The consequences of the extremely low frequency electromagnetic field (ELF-EMF) exposure on physiological processes in the uterus: novel insights and implications.

Extremely low frequency electromagnetic field (ELF-EMF), ranging from 1 Hz to 300 Hz, is prevalent in modern environments, yet its biological effects on the production remain insufficiently explored. This mini-review summarizes recent findings on ELF-EMF-induced alterations in the biological processes involved the uterus. It has been documented that the exposure to the ELF-EMF has been linked to significant changes in alterations of transcriptomic profile, histone modifications, DNA methylation, and microRNA pathways in the uterus, which may disrupt uterine contractility, secretory activity an

Revised Two-Stage Model of Preeclampsia Based on Autophagic Dysfunction: A Comprehensive Review.

A revised two-stage model of preeclampsia is proposed, centering on an autophagy-dependent requirement for extravillous trophoblast entry into the proximal one-third of the myometrium. The One-Third Myometrium Enigma, introduced here, denotes the unresolved physiological rule that early placentation requires trophoblasts to traverse decidua and reach the proximal one-third of myometrium under hypoxia and nutrient scarcity. The hypothesis posits a timed rise in basal autophagy to sustain trophoblast energy homeostasis and invasion, accompanied by TFEB-driven lysosomal programs that enable villo

Probiotic Modulation of the Gut-Ovary and Gut-Myometrium Axes: An In Vitro Study.

Emerging evidence suggests that gut microbiota significantly influence female reproductive health by affecting hormonal, immune and metabolic processes. This research explored how a probiotic blend comprising Lactobacillus crispatus novaLCR6, Limosilactobacillus fermentum novaLF58 and Bifidobacterium bifidum novaBBF9 affects the gut-myometrium and gut-ovary axes. Intestinal epithelial cells were exposed to individual probiotics or their combination using a Transwell® setup; their effects on barrier integrity, probiotic activity and short-chain fatty acid production were measured. Subsequently,

Does contained manual morcellation in minimally invasive hysterectomy prevent myometrial cell spillage? Evidence from sequential pelvic washings.

BACKGROUND: Concerns regarding tissue dissemination during morcellation have led to the adoption of containment systems during minimally invasive hysterectomy. However, the timing and extent of potential myometrial cell spillage during contained manual morcellation remain unclear. OBJECTIVE: To evaluate the presence and timing of myometrial cell spillage during minimally invasive hysterectomy with contained sharp morcellation using sequential pelvic washings. STUDY DESIGN: This was a prospective single-center observational study of premenopausal patients undergoing minimally invasive hystere

Selected calix[4]arene as an effective modulator of mitochondrial function and smooth muscle contractility.

Macrocyclic nanocompounds, particularly calix[4]arenes, are attracting growing interest in biomedical and molecular biotechnology research. Calix[4]arene C-715, functionalized with trifluoroacetamide groups on the upper rim, does not affect Ca2+ transport systems of the plasma membrane or sarcoplasmic reticulum, nor the ATPase activity of the myosin head, suggesting a selective action on mitochondrial Ca2+ transport. This study investigated the effects of C-715 on electron transport chain activity, Ca2+ handling, and nitric oxide production in isolated mitochondria of uterine smooth muscle, al

The impact of adenomyosis localization in myometrium on fertility and pregnancy outcomes: a narrative systematic review of the literature.

BACKGROUND: Adenomyosis can be categorized using various systems based on its extent, location, and severity. Depending on its location within the endometrium adenomyosis can be classified as intrinsic (inner myometrium) or extrinsic (outer myometrium). This narrative systematic review aims to comprehensively analyze existing literature to determine whether a significant correlation exists between the localization of adenomyosis within the uterus and infertility. Additionally, it evaluates the impact of intrinsic and extrinsic adenomyosis on outcomes of medically assisted reproduction, with a

A novel technique for transcervical biopsy of the myometrium†.

Our objective was to establish a minimally invasive technique for transcervical myometrial biopsy suitable for tissue collection from the nonpregnant or immediately post-pregnancy human uterus. We recruited pregnant patients undergoing surgical abortion up to 26 weeks, pregnant patients undergoing cesarean section at term, and nonpregnant patients of reproductive age undergoing laparoscopic bilateral salpingectomy for permanent contraception. Immediately following completion of the primary procedure, we introduced the CorVocet™ Biopsy System (Merit Medical, South Jordan, Utah) transcervically

Trophoblast invasion biology: from normal implantation to accreta spectrum and choriocarcinoma.

Trophoblast invasion is a finely regulated physiological process that demonstrates controlled cellular migration and vascular remodeling during human pregnancy. During normal implantation, cytotrophoblasts differentiate into extravillous trophoblasts (EVTs), which enter the maternal decidua and remodel spiral arteries, converting them into low-resistance vessels required for fetal nutrition. This balance of invasion and restraint promotes appropriate vascular adaptation at the maternal-fetal interface. Aberrations in this mechanism result in a range of pathological states, from excessive but n

Adenomyosis Pathophysiology, Diagnostic Advances, and Therapeutic Options.

Adenomyosis is a heterogeneous, nonmalignant uterine disorder characterized by ectopic endometrium within the myometrium, leading to abnormal uterine bleeding, dysmenorrhea, chronic pelvic pain, and subfertility. Once considered a disease of multiparous women older than age 40 years, advances in imaging have expanded recognition to younger reproductive-aged patients. Pathogenesis involves sex steroid imbalance, inflammation, fibrosis, neuroangiogenesis, and stem cell-mediated mechanisms. Clinical presentation ranges from asymptomatic to severe pain and bleeding. Coexisting leiomyomas and endom

Placenta Accreta Spectrum in a Patient Without Prior Cesarean Delivery: A Case of Posterior Placenta Previa.

Placenta accreta spectrum (PAS) is a potentially fatal obstetric condition characterized by abnormal placental adherence to the myometrium. It is strongly associated with prior cesarean delivery or major uterine surgery; however, a minority of cases occur in patients without these histories. We report the case of a 34-year-old, gravida 3, para 2 (G3P2002) woman with no history of cesarean delivery and only a prior dilation and curettage, who presented at 32 weeks and 3 days of gestation with complete posterior placenta previa and imaging findings concerning for PAS. Ultrasonography revealed pl

Clinical Outcomes of Laparoscopic Folding Suture for Cesarean Scar Defect and Its Impact on Pregnancy Outcomes: A Retrospective Study.

BACKGROUND: Cesarean scar defect (CSD) is characterized by localized thinning of the myometrium due to defective healing of a previous cesarean section incision. It is frequently associated with abnormal uterine bleeding (AUB) and secondary infertility, and high-quality evidence regarding its surgical management is lacking. As a novel reconstructive technique, the efficacy of laparoscopic CSD folding suture in alleviating AUB and improving fertility outcomes remains to be validated. PURPOSE: In the absence of high-quality evidence, this study aimed to provide preliminary evidence on the effic

Posterior placenta accreta spectrum: unraveling distinctive clinical features and diagnostic challenges.

BACKGROUND: Posterior placenta accreta spectrum (PAS) represents a diagnostically challenging subtype of PAS, which has long been overlooked in existing literature due to its relatively low incidence and nonspecific clinical manifestations. METHODS: A comprehensive literature search was conducted in PubMed, Web of Science, and other databases, focusing on studies related to the diagnosis and management of posterior PAS. This review systematically analyzes the risk factors, prenatal diagnostic characteristics, and pregnancy outcomes of posterior PAS, with a focus on clarifying the characterist

Pathogenesis of Adenomyosis: An Integrated Review of Cellular Origins, Molecular Mechanisms, and Intersecting Diseases.

Adenomyosis is a prevalent disorder of the archimetra, historically conflated with endometriosis but possessing a unique pathobiological trajectory. This review synthesises current molecular evidence to propose a unified mechanistic framework initiated by tissue injury and repair (TIAR), aberrant stem cell activation, or de novo metaplasia, all of which converge to disrupt the endometrial-myometrial interface disruption (EMID). Following this structural breach, a self-perpetuating 'vicious cycle' is triggered, driven by four interconnected axes: (1) Hormonal Dysregulation, characterised by loc

Case Report: presence of granulosa cells in the uterine tissue of a spayed Labrador Retriever bitch.

A 10-year-old Labrador Retriever bitch, spayed at 2 years of age, was presented to our veterinary care facility with a history of vulvar swelling, vaginal discharge, and male attraction for over 1 month. Vaginoscopy revealed edema and reddening of the vaginal mucosa. Vaginal cytology was consistent with estrus, while serum progesterone levels remained at baseline. On abdominal ultrasound, the uterus was hyperplastic, and fluid-filled cysts were visualized at both ovarian anatomic sites. Hysterectomy and excision of tissue from both ovarian sites were performed 10 days after the initial present

A novel technique of robotic excision of caesarean scar ectopic pregnancy with near-infrared guidance using the da Vinci Xi system.

Surgical management of caesarean scar ectopic pregnancy (CSEP) can cause severe bleeding and bladder injury, especially when the vesicouterine plane is scarred with minimal residual myometrium. A 36-year-old multigravida presented at 5 weeks 3 days period of gestation with vaginal bleeding. Beta-human chorionic gonadotropin (β-hCG) level was 23,190 mIU/mL. Imaging confirmed CSEP in the lower uterine segment with residual myometrial thickness of 1 mm. We performed a robotic-assisted excision of CSEP with Firefly technology for delineation of bladder margins, hence improving procedure safety and

Potassium depolarized rat uterus in response to oxytocin and its structural analogues: kinetic analysis.

The subject of this communication is the kinetic analysis of dynamic processes in the myometrium of rats in a depolarized state, with a focus on their biomechanics and possible implications in obstetrics. Depolarization was achieved by increasing the concentration of K+ in the organ bath. It is attended by a fast transient increase in muscle tension and the slower establishment of a new resting state in which the phasic component of contractile activity is suppressed. The myometrium remains stimulable by oxytocin and its structural analogues. Time-tonic response profiles indicate participation
