Perineal and Rectal Nerve Recruitment Order Varies During Pudendal Neurostimulator Implant Surgery.

INTRODUCTION: Pudendal nerve stimulation (PNS) is an off-label therapy for patients experiencing pelvic pain and voiding dysfunction. The pudendal nerve has two efferent branches to the rectum and perineum. Only the rectal branch is monitored via external anal sphincter electromyography during the implant procedure to help determine the lead position. We examined intraoperative PNS-driven urethral pressures to infer nerve recruitment order and tracked patient reported outcomes. METHODS: Patients receiving PNS for pelvic pain and/or urinary symptoms were recruited. During the implant surgery,

Internal Hernia Beneath the Obturator Nerve After Robot-Assisted Radical Cystectomy and Pelvic Lymphadenectomy: A Case Report With Literature Review.

A man in his 40's presented with vomiting and numbness in the right thigh. Eight months earlier, he had undergone robot-assisted radical cystectomy and pelvic lymphadenectomy for bladder cancer. Computed tomography (CT) revealed a caliber change and a closed loop of the small intestine in the right pelvis, prompting emergency diagnostic laparoscopy. Intraoperative findings showed that the small intestine had herniated into the space between the right obturator nerve and pelvic wall, resulting in strangulated intestinal obstruction. The necrotic small intestine was resected and reconstructed, w

Effect of Transcutaneous Tibial Nerve Stimulation in Multiparous Egyptian Women with Vaginal Laxity: A Randomized Controlled Trial.

Vaginal laxity (VL) is a common symptom of pelvic floor disorder, an underreported condition, and has a substantial impact on women's sex lives and relationships. The study aimed to determine the effect of transcutaneous tibial nerve stimulation (TTNS) on women with VL. Fifty women with vaginal laxity were chosen from an outpatient clinic in Egypt. They were randomly split into two equal categories (A and B). Group A (n = 25) received pelvic floor exercises (PFE), while Group B (n = 25) received TTNS and PFE. Every week, both groups performed three sessions for three months. The outcomes were

Comparing the Anatomy of the Lateral Femoral Cutaneous Nerve in Patients With and Without Meralgia Paresthetica: A Systematic Review and Meta-Analysis.

The anatomical variations of the lateral femoral cutaneous nerve (LFCN) are well documented, with several studies hypothesizing that specific variants may increase the LFCN's susceptibility to meralgia paresthetica (MP). This systematic review and meta-analysis aimed to compare the prevalence of LFCN anatomical variations among limbs with and without MP. It also sought to determine if the LFCN's cross-sectional area (CSA) on ultrasound differed between limbs with and without MP. PubMed, SCOPUS, CINAHL, and Cochrane databases were used to query publications from inception to June 13, 2024. Incl

Impact of laparoscopic pudendal nerve decompression on quality of life in patients suffering from pudendal neuralgia caused by entrapment syndrome.

BACKGROUND: Pudendal neuralgia caused by entrapment syndrome (PNE) is an underdiagnosed condition with severe quality of life (QoL) impact. In cases unresponsive to medical treatment, a minimally invasive surgical approach, such as laparoscopic surgery, is preferred, particularly in patients presenting central sensitisation (CS) and myofascial syndrome (MS). Although the procedure has proven to be safe and feasible, limited studies examinate its impact on QoL. OBJECTIVES: This retrospective observational study was conducted at the Résilience Center, Axium Clinic, Aix-en-Provence, France, with

MR Neurography of the Lumbosacral Plexus: Technique and Disease Patterns.

The lumbosacral plexus (LSP) comprises a complex network of nerves supplying the pelvis and lower extremities and may be affected by a wide range of diseases. Lumbosacral plexopathy can be challenging to diagnose due to overlapping clinical presentations and difficulty performing electrodiagnostic testing of the deep pelvic structures. MRI-more specifically, MR neurography (MRN)-can readily depict most LSP segments. MRN techniques, preferably performed at 3.0 T, continue to evolve, with most protocols including two-dimensional and optionally three-dimensional, heavily T2-weighted fat-suppresse

Anatomical structures of the obturator foramen and its importance for pelvic interventıons.

BACKGROUND: In clinical scenarios such as pelvic fractures, damage to the obturator neurovascular bundle, obturator neuropraxia, and mesh placement in patients with incontinence, preserving the anatomical structures related to the obturator foramen (OF) is crucial. This study aims to investigate the anatomical variations of the OF as a guide for identifying failure criteria and improving success parameters in clinical practice. METHODS: The OFs of one hundred adult dry pelvic bones were examined. Measurements were evaluated using ImageJ software. The morphology of the OF and the presence of b

The effect of high frequency sacral nerve stimulation on lower urinary tract function in awake, healthy animals.

Many individuals with neurological disorders rely on using catheters to empty their bladder. However, catheters are associated with urethral trauma and urinary tract infections. Peripheral nerve stimulation at frequencies of 500 - 10,000 Hz is associated with reduction of muscle contraction without causing fatigue. We hypothesized that high frequency sacral nerve stimulation would be associated with reduced pelvic muscle activity without reduced bladder pressures. We implanted five healthy cats with pulse generators connected to nerve cuff electrodes on sacral nerves S1 and S2. We applied stim

Hip abduction with hip extension produces the largest obturator nerve displacement: a cadaveric investigation.

BACKGROUND: The obturator nerve may be injured during surgery or trauma. Neurodynamic testing (NDT) is theorized to assist in obturator nerve injury identification, but the optimum hip position for NDT has not been biomechanically supported. OBJECTIVES: This study evaluated the displacement and strain of the obturator nerve within the pelvis using three NDT positions of the hip (neutral, flexion, and extension) combined with hip abduction. DESIGN: One-way repeated measures. METHODS: Nine cadavers were selected by consecutive sampling, and metal markers were glued to the exposed intrapelvic

Pedicled Vascularized Common Peroneal Nerve Graft in Sciatic Nerve Reconstruction With Involvement of Inner Pelvic Lumbar and Sacral Nerve Roots: A Case Report and Literature Review.

Long segment losses exceeding 10 cm in the sciatic nerve are challenging in both the reconstructive techniques and optimizing sensory and motor function recovery. This case report and literature review describes our experience of using a pedicled vascularized common peroneal nerve graft to repair a 14-cm sciatic nerve defect, involving L4, L5, and S1 nerve roots. Additionally, we conducted a literature review of various types of nerve autografts for large sciatic nerve defects, summarizing their characteristics and outcomes to aid clinicians in decision-making and expected results in different

Lipomatosis of nerve and osseous nerve territory overgrowth patterns affecting knee: illustrative cases.

BACKGROUND: Lipomatosis of the nerve (LN) is a rare peripheral nerve pathology characterized by abundant proliferation of fat within the epineurium giving it pathognomonic features on MRI. Any nerve may be affected, but major nerves such as the median and sciatic nerves are most reported. One of the clinical hallmarks of LN is associated bony and soft tissue overgrowth occurring distal to the nerve lesion. While macrodactyly and limb length overgrowth are well known in LN, this study aims to look at nerve territory bony overgrowth affecting the knee. OBSERVATIONS: An initial search revealed 5

Monopolar electrocautery causes pelvic neurovascular injury: a histological and stereological study in rabbits.

BACKGROUND: Electrosurgical devices are widely used in penile surgery for hemostasis and tissue dissection. However, the neurovascular risks associated with monopolar electrocautery, particularly in the context of pelvic innervation and vascular supply, remain unclear. This study investigates the histopathological effects of monopolar versus bipolar electrocautery on pudendal ganglia and prostatic arteries in an experimental model. MATERIALS AND METHODS: Seventeen male New Zealand rabbits were randomly assigned to three groups: Group I (control, surgical dissection = 5), Group II (bipolar ele

Splanchnic and Pelvic Spinal Afferent Pathways Relay Sensory Information From the Mouse Colorectum Into Distinct Brainstem Circuits.

The distal colon and rectum (colorectum) are innervated by two distinct spinal (splanchnic and pelvic) afferent nerve pathways. This study aimed to identify where the sensory information relayed by splanchnic and pelvic afferents integrates within the brainstem. Microinjection of transneuronal viral tracer (herpes simplex virus-1 H129 strain expressing EGFP, H129-EGFP) into the distal colon was used to assess the brainstem structures receiving ascending input from the colorectum. H129-EGFP+ cells were distributed in structures involved in ascending sensory relay, descending pain modulation, an

Autonomic Plexuses of the Female Sexual Organs Travel Within Pubocervical and Rectovaginal [Endopelvic] Fascia: A Rationale for 'Nerve-Aware' Vaginal Prolapse Repair.

The connective tissue support of female pelvic viscera-endopelvic fascia-has been studied in fetal and immunohistochemical models to demonstrate its relationship with the autonomic nerves of the female pelvis. Due to a paucity of literature examining the gross anatomical relationships between endopelvic fascia and autonomic nerves in adult female pelvises, it remains unknown whether defects in endopelvic fascia predisposing pelvic organ prolapse and/or manipulation of endopelvic fascia during prolapse repair may be the cause of prolapse-related pelvic pain and sexual dysfunction. Through the d

Bilateral posterior tibial nerve stimulation as a neuromodulation strategy for obstructed defecation: a randomized controlled trial.

BACKGROUND: Obstructed defecation syndrome (ODS) is a prevalent pelvic floor disorder, often impairing patients' quality of life. Noninvasive therapies, including posterior tibial nerve stimulation (PTNS), have been explored as alternative treatments. This study evaluates the efficacy of bilateral transcutaneous posterior tibial nerve stimulation (BT-PTNS) compared to medical treatment alone in patients with ODS without anatomical abnormalities. METHODS: A prospective randomized controlled study was conducted on 50 patients diagnosed with ODS. Patients were randomly assigned into two groups:

Magnetic Resonance Neurography of the Lumbosacral Plexus.

Pain and weakness in the low back, pelvis, and lower extremities are diagnostically challenging, and imaging can be an important step in the workup and management of these patients. Technical advances in magnetic resonance neurography (MRN) have significantly improved its utility for imaging the lumbosacral plexus (LSP). In this article, the authors review LSP anatomy and selected pathology examples. In addition, the authors will discuss technical considerations for MRN with specific points for the branch nerves off the plexus.

The lumbosacral plexus: Anatomy, clinical imaging, and proof-of-concept insights with ultra-high-field 7 Tesla MRI.

The lumbosacral plexus is the human neural network that provides motor and sensory innervation to the pelvis and lower extremities. Injury to the lumbosacral plexus may result due to trauma, compression, diabetes, infection, inflammation, or demyelination. Patients may present with persistent or progressive pain, paresthesia, or paresis. A diagnosis can typically be established based on correlation of clinical history, physical examination, and electromyography. However, although conventional diagnostic modalities provide vital functional information pertaining to motor unit activity and muscl

Electrical pudendal nerve stimulation versus pelvic floor muscle training with transrectal electrical stimulation for post-radical prostatectomy incontinence: a cohort study.

Electrical pudendal nerve stimulation (EPNS) has been proposed as a clinic-based option for post-prostatectomy incontinence (PPI), but comparative evidence is limited. We conducted a retrospective cohort study across three medical centers (2018-2022) comparing EPNS (24 sessions over eight weeks) with pelvic-floor muscle training plus transrectal electrical stimulation (PFMT + TES). Primary outcomes were changes from baseline to end of treatment in the International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form (ICIQ-UI SF) score and 24-hr pad-test urine loss.
