Pneumonic plague is rare compared to bubonic plague; data on clinical presentation and treatment outcomes are scant. We report on 13 prospectively-enrolled suspected pneumonic plague cases (7 laboratory-confirmed, one probable, 5 not-plague) treated with aminoglycoside plus ciprofloxacin for 10 days and discuss findings in the context of the available literature.

Emerging respiratory infectious diseases increasingly arise at the human-animal-environment interface and challenge clinicians with rapidly evolving diagnostic, therapeutic, and infection-control decisions. This narrative review summarizes clinically relevant viral and bacterial pathogens that may present as severe respiratory disease, but are easily mistaken for common community-acquired infections during early illness. We focus on zoonotic influenza A viruses, including contemporary H5N1, Middle East respiratory syndrome coronavirus (MERS-CoV), hantaviruses, Nipah virus, and inhalational bac

Pneumonic plague remains a high-consequence respiratory disease, highlighting the need for vaccines that induce durable lung-localized immunity. We show that intramuscular prime-boost vaccination with Alum-absorbed OMV46-LcrV (OMV46-LcrV/Alum) is safe but provides limited long-term protection against respiratory Yersinia pestis challenge in mice. To enhance lung immunity, we deliver an adjuvant-free intranasal LcrV "spike" following IM OMV46-LcrV/Alum priming. This "prime-spike" regimen is well tolerated and confers complete short- and long-term protection against high-dose pulmonary Y. pestis

Pneumonic plague, caused by Yersinia pestis, remains a deadly threat due to its high mortality and rapid progression. In our previous study, Yersinia pestis antigens formulated with CpG oligodeoxynucleotides show great potential against pneumonic plague, but these agonists display low TLR9 affinity, poor stability, limited cellular uptake, and weak induction of tissue-resident memory immunity. Here, we introduce AdjCRU, a cruciform DNA nano-adjuvant built on a four-way junction with each arm presenting looped CpG motifs (LoDNA) for TLR9 engagement. Guided by computational design and molecular

Pneumonic plague is the most fatal form of plague, with a mortality rate approaching 100% within 24 h if untreated. The emergence of drug-resistant Yersinia pestis strains capable of human-to-human transmission poses a significant challenge to conventional antibiotic therapies. While therapeutic antibodies present a promising alternative, their intravenous administration is limited by low pulmonary bioavailability and suboptimal efficacy. Targeting LcrV, a key virulence factor of Yersinia pestis type III secretion system, we developed a humanized heavy-chain antibody designated LcrV-X19-R1, an

The TYK2 P1104A variant has been cast as a textbook example of evolutionary compromise-protecting against autoimmunity whilst increasing susceptibility to tuberculosis (TB). This narrative has been invoked to explain its decline in frequency in European populations since the Bronze Age. However, accumulating evidence suggests that this widely accepted trade-off may be more complex than originally proposed. At the mechanistic level, P1104A reduces TYK2 catalytic activity without abolishing cytokine signalling, owing to compensatory, non-hierarchical JAK activation. Critically, IL-12-dependent i

BACKGROUND: Yersinia pestis, a category A infectious organism, is known to cause bubonic, septicemic, and pneumonic plague. With the emergence of streptomycin-resistant strains, there is an urgent need for new therapeutic strategies that can protect populations from Y. pestis infection. The main strategy for developing vaccines and therapeutic antibodies involves F1 antigen. Previous research has demonstrated that both human and murine monoclonal antibodies(mAbs) confer protective effects against Y. pestis infection. While, no relevant studies were identified on mAbs against drug-resistance Y.

Two live-attenuated vaccines, LMA and LMP, were evaluated alone or in combination with a trivalent adenoviral vector-based vaccine (Ad5-YFV) for their protective efficacy against pneumonic plague in wild-type (WT) and interferon-γ (IFN-γ) knockout (KO) mice. LMA and LMP comprise triple deletion mutants of Yersinia pestis CO92, which causes pneumonic plague, and Ad5-YFV incorporates three protective plague immunogens. Protection of 80 to 100% was observed in vaccinated mice when challenged with highly lethal intranasal doses of parental Y. pestis CO92. All vaccinated mice generated robust humor

INTRODUCTION: Yersinia pestis is the etiologic agent of plague, and the disease is categorized into several forms, including bubonic, septicemic, and pneumonic. Plague meningitis is a rare but severe complication and estimated to occur in 6-11% of documented cases. It is most frequently observed in bubonic plague patients under 15 years old that receive inadequate or no antibiotic treatment. To date, there are no reports describing plague meningitis in laboratory animal models of pneumonic plague.
METHODS: Therefore, we sought to use the BALB/c mouse pneumonic plague model to investigate cent

Infectious bacteria remain among the most plausible agents for deliberate biological attack because they combine environmental robustness, low infectious dose, and the capacity for rapid, severe disease. This review synthesizes current knowledge on major bacterial biowarfare threats including Bacillus anthracis, Yersinia pestis, Francisella tularensis, Brucella spp., and Clostridium botulinum across three levels: transmission routes, molecular pathogenesis, and defense strategies. We summarize how aerosol, food-water, and vector-borne pathways, together with globalization and urban crowding, s

The management of patients with acute infectious diseases can present significant challenges, especially if the causative agent has a propensity for person-to-person transmission. In such cases, effective patient management is dependent on both rapid identification of disease and the provision of necessary medical care while adhering to suitable infection prevention and control measures to reduce the potential for onwards transmission. The UK has operated a defined system for managing patients with high-consequence infectious diseases (HCIDs) since the 1970s, when protocols were first implemen

Yersinia pestis, the causative agent of plague, remains a significant global health hazard and a potential top-tier biothreat despite modern medical advances. Here, two mRNA constructs encoding different versions of the low-calcium response virulence (LcrV) protective antigen, an essential virulence factor of Y. pestis, are designed and evaluated. Next, the immunogenicity and protective efficacy both independently and in combination is assessed with the previously reported F1-encoding mRNA construct in the well-established mouse model of pneumonic plague. The findings reveal that human Fc-conj

BACKGROUND: The Athens Plague (430-427 BCE) was a devastating outbreak during the Peloponnesian War, described in detail by Thucydides. Although there is ongoing discussion, the specific cause of the epidemic remains undetermined.
OBJECTIVES: This review re-examines the Athenian Plague through historical and microbiological lenses, reassessing diagnostic hypotheses and proposing the plausibility of an ancient pneumonic strain of Yersinia pestis.
SOURCES: We analysed Thucydides' original account alongside interdisciplinary literature from classical studies, paleopathology, epidemiology, and m

Yersinia pestis, the causative agent of plague, is classified as a Tier I Select Agent of bioterrorism and is among a few pathogens of high concern as a potential cause of a future pandemic. Currently, there is no licensed vaccine against plague. Previously, we developed a live attenuated vaccine candidate, rLVS ΔcapB/Yp F1-V, that utilizes a highly attenuated capB mutant of Francisella tularensis Live Vaccine Strain as a vector to express a fusion protein of Y. pestis F1 and LcrV antigens. We showed that homologous prime-boost vaccination with this vaccine provided potent protection in mice a

Madagascar is the most plague-affected country globally, yet the phylogenetic diversity of Yersinia pestis in this country remains insufficiently characterized. In this study, we analyzed whole-genome sequences of 614 Y. pestis strains, with 141 strains newly sequenced, collected over 96 years across Madagascar. All isolates emerged in 1898 and belong to the phylogroup 1.ORI3, supporting a single introduction event to Madagascar, followed by local diversification. We identified 23 distinct subgroups, including eight previously undescribed. Although most novel subgroups were rarely detected or

Although often considered a historical disease, plague remains a modern-day public health concern with global relevance. Caused by the bacterium Yersinia pestis and transmitted to humans primarily through infected flea bites or contact with infected animals, plague persists worldwide, including parts of the southwestern United States. The disease presents in three main forms: bubonic, septicemic, and pneumonic. Bubonic plague, the most common form, is characterized by painful swollen lymph nodes known as buboes. Septicemic plague occurs when the infection enters the blood stream, while pneumon

INTRODUCTION: Yersinia pestis is the etiological agent of plague, a disease that remains a concern as demonstrated by recent outbreaks in Madagascar. Infection with Y. pestis results in a rapidly progressing illness that can only be successfully treated with antibiotics given shortly after symptom onset. Live attenuated or whole cell inactivated vaccines confer protection against bubonic plague, but pneumonic plague has been more difficult to prevent. Novel effective subunit vaccine formulations may circumvent some of these shortfalls. Here, we compare the immunogenicity generated by an advanc

BACKGROUND: Plague is a high-consequence infectious disease with epidemic potential. Current treatment guidelines are based on weak evidence.
METHODS: We enrolled persons (excluding pregnant persons) in Madagascar who had clinically suspected bubonic plague during 2020-2024. Using an open-label noninferiority design, we compared two treatments included in the national plague guidelines: oral ciprofloxacin for 10 days (ciprofloxacin monotherapy) or injectable aminoglycoside for 3 days followed by oral ciprofloxacin for 7 days (aminoglycoside-ciprofloxacin). The primary end point was treatment
