Role of Radiotherapy After Breast-Conserving Surgery in T1-2N0M0 Luminal (HR+/HER2-) Breast Cancer Stratified by Tumor Quadrant Location.

BACKGROUND The survival association of adjuvant radiotherapy (RT) in early-stage node-negative luminal (HR+/HER2-) breast cancer remains under evaluation, and whether this association is modified by tumor quadrant location is unknown. MATERIAL AND METHODS We identified 5831 patients with T1-2N0M0 luminal breast cancer treated with breast-conserving surgery from the Surveillance, Epidemiology, and End Results (SEER) database (2010-2017), stratified by RT receipt. Breast cancer-specific survival (BCSS) was analyzed using Kaplan-Meier, Cox regression, and Fine-Gray competing-risk models, with a p

Primary Multifocal Neuroendocrine Tumor of the Breast: A Diagnostic Dilemma.

Primary neuroendocrine neoplasms (NENs) of the breast are extremely rare and diagnostically challenging tumors. We are reporting a female patient in her 40s with progressively enlarging, painless lumps measuring 30 mm and 15 mm in the upper and lower inner quadrants of the right breast for three months. Mammography reported it as the Breast Imaging Reporting and Data System (BIRADS) IVC lesion. A fine needle aspiration cytology (FNAC) done on the right breast lump and right axillary lymph node was reported as carcinoma with metastasis. Subsequent biopsy and immunohistochemistry (IHC) were repo

The anterior intercostal artery perforator flap in immediate oncoplastic breast reconstruction: current applications and future perspectives.

In recent years, the anterior intercostal artery perforator (AICAP) flap has gained increasing attention for its minimal donor-site morbidity, natural contour, and excellent scar concealment. This review begins by outlining the vascular anatomy of the anterior intercostal perforators, including their distribution, perfusion characteristics, and key considerations for preoperative imaging. We then summarize the indications and limitations of other commonly used chest wall perforator flaps, such as the lateral intercostal artery perforator (LICAP) and thoracodorsal artery perforator (TDAP) flaps

Pinch and quadrants: a mechanical hypothesis for the site-specific incidence of breast cancer.

Breast cancer exhibits a well-documented but incompletely explained predilection for the upper outer quadrant (UOQ), accounting for 45-50% of all cases, followed by the lower inner quadrant (LIQ) as the second most common site. We propose a mechanical hypothesis: chronic, repetitive mechanical stress from sexual partner-initiated breast manipulation - specifically the four-finger pinch grasp - creates sustained micro-injury and subsequent reparative proliferation preferentially targeting the UOQ, thereby increasing local mutational burden. The thumb opposes on the LIQ, offering a natural expla

Unmasking the Myositis: A Diagnostic Clue to Occult Breast Cancer.

Dermatomyositis (DM) is an idiopathic inflammatory myopathy characterised by proximal muscle weakness and pathognomonic cutaneous features. A well-recognised but underappreciated association exists between DM and underlying malignancy, termed cancer-associated myositis (CAM). The prevalence of malignancy in DM patients is high, with breast, ovarian, lung, and gastrointestinal cancers most commonly implicated. However, the detection of occult malignancy in young women presenting with DM remains particularly challenging due to atypical epidemiology and the limitations of standard imaging in dens

Diagnosis of Extensive DCIS with Progressive Microcalcifications over a Period of 11 Years.

A 53-year-old woman presented complaining of enlargement of her right breast; 12 years earlier she was treated elsewhere for an invasive ductal carcinoma of the right breast with breast-conserving surgery, adjuvant chemotherapy, anti-HER2 therapy, radiotherapy and endocrine therapy. One year postoperatively, a group of microcalcifications was seen in the ipsilateral breast on mammography. In the following years, the area of microcalcifications progressively increased, but instead of vacuum-assisted biopsy, the treating physicians performed core needle biopsies that were negative for malignancy

Differences in distant metastasis risk of breast cancer originating from various primary sites: a retrospective analysis based on the SEER database.

Breast cancer's primary site diversity may influence distant metastasis, yet research is scant. This study explores primary site impacts on metastasis and patient survival. We included 240,943 female breast cancer patients in the Surveillance, Epidemiology, and End Results Program (SEER) data between 2010 and 2019. The primary sites were classified as upper-inner quadrant (UIQ, C50.2), lower-inner quadrant (LIQ, C50.3), upper-outer quadrant (UOQ, C50.4), lower-outer quadrant (LOQ, C50.5), axillary tail (C50.6), overlapping region (C50.8), and the central and nipple portion (TCNP, C50.0-C50.1).

Metaplastic carcinoma masquerading as fibromatosis: A diagnostic dilemma.

Metaplastic breast carcinoma (MBC) is a rare, heterogeneous group of neoplasm of breast, accounting for about 0.2-1% of all invasive breast carcinomas. Its variants include low grade adenosquamous carcinoma, fibromatosis like metaplastic carcinoma, spindle cell carcinoma, squamous cell carcinoma and mixed metaplastic carcinoma. We report a case of a 48 year old female, who presented with painful lump in lower inner quadrant of right breast since one and half month, gradually increasing in size. Trucut biopsy was reported as Fibromatosis of breast. Excisional biopsy was consistent with malignan

Schwannoma in the lower inner quadrant of the right breast in a male patient: A rare case report.

RATIONALE: Lower inner-quadrant breast schwannomas are exceedingly rare, with no reports of their occurrence in male patients in the literature. In this report, we describe a male patient with a schwannoma in the lower inner quadrant of the right breast. PATIENT CONCERNS: A 26-year-old man presented at our hospital with a 6-month history of a lump in the right breast and reported pain in the area 3 days prior to presentation. Ultrasonography identified a 0.86 × 0.64 cm hypoechoic nodule 0.41 cm beneath the skin in the lower inner quadrant of the right breast. The nodule exhibited clear bounda

Immediate Breast Reconstruction with Latissimus Dorsi Flap and Implant: The Innovation of Flap Design and Transfer.

BACKGROUND: Modified radical mastectomy is the mainstream surgical treatment for breast cancer in which nipple-areolar complex and fusiform skin may be excised due to cancer invasion. Latissimus Dorsi (LD) flap combined with prosthesis implantation is a common method of immediate breast reconstruction after modified radical mastectomy. In this study, we introduced the design and transfer of flap based on the mastectomy incision for better practicality and appearance. METHODS: From January 2016 to December 2022, a retrospective analysis was performed in patients who received immediate breast r

Trends in Dose to the Contralateral Breast from Breast Cancer Radiotherapy in the United States.

Contralateral breast (CB) cancer is the most common subsequent cancer among breast cancer survivors, and radiotherapy has been linked to CB cancer risk. The purpose of this work was to evaluate doses to subregions of the contralateral breast from historical breast cancer treatments carried out in the United States between 1990 and 2012. We extracted treatment data from radiation therapy summaries for 2,442 radiotherapy patients during that period. We estimated CB doses for five breast regions: the upper inner quadrant (UIQ), lower inner quadrant, upper outer quadrant, lower outer quadrant (LOQ

Combination of Hatchet and Anterior Intercostal Artery Perforator Flaps in Oncoplastic Breast-conserving Surgery.

Oncoplastic breast-conserving surgery aims to achieve oncological safety and aesthetic preservation. Here, we report a novel hybrid oncoplastic breast-conserving surgical technique that combines volume displacement and replacement using hatchet and anterior intercostal artery perforator (AICAP) flaps. A 71-year-old woman with centrally located breast cancer extending to the lower inner quadrant underwent wide local excision, incorporating the nipple-areolar complex, resulting in a 90-mm cylindrical defect. Reconstruction was planned using a hatchet flap elevated from the inferolateral breast a

Surgical Management of Ipsilateral Breast Cancer Recurrence After Conservative Mastectomy and Prepectoral Breast Reconstruction: Exploring the Role of Wide Local Excision.

Background: Conservative mastectomy with prepectoral breast reconstruction is becoming increasingly widespread and validated in recent years. Today, while aesthetic advantages and improvement in quality-of-life outcomes are widely acknowledged, oncological safety remains subject of debate. There is limited evidence on residual breast tissue after conservative mastectomy, and it still represents an unknown risk for local recurrence. The recent spread of this surgical technique precludes a standardized surgical approach in case of local recurrence of ipsilateral breast cancer, and the lack of ev

Radiation Therapy for Breast Cancer With Oligometastatic Cervical Lymph Nodes: A Case Report.

Stage IV breast cancer is difficult to cure and is mainly treated with systemic therapy. However, when distant metastasis is oligometastatic, proactive treatment including local therapies for the primary lesion and distant metastases has been reported to improve prognosis. We encountered a patient who had left breast cancer with ipsilateral cervical lymph node metastases. The metastases were oligometastatic, and we treated them curatively. The patient was a female in her 50s who had been aware of a lump in the lower inner quadrant of the left breast for a few years. A biopsy was performed and
