OmniLAMP® - A portable colorimetric LAMP system for fast and reliable clinical molecular detection of SARS-CoV-2.

Despite the transition of COVID-19 from a global emergency to sustained circulation, SARS-CoV-2 continues to evolve, reinforcing the need for rapid, reliable, and decentralized molecular diagnostic tools. Reverse transcription loop-mediated isothermal amplification (RT-LAMP) represents an alternative to RT-qPCR, particularly for point-of-care (PoC) applications. In this study, we evaluated the OmniLAMP® portable PoC platform for SARS-CoV-2 detection in clinical saliva samples using colorimetric RT-LAMP targeting the SARS-CoV-2 N/E genes, using either extracted RNA or an extraction-free saliva

Structural basis for HKU5‑CoV main protease inhibition by the clinical antivirals nirmatrelvir and ensitrelvir.

The identification of Pipistrellus bat coronavirus HKU5 lineage 2 (HKU5-CoV-2) as a human ACE2-adapted virus highlights the need for antiviral strategies to control emerging HKU5-related merbecoviruses. With no specific antivirals approved for merbecoviruses, repurposing SARS-CoV-2 Mpro inhibitors is a promising strategy. However, their inhibitory activity and binding mechanisms against HKU5-CoV Mpro remain poorly understood. Here, we obtained high-resolution crystal structures of HKU5-CoV-1 Mpro in its apo state (1.75 Å) and in complex with the approved covalent inhibitor nirmatrelvir (1.91 Å

Analytical Validation of MethyLiverDx-Sigma, a Plasma cfDNA Methylation Assay for Hepatocellular Carcinoma Detection.

Hepatocellular carcinoma surveillance relies on ultrasonography and alpha-fetoprotein, both limited in sensitivity for early-stage disease; aberrant methylation of circulating cell-free DNA (cfDNA) offers a promising blood-based approach. Reported here is the analytical validation of MethyLiverDx-Sigma (MLDx-s), an assay that detects methylated RNF135 and DNMT3A in bisulfite-converted plasma cfDNA by methylation-specific PCR with high-resolution melting (HRM), with GAPDH as an endogenous control. Performance was evaluated following CLSI EP05-A3, EP07, EP12-A2, and EP37. Precision of quantifica

Test-retest reliability and interpretability of Long COVID Burden Index: A prospective observational study.

The Long COVID Burden Index (LCBI) is an 11-item patient-reported outcome measure developed to assess the burden of long COVID symptoms on daily life. Although its cross-sectional validity and internal consistency have been demonstrated, its test-retest reliability, measurement error, and minimal important change (MIC) have not yet been evaluated. We conducted a longitudinal postal survey among adults with a history of COVID-19 hospitalization. The LCBI was administered at the initial survey, 2-week follow-up, and 3-month follow-up. Test-retest reliability was assessed among participants who c

Obstructive sleep apnea is frequent in patients with Long COVID fatigue but is not significantly associated with alterations of quality of life and cognition.

Neurologic manifestations of Long COVID, also called post-acute sequelae of SARS-CoV-2 infection (PASC), include fatigue, cognitive dysfunction, sleep disturbances, and headaches, which can be debilitating. Since these symptoms overlap with those of obstructive sleep apnea (OSA), we sought to determine the prevalence of OSA in the population of a Neuro-COVID-19 clinic. We performed home sleep testing in 100 consecutive patients with Long COVID and no prior history of OSA who reported fatigue after an episode of COVID-19 not requiring hospitalization. The average age was 46.4 years and 70% of p

Fast and inexpensive visualization of genome collection at scale.

BACKGROUND: Investigating genetic progression and lineage using sequence data is a fundamental focus within the realm of viral genome research. Given the substantial increase in publicly accessible genome sequences, tools designed for extensive data analytics, particularly insightful visualizations play an important role in elucidating the spatio-temporal evolution as well as the diversity of viral lineages. Existing genome data analysis methods frequently rely on multiple sequence alignment (MSA) and phylogeny computations, making integration of new sequences computationally expensive. Such m

Transcription Regulatory Sequences in Coronaviruses: Mechanistic Foundations for Antivirals and Vaccines.

Coronaviruses employ a distinctive discontinuous transcription mechanism to generate a nested set of subgenomic mRNAs (sgRNAs) from a single large positive-strand RNA genome. Transcription regulatory sequences (TRSs)-comprising a leader TRS (TRS-L) near the 5' end and multiple body TRSs (TRS-Bs) upstream of downstream open reading frames-serve as the primary cis-acting signals that direct polymerase pausing and template switching during negative-strand synthesis. The COVID-19 pandemic spurred intensive investigation of SARS-CoV-2 gene expression, yielding high-resolution transcriptome maps, ge

Evaluating Classifier Derivation and Validation Beyond the Receiver Operating Characteristic Curve.

The area under the receiver operating characteristic curve (AUROC) summarizes a classifier's discrimination across all possible thresholds, but it does not establish calibration, clinical utility, or performance at the threshold used in practice. This editorial explains the interpretation and limitations of AUROC using a hypothetical genomic enzyme test for susceptibility to symptomatic COVID-19. It emphasizes that threshold selection must be justified according to the intended use, outcome prevalence, and consequences of false-positive and false-negative results. Threshold derivation and vali

Bridging spatial scales in respiratory viral infection.

Respiratory viruses spread within the host through a combination of local expansion and occasional long-range dissemination that seeds new infection foci. We present LEAP (Lotka-Euler Airway Pathogen model), an analytically tractable within-host model that captures this two-scale process. The model takes the form of an age-dependent branching process and yields a closed-form expression for the exponential growth rate during early infection. Using empirical data to parameterize the model, we find that productive dissemination requires only a small number of successful long-range seeding events

A multi-objective sequence engineering framework for rational genetic vaccine design.

Genetic vaccine performance is determined not only by the delivery platforms and antigen choice but also by the coding sequence that influences antigen expression, innate immune sensing, transcript stability and sequence-associated risks. However, the design principles governing these sequence-level effects remain insufficiently defined. Thus, we consider vaccine coding-sequence design as a multi-objective engineering problem that requires simultaneous consideration of multiple, potentially competing biological properties. We present a multi-parameter computational framework for analysing SARS

Socioeconomic transitions and the spatial dynamics of syphilis transmission in urbanizing China.

BACKGROUND: Syphilis remains a re-emerging sexually transmitted infection in China, with transmission increasingly shaped by regional socioeconomic disparities. Understanding spatial heterogeneity and inter-provincial inequality is essential for equitable disease control. METHODS: A balanced panel dataset of syphilis incidence was constructed across 31 provinces in mainland China from 2004 to 2020. Temporal trends were stratified by age and disease stage using joinpoint regression analysis. Spatial clustering patterns of syphilis incidence were evaluated through Global Moran's I and Local Mor

Longitudinal BCR repertoire dynamics reveal the evolution of broadly binding antibodies following heterologous SARS-CoV-2 vaccination and breakthrough infection: a single-donor case study.

Continued SARS-CoV-2 antigenic drift and the persistent risk of zoonotic sarbecovirus spillover highlight the need for broadly protective countermeasures. Here, we longitudinally profiled the B-cell receptor (BCR) repertoire of a single volunteer who received heterologous booster vaccination followed by Omicron BF.7 breakthrough infection, integrating bulk BCR sequencing with single B-cell culture and antibody binding analysis. Breakthrough infection was associated with broadened serum neutralization across SARS-CoV-2 variants and related sarbecoviruses, consistent with recall of antibodies ta

Aptamer-CRISPR/Cas as a tool in point-of-care testing of infectious diseases.

Aptamers and the CRISPR/Cas system have become the cornerstones of target detections due to their numerous advantages. Aptamers enable direct recognition of non-nucleic acid targets, such as pathogens and antigens, while CRISPR/Cas system drives signal amplification and rapid detection. The combined aptamer-CRISPR/Cas system can overcome the limitations of traditional target detection by realizing field-deployable, highly sensitive, specific, versatile, and rapid assays. Given the increasing number of different aptamer-CRISPR/Cas systems developed recently, particularly for infectious diseases

Evaluation of mRNA expression and DNA methylation status of immune checkpoints in the peripheral blood of COVID-19 patients.

BACKGROUND: COVID-19 immunopathology involves dysregulated immune responses, partly mediated by inhibitory immune checkpoints (ICs). Epigenetic mechanisms, particularly DNA methylation by DNA methyltransferases (DNMTs), regulate gene expression. However, the interplay between IC transcription, promoter methylation, and DNMT expression across disease severities remains unclear. RESEARCH DESIGN AND METHODS: This cross-sectional study enrolled 208 COVID-19 patients (mild to critical) and 93 healthy controls. mRNA expression of PDCD-1, HAVCR-2, LAG-3, CTLA-4, DNMT-1, DNMT-3a, and DNMT-3b was meas
