Limited incision plating of midshaft clavicle fractures: a case series of 1,038 patients.

BACKGROUND: Clavicle fractures are a common injury in young and active patients. Nonoperative management has historically been preferred; however, recent studies have challenged this paradigm with evidence of lower nonunion rates and improved functional outcomes following surgical treatment. This article describes a limited incision approach to clavicle plating and reports long-term functional and radiographic outcomes for a large cohort of operative midshaft clavicle fractures. METHODS: A prospectively maintained database was used to identify 1,038 patients who underwent surgical treatment f

Single-Entry Selective Trunk Block (S-SeTB): An Innovative Approach to Complete Upper Extremity Anesthesia in Weapon-Wounded Patients in Low-Resource Settings.

BACKGROUND: Regional anesthesia for upper extremity surgery in weapon-wounded patients is challenging, particularly in low-resource settings. Existing techniques often require multiple needle entries and ultrasound probe repositioning, increasing complexity and risk. There is a need for a simpler, effective technique providing complete anesthesia from the shoulder to the hand. METHODS: We developed a novel ultrasound-guided regional anesthesia technique-the Single-entry Selective Trunk Block (S-SeTB)-which targets the superior trunk (ST), middle trunk (MT), and C8 ventral ramus (C8VR) with lo

Humeral shaft reconstruction surgery following military combat gunshot injury using a titanium spinal mesh cage - A case report.

INTRODUCTION: High-velocity gunshot injuries to the upper extremity are rare but devastating, particularly in military personnel, and are associated with high risks of infection, instability, and limb loss. Reconstruction of extensive segmental bone defects in such cases is a complex surgical challenge. CASE PRESENTATION: We report a challenging case of a 32-year-old Indian Air Force soldier who sustained a Grade 3B open right humeral shaft fracture from a gunshot wound during military combat. Initial management included wound debridement, skin grafting, and external fixation at military hosp

Using a wearable system combining inertial and force sensing for simultaneous detection of limb motion and grasping actions in the workplace.

Using wearable sensors to evaluate workers' performance is challenging with existing sensor techniques. It requires detecting not only limb motions but also the onset and offset of specific actions. Commonly used inertial measurement units (IMUs) can be combined with surface electromyography (sEMG) to detect muscular activity. However, sEMG requires skin preparation and careful sensor placement, and can be affected by sweat or motion artifacts. To address these limitations, we used a wearable system combining IMUs and force-sensing resistors (FSRs), where IMUs capture joint kinematics and FSRs

Ultrasound Applications in Adaptive Sports Medicine.

The use of ultrasound in adaptive sports has not been well studied. There is more literature for use in wheelchair athletes who suffer overuse upper extremity injuries, including shoulder and biceps tendon injuries, lateral epicondylosis, ulnar neuropathy at the elbow, and carpal tunnel syndrome. A few key lower extremity injury phenotypes have been studied with ultrasound in this population, including HO and skin complications. There are also unique applications of ultrasound, including boosting, respiratory muscle training, and evaluation of deep tissue injuries. Further ultrasound studies f

Transcutaneous Osseointegrated Prosthesis Systems (TOPS) for Rehabilitation After Lower Limb Loss: Surgical Pearls.

BACKGROUND: The biology of osseointegration of any intramedullary implant depends on the design, the press-fit anchoring, and the loading history of the endoprosthesis. In particular, the material and surface of the endoprosthetic stem are designed to stimulate on- and in-growth of bone as the prerequisite for stable and long-lasting integration1-8. Relative movement between a metal stem and the bone wall may stimulate the formation of a connective-tissue interface, thereby increasing the risk of peri-implant infections and implant loss9-12. The maximum achievable press-fit (i.e., the force cl

Milia en plaque on the shoulder as an early manifestations of mycosis fungoides.

Milia en plaque (MEP) is an uncommon skin condition identified as retroauricular confluent milium by Boulzer and Fouqet in 1903 [1]. It can be mistaken for other dermatoses like Favre-Racouchot nodular elastosis, steatocystoma multiplex, and nevus comedonicus. Milia en plaque can either be primary or secondary and is typically benign, often triggered by dermatological procedures like cryotherapy, as reported in this journal. In some cases, MEP can arise as a secondary manifestation of other diseases, including folliculotropic mycosis fungoides (FMF). Despite this association, there are few doc

An Innovative Approach to Classifying and Treating Axillary Scar Contracture.

BACKGROUND: Axillary cicatricial contracture is a debilitating condition that can greatly impair shoulder joint function. Therefore, timely correction of this condition is imperative. In light of Ogawa's prior classification of axillary cicatricial contracture deformities, we have proposed a novel classification system and reconstruction principles based on a decade of treatment experience. Our proposed system offers a more comprehensive approach to correcting axillary cicatricial contracture deformities and aims to improve patient outcomes. METHODS: Our study included 196 patients with a tot

Bayesian inference in arm posture perception.

To configure our limbs in space, the brain must compute their position based on sensory information provided by mechanoreceptors in the skin, muscles, and joints. Because this information is corrupted by noise, the brain is thought to process it probabilistically and integrate it with prior belief about arm posture, following Bayes' rule. Here, we combined computational modeling with behavioral experimentation to test this hypothesis. The model conceives the perception of arm posture as the combination of a probabilistic kinematic chain composed by the shoulder, elbow, and wrist angles, compro
