Sensitivity and Specificity of the Brachial Plexus Outcome Measure Activity Scale: A Retrospective Cohort Study.

PURPOSE: Upper limb impairments occur after incomplete recovery from brachial plexus birth injury (BPBI). Reconstructive surgeries are offered to address specific joint-level impairments; however, complete restoration is not possible. Thus, surgical decisions must carefully consider the goal of improving activity function within the child's life context. The Brachial Plexus Outcome Measure (BPOM) Activity scale is an outcome measure of upper limb activity limitation in children with BPBI. It measures the upper limb function on a 5-point ordinal scale, where a score of ≤3 is theoretically an in

Single-shot versus slow infusion interscalene block and its impact on diaphragmatic function in patients undergoing shoulder surgery, a double blind randomized controlled trial.

BACKGROUND: The Interscalene block offers adequate anesthesia and analgesia to the shoulder. Ipsilateral phrenic nerve paralysis is a common and generally well-tolerated consequence of the interscalene block. However, it may become clinically significant in patients with pre-existing respiratory conditions or limited pulmonary reserve. We hypothesized that a slow infusion of an interscalene block over 10 min would reduce the incidence of a complete phrenic nerve block. METHODS: This randomized controlled trial included 83 patients who underwent arthroscopic shoulder surgery. Patients were ran

Evaluation of nerve transfer techniques for shoulder function recovery in traumatic brachial plexus injuries: A Systematic review and individual patient data meta-analysis.

INTRODUCTION: Traumatic brachial plexus injuries can significantly impair shoulder function, particularly abduction and external rotation. Various nerve transfer techniques are available to restore these functions. This meta-analysis evaluates the efficacy of different nerve transfers for shoulder abduction and external rotation recovery to identify the most effective surgical approaches. METHODS: A literature search was conducted in PubMed and Mendeley up to June 2025 following PRISMA guidelines. Studies were included if they involved nerve transfers to the suprascapular and/or axillary nerv

Neck extension during thyroidectomy: effects on early postoperative cognitive function and optic nerve sheath diameter- a randomized controlled trial.

BACKGROUND: During thyroidectomy, patients are commonly positioned with neck extension using a shoulder roll. Excessive extension may impair arterial and venous outflow, compromise cerebral perfusion, and elevate intracranial pressure, potentially leading to postoperative cognitive impairment. This study investigated the effect of shoulder-roll-assisted neck extension on cognitive outcomes and on optic nerve sheath diameter (ONSD). METHODS: After tracheal intubation, patients were randomly assigned to either a shoulder roll group or a control group without a roll. The primary outcome was the

Preliminary Results of Coracoid Excision for Anterior Shoulder Pain in Brachial Plexus Birth Injury.

BACKGROUND: Secondary shoulder deformities are the most frequent sequelae of upper trunk-type brachial plexus birth injury (BPBI). Coracoid process dysplasia was recently described as being associated with glenoid retroversion and was recognized to block successful humeral head relocation. There are little to no reports on coracoid process deformity causing anterior shoulder pain. Our hypothesis is that coracoid process impingement causes anterior shoulder pain in BPBI, which is relieved by coracoid excision. This study presents the short-term results of this new technique in BPBI patients. M

Telehealth-Enabled In-Home Elbow Rehabilitation for Brachial Plexus Injuries Using Deep-Reinforcement-Learning-Assisted Telepresence Robots.

Due to damage to the network of nerves that regulate the muscles and feeling in the shoulder, arm, and forearm, brachial plexus injuries (BPIs) are known to significantly reduce the function and quality of life of affected persons. According to the World Health Organization (WHO), a considerable share of global disability-adjusted life years (DALYs) is attributable to upper limb injuries, including BPIs. Telehealth can improve access concerns for patients with BPIs, particularly in lower-middle-income nations. This study used deep reinforcement learning (DRL)-assisted telepresence robots, spec

Lower trapezius transfer to infraspinatus in cases of sequelae of obstetric brachial plexus injury.

BACKGROUND: Deficient shoulder function is a common and exhausting issue in children with obstetric brachial plexus injuries. Even with functioning elbow, wrist, and fingers, upper limb function is markedly disabled by limited shoulder abduction external rotation. Lower trapezius transfer carries many advantages; simple and safe technique, same line of pull as donor; reliable nerve supply (extraplexal from spinal accessory nerve), and not acting on rotation of the shoulder, mostly it will not adversely affect internal rotation range after the transfer. This study aims to evaluate the role of i
