Spectral Characteristics and Parameters of Pulse Oximetry Waveforms for Assessing Resuscitation Status.

BACKGROUND: Real-time assessment of physiological resuscitation status is critical for guiding personalised cardiopulmonary resuscitation (CPR) and avoiding unnecessary harm from prolonged chest compressions. Non-invasive, objectively trackable tools for this purpose remain limited. METHODS: This multicentre retrospective study (2013-2018) included 451 adult patients with cardiac arrest (497 CPR episodes) from 14 Chinese tertiary hospitals, comprising 158 Out-of-Hospital Cardiac Arrest (OHCA) episodes with prehospital SpO2/ECG monitoring continued upon ED arrival and full pulse oximetry pleth

Developmental toxicities of albendazole and its metabolites in zebrafish (Danio rerio) embryos.

Albendazole (ABZ) is a benzimidazole anthelmintic medicine widely used in livestock and, especially, aquaculture industries. Given its widespread use, ABZ commonly enters waterways, negatively impacting aquatic organisms. Classified as Pregnancy Category C because of its teratogenic properties, ABZ poses a high risk of developmental toxicity. In this study, zebrafish (Danio rerio) embryos were used to assess the developmental toxicity of ABZ and its two metabolites, ABZ-sulfoxide and ABZ-sulfone. Among these, only ABZ exhibited strong lethality, demonstrating an LC50 value of 83.3 μg/L, and in

Long-Term Durability of the Sorin Soprano Aortic Bioprosthetic Valve-A Swedish Nationwide Study.

OBJECTIVES: Structural valve deterioration is an inherent limitation of bioprosthetic valves; however, the time it takes to develop varies depending on the valve model. The Sorin Soprano aortic bioprosthetic valve (Sorin Group/LivaNova, Milan, Italy) lacks reported long-term results. This study aims to establish the long-term durability of this bioprosthetic valve in a nationwide cohort. METHODS: All patients who had received the Soprano bioprosthesis during aortic valve replacement in Sweden between 2005 and 2013 were included in the study. Patient characteristics, outcomes, and echocardiogr

In silico modelling of aortic annuloplasty: hemodynamic assessment through in vitro experiments and in vivo MRI.

Aortic annuloplasty (AA) is an innovative surgical technique for aortic root (AR) enlargement. It is performed by implanting sutures, bands, or rings, either externally or internally the AR, hereby reducing its diameter. This study evaluates the impact of AA approaches on AR hemodynamic by employing a porcine-specific workflow combining in vivo magnetic resonance imaging (MRI), in vitro experiments and in silico fluid-structure interaction (FSI) simulations investigating external single ring AA. CAD models of native and post-annuloplasty ARs were segmented from in vivo porcine MRI data and ser

Triple cardiovascular disease detection with an artificial intelligence-enabled stethoscope (TRICORDER) in the UK: a cluster-randomised controlled implementation trial.

BACKGROUND: Early detection of cardiovascular disease is a global public health priority. Artificial intelligence (AI)-enabled stethoscopes offer robust performance characteristics in point-of-care detection of heart failure, atrial fibrillation, and valvular heart disease (VHD). We conducted a pragmatic, cluster-randomised controlled implementation trial to determine the real-world effect and implementation challenges of AI-stethoscopes. METHODS: UK primary care practices were cluster randomised 1:1 to intervention (training and implementation in use of AI-stethoscopes in routine care) or co

First-in-human pilot study of broadband optical spectroscopy (BOS) as noninvasive surveillance for necrotizing enterocolitis (NEC).

BACKGROUND: Necrotizing enterocolitis (NEC) lacks predictive biomarkers and objective early diagnostic markers. Broadband optical spectroscopy (BOS), a transcutaneous noninvasive tool, has previously demonstrated diagnostic specificity and early predictive power for NEC in a mouse model. Here, in continuation of the translational development, we report a first-in-human observational study in premature infants. METHODS: An apparatus for BOS in infants was assembled comprising a handheld probe with broad-spectrum light source coupled to a laboratory-grade spectrometer (ASD LabSpec 4, Malvern Pa

Long Term Target Vessel Outcomes in Patients Treated with Fenestrated Endovascular Aortic Repair.

OBJECTIVE: Fenestrated endovascular aortic repair (FEVAR) has been proposed as an alternative to open surgical repair (OSR) in complex aortic cases. Target vessel outcomes have shown good results, with re-interventions constituting an important FEVAR pitfall. This single centre, retrospective, observational cohort study presented the target vessel outcomes in patients treated with FEVAR in a high volume aortic centre. METHODS: Consecutive patients managed with custom made FEVAR from 2010 to 2024 were included. Vessels targeted through scallops or intentionally unstented fenestrations were exc

Sex Differences in Early Complications After Cardiac Implantable Electronic Device Implantation: Myth or Reality?

BACKGROUND: Early complications after cardiac implantable electronic device (CIED) implantation remain clinically relevant and have been widely examined in large registry studies. Advanced age, comorbidities, device complexity, and antithrombotic therapy are recognized predictors of early complications. This study planned to examine the effect of sex on early (≤ 30 days) device-related complications and to determine independent predictors of early major adverse cardiac events (MACE). METHODS: This retrospective, single-center study enrolled 1807 sequential patients (729 women, 1078 men) under

Valve Pop-Up and Infolding in Transcatheter Aortic Valve Replacement for a Heavily Calcified Bicuspid Valve.

BACKGROUND: Transcatheter aortic valve replacement (TAVR) in bicuspid aortic valve (BAV) anatomy remains challenging, especially in the presence of heavy calcification. CASE SUMMARY: A patient with severe aortic stenosis and a heavily calcified type 1 BAV with a calcified raphe underwent transfemoral TAVR with a 29-mm self-expanding Evolut FX + valve. At final release, delayed prosthesis pop-up toward the ascending aorta occurred. Recapture was unexpectedly successful, but redeployment from a deeper position led to frame infolding. The prosthesis was retrieved, and definitive bailout was achi

Differentiating LAA Thrombus From Spontaneous Echo Contrast for Safe LAA Occlusion.

Left atrial appendage (LAA) occlusion is a valuable therapeutic option for stroke prevention in patients with atrial fibrillation who have contraindication to long-term anticoagulation. However, the presence of LAA thrombus is a contraindication to device implantation. Differentiating a true thrombus from spontaneous echo contrast or "sludge" on transesophageal echocardiography is both challenging and crucial for procedural safety. We present a series of cases using adjunctive modalities that allowed safe LAA occluder deployment.

Decellularized extracellular matrix enhances hydrogel printability for bioprinting functional muscle constructs in a volumetric muscle loss model.

Volumetric muscle loss (VML) remains a major clinical challenge due to the limited regenerative capacity of skeletal muscle. Effective repair requires the use of biomaterials that support cell viability, promote myogenic differentiation and enable vascularisation to allow the formation of structurally aligned, functional muscle. Here, we have integrated a biomimetic bioink based on gelatin methacryloyl (GelMA) and methacryloyl-modified decellularized extracellular matrix (dECM-MA) with a micropost tension-assisted bioprinting strategy to engineer skeletal muscle constructs for VML repair. We s

Cardiac implantable structural devices (CISDs) and cardiac implantable electronic devices (CIEDs) on chest radiography: a pictorial review.

Implantable cardiac devices are increasingly used to manage cardiovascular conditions. Consequently, radiologists are more frequently encountering the ever-expanding range of both cardiac implantable electronic devices (CIEDs) and cardiac implantable structural devices (CISDs) on chest radiography. Chest radiography remains a widely available, low-cost modality for confirming device presence and detecting complications such as gross malposition or migration. This pictorial review demonstrates the characteristic appearances of established and emerging CIEDs and CISDs on chest radiography, rangi

Recommendations for sharing cardiovascular implantable electronic device data with patients based on clinician interviews.

BACKGROUND: Cardiovascular implantable electronic devices (CIEDs) capture an abundance of data that requires specific medical knowledge to interpret. Currently, there are no clear recommendations for sharing clinically meaningful CIED data with patients from the abundance of data in CIED remote monitoring reports. OBJECTIVE: The objectives of this study were to explore clinicians' perceptions of clinical care gaps related to CIED data that could be improved through patient access to data and to generate recommendations for sharing CIED data with patients. METHODS: In this institutional revie
