BACKGROUND: Tricuspid regurgitation in elderly patients with multiple comorbidities presents unique challenges, especially after prior valve interventions and when advanced heart failure symptoms persist. New device-based therapies such as bicaval valve stents (TricValve®) can offer alternatives for patients not suitable for surgery but may complicate pacemaker implantation.
CASE SUMMARY: We report an 85-year-old woman with a history of coronary artery disease, prior valve-in-valve transcatheter aortic valve implantation (TAVI) for a degenerated aortic prosthesis, and permanent atrial fibrill

Failure of tricuspid transcatheter edge-to-edge repair (T-TEER) with persistent severe tricuspid regurgitation (TR) is an increasingly encountered clinical scenario in contemporary structural heart disease treatment. In this setting, transcatheter tricuspid valve replacement (TTVR) is emerging as an effective therapeutic option for recurrent TR after T-TEER. However, the presence of pre-existing tricuspid edge-to-edge devices significantly alters valve geometry, creating a fixed multiple-orifice configuration that may impair coaxial device crossing, generate asymmetrical expansion forces, and

BACKGROUND: Secondary tricuspid regurgitation (TR) often develops or persists following left-sided valve surgery. However, its impact on outcomes and the optimal timing of intervention is unclear. This study examined the association between isolated postoperative secondary TR and outcomes in patients with a history of mitral or aortic valve surgery.
METHODS: This observational single-centre cohort study included patients with left-sided valve surgery and less than moderate preoperative TR who underwent follow-up echocardiography between 2002 and 2024. The presence of isolated postoperative se

BACKGROUND: Rheumatic heart disease predominantly affects left-sided cardiac valves. Quadrivalvular involvement with clinically significant pulmonary valve stenosis is exceedingly rare, and evidence regarding minimally invasive management strategies in such complex cases is limited.
CASE SUMMARY: A 30 year-old woman with chronic rheumatic heart disease and prior mitral and aortic bioprosthetic valve replacement with tricuspid valve repair presented with a four-month history of progressive exertional dyspnea and palpitations (New York Heart Association functional class lll). BACKGROUND AND OBJECTIVE: Although transcatheter aortic valve implantation (TAVI) generally provides favorable procedural outcomes, thrombotic complications remain an important concern for the long-term performance and durability of transcatheter heart valves. A subset of patients develops hypo-attenuated leaflet thickening (HALT), reduced leaflet mobility, or late thrombotic complications, often in the absence of overt clinical manifestations. This has motivated the use of computational modeling to investigate post-TAVI hemodynamics and valve mechanics and to identify flow- and device-related

BACKGROUND: Prosthetic valve endocarditis (PVE) after transcatheter aortic valve replacement (TAVR) is an uncommon but life-threatening complication associated with high embolic risk.
CASE PRESENTATION: A 82-year-old woman with prior TAVR presented with fever and malaise and was diagnosed with PVE due to Streptococcus Mutans. Transesophageal echocardiography revealed a large, highly mobile vegetation attached to the transcatheter valve, floating in the systolic jet of blood flow. Computed tomography demonstrated progressive splenic infarction and embolic involvement of the kidneys and mesente

The relationship between host microbiota and infective endocarditis (IE) has been investigated by few studies. We compared the oral, gut, and skin microbiota profiles of patients awaiting heart valve replacement, both with and without IE, and correlated the profiles with the patients' clinical data, including the etiological agents and antibiotics received. Thirty-six patients were enrolled in a prospective, observational pilot study: 25 with IE (cases) and 11 with non-infective valve disease requiring surgery (controls). Clinical data and oral, fecal, and skin samples were collected before su

After right ventricular outflow tract obstruction reconstruction for CHD, surgical pulmonary valve replacement, or transcatheter pulmonary valve replacement may be performed if complicated by moderately severe or severe right ventricular outflow tract dysfunction. However, for patients whose anatomy is not suitable for transcatheter pulmonary valve replacement, surgical pulmonary valve replacement is the only option, but it has a higher rate of perioperative complications and longer hospitalisation for patients. In two cases of severe transcatheter pulmonary valve replacement in which percutan

BACKGROUND: Increasing demand for transcatheter aortic valve implantation (TAVI) places greater emphasis on the efficiency of pathways and services. A significant limitation to increasing TAVI capacity is the availability of cardiac catheterisation laboratory time. We have developed a novel complexity scoring system (TAVI ComplEXity; TEX score) which can aid in planning lists with appropriate case selection. To validate the TEX score, we have undertaken a retrospective analysis of TAVI cases. The hypothesis is that increasing TEX score correlates with increased procedural duration and reduced

BACKGROUND: Lifetime treatment of aortic valve disease is a matter of increasing debate. Although the risks of a second aortic valve intervention are recognized, little attention has been given to the challenges of a third.
OBJECTIVES: This study delves into the clinical characteristics, indications, and outcomes of patients undergoing 3 aortic valve interventions.
METHODS: The THIRD (THree aortIc Reinterventions for valve Disease) registry is a retrospective multicenter, international study of patients who underwent a third procedure on the aortic valve, either surgically or transcatheter-b

Overwhelming post-splenectomy infection (OPSI) is a severe and potentially life-threatening condition that can occur in patients undergoing splenectomy. We report a case of a patient who had a splenectomy approximately 30 years ago during prosthetic valve insertion for infective endocarditis (IE). The patient later developed prosthetic valve endocarditis caused by Streptococcus pneumoniae associated with OPSI. Although IE due to Streptococcus pneumoniae is rare, the presence of a prosthetic valve may increase the risk of IE in splenectomized patients. This case highlights the need for increase

Bleeding is the most common side effect during treatment with vitamin K antagonists (VKAs). Sometimes, VKA use causes bleeding episodes due to rare variants in the factor IX (FIX) propeptide that modify the affinity of FIX propeptide to the binding of γ-glutamyl carboxylase. We report on a 51-year-old patient who presented with recurrent spontaneous and severe intramuscular and cutaneous bleedings during VKA (warfarin) treatment for the presence of a prosthetic mechanical aortic valve. Laboratory evaluation revealed INR within the therapeutic range with markedly prolonged aPTT and a large redu

Purpose. Transcatheter aortic valve implantation (TAVI) is increasingly performed in patients with longer life expectancies. The need for permanent pacemaker implantation (PPI) following TAVI has been associated with increased all-cause mortality and morbidity. This study aimed to identify ECG, CT, and procedural predictors of PPI following TAVI. Methods. We conducted a retrospective observational study at the University Hospital of North Norway. Patients who underwent TAVI with SAPIEN 3 or SAPIEN 3 Ultra valves were included, while those with prior pacemakers, self-expanding valves, or valve-

Transcatheter aortic valve replacement in the presence of a preexisting bioprosthetic mitral valve can lead to significant interaction of the valves. Simultaneous transcatheter aortic valve and mitral valve deployment using balloon-expandable systems further complicates the interaction. We describe preprocedural computed tomography planning using novel parameters and bench tests to predict these interactions and their clinical significance. On the basis of the available anecdotal evidence, we propose a new classification system for the various types of interactions.

BACKGROUND: Automated data extraction from echocardiography reports could facilitate large-scale registry creation and clinical surveillance of valvular heart diseases (VHD). We evaluated the performance of open-source large language models (LLMs) guided by prompt instructions and chain of thought (CoT) for this task.
METHODS: From consecutive transthoracic echocardiographies performed in our center, we utilized 200 random reports from 2019 for prompt optimization and 1000 from 2023 for evaluation. Five instruction-tuned LLMs (Qwen2.0-72B, Llama3.0-70B, Mixtral8-46.7B, Llama3.0-8B, and Phi3.0

There is limited evidence on the prognosis and long-term valve durability after transcatheter aortic valve replacement (TAVR) in patients with small aortic annulus (SAA) and large aortic annulus (LAA). This analysis was sought to evaluate the impact of annular size differences on patients' and valve outcomes. A total of 1,211 patients undergoing TAVR using self-expandable transcatheter heart valve (SE-THV) were retrospectively analyzed. The cut-off for SAA was defined as annulus perimeter of < 72 mm. The primary endpoints were all-cause mortality and bioprosthetic valve failure (BVF) between t

Right ventricular (RV) function is an important determinant of prognosis in pulmonary hypertension and valvular heart disease. The accurate assessment of RV function is however limited by the presence of concomitant tricuspid regurgitation (TR). Implantation of a novel transcatheter tricuspid valve device in this case highlights the importance of recognizing these limitations, and the need to define alternative ways to measure RV systolic function, contextualized to pulmonary pressure in patients with severe TR. It also demonstrates the clinical potential of perforation of the pop-off zone in

OBJECTIVES: Pulmonary allografts are commonly used for right ventricular outflow tract reconstruction. Cryopreserved allografts may develop degeneration and stenosis over time due to immune-related issues. Fresh decellularized aortic and pulmonary allografts have shown favourable early outcomes. We used a sheep model to evaluate the safety of a novel decellularization protocol combined with cryopreservation applied on pulmonary allografts, aiming to improve availability and reduce cost.
METHODS: Pulmonary valve replacement was performed in juvenile female sheep using cryopreserved pulmonary a

BACKGROUND: Patient-prosthesis mismatch (PPM) after trans-catheter aortic valve implantation (TAVI) is a risk factor for heart failure and mortality. Assessment of PPM using transthoracic echocardiography (TTE) and presence of hypo-attenuated leaflet thickening (HALT) may lead to overestimation. Our study aimed to assess the incidence of PPM using TTE and CTA after exclusion of patients with HALT and to evaluate predictors analyzing stent geometry.
METHODS: 444 patients were analyzed. PPM was calculated using the continuity equation from TTE (TTE-PPM) and CTA (CT-PPM). Regression analyses wer
