Surgical treatment of the "marginal mandibular lip" deformity.

Unilateral permanent paralysis of the lower lip after section or compression of the marginal mandibular branch of the facial nerve is not rare. It may occur after trauma or as a result of parotid, ear, or upper neck surgery. Surgical treatment of the deformity--which we term the marginal mandibular lip--is indicated to improve cosmesis and restore oral competence during eating and speech. We describe our experience with partial resection of the paralyzed lower lip as an effective yet simple method of surgical correction. We also review the alternate methods of treatment for comparison.

Mimetic surgery for the paralyzed face. Neural and plastic procedures.

One hundred and thirty-eight patients with facial nerve paralyses were treated by various surgical methods. One hundred and six patients had total facial nerve paralyses and thirty-two had segmental paralyses. In the total paralysis group, 80 patients had oncologic procedures or tumor induced paralysis. In the segmental paralysis group, 21 patients had malignancies. Two patients had idiopathic facial nerve paralysis. In the total paralysis group, 75 had neural reconstructions, 7 neuromuscular pedicles and 26 muscle-fascia transpositions. The segmental paralysis group was divided into an upper

The location of descending fibres to sympathetic neurons supplying the eye and sudomotor neurons supplying the head and neck.

Evidence is given of the location in man of the fibres going to the sympathetic neurons of the lateral horn that supply the intrinsic and extrinsic muscles of the eye and the sweat glands of the head and neck. For the region of the pons and medulla, the evidence is abstracted from the literature. For the cervical spinal cord, the evidence is from our cases of anterolateral cordotomy. In the medulla, thrombosis of the artery of the fossette latérale destroys the fibres; this locates the fibres in the posterolateral retro-olivary area. But not all fibres to the sudomotor neurons lie there: some

Preliminary observations after facial rehabilitation with the ansa hypoglossi pedicle transfer.

Facial paralysis is a very disabling condition, both functionally and cosmetically. Despite the different methods of facial reanimation that have been described, there is no single method that will restore normal facial tone and motion. Of the methods available, primary neurorrhaphy is probably the most effective. The recovery period, however, is prolonged and, as a result, muscle tone and bulk may be lost. The hypoglossal-facial anastomosis is also a very reliable and effective technique but requires necessary interruption of both major cranial nerve trunks. Transfer of a neuromuscular pedicl

Surface landmarks for the identification of key anatomic structures of the face and neck.

The location of many deep anatomic structures of the face and neck can be identified by their relationship to surface landmarks. By systematically identifying the important anatomic landmarks, one can perform surgical operations with greater confidence of avoiding inadvertent damage to underlying structures.

The anatomy of the fasciae of the face and neck with particular reference to the spread and treatment of intraoral infections (Ludwig's) that have progressed into adjacent fascial spaces.

Descriptions of the fasciae of the lower half of the face and of the adjacent cervical fasciae have long been puzzling and descriptively much too complex. For this reason, medical students, young medical and dental practitioners, and at times even senior surgeons frequently do not understand the anatomy of the cervicofacial fasciae, which plays such an important role in the spread and subsequent final localization of primary intraoral infections. This article attempts to simplify the descriptions of these fasciae, in particular, their sites of origin and insertion.

Effects of nasal and laryngeal stimulation upon peripheral lung function.

Arterial hypoxemia and sudden death have been associated with the placement of posterior packs for control of severe epistaxis. There exists some controversy as to the mechanism underlying the clinically observed hypoxemia; some authors postulate a nasal pulmonary reflex as the basis for the hypoxemia. In an attempt to further investigate this theory, collateral ventilation was measured in the dog model during nasal and laryngeal stimulation. Collateral ventilation is thought to be one of the most sensitive techniques available to assess the resistive and elastic properties of the lung periphe

CT of perineural tumor extension: pterygopalatine fossa.

Tumors of the oral cavity and paranasal sinuses can spread along nerves to areas apparently removed from the primary tumor. In tumors of the palate, sinuses, and face, this "perineural" spread usually involves the maxillary division of the trigeminal nerve. The pterygopalatine fossa is a pathway of the maxillary nerve and becomes a key landmark in the detection of neural metastasis by computed tomography (CT). Obliteration of the fat in the fossa suggests pathology. Case material illustrating neural extension is presented and the CT findings are described.

Regional anesthesia in head and neck surgery.

Regional anesthesia in the head and neck area is a common and effective tool for the plastic surgeon. For short procedures, it offers the advantages of convenience and comfort to the patient. In many procedures, in addition, it offers advantages to the plastic surgeon over general anesthesia. A knowledge of anatomy of the course of the sensory nerves in the head and neck is essential to execute regional anesthesia. Some variability is always present. However, the most important factor in success is practice with a specific technique until it is mastered. No attempt was made in this article to

Major motor atrophic patterns in the face and neck: CT evaluation.

Cranial nerve deficits from various pathologic processes of the head and neck may result in characteristic patterns of denervation muscular atrophy. Such atrophic patterns may be clues to the location and extent of the lesion, particularly when cranial nerves are involved early in the course of the disease process. Thirty-six patients with computed tomographic (CT) evidence of muscular atrophy secondary to pathologic conditions involving the motor division of cranial nerves were examined. Five characteristic denervation muscular atrophy patterns seen on CT scans were identified. In several pat

Facial nerve recording from the eardrum: a possible method for evaluating idiopathic facial nerve paralysis.

A noninvasive technique has been developed for recording from the the facial nerve within the fallopian canal. Following transcutaneous stimulation of the nerve on the face, an antidromic nerve potential can be detected with electrodes placed on the eardrum and enhanced by the technique of averaging. In studies conducted on cats and normal human subjects it has been determined that the primary recording site of the nerve potential is along the tympanic segment of the nerve just distal to the geniculate ganglion. Disturbances in nerve conduction caused by an experimental lesion produced changes

Percutaneous trigeminal thermorhizotomy in treatment of malignant facial pain.

Fifty-eight patients suffering from intractable pain in the trigeminal distribution due to neoplasms of the intracranial or facial regions underwent controlled percutaneous coagulation of the Gasserian ganglion (trigeminal thermorhizotomy). Of them, 35 were evaluated at varying time intervals from the intervention (mean: 7 months). While pain was completely relieved in 71 per cent of cases at discharge, the follow-up examination was consistent with 49 per cent of analgesias. The rationale of percutaneous thermocoagulation in malignant facial pain is given and the pertinent literature summarize

Superficial fascia rhytidectomy. A restoration of the SMAS with control of the cervicomental angle.

The surgical correction of an anatomic defect should re-establish the normal relationship of the tissues which contribute to the defect and restore or improve function. When the repair of the anatomic defect is not maintained, it may be necessary to alter the tissues to accomplish a more lasting result. Restoration of the SMAS cephalad to the cervicomental angle by superficial fascia rhytidectomy with a complete transverse resection of a segment of the platysma muscle at the level of the cervicomental angle fulfills the criteria of the young face by improving the function and contours of the f

Effect of trigeminal tractotomy on dental sensation in humans.

Two patients with intractable pain of the head and neck due to cancer underwent trigeminal tractotomy and rhizotomy of the ninth and 10th cranial nerves, as well as an upper cervical dorsal rhizotomy. Postoperatively, cutaneous analgesia ws present ipsilaterally in peripheral portions of the face and the neck, but paramedian facial regions and the oral mucosa were hypalgesic. The procedures had no significant effect on dental pain perception in these patients, as tested electrically and thermally. These results are similar to those in recent reports of dental sensation after trigeminal tractot

Stimulation sites in periaqueductal gray, nucleus raphe magnus and adjacent regions effective in suppressing oral-facial reflexes.

Electrode penetrations were made in the mesencephalon and caudal brainstem at the levels of the periaqueductal gray matter (PAG) and nucleus raphe magnus (NRM) in chloralose-anaesthetized or decerebrate cats. In a systematic fashion, mesencephalic and brainstem loci at every 1 mm of vertical depth were electrically stimulated in a series of mediolateral or anteroposterior electrode penetrations to determine the lowest stimulation threshold at each locus for suppressing the digastric jaw-opening reflex evoked by tooth pulp or infraorbital nerve stimulation; at some loci, the threshold current r
