexplored. STUDY DESIGN AND METHODS: This prospective study enrolled patients with PAH who underwent cardiovascular magnetic resonance (CMR) within 72h of right heart catheterization (RHC). Myocardial native T1, extracellular volume fraction (ECV) and entropy were calculated in mid-ventricular short-axis slice. The primary endpoint was defined as all-cause mortality or lung transplantation. Outcomes were further analyzed across groups stratified by survival statu

Impact of Initial Treatment Setting on Survival in Relapsed Pediatric and AYA Bone Sarcomas: A Secondary Analysis of a Prospective Clinical Trial.

BACKGROUND: Outcomes in relapsed or refractory pediatric and AYA bone sarcomas remain poor, and the impact of early referral to specialized sarcoma centers on survival is unclear. Early centralization of care may improve local and systemic treatment quality, but evidence in this population is limited. METHODS: This secondary analysis of a prospective single-center regorafenib trial in children and adolescents/young adults with relapsed or refractory bone sarcomas compared outcomes between patients managed at a specialized high-volume sarcoma center from initial diagnosis ("internal" group) an

Cutaneous Lymphoma International Prognostic Index and Risk Stratification in Advanced Mycosis Fungoides and Sézary Syndrome.

IMPORTANCE: Risk stratification in advanced mycosis fungoides and Sézary syndrome (MF/SS) remains suboptimal given that current TNMB (tumor, node, metastasis, blood) staging has limited prognostic value for clinically relevant outcomes. OBJECTIVE: To determine whether the Cutaneous Lymphoma International Prognostic Index (CLIPI) may be used to predict progression-free survival (PFS) and best objective response (BOR) in patients with advanced MF/SS. DESIGN, SETTING, AND PARTICIPANTS: This international prospective cohort study included patients with stage IIB to IVB MF/SS from the PROCLIPI tr

Myelodysplastic Syndromes-Transformed Acute Myeloid Leukemia With Concurrent DEK::CAN Fusion Gene Positivity and WT1 Overexpression: A Case Report and Literature Review.

BACKGROUND: Myelodysplastic syndromes (MDS) are a group of clonal hematopoietic stem cell disorders with a risk of progression to acute myeloid leukemia (AML). MDS-transformed AML (MDS-AML) has a poor prognosis. The concurrent presence of DEK::CAN fusion gene positivity and WT1 overexpression is extremely rare and likely denotes a highly aggressive disease. CASE: A 57-year-old Han Chinese female with a 5-year history of MDS (DEK::CAN status at initial diagnosis unknown) presented with fatigue and pancytopenia. Bone marrow blasts accounted for 81% (AML-M5), and karyotype showed t(6;9)(p23;q34)

MAIA frailty subgroup analysis: long-term follow-up with daratumumab plus lenalidomide/dexamethasone in transplant-ineligible newly diagnosed myeloma.

In MAIA (Daratumumab, lenalidomide, and dexamethasone versus lenalidomide and dexamethasone alone in newly diagnosed multiple myeloma), daratumumab plus lenalidomide/dexamethasone (D-Rd) improved progression-free survival (PFS) and overall survival (OS) versus Rd in transplant-ineligible newly diagnosed multiple myeloma (NDMM). Here, we report an updated subgroup analysis by frailty status. We retrospectively performed frailty assessments using age, Charlson Comorbidity Index, and baseline Eastern Cooperative Oncology Group performance status score; patients were classified as fit, intermediat

Fragmentation patterns of human telomeric chromatin in plasma cfDNA.

The specialized chromatin structure at telomeres protects the linear ends of chromosomes from DNA surveillance machineries. Here, we demonstrate that circulating cell-free DNA (cfDNA) from plasma can map chromatin structure at telomeres. We find that the telomeric 6-mer repeats (TTAGGG/CCCTAA) are the most abundant circulating 6-mers in cfDNA. Telomeric sequences in cfDNA contain subnucleosomal footprints distinct from the rest of the genome, arising from specific cleavages in the C-rich strand, and a nucleosome repeat length of 145 bp, markedly shorter than the ~170 bp observed genome-wide.
