Permissive versus conventional weaning from venovenous ECMO in patients with ARDS: a retrospective cohort study.

BACKGROUND: Criteria for weaning from venovenous extracorporeal membrane oxygenation (V-V ECMO) in acute respiratory distress syndrome (ARDS) remain poorly standardized. Some patients are decannulated despite persistent abnormalities in respiratory mechanics once oxygenation and carbon dioxide clearance are considered adequate. We aimed to compare outcomes associated with permissive versus conventional V-V ECMO weaning. METHODS: We conducted a retrospective single-center cohort study including adult patients with refractory ARDS treated with V-V ECMO between January 2015 and December 2022 who

Preliminary stability studies of a β-SARS-CoV-2 virus-like particle vaccine.

We aimed to study the stability of a Beta (β)-SARS-CoV-2 virus-like particle (VLP) vaccine in a series of preliminary experiments using selective stabilizing excipients. β-SARS-CoV-2 VLPs were produced and purified using established methodologies. The thermostability of VLPs was tested at 4°C and -30°C in the presence or absence of stabilizers polysorbate 80, sorbitol, or L-histidine in the presence of a physiological NaCl concentration of 137 mM. The integrity of VLPs was assessed using ELISA, Western immunoblot, and dynamic light scatter (DLS). β-SARS-CoV-2 VLPs were stable at 4°C for 14 day

Unveiling structural determinants of peptide recognition by public and private SARS-CoV-2-specific T-cell receptors.

Public T cell receptors (TCRs) recurrently emerge across individuals in response to common pathogens, yet the structural and biophysical basis distinguishing public from private clonotypes remains incompletely defined. Here, we combine epitope mapping, single-cell TCR sequencing, and single-particle cryo-electron microscopy to dissect CD8+ T cell responses to the immunodominant SARS-CoV-2 ORF3a(207-215) epitope presented by HLA-A*01:01. Among responding clonotypes, we identify a public TCR (TCRpub), shared across donors, and a private TCR (TCRpriv) that uses nearly identical β chains but disti

Clinical validation of a novel multiplex real-time PCR for diagnosing community-acquired pneumonia in the emergency department: a promising diagnostic tool for Mycoplasma pneumoniae.

Community-acquired pneumonia (CAP) is a global public health burden with high morbidity, but timely diagnosis is hindered by limitations of conventional methods. We validated a novel multiplex real-time PCR (mRT-PCR) for CAP diagnosis in the emergency department, focusing on Mycoplasma pneumoniae (M. pneumoniae). A retrospective cohort of 371 CAP patients (2019-2021) from the Chinese PLA General Hospital was enrolled. The panel targeted 85 pathogens (40 bacteria, 18 viruses, 17 fungi, 10 other) plus 8 resistance genes, with a turnaround time (TAT) under 4 h and a cost of approximately US$100 p

Four-Year Longitudinal Phenotyping of Post-Acute Sequelae of SARS-CoV-2 Infection (PASC): A Single-Centre Cohort Study.

Background: Although an increasing number of studies have reported 2- and 3-year outcomes of post-acute sequelae of SARS-CoV-2 infection (PASC), the long-term symptom patterns of and their clinical phenotypes beyond four years remains unknown. We aimed to characterize 48-month clinical outcomes and symptom profiles using hierarchical cluster analysis. Methods: We conducted a longitudinal cohort study of 79 patients with persistent PASC enrolled between March 2021 and March 2022. Of these, 77 patients completed the 48-month follow-up. Symptoms were assessed at 11 and 48 months post-infection. H

Cell DiffErential Expression by Pooling (CellDEEP) highlights issues in differential gene expression in scRNA-seq.

Accurate identification of differentially expressed genes (DEGs) in single-cell RNA sequencing data remains challenging. Single-cell-specific statistical models often report large numbers of candidate genes but can exhibit inflated false positive rates, whereas pseudobulk approaches improve false discovery control at the cost of reduced sensitivity. To overcome the noise and bias that other tools have, and allow the user to have more control of the DEG process, we present CellDEEP, which uses a cell aggregation (metacell) approach. This tool provides a framework for flexible selection of pooli

Viral Infection-Induced Epilepsy and the Gut Microbiome Crosstalk.

Viral infections of the central nervous system (CNS) pose a significant risk for seizures, and in some cases, long-term epileptogenesis. Seizure development following viral infection involves diverse CNS mechanisms, with emerging evidence that peripheral factors, including gut microbiome composition and immune-inflammatory responses, also contribute. The gut microbiome is a multifaceted ecosystem with important roles in CNS health, and advances in sequencing have enabled investigation of host and microbial genomic changes associated with seizure development. While both animal and human studies

Protective Effect of COVID-19 Vaccination on Post-Infection Visuospatial Deficits.

OBJECTIVE: COVID-19 has been associated with persistent cognitive deficits months after SARS-CoV-2 infection. Vaccination reduces severe disease, but its effect on post-infection cognition remains uncertain. We evaluated whether COVID-19 vaccination attenuates cognitive deficits after SARS-CoV-2. METHODS: We assessed 237 adults with COVID-19 and 105 controls. Patients were grouped by vaccination status at infection (vaccinated n=87; unvaccinated n=150). The neuropsychological assessment covered language, visuospatial ability, visual memory, verbal memory, attention, executive functions, and w

Prolonged Active SARS-CoV-2 Infection in a Patient With Chronic Lymphocytic Leukemia on Venetoclax.

We report a case of prolonged active SARS-CoV-2 infection in a patient with chronic lymphocytic leukemia (CLL) receiving venetoclax. The patient demonstrated persistent symptoms over a span of 105 days and repeatedly positive SARS-CoV-2 polymerase chain reaction (PCR) testing. The case highlights the diagnostic and therapeutic challenges in immunocompromised hosts, including limited guidance and drug availability. Management required multiple antiviral regimens and consideration of alternative therapies. This case underscores the need for further research that can inform more robust guidelines

Golgi-derived COPI vesicles fuel the biogenesis of coronavirus replication organelles.

Positive-strand RNA viruses remodel host endomembranes to form replication organelles (ROs). However, the membrane sources for coronavirus double-membrane vesicle (DMV) biogenesis remain unclear. Here, we used proteomic, genetic, pharmacological, and imaging approaches to identify the central membrane supply route for DMV formation. APEX2-based proximity proteomics first identified candidate pathways, followed by functional validation using siRNA knockdown, CRISPR/Cas9 editing, and Golgicide A (GCA) inhibition. DMV formation and viral RNA synthesis were assessed by electron microscopy, immunof

Intranasal gold nanoparticle-antigen induces mucosal and systemic immunity against β-coronaviruses.

The continued emergence of diverse beta-coronaviruses (β-CoVs) from animal reservoirs underscores the urgent need for broad-spectrum mucosal vaccines, yet their development is hindered by physiological barriers, rapid antigen clearance, and suboptimal multivalent display strategies. To address this, we intranasally immunized BALB/c mice (n = 3 or 4 per group) with 5 or 15 nm gold nanoparticles (AuNPs) displaying the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) receptor-binding domain (RBD, 5 μg per mouse) and adjuvanted with the STING agonist cGAMP (20 μg per mouse). Crystal structures of SARS-CoV-2 main protease and its complex with PBI-0451.

The SARS-CoV-2 main protease (Mpro) has a pivotal role in mediating viral genome replication and transcription of the coronavirus, making it a prime target for antiviral drug development. This study presents four high-resolution crystal structures of SARS-CoV-2 Mpro in the apo state. Among these structures, we identify a previously unobserved, stable inactive conformation, characterized by a major displacement of the oxyanion loop (especially Phe140, Leu141 and Asn142), the rotation of some important residues (including His163 and His172) and the loss of many key interactions around the active

Identifying Public Response Topics to the Centers for Disease Control and Prevention's COVID-19 Communications on Social Media: Infoveillance Study Using Large Language Model-Based Rephrasing.

BACKGROUND: Public health agencies increasingly use social media platforms such as Twitter (subsequently rebranded X, X Corp) to monitor public responses to official health communications and support timely decision-making during health crises. Public replies to communications from official agencies provide valuable insights into population-level discourse and engagement with health policies. However, analyzing large-scale short-text responses remains challenging because social media replies are often brief, informal, ambiguous, and linguistically fragmented. Topic modeling offers a scalable a
