Performance and Safety of the Extravascular Implantable Cardioverter-Defibrillator in Patients With Hypertrophic Cardiomyopathy.

BACKGROUND: The automatic implantable cardioverter-defibrillator (ICD) is effective in preventing arrhythmic sudden cardiac death in patients with hypertrophic cardiomyopathy (HCM). The extravascular (EV) ICD provides antitachycardia pacing and defibrillation capabilities while avoiding transvenous lead complications. OBJECTIVES: This study sought to evaluate the safety and performance of the novel EV-ICD in patients with HCM. METHODS: This analysis was derived from the Pivotal EV-ICD Study, an international, multicenter, interventional trial evaluating the EV-ICD in patients with guideline-

Outcomes of Sternal-Sparing Left Ventricular Assist Device Insertion in Obese Patients.

INTRODUCTION: Sternal-sparing left ventricular assist device (ssLVAD) insertion avoids the morbidity of median sternotomy for patients with heart failure. Obesity may present challenges in this minimal access operation. We present our experience with ssLVAD in patients with body mass index (BMI) of > 35 kg/m2 or greater. METHODS: A single-center retrospective review of durable ssLVADs implanted from 2019 to 2024 was performed. Patients were stratified by the presence of World Health Organization obesity grade II (BMI >35 kg/m2). A composite outcome was created, including the presence of posto

Hypovolemia assessment with inferior vena cava collapsibility index in cardiac surgery patients: a cross-sectional study.

BACKGROUND: Patients who undergo cardiac surgery are at high risk of developing hemodynamic instability during induction of anesthesia, with hypovolemia being a possible trigger. The diagnosis of this condition usually depends on the anesthesiologist’s clinical judgment and ability to interpret hemodynamic parameters once induction has begun. Assessment of inferior vena cava collapsibility and diameter has been correlated with identification of patient volume status. The aim of this study was to describe hemodynamic behavior during anesthetic induction based on the presence of hypovolemia, ass

Impact of unfavorable aortic anatomy on transcatheter aortic valve replacement outcomes.

BACKGROUND: Despite device improvements for transcatheter aortic valve replacement (TAVR) and increased operator expertise, unfavorable aortic root anatomy might subtend worse procedural and clinical outcomes. The aim of the study is to assess procedural and clinical outcomes of transfemoral TAVR in patients with and without unfavorable aortic root anatomy and receiving Evolut PRO/PRO+ or SAPIEN 3 Ultra devices in contemporary clinical practice. METHODS: Patients enrolled in the multicenter OPERA-TAVI registry were considered. Patients were compared using propensity score matching according t

Performance of the DERIVATE Risk Score 2.0 for predicting major arrhythmic events in non-ischemic cardiomyopathy: A Portuguese validation study.

INTRODUCTION AND OBJECTIVES: Sudden cardiac death (SCD) risk stratification in non-ischemic cardiomyopathy (NICM) remains challenging. The recently proposed Derivate Risk Score 2.0 (DRS 2.0) seeks to enhance risk prediction using three variables: sex, cardiac magnetic resonance-derived left ventricular ejection fraction (CMR-LVEF), and the presence of midwall late gadolinium enhancement (LGE). The aim of this study was to assess the accuracy of the DRS 2.0 for predicting Major arrhythmic events (MAACE) in a Portuguese NICM cohort. METHODS: Patients with the clinical diagnosis of NICM who unde

What most captures the physician's interest when evaluating a multiparameter monitor in a Neonatal ICU? - A simulation study.

OBJECTIVE: To assess the gaze of physicians in the Neonatal ICU setting when evaluating a multiparameter monitor to determine the presence/absence of vital sign alterations. METHODS: Pediatricians' gaze was tracked when they evaluated a 30-second video of a multiparametric monitor on a screen. They assessed one of three scenarios: Normal, Tachycardia, or Hyperoxia. Eight areas of interest (AOI) were defined: Heart rate(HR), Electrocardiogram(ECG), SpO₂, Pulse wave(PulseW), Respiratory rate(RR), Respiration curve(Resp.C), Blood pressure(BP), and Temperature(Temp.). Gaze fixation on each AOI wa

Residual tiny pulmonary vein potentials on high-resolution mapping after pulsed field ablation predict atrial arrhythmia recurrence and poor durability of pulmonary vein isolation.

BACKGROUND: Pulsed field ablation (PFA) enables safe and efficient achievement of acute pulmonary vein (PV) isolation, with apparent elimination of PV potentials (PVPs) as assessed by the relatively coarse electrogram evaluation using single-shot PFA ablation catheters. Nevertheless, late-phase left atrium (LA)-PV reconnection remains a clinically relevant issue. OBJECTIVE: This study aimed to elucidate the potential role of residual tiny PVPs observed on high-resolution mapping after the elimination of overt PVPs by single-shot PFA for the prediction of atrial arrhythmia recurrence and late-

Contrast-free postoperative imaging of the pulmonary arteries: Intraindividual comparison of relaxation-enhanced angiography without contrast and triggering and time-resolved contrast-enhanced magnetic resonance angiography.

BACKGROUND: Flow-independent, three-dimensional isotropic non-contrast magnetic resonance angiography (MRA) such as relaxation enhanced angiography without contrast and triggering (REACT) are increasingly established as alternatives to contrast-enhanced MRA (CE-MRA) for assessment of the pulmonary arteries (PAs). However, their performance for dedicated postoperative assessment of the PAs in patients with congenital heart disease (CHD), particularly in the presence of different implant types, remains unclear. METHODS: In this retrospective single-center study, 53 patients with CHD underwent c

An unusual case of Streptococcus pyogenes infective endocarditis demonstrating the usefulness of repeated echocardiography.

Infective endocarditis (IE) is most often caused by alpha-hemolytic streptococci or Staphylococcus aureus and is characterized by the presence of vegetations on the heart valves. Here we present a case of IE caused by Streptococcus pyogenes, a distinctly uncommon IE-pathogen, where no vegetations could be visualized on repeated echocardiography. Diagnosis was instead evident from septic embolizations and progressive aortic insufficiency and IE was verified upon heart valve surgery. The patient was a 69-year-old man who presented with a two-day-history of fever and confusion. Several lesions on

Pictorial Essay of [18F]FDG PET/CT in the Evaluation of Suspected Infection of Cardiac Devices and Infective Endocarditis: Patient Preparation, Pathophysiologic Findings, and Interpretation.

In addition to the standard viability assessment, cardiac [18F]FDG PET/CT is used in cases of infection, inflammation, innervation, and infiltrative pathologies. Unlike for myocardial viability assessment, these conditions require a specific patient preparation protocol aimed at suppressing physiologic myocardial glucose uptake. Despite adequate patient preparation, various patterns of [18F]FDG uptake are noted in the myocardium, which correlate with the cardiac physiology as reflected by the clinical, echocardiographic, and anatomic findings. Surgical manipulation with implantation of various

Subclinical Venous Injury in Transvenous Lead Extraction: Histological Correlation With Clinical Risk Factors.

BACKGROUND: Transvenous lead extraction (TLE) is a commonly performed and generally safe procedure, though procedural complexity and risk can vary based on clinical and procedural factors. OBJECTIVE: This study aims to identify clinical and procedural factors associated with histologic evidence of subclinical vascular injury, defined by elastin positivity on extracted leads and its clinical significance. METHODS: Patients undergoing TLE with a lead dwell time of ≥ 1 year were included. Extracted leads with tissue were analyzed histologically using Verhoeff-Van Gieson staining to assess the p

Failure of Tricuspid Bioprosthetic Valve After Concomitant Use With a Continuous Flow Temporary Right Heart Pump.

Managing right ventricular heart failure (RVHF) is a challenging issue, requiring balancing volume status, increasing right ventricular (RV) contractility, and reducing RV afterload. When RVHF is refractory to medical management, surgical intervention is required. Current studies have documented the success of Impella RP for RVHF. However, there are no reports regarding the concomitant implantation of an Impella RP and a bioprosthetic tricuspid valve. A 79-year-old man presented with a pseudoaneurysm of the left atrioventricular junction. On physical exam he had 4+ diffuse pitting edema and as

Incremental value of implantable loop recorders in arrhythmia detection and management in cardiomyopathies: Prospective study.

INTRODUCTION AND OBJECTIVES: Implantable loop recorders (ILRs) are increasingly being used for the detection of infrequent arrhythmias in patients with cardiomyopathies, especially in the presence of risk markers. The role of these devices in improving the detection of significant arrhythmias requiring a change in clinical management remains to be determined. Our purpose was to evaluate the diagnostic yield, types of arrhythmia, and subsequent management in cardiomyopathy patients receiving an ILR. METHODS: Prospective single-centre study in cardiomyopathy patients considered at borderline ri

Percutaneous Device Closure of Perimembranous Ventricular Septal Defect (VSD) in Congenitally Corrected Transposition of Great Arteries (cc-TGA): A Rare Case Report.

Congenitally corrected transposition of great arteries (cc-TGA) is often accompanied by associated lesions with a wide spectrum of presentations. Traditionally, ventricular septal defects (VSDs) in such patients have been managed surgically with either physiologic repair or, more recently, there is a paradigm shift towards anatomical repair, such as the double switch operation. Percutaneous VSD device closure in cc-TGA physiology is extremely rare, with only isolated case reports in the literature. The present case report highlights a 15-year-old female, previously diagnosed with cc-TGA with m

Coronary Computed Tomography Angiography Identifying a Single Coronary Artery With a Critical Left-Main-Equivalent Stenosis Treated by Elective PCI.

Single coronary artery (SCA) is a rare congenital anomaly in which the entire myocardium is supplied by a single coronary ostium, and clinical significance depends largely on the anatomical course and presence of concomitant atherosclerotic disease. We report the case of a 58-year-old man presenting with exertional chest pain whose coronary computed tomography angiography revealed an SCA arising from the right coronary sinus with a critical 98% stenosis in the left main equivalent trunk supplying both the left anterior descending and circumflex territories. Although coronary artery bypass graf

Association of decompensated cirrhosis with acute clinical and financial outcomes following cardiac operations.

BACKGROUND: Patients with cirrhosis undergoing cardiac surgery comprise a high-risk population. Despite advances in surgical care, traditional risk models often underestimate risk in this cohort. We used a nationally representative sample to assess the association of cirrhosis with in-hospital mortality, postoperative complications, and resource utilization. METHODS: All elective adult (≥18 years) hospitalizations for coronary, valvular, and proximal aortic operations were tabulated from the 2016 to 2022 Nationwide Readmissions Database. Multivariable regression models were developed to asses

Patent ductus arteriosus in Qatar: a retrospective study on epidemiology and neonatal outcomes following transcatheter closure.

INTRODUCTION: Patent ductus arteriosus (PDA) is a congenital heart defect resulting from failure of the ductus arteriosus to close after birth. It is particularly common in preterm infants and may become hemodynamically significant, requiring intervention. Transcatheter device closure offers a minimally invasive treatment option, though it can be technically challenging in this population. METHODS: This retrospective study included 80 infants diagnosed with PDA at Sidra Medicine, Qatar, between 2018 and 2024. Data were obtained from electronic health records. Patients who underwent medical tr

Anaesthetic management of a child with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA) and supravalvular aortic stenosis (SVAS) associated with Williams-Beuren syndrome undergoing combined coronary reimplantation and supravalvular aortoplasty.

A child with genetically confirmed Williams-Beuren syndrome, having moderate supravalvular aortic stenosis, was diagnosed with anomalous origin of the left coronary artery from the pulmonary artery (ALCAPA), a rarely described combination. This coexistence created competing physiological stress on coronary perfusion as myocardial oxygen delivery to the left ventricle depended on right coronary collateral circulation in the presence of fixed left ventricular outflow obstruction. The child underwent combined surgical repair with reimplantation of the left coronary artery into the aorta and supra
