Impact of tranexamic acid on the efficacy of vancomycin and meropenem against periprosthetic joint infections : an in vitro analysis.

AIMS: This study investigated the effects of tranexamic acid (TXA) on the efficacy of vancomycin and meropenem against common periprosthetic joint infection (PJI)-associated pathogens in vitro. The aim was to uncover valuable insights that can be used for clinical decision-making and enhanced management of PJI in orthopaedic surgery. METHODS: We evaluated the minimum inhibitory concentration (MIC), minimum bactericidal concentration (MBC), minimum biofilm inhibitory concentration (MBIC), and minimum biofilm eradication concentration (MBEC) for vancomycin and meropenem, both with and without T

Vancomycin-resistant enterococci utilise antibiotic-enriched nutrients for intestinal colonisation.

Antibiotic treatment significantly disrupts the gut microbiome and promotes vancomycin-resistant enterococci (VRE) intestinal colonisation. These disruptions cause the intestine to act as a reservoir for VRE that seed difficult-to-treat infections. Here we show that antibiotics that promote VRE intestinal colonisation increase the concentration of a wide range of nutrients and decrease the concentration of a wide range of microbial metabolites. We show significant but incomplete suppression of VRE growth by individual short chain fatty acids that were decreased in antibiotic-treated faecal mic

Time-Varying Bayesian Network Meta-Analysis.

The presence of methicillin-resistant Staphylococcus Aureus (MRSA) in complicated skin and soft structure infections (cSSSI) is associated with greater health risks and economic costs to patients. There is concern that MRSA is becoming resistant to other "gold standard" treatments such as vancomycin, and there is disagreement about the relative efficacy of vancomycin compared to linezolid. There are several review papers employing Bayesian Network Meta-Analyses (BNMAs) to investigate which treatments are best for MRSA-related cSSSIs, but none address time-based design inconsistencies. This pap

Virulence signature of the endemic vancomycin-resistant Enterococcus faecium clones in Denmark, 2015-2023.

Since 2012, Denmark has seen a significant rise in vancomycin-resistant Enterococcus faecium (VREfm) cases, mirroring trends in other countries, though exceptions occurred during the coronavirus disease 2019 (COVID-19) lockdown and between 2022 and 2023. This rise has been accompanied by ongoing changes in the endemic VREfm clones, reflecting the species' high genetic plasticity. VREfm rapidly acquires plasmids and mobile genetic elements, enriching it with putative virulence markers (PVMs), including surface proteins, pili, and factors encoding biofilm production and adhesion. Antimicrobial susceptibility and genomic characterization of Lactococcus formosensis, Lactococcus garvieae, and Lactococcus petauri in Hong Kong.

UNLABELLED: Recent studies have shown that many isolates previously classified as Lactococcus garvieae were L. petauri and L. formosensis, complicating the understanding of the ecological niches, phylogeny, and phenotypic characteristics of these organisms. In this study, a total of 94 human feces and 166 food samples, collected from Hong Kong in 2021-2023, were cultured using selective media, with a positive rate of 21% for human feces, 37% for sashimi, 8% for cut fruits, and 8% for salad vegetables. The isolates (n = 67, including 43 from sashimi and four from ready-to-eat cut fruits and veg

Peritoneal Dialysis-Related Peritonitis Caused by Staphylococcus caprae: A Case Report.

Herein, we report a case of peritoneal dialysis (PD)-related peritonitis caused by Staphylococcus caprae (S. caprae). An 88-year-old man who had been receiving PD presented with cloudy effluent and an elevated dialysate white blood cell (WBC) count of 800/μL. He was diagnosed with PD-related peritonitis and treated with intraperitoneal cefazolin and ceftazidime, which promptly cleared the effluent by day 3 and reduced the WBC count in the dialysate to 0/μL by day 5. Culturing of the effluent identified S. caprae (methicillin-resistant coagulase-negative Staphylococcus), so the antibiotic regim

Short-Course Therapy for Uncomplicated VRE Bacteremia: This is Us Trying (to Shorten Therapy).

Background: The optimal treatment duration for uncomplicated vancomycin-resistant Enterococcus (VRE) bacteremia is unclear. Shorter courses may reduce adverse events (AEs) and hospital length of stay (LOS) without compromising clinical outcomes. Objective: To evaluate clinical outcomes associated with short-course (≤6 days) vs long-course (>6 days) therapy in hospitalized adults with uncomplicated VRE bacteremia. Methods: We conducted a single-center, retrospective study of adults with uncomplicated VRE bacteremia between 2014 and 2024. Patients who received ≤6 days of definitive antibiotic th

Antibiotic and Heavy Metal Resistance in Marine Bacteria from Terra Nova Bay (Ross Sea): Insights from Wild Fish and Environmental Samples.

This study examines the occurrence of bacteria resistant to antibiotics and heavy metals in Terra Nova Bay, a coastal area of the Ross Sea in Antarctica that is increasingly recognised as vulnerable to human influence. During the 37th Italian Antarctic Expedition (2021-2022), researchers collected seawater, sediment, and fish samples from the notothenioid species Trematomus bernacchii to evaluate microbial resistance in an environment once considered largely pristine. Fifty heterotrophic bacterial isolates were obtained and tested against twenty-eight antibiotics, revealing a notable presence

Reversible Severe Cardiomyopathy Associated With Supratherapeutic Vancomycin Levels: A Case Report.

Vancomycin is a widely used glycopeptide antibiotic, and while nephrotoxicity and infusion-related reactions are well-established adverse effects, direct reversible cardiomyopathy associated with extreme vancomycin levels has not been previously reported in humans. We describe a 59-year-old woman treated for methicillin-resistant Staphylococcus aureus (MRSA) bacteremia who developed acute heart failure, hypotension, and severe left ventricular systolic dysfunction (ejection fraction 27%) while her vancomycin level exceeded 80 mg/L. Multiple left ventricular thrombi were also identified, likely

Nephrocast-V: A Deep Learning Model for the Prediction of Vancomycin Trough Concentration Using Electronic Health Record Data.

INTRODUCTION: Vancomycin is a critical antibiotic for treating methicillin-resistant Staphylococcus aureus and other gram-positive bacterial infections, but achieving and maintaining therapeutic trough concentrations is challenging. OBJECTIVES: We hypothesized that a deep learning model could accurately predict vancomycin trough concentrations 2 days in advance in critically ill patients and provide recommendations for optimal dosing adjustments to achieve target drug concentrations. METHODS: We trained and validated the model using electronic health record (EHR) data from adults admitted to

First comprehensive genome analysis and antimicrobial resistance profile of Clostridium innocuum from a domestic cat.

This study aimed to investigate the molecular and phenotypic properties of a C. innocuum isolate from a healthy cat, which was previously misidentified as C. difficile by molecular testing (tpi-PCR). To resolve the misidentification and obtain more information on the isolate at the genetic level, whole genome sequencing (WGS) on the long-read sequencing platform Oxford Nanopore PromethION was performed. WGS data were analysed to identify virulence factors, antibiotic resistance, and hypothetical prophage genes. The isolate was identified as C. innocuum as a result of WGS. While several virulen

Discovery of MBL-AB01: a novel antibacterial xanthone antibiotic with high activity against methicillin-resistant Staphylococcus aureus.

A novel compound denoted MBL-AB01 was isolated from a marine Actinoalloteichus, which belongs to a rare and underexplored class of Actinobacteria. This work demonstrates that the novel compound MBL-AB01 shows very high activity in vitro against six methicillin-resistant Staphylococcus aureus strains, and high activity against a panel of three other Gram-positive strains, including a vancomycin-resistant Enterococcus faecium. Structure elucidation of the compound revealed that MBL-AB01 is a polycyclic xanthone antibiotic closely related to the bioactive compounds: xantholipin and lysolipin. Thi

Polymicrobial periprosthetic joint infection: eradication with different antimicrobials.

Antibiotics incorporated into spacers do not guarantee complete eradication of infection. This study aimed to compare, in vitro, the antimicrobial activity of antibiotic-loaded cement with and without additional alternative antimicrobial molecules for the eradication of polymicrobial biofilms. Methicillin-resistant Staphylococcus aureus (MRSA), Pseudomonas aeruginosa (PA), and Candida albicans (CA) were selected to form mono- and polymicrobial biofilms on polyurethane sponges. Bone cement was supplemented with vancomycin, imipenem, silver nitrate (SN), and xylitol in different combinations. In

Photochemical Transformation of Rearranged Pregnane Steroids from the Soft Coral Scleronephthya gracillimum and Their Antibacterial Properties.

Three uncommon rearranged pregnane steroids, namely, sclerogroids A-C (1-3), along with seven known related ones 4-10, have been isolated from the soft coral Scleronephthya gracillimum collected off Ximao Island. The photoreactions of four α,β-unsaturated ketosteroids (8, 11-13) have been successfully performed, leading to the generation of two novel carbon skeleton pregnane steroids with an unprecedented 5/3/6/6/5 pentacyclic ring scaffold, namely, sclerogranes A (8b) and B (11f), together with a series of rearranged steroids and oxidation products. The structures of the new compounds were el

Redesigning oxazolidinones as carbonic anhydrase inhibitors against vancomycin-resistant enterococci.

The rise of vancomycin-resistant enterococci (VRE) as a leading cause of hospital-acquired infections underscores the urgent need for new treatment strategies. In fact, resistance has developed not only to vancomycin but also to other clinically used agents, such as daptomycin and linezolid. We propose a novel drug design approach merging tedizolid, a second-generation oxazolidinone used as an unapproved salvage therapy in clinical settings, with carbonic anhydrase inhibitors (CAIs) recently validated as functioning decolonization agents. These sulfonamide derivatives showed potent inhibition
