Multiparameter assessment of vasodilatory endothelial dysfunction in preclinical and severe cardiovascular risk stages.

BACKGROUND: Vasodilatory endothelial dysfunction (VED) is identified as a precursor to cardiovascular diseases (CVDs) and atherosclerosis. Therefore, early detection of VED is important to prevent the development of severe CVDs and prescribe lifestyle changes or make clinical interventions. Though Acetylcholine-induced coronary vasodilation test is the invasive gold standard to detect VED, specialized devices have provided alternative means of detecting VED non-invasively. With these, they have already detected presence of VED in patients who are already diagnosed with CVD or Diabetes Mellitus

Facial petechial hemorrhages after cardiopulmonary resuscitation in adults: A pilot study.

INTRODUCTION: Facial petechial hemorrhages (FPH) are small dermal or mucosal hemorrhages traditionally associated with impaired venous return, including neck or thorax compression in forensic contexts. Cardiopulmonary resuscitation (CPR) generates major intrathoracic pressure fluctuations and mechanical forces that could theoretically contribute to FPH formation, but available evidence is limited and confounded. This pilot study aimed to determine the prevalence and duration of FPH after CPR in adults and to explore patient and treatment characteristics associated with their occurrence. METHO

The Alpha-Gal Syndrome and Hypersensitivity to Biomaterials: Understanding Xenoimmunity.

Biomaterials derived from nonhuman mammals are increasingly used in cardiovascular interventions, yet xenoimmune responses to these materials can complicate these procedures. Xenoimmunity describes the immune response mounted against foreign cells, tissues, or organs from different species. Alpha-gal syndrome, a tick-acquired IgE-mediated allergy to galactose-alpha-1,3-galactose, exemplifies this challenge. Patients face risk when exposed to porcine heparin, gelatin-based hemostatic agents, or bioprosthetic heart valves, with reactions ranging from delayed urticaria to immediate anaphylaxis. A

Genetic Determinants of Early Heart Failure in Hypoplastic Left Heart Syndrome: A Prospective NC-DEFINE Study.

BACKGROUND: Survivors of hypoplastic left heart syndrome (HLHS), the most severe form of congenital heart disease, are at high risk for heart failure (HF). HF in early life is a major contributor to mortality in this vulnerable population. However, reliable approaches to identify infants at highest risk for early HF are currently lacking. OBJECTIVES: The purpose of this study was to evaluate whether ultra-rare variants in cardiomyopathy-associated genes are associated with HF risk in HLHS. METHODS: Neonates with HLHS were prospectively enrolled within the first 21 days of life at Duke Univer

Feasibility of 35-mm Balloon-Expandable Transcatheter Pulmonary Valve Implantation in Extra-Large Native Right Ventricular Outflow Tracts.

BACKGROUND: Transcatheter pulmonary valve implantation is established for right ventricular outflow tract dysfunction but remains limited in extra-large native outflow tracts because of device size constraints. CASE SUMMARY: This case series describes implantation of a custom 35-mm balloon-expandable Myval valve in 4 patients with severe pulmonary regurgitation and right ventricular dilation who were unsuitable for conventional transcatheter therapy. Multimodality imaging and balloon interrogation consistently identified a functional landing zone of approximately 32 mm, guiding valve selectio

Impaired hemostasis in mechanical circulatory support systems: Monitoring with T-TAS® 01, in vitro correction with VWF concentrates, and impact of membrane oxygenators.

BACKGROUND: Mechanical circulatory support (MCS) systems assist patients with severe cardiac or respiratory failure, but bleeding linked to acquired von Willebrand disease remains a major complication. The T-TAS® 01 system has emerged as a rapid tool for assessing MCS patients' hemostasis. OBJECTIVES: To evaluate hemostatic changes during and after MCS support. Secondary objectives included assessing the impact of the in vitro addition of von Willebrand factor concentrates and exploring differences according to the presence of a membrane oxygenator (MO). METHODS: This prospective, bicentric

A Pascal in ring: Transcatheter edge-to-edge tricuspid repair for severe regurgitation within a surgical annuloplasty ring.

BACKGROUND: Severe tricuspid regurgitation (TR) carries a poor prognosis and its management is particularly challenging in patients with recurrent TR after prior surgical annuloplasty. For those patients redo surgery is often high-risk and transcatheter valve replacement may be anatomically precluded by the presence of a rigid or semi-rigid ring. CASE SUMMARY: We report the case of a 69-year-old woman with a history of rheumatic heart disease, multiple previous cardiac surgeries who presented with heart failure related to severe, symptomatic tricuspid regurgitation caused by leaflet restricti

Impact of End-Stage Renal Disease on Outcomes in Mechanical Versus Bioprosthetic Valve Replacement.

BACKGROUND: Choosing between mechanical and bioprosthetic valves for aortic valve replacement (AVR) or mitral valve replacement (MVR) remains challenging, particularly for patients with end-stage renal disease (ESRD). Patients with ESRD have an increased risk of bleeding due to simultaneous anticoagulation therapy and accelerated valve calcification, which can complicate valve selection. METHODS: In this nationwide retrospective cohort study, we used data from Taiwan's National Health Insurance Research Database. We included patients who underwent AVR or MVR from 2001 to 2020. Propensity scor

Atrial Fibrillation and Tricuspid Regurgitation in Patients Undergoing Mitral Valve Transcatheter Edge-to-Edge Repair.

BACKGROUND: The interplay between atrial fibrillation (AF) and tricuspid regurgitation (TR) has been recognized. However, it is not simple and has not been fully investigated. OBJECTIVES: The aim of this study was to examine the association among AF, TR, and outcomes in patients undergoing mitral valve transcatheter edge-to-edge repair (M-TEER). METHODS: Patients were divided into 4 groups according to the presence of AF and postprocedural significant TR (moderate or greater): no AF and no TR, no AF with significant TR, AF and no TR, and AF with significant TR. The primary outcome was cardio

[Guidelines for the diagnosis and treatment of obstructive sleep apnea in adults (2025)].

Obstructive sleep apnea (OSA) is a highly prevalent sleep-related breathing disorder with a considerable healthcare burden. Previously, the 2011 guideline for adult obstructive sleep apnea was largely formulated on the basis of expert opinion and consensus, which played an important role in guiding clinical practice. With the growing body of evidence from high-quality clinical studies, however, guideline development models that rely primarily on expert consensus are no longer adequate to support contemporary clinical decision-making. To further standardize the screening, diagnosis, treatment,

The ELASTA-T Technique for TTVR After Failed T-TEER: An Operator-Oriented Guide to Planning, Execution, Pitfalls.

Failure of tricuspid transcatheter edge-to-edge repair (T-TEER) with persistent severe tricuspid regurgitation (TR) is an increasingly encountered clinical scenario in contemporary structural heart disease treatment. In this setting, transcatheter tricuspid valve replacement (TTVR) is emerging as an effective therapeutic option for recurrent TR after T-TEER. However, the presence of pre-existing tricuspid edge-to-edge devices significantly alters valve geometry, creating a fixed multiple-orifice configuration that may impair coaxial device crossing, generate asymmetrical expansion forces, and

Natural History of Asymptomatic Phenotypically Mild HCM: Insights From the SHaRe Registry.

BACKGROUND: Patients with phenotypically mild hypertrophic cardiomyopathy (HCM) do not require symptom management, but may be at an earlier stage in the disease course, with potential to benefit from disease-modifying therapies. However, little is known about the natural history and predictors of major adverse cardiovascular events (MACE). OBJECTIVES: Using the Sarcomeric Human Cardiomyopathy Registry, we identified predictors of incident MACE and characterized disease progression in phenotypically mild HCM. METHODS: Phenotypically mild HCM was defined as: having shorter disease duration (<1

From Rescue to Interception: The PREVENT 2-LATE Framework for Advanced Heart Failure.

The management of advanced heart failure has been transformed by two parallel developments that have not yet been fully translated into clinical practice. Heart transplantation and durable left ventricular assist device (LVAD) support confer contemporary 1-year survival approaching 90%; yet these therapies are frequently deployed only after the onset of progressive right ventricular dysfunction, cardiorenal injury, severe frailty, and hemodynamic instability which are conditions that elevate procedural risk, impair long-term outcomes, and may ultimately preclude candidacy for advanced therapie

Computational modeling of thrombosis after transcatheter aortic valve implantation: A systematic review of hemodynamic and mechanical risk metrics.

BACKGROUND AND OBJECTIVE: Although transcatheter aortic valve implantation (TAVI) generally provides favorable procedural outcomes, thrombotic complications remain an important concern for the long-term performance and durability of transcatheter heart valves. A subset of patients develops hypo-attenuated leaflet thickening (HALT), reduced leaflet mobility, or late thrombotic complications, often in the absence of overt clinical manifestations. This has motivated the use of computational modeling to investigate post-TAVI hemodynamics and valve mechanics and to identify flow- and device-related
