US-based Delphi Consensus Study on Adjuvant Radiation Therapy (ART) in Node-Negative Cutaneous Squamous Cell Carcinoma (CSCC).

PURPOSE: There is a paucity of high-level evidence for adjuvant radiation therapy (ART) in cutaneous squamous cell carcinoma (CSCC) creating practice variations. This study provides expert consensus recommendations on patient and tumor selection, timing, duration, dosage, and additional considerations for ART in node-negative CSCC. METHODS: The Delphi panel included 35 experts in radiation oncology (15), dermatology/Mohs micrographic surgery (9), medical oncology (7), head and neck surgery (3), and surgical oncology (1). Consensus was defined as ≥80% concordance; near-consensus 70-79% concord

Fecal microbiota transplantation for the treatment of neurogenic rosacea: protocol of a randomized, blinded, placebo-controlled explorer study.

INTRODUCTION: Neurogenic rosacea is a distinct phenotype of rosacea characterized by persistent facial erythema, flushing, burning, and stinging, and it often responds poorly to conventional therapies. Increasing evidence suggests that gut microbiota dysbiosis may be involved in the pathogenesis of NR. Fecal microbiota transplantation may provide a novel therapeutic strategy by restoring microbial homeostasis. OBJECTIVE: This study aims to evaluate the efficacy and safety of FMT in patients with NR and to explore its potential mechanisms through modulation of the gut microbiota. PATIENTS AND

Organ Preservation and Quality of Life After Total Neoadjuvant Treatment in Rectal Cancer.

IMPORTANCE: Data on quality of life (QOL) after total neoadjuvant therapy (TNT) are important for counseling patients on the benefits of watch-and-wait (WW) vs total mesorectal excision (TME) for rectal cancer. A thorough understanding of both options and their long-term impact on bowel, urinary, sexual function, and other QOL domains remains limited. OBJECTIVE: To examine the association of organ preservation with QOL and functional outcomes among patients with rectal cancer treated in 2 phase 2 clinical trials. DESIGN, SETTING, AND PARTICIPANTS: This unplanned, post hoc, cohort study used

Case report: Maternal engraftment of T-cells in a patient with Severe Combined Immunodeficiency due to DCLRE1C pathogenic variant - diagnostic and treatment challenges.

BACKGROUND: Severe combined immunodeficiency (SCID) is a rare but serious congenital condition caused by genetic defects that severely impair the development and function of T cells, B cells, and natural killer (NK) cells. Without early diagnosis and immune system reconstitution, SCID is usually fatal in infancy. The condition requires prompt recognition and hematopoietic stem cell transplantation (HSCT) to restore immune function. This report describes the first genetically confirmed case of SCID in Latvia - caused by a DCLRE1C gene pathogenic variant and diagnosed prior to the implementation

Cutaneous Lymphoma International Prognostic Index and Risk Stratification in Advanced Mycosis Fungoides and Sézary Syndrome.

IMPORTANCE: Risk stratification in advanced mycosis fungoides and Sézary syndrome (MF/SS) remains suboptimal given that current TNMB (tumor, node, metastasis, blood) staging has limited prognostic value for clinically relevant outcomes. OBJECTIVE: To determine whether the Cutaneous Lymphoma International Prognostic Index (CLIPI) may be used to predict progression-free survival (PFS) and best objective response (BOR) in patients with advanced MF/SS. DESIGN, SETTING, AND PARTICIPANTS: This international prospective cohort study included patients with stage IIB to IVB MF/SS from the PROCLIPI tr

Chimeric antigen receptor (CAR) T-cell therapy in dermatology: A narrative review.

Chimeric antigen receptor (CAR) T-cells are autologous T-cells genetically engineered to express an antigen receptor that can recognise and kill cells expressing that target antigen. Originally developed for refractory or relapsed B-cell and plasma cell malignancies, CAR T-cell therapy is now being explored as a promising treatment for B-cell-mediated autoimmune diseases. CAR-T cells produce 'deep' B-cell depletion and cause 'resetting' of the immune system, thereby achieving long-lasting remissions and potentially even 'cure', addressing some major limitations of current immunosuppressive and

The role of a multidisciplinary approach in non-melanoma skin cancer management.

A multidisciplinary team (MDT) is fundamental for properly managing non-melanoma skin cancer (NMSC). Surgery is the first treatment choice for early-stage disease; however, in cases with a high risk of recurrence or when demolitive approaches would not achieve safe margins, radiotherapy (RT) or an MDT approach is required. We retrospectively revised the population evaluated at our weekly MDT meeting, and all patients were discussed. A case series of 130 patients visited from July 2021 to March 2024 was collected. In-person visits were performed. The male/female ratio was 69.5/31.5%. Elderly pa

Intent to Treat Analysis of the Primary and Secondary Outcomes for the ODINN Intact Fish Skin Graft for Deep Diabetic Foot Wounds Trial.

There is a significant need for trials that evaluate the treatment of University of Texas (UT) grade 2 and 3 diabetic foot ulcers (bone, joint, or tendon exposed wounds). We undertook a trial looking at the effect of intact fish skin graft (IFSG) on these deep and difficult-to-heal ulcers. 262 patients Intent to Treat (ITT) patients with UT grade 2 and 3 DFUs were randomised to receive intact fish skin graft (IFSG) or a standardised treatment (SOC) that adhered to the International Working Group on the Diabetic Foot (IWGDF) guidelines. The secondary endpoints that were measured included wound

Desmosine as a Novel Biomarker Candidate for Pulmonary and Cutaneous Chronic Graft-Versus-Host Disease.

Pulmonary and cutaneous fibrotic manifestations are common in chronic graft-versus-host disease (cGVHD) and can significantly reduce patients' quality of life and functional status. Plasma desmosine (pDES), a byproduct of elastin breakdown, may serve as a biomarker for fibrotic tissue involvement in cGVHD. The primmary objective was to evaluate the association between plasma desmosine levels and pulmonary and cutaneous manifestations in patients with cGVHD. We measured pDES levels in 81 cGVHD patients enrolled in the NIH cGVHD Natural History Study using liquid chromatography-tandem mass spect

Therapeutic Outcomes in VEXAS Syndrome: A Multicenter Comparative Cohort of Allogeneic Hematopoietic Stem Cell Transplantation and Hypomethylating Agents.

Hypomethylating agents (HMA) and allogeneic hematopoietic stem cell transplantation (alloHSCT) have both demonstrated remissions in VEXAS; however, comparative data is lacking. We conducted a multicenter, retrospective analysis of 66 patients diagnosed with VEXAS syndrome treated with HMA (n = 35) or alloHSCT (n = 31). Baseline characteristics such as genetics, co-morbidities, and performance status were balanced between the groups, except older age in the HMA group. Median follow-up from therapy initiation was 18 months (95% CI: 11-26), and 14 (21%) deaths were reported (alloHSCT n = 3; HMA n

Reconstruction of a Complex Pretibial Wound Using Dermal Regeneration Template and Split-Thickness Skin Graft.

The reconstruction of the distal leg is challenging due to watershed vascularity and scarce soft-tissue bulk. While traditional reconstruction guidelines rely on vascularized flap coverage, such interventions are higher-risk options in certain patients with significant medical co-morbidities. This case report describes non-microsurgical reconstruction of a distal-third pre-tibial wound in a patient with a history of renal transplant, type 2 diabetes, coronary artery disease, and peripheral vascular disease. In this patient, chronic immunosuppression induced blunt wound healing, while concurren

Beyond the First Graft: Skin Cancer Risk and Risk Stratification in Retransplanted Solid Organ Recipients.

Solid organ transplant recipients (SOTRs) face a markedly elevated risk of post-transplant skin cancer, particularly cutaneous squamous cell carcinoma (cSCC), driven by chronic immunosuppression, including the direct oncogenic effects of some immunosuppressive drugs, and cumulative carcinogenic risk factors including ultraviolet radiation exposure. Retransplanted solid organ transplant recipients (R-SOTRs) represent a growing yet under-recognised subgroup, carrying additional vulnerability as the second or third transplant will often require more profound immunosuppression. Despite this, no re

Sustained-release oxymatrine hydrogel enhances the survival of transplanted placental mesenchymal stem cells and promotes wound healing in diabetic mice.

Chronic diabetic wounds represent a significant clinical challenge due to impaired healing processes characterized by persistent inflammation, compromised angiogenesis, and a hostile microenvironment. Despite the therapeutic potential of mesenchymal stem cells (MSCs), their post-transplantation survival remains suboptimal in such pathological conditions. Here, we developed a sustained-release oxymatrine-loaded hyaluronic acid methacryloyl (HAMA) hydrogel to enhance placental mesenchymal stem cell (PMSC)-mediated repair in diabetic wounds. In a streptozotocin-induced diabetic mouse model with f

VEXAS syndrome.

VEXAS (vacuoles, E1 enzyme, X-linked, autoinflammatory, somatic) syndrome is a disorder discovered in 2020 that bridges haematology, immunology and genetics. VEXAS syndrome arises from somatic mutations in UBA1, which encodes an E1 ubiquitin-activating enzyme, acquired in haematopoietic stem cells. These mutations disrupt ubiquitin-dependent protein homeostasis, triggering proteotoxic and inflammatory stress that drives systemic inflammation, cytopenias and clonal haematopoiesis. Clinically, VEXAS syndrome presents predominantly in older men with glucocorticoid-dependent inflammation, neutroph

Self-Reported Dietary Carotenoid Intake and Skin Carotenoid Score Status in a Geographically Diverse U.S. Sample.

Background: The reflection spectroscopy (RS)-based skin carotenoid score (SCS) has been validated to assess carotenoid intake. However, associations between the SCS and individual dietary carotenoid species have been inconsistently reported in the previous literature. This study examined the utility of using a carotenoid screener along with RS to assess individual dietary carotenoid species intake among U.S. adults. Methods: Adults (N = 280) from eight U.S. sites completed a 44-item carotenoid intake screener to estimate total and individual dietary carotenoid intake (α-carotene, β-carotene, β
