Laparoscopic transabdominal pre-peritoneal hernia repair with bilateral orchidectomy in partial androgen insensitivity syndrome.

Partial androgen insensitivity syndrome (PAIS) is a rare X-linked recessive disorder in which individuals with a 46,XY karyotype exhibit a phenotypically female appearance due to end-organ resistance to androgens. We present a 21-year-old phenotypic female with left groin pain and a palpable right labial mass. Clinical findings included normal breast development, scant pubic and axillary hair, primary amenorrhea, clitoromegaly resembling a microphallus and bifid scrotum-like labia. Imaging revealed a left inguinal undescended testis with hernia and a right testis in the right labia majora, wit

Recurrent pancreatitis and atypical hemolytic uremic syndrome (aHUS): an unusual presentation in childhood.

Hemolytic uremic syndrome (HUS) is a thrombotic microangiopathy characterized by the classical triad of acute hemolytic anemia, thrombocytopenia, and kidney impairment. We report a 10-year-old boy with acute pancreatitis presenting simultaneously with atypical HUS (aHUS) with two such episodes occurring 1 year apart. The child presented with abdominal pain, vomiting, oliguria, epigastric tenderness, and had a right undescended testis. During the initial episode, anti-factor H antibodies were mildly elevated while they were normal in the subsequent episode with normal complement components. Who

An international multicenter cohort study of management and outcomes of undescended testis in German-speaking countries.

BACKGROUND: Undescended testis (UDT) is the most common congenital genitourinary anomaly in boys. Guidelines recommend orchidopexy by 12-18 months to optimize fertility and reduce malignancy risk, but adherence is often poor. The objectives of this study were to asses institutional management for UDT and evaluate surgical practices and long-term patients outcomes following orchidopexy. METHODS: We conducted a multicenter observational study in German-speaking countries using a standardized questionnaire on UDT management and collected retrospective data on all patients who underwent orchidope

Investigation of ELF5, KIF18A, NPTX1 and COL23A1 genes in the pathophysiology of indirect inguinal hernia in children.

PURPOSE: In this study, we investigated the effect of ELF5, KIF18A, NPTX1 and COL23A1 genes in residual processus vaginalis (PV) the main factor in the development of indirect inguinal hernia (IIH) in children. METHODS: Cases operated for IIH in children aged 0–18 years between 2018 and 2021 constituted the study group, and cases with undescended testis without hernia the control group. Protein levels of KIF18A, which has the highest gene expression in tissue samples, were also analyzed by ELISA method. Polymorphisms of ELF5, KIF18A and COL23A1 genes with the highest level of expression change

Prenatal exposure to pesticides and the risk of male genital malformations: A scoping review.

INTRODUCTION: Hypospadias and undescended testis (UDT) are common genital malformations with multifactorial origins. Endocrine-disrupting chemicals, including pesticides, are suspected contributors, but prior reviews are outdated, and none has mapped evidence for both conditions together. OBJECTIVE: To map evidence on the association between prenatal pesticide exposure and hypospadias/UDT, with particular attention to underexplored exposures such as pyrethroids and household pesticides, and to identify research gaps. METHOD: Following PRISMA-ScR guidance, we searched PubMed, Web of Science,

Anatomical position as a predictor of hormonal therapy failure in palpable undescended testes.

BACKGROUND: Hormonal therapy for cryptorchidism shows limited long-term efficacy and is not routinely recommended by current guidelines, yet it continues to be used in practice. Evidence identifying anatomical subgroups with predictably poor outcomes remains limited. This study evaluated whether anatomical position predicts failure of hormonal therapy in palpable undescended testes. METHODS: This single-center retrospective cohort study included boys ≤ 14 years who received human chorionic gonadotropin (hCG) therapy for palpable undescended testes between 2015 and 2024. Testes were classified

Unusual Site of Solitary PSMA-Positive Recurrence of Prostate Cancer in an Undescended Testis.

Prostate cancer is the most frequently diagnosed malignancy among men. Biochemical recurrence occurs in ∼20%-40% of patients and poses a significant clinical challenge. PSMA-PET/CT has emerged as a highly sensitive and specific imaging modality for detecting recurrent and metastatic prostate cancer, outperforming conventional imaging, especially at low PSA levels. We report an unusual site of metastatic recurrence detected on [¹⁸F]-Piflufolastat PSMA PET/CT in a patient with rising PSA. This case highlights the crucial role of advanced molecular imaging in accurately identifying atypical metas

Yolk sac tumor after post-detorsion changes in a torsed undescended testis: a case report.

BACKGROUND: Cryptorchidism complicated by testicular torsion is an uncommon but clinically important emergency in pediatric urology. Cryptorchidism is also a recognized risk factor for testicular germ-cell tumors. However, malignancy arising or becoming clinically apparent after testis-sparing detorsion in a torsed undescended testis is rarely reported. In particular, postoperative heterogeneous parenchymal echogenicity is often interpreted as benign post-ischemic change, which may reduce vigilance for occult malignancy. CASE DESCRIPTION: We report the case of a 2-year-5-month-old boy who und

[A CASE REPORT OF A METACHRONOUS BILATERAL TESTICULAR TUMOR AFTER LEFT UNDESCENDED TESTIS SURGERY].

A 53-year-old man underwent testicular fixation for a left undescended testicle at the age of 10 and a right high orchiectomy for a testicular tumor at the age of 35. He presented to our hospital with awareness of swelling in the left scrotal contents for the past month. Scrotal ultrasonography revealed a left testicular tumor, and contrast-enhanced abdominal CT showed a heterogeneous mass in the left scrotum with infiltration of the scrotal skin, as well as pathologically enlarged left inguinal and para-aortic lymph nodes. The pathology confirmed seminoma. The patient was treated with four co

Could miRNAs be used as markers for distinguishing undescended testes from retractile testes? A prospective controlled pilot/exploratory study.

BACKGROUND: Undescended testes (UDTs) are common in male infants. Untreated UDT poses risks such as infertility (IF), testicular cancer (TC), and testicular torsion (TT). Retractile testes (RTs) sporadically ascend from the scrotum. UDT requires early surgical correction, whereas RT requires only periodic follow-up. Differentiating these conditions is challenging, making clinical biomarkers potentially useful. The aim of our study was to examine the use of miRNAs as biomarkers in the differential diagnosis of UDT and RT which are difficult to differentiate. METHODS: This prospective pilot/exp

Optimizing Care for Undescended Testicles in Children and Adolescents-Diagnosis, Management, and Outcomes: A Narrative Review of Current Evidence.

Cryptorchidism is the most prevalent congenital anomaly of the male genitourinary tract, with an incidence of approximately 1 to 9 percent in full-term male infants, decreasing with age due to spontaneous descent. It encompasses testes that fail to descend into the scrotum, which may be intra-abdominal, inguinal, or ectopic, and can be associated with syndromic, genetic, or environmental factors. The descent process occurs in two phases: intra-abdominal, driven by gubernacular development and androgen-independent mechanisms, and inguinoscrotal, regulated by hormonal and mechanical factors incl

Accuracy of Referral for Undescended Testes in Paediatric Patients Referred to Specialist Paediatric Surgical Centres: A Multicentre Prospective Cohort Study.

AIM: Undescended testes (UDT) are a common congenital disorder in boys, yet delays in diagnosis and referral still occur, especially in low-and middle-income countries where access to specialist care may vary. Our study aimed to examine the accuracy and timing of referrals of UDT to paediatric surgical services across different healthcare settings and to evaluate knowledge regarding UDT among paediatric and family medicine trainees. METHOD: A prospective cohort study was conducted from January to December 2022 involving patients referred for UDT at three paediatric surgical centres representi

The potential role of dynein heavy chain genes in the pathophysiology of undescended testis.

BACKGROUND: Failures of testicular descent may result in the condition known as an undescended testis (UDT). While mechanical and hormonal causes have been researched quite extensively, not much is known about the underlying genetics. Developmental processes dependent on ciliary function may involve outer dynein arm heavy chain genes. Its role in testicular descent has never been explored. This study attempts to investigate the potential role of Outer Dynein Arm Heavy Chain (ODNAH) genes in UDT. METHODS: 24 male juvenile UDT patients and 24 age-matched controls undergoing circumcision were in

Transverse testicular ectopia with fused vas deferens: A systematic review.

BACKGROUND: Transverse testicular ectopia (TTE) with fused vas deferens is an extremely rare anomaly, often diagnosed intraoperatively. Current TTE classifications do not address internal ductal variations, limiting surgical guidance. OBJECTIVE: To systematically review cases of TTE with fused vas deferens, summarize presentation, operative strategies, outcomes and identify patterns that highlight the need for classification refinement. METHODS: A PRISMA 2020-compliant systematic review (PROSPERO; CRD420251247785) was performed across PubMed, ScienceDirect and citation of included articles t

Comparison of Microvascular Flow Imaging With Other Doppler Methods in Ultrasonography for Assessment of Parenchymal Perfusion in Undescended Testis in Young Children.

BACKGROUND: Cryptorchidism is a common congenital anomaly in male children, where accurate assessment of testicular vascularity is important for imaging evaluation and characterization of intratesticular blood flow. Conventional Doppler techniques have limitations in detecting low-velocity microvascular flow, particularly in small pediatric testes. These limitations may affect the ability to adequately visualize subtle vascular signals using standard ultrasound techniques. METHODS: This cross-sectional study was conducted at a tertiary care center over six months and included 40 children aged

Intratesticular Fetus in Fetu Arising from an Intra-Abdominal Undescended Right Testis: A Case Report.

INTRODUCTION: Fetus in fetu (FIF) is a rare congenital anomaly. The most common location of FIF is the retroperitoneum. FIF of testicular origin is extremely rare, with only 9 cases reported previously. We present a case of intratesticular FIF within an intra-abdominal undescended right testis and review previously reported cases to discuss the possibility of intratesticular FIF. CASE PRESENTATION: A male neonate was born through normal vaginal delivery at 37 weeks and 5 days of gestation, with a birth weight of 2648 g. An abdominal mass suspected to be a FIF was prenatally detected. At 26 we

Influence of Epididymal Anomalies and Development of Processus Vaginalis on Testicular Migration And Cryptorchidism. A Narrative Review.

The objective of this narrative review is to show the most relevant aspects of the structure of epidydimal anomalies (EAs) and processus vaginalis (PV) during testicular migration and in patients with undescended testis. Smooth and striated muscles develop around the processus vaginalis (PV) within the gubernaculum during fetal life. The testis migrates through the PV by the means of peristaltic activity generated by those muscles. After migration is completed, the smooth muscle undergoes programmed cell death, resulting in obliteration of PV and leaving only the dartos muscle. Initiation of p
