PURPOSE: The left internal mammary artery (IMA) is the gold standard in coronary artery bypass grafting (CABG) to the left anterior descending (LAD) coronary artery. The presence of the lateral costal artery (LCA) has been identified as the culprit for post-operative steal phenomenon in some patients. Direct ligation of LCA in minimally invasive CABG (mini-CABG) may be challenging due to limited surgical access. This study aims to evaluate the presence, length, and size of the LCA and to determine whether it plays a role in LAD territory ischemia after mini-CABG.

BACKGROUND In coronary artery bypass grafting, hyperthyroidism with autoantibodies, as seen in Basedow disease, is uncommon and rarely leads to severe thyrotoxicosis. However, surgery can trigger this condition, and documented cases have resulted in poor outcomes. We describe the case of an incidental hyperthyroidism finding on preoperative evaluation for coronary artery bypass grafting for triple-vessel coronary artery disease. CASE REPORT A 58-year-old man presented with exertional dyspnea, chest pain, and postprandial vomiting. Coronary angiography revealed severe triple-vessel disease. Aft

Coarctation of the aorta (CoA) persisting into adulthood is uncommon and may be complicated by hypertension and aortic pathology. Aortic dissection occurring distal to a coarctation segment in the presence of advanced coronary artery disease (CAD) is not uncommon, but its description in the literature - especially the management - is rarely reported. A 46-year-old male with no prior symptoms or history of comorbid illness presented with acute, tearing chest and back pain. Echocardiography revealed a CoA. Further evaluation with computed tomography angiography demonstrated a Stanford type B aor

BACKGROUND AND AIMS: Transthyretin amyloid cardiomyopathy (ATTR-CM) is an increasingly recognised albeit rare and frequently delayed diagnosis in patients with a variety of underlying cardiac presentations. This cross-sectional observation study aimed to determine the prevalence of cardiac amyloidosis in an undifferentiated adult population with coronary artery disease undergoing coronary artery bypass graft surgery (CABG) using direct histological analysis of cardiac tissue.
METHODS: Consecutive eligible patients, with no known history of ATTR-CM or monoclonal gammopathy, aged 60 years and o

BACKGROUND: Peripheral arterial disease (PAD) is a common comorbidity in patients undergoing coronary artery bypass grafting (CABG) and is associated with increased morbidity. Early identification of PAD in this population is clinically important; however, the relationship between coronary disease complexity and peripheral arterial involvement remains unclear. This study aimed to evaluate the association of the SYNTAX score, ankle-brachial index (ABI), and toe-brachial index (TBI) with the development of peripheral arterial disease following CABG.
METHODS: In this retrospective study, 87 patie

BACKGROUND Chylothorax is one of the rarest postoperative complications observed in patients undergoing coronary artery bypass grafting (CABG). Most reported cases of post-CABG chylothorax are associated with left internal mammary artery (LIMA) harvesting, which can injure the thoracic duct or its branches because of their close anatomical proximity. In contrast, the occurrence of chylothorax without LIMA grafting is extremely rare and can lead to diagnostic uncertainty. CASE REPORT We report the case of a 73-year-old man with triple vessel disease who underwent elective CABG via median sterno

BACKGROUND: Elevated homocysteine (Hcy) has been linked to endothelial dysfunction and adverse cardiovascular outcomes. However, its specific role as a risk factor for postoperative complications, especially acute kidney injury (AKI), in patients undergoing coronary artery bypass grafting (CABG), remains insufficiently defined. What's more we took into consideration presence of diabetes mellitus as an independent risk factor of AKI or the exacerbation of pre-existing chronic kidney disease (CKD), what can modify the prevalence of this complication.
METHODS: We conducted a prospective analysis

OBJECTIVE: Compared with the traditional open approach, minimally invasive vascular surgery techniques may offer benefits to patients undergoing lower-extremity arterial reconstruction. We sought to determine whether endoscopically-assisted in-situ lower-extremity bypass for chronic limb-threatening ischemia is as safe and effective as traditional approaches.
METHODS: From 2021 to 2024, we performed a retrospective cohort study of endoscopically-assisted femoropopliteal or femorotibial bypass on 10 patients in an academic hospital in Ontario, Canada. Patients were selected based on the presen

BACKGROUND Lipomatous atrial septal hypertrophy (LASH) is a benign but uncommon condition characterized by excessive adipose tissue accumulation within the interatrial septum. Although most cases remain asymptomatic, large lesions may produce superior vena cava (SVC) obstruction. Acute pulmonary embolism as an initial presentation is rare. CASE REPORT A 75-year-old woman with a history of chronic obstructive pulmonary disease and hypertension experienced 7 days of progressively worsening exertional dyspnea, orthopnea, and lower-extremity edema, without chest pain, cough, or fever. BACKGROUND: A prolonged QTc interval has been reported in patients with liver disease. The objectives of our study were to determine whether a prolonged QTc interval was an independent predictor of mortality in patients with cirrhosis and to examine the effect of liver transplantation (LT) on QTc interval.
PATIENTS AND METHODS: We retrospectively studied two cohorts of patients - QTc interval was measured in 409 patients (pre-transplant group), and in 162 patients (transplant group) before and 6 months after LT. QT interval (mean) corrected (QTc) for ventricular rate was read from a 12-lead E

BACKGROUND: Secondary tricuspid regurgitation (TR) often develops or persists following left-sided valve surgery. However, its impact on outcomes and the optimal timing of intervention is unclear. This study examined the association between isolated postoperative secondary TR and outcomes in patients with a history of mitral or aortic valve surgery.
METHODS: This observational single-centre cohort study included patients with left-sided valve surgery and less than moderate preoperative TR who underwent follow-up echocardiography between 2002 and 2024. The presence of isolated postoperative se

BACKGROUND: The growing adoption of transcatheter aortic valve replacement (TAVR) in younger and lower-risk patients has increased the number of patients with other cardiac diseases requiring surgical intervention. Such surgeries pose unique technical challenges due to the presence of the TAVR, often requiring explantation. Despite increasing clinical relevance, outcomes of these operations remain poorly characterized. We sought to assess the incidence, indications, and short-term results of cardiac surgery following TAVR at a high-volume institution.
METHODS: This was a retrospective single-

Electron-mediated transport is fundamental to the thermal and dynamical evolution of plasmas across a vast range of physical environments, from the solar wind and solar atmosphere to astrophysical systems such as galaxy clusters and accretion flows, to laboratory and fusion plasmas. This article reviews the current state of understanding of electron transport, drawing together observations, simulations, and theory to contextualise key challenges and open questions. We characterise plasma environments using the Knudsen number and electron beta, and highlight that some systems fall in an interme

Ischaemic left ventricular dysfunction (ILVD) is the leading cause of heart failure and is commonly encountered in cardiac catheterization laboratories. Patients presenting with ILVD are typically older and often have complex coronary anatomy, making revascularization challenging. Although coronary artery bypass grafting improves long-term survival in patients with ILVD compared with medical therapy, the benefit emerges only after a decade and appears limited to younger patients, leaving many older, frail, or comorbid individuals at untenable risk. Such patients are frequently considered for c

INTRODUCTION: Postoperative atrial fibrillation (POAF) and pericardial effusion remain common complications after coronary artery bypass grafting (CABG).
METHODS: Posterior pericardiotomy has been proposed to reduce these events, but long-term data remain limited. We evaluated the impact of triangular posterior pericardiectomy (TPP) on early and 12-month outcomes after CABG using unsupervised taxonomic analysis of postoperative responses. This retrospective study included 489 patients undergoing CABG with cardiopulmonary bypass between 2017 and 2023. Patients underwent standard surgery (no pe

PURPOSE: Increased reactive oxygen species (ROS) released by NADPH oxidase and inflammation are associated with arterial hypertension and eye diseases associated with high blood pressure, including glaucoma, retinopathies (e.g., age-related macular degeneration), and choroidopathies affecting ocular function; however, the mechanisms underlying these adverse outcomes remain undefined. The present study was designed to highlight the importance of oxidative stress in severe hypertension-related eye damage.
METHODS: Male Wistar rats (n = 7, unless otherwise specified for specific experiments) wer

BACKGROUND: Mirtazapine has a unique mechanism of antidepressant action, and thus is thought to have a different profile of adverse events from that of other antidepressants.
OBJECTIVE: To present a methodologically rigorous systematic review of the adverse event profile of mirtazapine and point to possible problems with safety reporting in randomized controlled trials (RCTs) of the acute-phase treatment of major depression in adults with mirtazapine in comparison with other types of antidepressant.
METHODS: The Cochrane Collaboration Depression, Anxiety and Neurosis Controlled Trials Regist

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

We present the case of a 62-year-old man with severe coronary artery disease who presented to the hospital in refractory ventricular fibrillation cardiac arrest. He showed signs of life despite prolonged resuscitation. We thus decided to initiate extracorporeal cardiopulmonary resuscitation (ECPR). The patient had a known total occlusion of his infrarenal aorta that had been surgically bypassed with a bifemoral-axillary graft. We successfully initiated ECPR via the surgical graft, establishing blood flow to the central circulation through the axillary artery in a peripheral configuration while
