PURPOSE: Transcatheter aortic valve replacement (TAVR) is the standard treatment for patients with aortic diseases at high surgical risk. Transcatheter heart valve prostheses (THV) are inserted into the aortic valve, creating a new area between the native and artificial leaflets. This area, known as neo-sinus, increases the thrombogenicity of THVs. But there is a lack of testing methods that evaluate thrombogenicity in vitro.
METHODS: To analyze the flow field within the native sinus and the neo-sinus, Particle Image Velocimetry (PIV) was performed with a thrombosis tester. Additionally, a co

Surgical aortic valve replacement (SAVR) is the treatment of choice for young patients with severe aortic stenosis (AS). Left ventricular (LV) reverse remodeling (RR) after surgery is expected to occur, even though its definition is largely heterogenous and ill-defined. However, LV RR not always occurs following afterload relief, and such may impact the prognosis. Single-centre prospective study including patients referred for SAVR due to severe symptomatic AS, with no previous history of ischemic cardiomyopathy. Both pre- and post-operative transthoracic echocardiographic (TTE) and cardiac ma

This study aimed to determine the mechanical stresses experienced by third-generation, balloon-expandable transcatheter aortic valves in the presence of aortic calcification, defined by an elevated Agatston score index for calcium. The device under investigation was the Edwards SAPIEN 3 valve (referred to as S3-TAV henceforth), a 26-mm commercial device. Five steps were undertaken. The first was the reconstruction of the three-dimensional (3D) computer-aided design (CAD) model of the device. The second was a simulation of the crimping and reopening process. The third was evaluating the crimpin

Tumor necrosis factor-alpha (TNF-α) inhibitors and other biologics used for autoimmune diseases are associated with low-grade immunosuppression. Treatment with these and the presence of prosthetic mechanical cardiac valves both increase the risk of infective endocarditis (IE). However, evidence on the risk of prosthetic valve endocarditis (PVE) among patients treated with TNF-α inhibitors is limited. This study reports a 41-year-old man with a prosthetic aortic valve who was on treatment with golimumab and presented with low-grade fevers and positive blood cultures for Staphylococcus epidermid

OBJECTIVES: Indications for aortic valve replacement (AVR) in severe asymptomatic aortic stenosis (AS) differ for individuals with normal versus low or decreasing left ventricular ejection fraction (LVEF). Conceptually, the development of left ventricular hypertrophy (LVH) could also indicate ventricular injury and potential need for earlier AVR.
METHODS: Initially, 1 232 492 echocardiography reports from 12 hospitals were identified; then previously validated, open-source natural language processing modules were applied to identify aortic gradients, LVEF, and the presence of LVH. These repor

Human parvovirus B19 infection in immunocompetent adults predominantly manifests as arthropathy or erythema infectiosum, with transient aplastic crisis being a rare hematological complication. We report a diagnostically challenging case of a man in his late 40s who developed recurrent severe aplastic anemia following mechanical valve replacement. Notably, the patient presented with progressive pancytopenia despite optimal anticoagulation and normally functioning prosthetic valves on serial echocardiograms. Critical diagnostic differentiation was achieved through comprehensive laboratory evalua

Oscillations of aortic bioprosthetic heart valve (BHV) leaflets during systole are known as leaflet fluttering (LF). LF may be relevant for assessing valvular function and could play a role in structural valve deterioration. However, a quantitative characterization of LF and its underlying physical processes is still missing. The objectives of this study are to systematically characterize LF for a BHV in vitro and to investigate the associated flow structures using a computational model. Leaflet motion of a bovine BHV was captured with high-speed cameras in an in vitro flow loop under varying

OBJECTIVE: To study hemodynamic parameters according to the data of echocardiography, as well as to evaluate the incidence of complications in the early and late post-operative periods in patients who underwent septal myectomy during aortic valve replacement.
METHODS: A search for articles in the PubMed database was conducted considering the PRISMA guidelines. The clinical trial had to meet the following criteria: 1) the presence of two comparison groups: aortic valve replacement (AVR) and AVR+septal myectomy (SME); 2) hypertrophy of the left ventricular outflow tract (LVOT) should be associa

Patients undergoing surgical aortic valve replacement (SAVR) are at risk of developing stroke after the intervention, usually detected via diffusion-weighted magnetic resonance imaging (DW-MRI). Previous studies suggested the maintenance of cerebral autoregulation (CA) as derived from time series analysis of mean arterial pressure (MAP) and mean cerebral blood velocity (MCBv) after SAVR, but the association of CA markers with the presence of recent ischemic lesions was not assessed so far. This study investigates closed-loop cerebrovascular control via causal cross-spectral analysis in 23 subj

The aim of this study was to assess the impact of left ventricular outflow (LVOT) calcification on early device outcomes in a contemporary patient cohort with bicuspid aortic valve undergoing transcatheter aortic valve implantation (TAVI), and to compare outcomes between balloon-expandable (BE) and self-expandable (SE) prostheses in this population. LVOT calcification is known to be associated with adverse outcomes after TAVI in patient with tricuspid anatomy. However, its impact on patients with bicuspid anatomy remains less explored. Additionally, the differential effect of LVOT calcificatio

BACKGROUND: Infective endocarditis (IE) caused by Gram-negative bacteria is a rare but increasingly recognized emerging infectious disease. Achromobacter xylosoxidans, a Gram-negative non-fermenting opportunistic bacterium, has rarely been associated with IE and its biofilm potential in vivo is unknown.
METHODS: Specimens from a patient with A. xylosoxidans IE were obtained during cardiac surgery. Fluorescence in situ hybridization was used for visualization of microorganisms within heart valve tissues in combination with 16S rRNA gene PCR and sequencing (FISHseq) for correlation with prior b

INTRODUCTION AND OBJECTIVES: Suboptimal outcomes with the ACURATE neo2 valve have been attributed to valve frame underexpansion, reported in approximately 20% of cases according to 3 recently proposed angiographic criteria. We aimed to validate these criteria in an independent registry with a 2-year follow-up.
METHODS: This registry included 696 patients with severe aortic stenosis treated with the ACURATE neo2 valve at 7 experienced centers. Underexpansion was defined as the presence of at least 1 of the 3 criteria; the boundary box method was also applied to assess postparallelism. The prim

INTRODUCTION AND OBJECTIVES: This study aimed to assess the effectiveness and clinical outcomes of tricuspid transcatheter edge-to-edge repair (T-TEER) in patients with cardiac implantable electronic devices (CIEDs).
METHODS: This subanalysis of the Transcatheter Tricuspid Valve Repair in Spain (TRI-SPA) registry included patients with at least severe tricuspid regurgitation (TR) who were treated with T-TEER between June 2020 and May 2023 in Spain. Patients with CIEDs and no or mild tricuspid lead-leaflet interaction were compared with those without CIEDs. The primary composite endpoint was a

Severe tricuspid regurgitation (TR) is associated with significant morbidity and mortality. Management of severe TR includes treatment of the underlying cause(s), medical therapy and less commonly surgical tricuspid valve repair. Newer transcatheter tricuspid valve repair devices show promise for those patients who remain symptomatic with severe TR despite such efforts. The presence of a Cardiac Implantable Electronic Device (CIED) with leads across the tricuspid valve poses a significant challenge from both diagnostic and therapeutic standpoints. In this paper, we propose an algorithm to mana

BACKGROUND: Mitral valve transcatheter edge-to-edge repair (M-TEER) is increasingly applied in patients with high surgical risk. We aimed to evaluate whether the PASCAL system can be applied in an all-comers cohort irrespective of the underlying anatomy and whether technical features influence therapeutic success.
METHODS: In this prospective, observational study we enrolled consecutive patients (n = 80) with mitral regurgitation (MR) 3+ and 4+ scheduled for M-TEER. All patients were allocated to be treated with the PASCAL system irrespective of the underlying anatomy. Complexity of mitral va

BACKGROUND: Many patients with atrial functional mitral regurgitation are not suitable candidates for surgery or transcatheter repair. For transcatheter mitral valve replacement, a common contraindication is the risk of left ventricular outflow tract obstruction, particularly in patients with atrial functional mitral regurgitation, who have characteristically small left ventricles. Herein, we examine the outcomes of transcatheter mitral valve replacement using the AltaValve system, which employs atrial fixation thus minimizing left ventricular outflow tract obstruction risk.
METHODS: Patients

BACKGROUND: Patients requiring transcatheter tricuspid valve replacement (TTVR) often have a preexisting pacemaker lead crossing the tricuspid valve (TV). TTVR could also lead to heart block.
OBJECTIVE: We aimed to describe pacing considerations in patients referred for TTVR.
METHODS: We identified patients who underwent TTVR at Emory Healthcare. Clinical characteristics including presence of preexisting pacemaker leads, management of these TV leads at the time of TTVR, and pacing requirements after TTVR were collected.
RESULTS: Seventy-three patients underwent TTVR; 21 (29%) patients had p
