Ovarian endometrioid adenofibroma in a perimenopausal woman with intermittent vaginal bleeding: a case report.

Ovarian endometrioid adenofibroma (EA) is an exceptionally rare benign epithelial tumor of the ovary with nonspecific clinical and imaging manifestations, and its diagnosis relies mainly on pathological examination. This case describes a 55-year-old perimenopausal woman with a 1-year history of intermittent vaginal bleeding and a progressively enlarging right adnexal mass. Imaging revealed a solid mass in the right adnexal region with minimal blood flow signal. Serum tumor markers were within normal ranges, and the sex hormone profile showed no evidence of estrogen excess. Serial ultrasonograp

Beyond endometrial thickness: platelet-rich plasma and reproductive outcomes in recurrent implantation failure following euploid embryo transfer.

BACKGROUND: Recurrent implantation failure (RIF) after euploid embryo transfer suggests that endometrial receptivity, rather than embryo competence alone, may be rate-limiting for pregnancy. Autologous platelet-rich plasma (PRP) has been proposed as a biological adjuvant for reproductive outcomes, but evidence in women with normal endometrial thickness remains limited. We investigated whether intrauterine PRP is associated with higher implantation rates in women with RIF and normal endometrial thickness undergoing single euploid blastocyst transfer. METHODS: Prospective observational cohort s

Systemic complement C3 reduction in infertile women with endometrial dysbiosis: preliminary observations.

CONTEXT: Endometrial dysbiosis has been linked to infertility, but its relationship with systemic immune activity remains unclear. AIMS: To examine whether endometrial microbiota phenotype is associated with serum complement profiles in infertile women. METHODS: We enrolled 105 infertile women and classified endometrial microbiota by 16S rRNA gene sequencing as Lactobacillus-dominant microbiota (LDM) or non-LDM (NLDM). Complement data were available in 27 women: after excluding cases with missing C-reactive protein (CRP; n = 2) or CRP above the 95th percentile (n = 2), 23 women were analyzed

Effects of Progestins and Progesterone Receptor Modulators Used for Emergency Contraception on Endometrial Gene Expression and Implantation.

The human endometrium responds to progesterone, including emergency contraception (EC) drugs such as levonorgestrel (LNG), mifepristone, and ulipristal acetate (UPA). Here, we investigated gene expression during the window of implantation (WOI) in mammals and compared it with that of women receiving LNG, UPA, and mifepristone across a wide variety of experimental models and conditions. Evidence from in vivo models, ex vivo endometrial platforms, embryo-endometrium co-cultures, and stem cell-derived trophoblast systems is presented. The findings from single-cell and spatial transcriptomic studi

Endometrial Thickness and Ectopic Pregnancy Risk Following in vitro fertilization-embryo transfer: A Systematic Review and Meta-Analysis.

BACKGROUND: Endometrial thickness is a significant determinant of pregnancy outcomes following in vitro fertilization-embryo transfer (IVF-ET). However, the relationship between endometrial thickness and the risk of ectopic pregnancy after IVF-ET remains debated. METHODS: We conducted a comprehensive literature search of the PubMed, Cochrane Library, and Embase databases to identify relevant studies. In fresh embryo transfer cycles, endometrial thickness was measured on the day of hCG triggering; in frozen-thawed embryo transfer cycles, measurement was performed during the endometrial prepara

Ejiao (Asini Corii Colla) promotes thin endometrium repair via upregulation of the SDF-1/CXCR4 signalling axis in a mouse model.

BACKGROUND: Thin endometrium (TE) severely impairs embryo implantation and pregnancy outcomes in reproductive medicine, while first-line oestrogen-based therapy has marked limitations including receptor insensitivity and systemic side effects. Ejiao (Asini Corii Colla), a classic Traditional Chinese Medicine tonic, is widely used for gynecological disorders, yet its mechanism in endometrial regeneration remains unclear. This study aimed to investigate the therapeutic effect of Ejiao on TE and its regulation of the SDF-1/CXCR4 signalling axis. METHODS: A TE mouse model was established via intr

FSP1+ stromal cell-associated macrophage efferocytosis contributes to impaired decidualization in adenomyosis.

STUDY QUESTION: Does disrupted cell crosstalk contribute to embryo implantation failure in adenomyosis, and what is the underlying molecular mechanism? SUMMARY ANSWER: Expanded Fibroblast-Specific Protein 1 (FSP1)+ stromal cell populations in adenomyosis may promote GAS6-associated macrophage efferocytosis, which correlates with increased macrophage TNF-α secretion and senescence of adjacent stromal cells. WHAT IS KNOWN ALREADY: Adenomyosis impairs endometrial receptivity and decidual transformation, contributing to implantation failure and pregnancy loss. Women with this condition exhibit i

Endometrial tuberculosis mimicking gynecological malignancy in a postmenopausal woman.

BACKGROUND: Female genital tuberculosis is a rare form of extrapulmonary tuberculosis that may mimic gynecologic malignancy, particularly when clinical and imaging findings are nonspecific. Diagnosis can be especially challenging in postmenopausal women. CASE PRESENTATION: A 78-year-old postmenopausal woman presented with pelvic pain and was found to have marked endometrial cavity distention, hematometra, and a cervical lesion concerning for malignancy. Severe cervical stenosis prevented adequate endometrial sampling despite examination under anesthesia, attempted hysteroscopy, and dilation a

Cervico-vaginal ISGs and interferon tau: emerging biomarkers for early pregnancy detection in cattle.

Early pregnancy detection in cattle is essential for improving reproductive efficiency, reducing days open, and enabling timely rebreeding of non-pregnant animals. Although several pregnancy diagnostic methods are available, none can reliably identify pregnancy before Day 21 post-insemination. Interferon tau (IFNT), the pregnancy recognition signal produced by the ruminant conceptus, initiates maternal responses through the induction of interferon-stimulated genes (ISGs). While this response is most pronounced in the endometrium, recent studies have shown that the cervix and vagina also respon

Dynamic Changes in Endometrial Folding and Secretory Activity Across the Menstrual Cycle.

UNLABELLED: Embryo implantation remains a major limitation of assisted reproductive technology, with failure occurring in approximately 30% of euploid embryo transfers. Implantation requires a synchronized dialogue between the blastocyst and receptive endometrium during the window of implantation (WOI), yet minimally invasive approaches to characterize the structural and molecular features of receptivity remain limited. We analyzed paired sonohysterogram images and uterine lavage samples collected during the proliferative and mid-secretory phases from subjects with regular ovulatory cycles and

Quantitative photoacoustic endoscopy reveals early microvascular biomarkers of endometrial fibrosis in a rat model.

Endometrial fibrosis, the core pathological feature of intrauterine adhesion, lacks effective early assessment methods. This study used a quantitative photoacoustic endoscopy (PAE) method to assess the relationship between early PAE-derived features and late-stage fibrosis by characterizing microvascular and inflammatory dynamics in the injured endometrium. PAE captured dynamic endometrial microvascular perfusion changes post-injury, with distinct late vascular remodeling patterns under mild and severe injury. PAE-derived vessel area and mean E lacunarity correlated strongly with histologicall

A Single-Cell Transcriptomic Map Reveals Platelet-Rich Plasma-Induced Endometrial Microenvironment Remodeling in Recurrent Implantation Failure.

Intrauterine infusion of platelet-rich plasma shows clinical promise as a regenerative therapy for patients with repeated implantation failure, yet the precise cellular and molecular mechanisms governing its in vivo effects on the human endometrium remain poorly understood. To elucidate these mechanisms at high resolution, we performed single-cell RNA sequencing on paired endometrial biopsies collected from patients during the proliferative phase, both before and shortly after intrauterine platelet-rich plasma infusion. Our analysis reveals that platelet-rich plasma acts as a potent modulator

Trypanosoma brucei infection remodels the uterine immune environment and drives neuroendocrine dysfunction.

Human African Trypanosomiasis (HAT) or sleeping sickness is a systemic parasitic infection caused by the protozoan parasite Trypanosoma brucei. HAT is associated with substantial immunological, metabolic, and neurological pathology. Although reproductive dysfunction has previously been recognised in both human and experimental T. brucei infection, whether parasites can directly infiltrate the female reproductive tract (FRT), and how infection may reshape the FRT immune landscape remains poorly understood. Using a murine model of T. brucei infection we reveal that parasites are localised in the

Pregnancy establishment in cattle without embryonic interferon tau.

In ruminants, embryonic interferon tau (IFNT) is widely considered the indispensable signal for maternal recognition of pregnancy, driving endometrial gene expression to prevent luteolysis. However, this canonical paradigm has never been directly tested using genomic loss-of-function models. Here we show that IFNT is completely dispensable for pregnancy recognition, luteal maintenance, and full-term gestation in cattle. Using CRISPR-Cas9, we generate bovine fibroblasts lacking all functional copies of the multigene IFNT locus for somatic cell nuclear transfer. IFNT-null embryos develop into bl

Clinicopathological Profile of Disordered Proliferative Endometrium in Anovulatory Abnormal Uterine Bleeding and Determinants of Response to Levonorgestrel-Releasing Intrauterine System Therapy.

Disordered proliferative endometrium is an under-recognized endometrial lesion, with limited evidence regarding its clinicopathological characteristics and predictors of treatment response. The present study aimed to characterize the clinical and histomorphological features of disordered proliferative endometrium, evaluate the response to treatment with a levonorgestrel-releasing intrauterine system, and identify the clinicopathological determinants of progesterone responsiveness. A 3-year cross-sectional study was conducted involving 124 women with histopathologically confirmed disordered pro

Development of a procedure for the simultaneous detection of mRNA markers in menstrual blood, vaginal fluid, and peripheral blood for the purpose of discriminating between menstruation and traumatic vaginal injury in sexual assault cases.

Blood from menstruation or traumatic hemorrhage is often detected in vaginal fluid in sexual assault cases. Discriminating peripheral blood from menstrual blood in a biological sample collected from a crime scene can help to determine whether sexual assault with traumatic injury has occurred. In a previous study, novel mRNA markers for menstrual blood that are specifically expressed in the endometrium were investigated and evaluated for their detectability and specificity by quantitative RT-PCR. In the present study, we developed the MB-VF-PB multiplex RT-PCR procedure for the simultaneous det

Heat shock transcription factor-1 and heat shock protein 90: insights into post-insemination endometritis.

CONTEXT: In mares, post-breeding endometritis is a major cause of embryo mortality. In other species, endometrial function and pathology seem to involve heat shock protein-90 (HSP90), a factor regulated by heat shock transcription factor-1 (HSF-1). AIMS: To study the relationships between HSP90 and HSF-1 and post-insemination endometritis. METHODS: Endometrium was sampled at oestrus without artificial insemination (AI) and at 2 h and 4 h after AI. Immunohistochemistry and image analysis were used to localise and quantify HSP90 and HSF-1 in luminal and glandular epithelium; in stroma, immuno-

What Stops Menstrual Bleeding? Hemostatic Mechanisms in the Menstruating Endometrium.

Menstrual bleeding is one of the most common physiological bleeding events in humans, yet the mechanisms that terminate bleeding remain poorly defined. Current models of menstruation focus primarily on progesterone withdrawal, inflammation, vascular changes, tissue breakdown, local coagulation and fibrinolysis, and endometrial repair. However, considerably less is known about how hemostasis is controlled at individual bleeding vessels exposed during endometrial shedding. The functional endometrium is supplied by spiral arterioles that terminate in a dense capillary network, and their distal se
