Clinical efficacy of single-port transscrotal laparoscopic orchiopexy combined with hernia needle for palpable undescended testes in children.

OBJECTIVE: To investigate the clinical efficacy and safety of single-port transscrotal laparoscopic orchiopexy combined with a hernia needle (SPLT-Orchiopexy) in children with palpable middle- and low-position undescended testes. METHODS: A retrospective analysis was conducted on 200 children with palpable middle- and low-position undescended testes who underwent surgical treatment at our hospital between March 2021 and April 2023. Patients were categorized into two groups according to the surgical technique used: the conventional laparoscopic surgery (CLS, n = 120) group and the SP-Orchiopex

Modified testicular traction in intra-abdominal testes, do we really need to wait for 6 weeks?

INTRODUCTION: We previously published our modified testicular traction technique, including an intracorporeal suture, gubernacular preservation, and minimal dissection. We used to fix the testis to a mobile part of the abdominal wall to enhance lengthening with respiratory movement rather than fixing near a bony landmark. This study compares our approximately 7-day traction period to the traditional 6-week period adopted earlier. PATIENTS AND METHODS: Patients with intra-abdominal testes who underwent intracorporeal placement of a traction suture, as a modification of staged traction laparosc

Parascrotal Approach for Orchiopexy in Perineal Ectopic Testes: Our Experience Over 10 Years.

BACKGROUND/AIM: Perineal ectopic testis (PET) is a very rare condition in the spectrum of abnormally located testis and should be looked for in all children with an empty scrotum who do not have undescended testis (UDT). While the etiology of testicular ectopia is not completely known, the complications are similar to that of UDT, and timely placement in scrotum is critical. Single incision scrotal orchiopexy is increasingly used for low-lying UDT. The aim of this study is to present our experience with parascrotal orchiopexy in the management of PET. MATERIALS AND METHODS: A retrospective re

Testicular Regression Syndrome Presenting as a Nonpalpable Undescended Testis in Adulthood: A Case Report and Literature Review.

Vanishing testis is also known as testicular regression syndrome, which is atrophy and disappearance of one testis during fetal life. Vanishing testis accounts for < 5% of undescended testis. Vanishing testis is a rare condition that must be remembered in the differential diagnosis of undescended testis. We report a case of vanishing testis in a young man of 29 years old who underwent inguinal exploration for nonpalpable undescended testis. There was no obvious testis at exploration and instead the vas deferens together with cord vessels ended blindly distally. The spermatic cord was excised a

Is torsion of the undescended testis intrinsically associated with worse salvage? A matched cohort analysis.

BACKGROUND: Testicular torsion in undescended testes (UDT) has poor salvage rates, but it remains unclear if the cryptorchid state itself confers an intrinsic disadvantage. We aimed to compare testicular salvage between UDT and descended torsion while controlling for presentation variables. METHODS: In a single-institutional retrospective cohort (2010-2024), using propensity score matching, 25 UDT torsion cases were matched 1:2 to 129 descended torsion controls on age, ischemia time, and testicular volume. The primary outcome was salvage on post-operative imaging (≥50% volume of contralateral

The great pretender: Torsion of a giant intra-abdominal seminoma presenting as acute appendicitis-A rare diagnostic trap.

Acute appendicitis may mask rare surgical emergencies. Torsion of an intra-abdominal undescended testis with malignancy is exceptionally uncommon. A 27-year-old man presented with 3 days of right lower quadrant pain, vomiting, and anorexia. Examination showed peritonitis and an absent right testis. Laparotomy for presumed appendicitis revealed a normal appendix and an 11 × 13 × 12 cm intra-abdominal mass with a 540° torsed spermatic cord. Right radical orchiectomy was performed. Histopathology confirmed classical seminoma. Final stage was pT2N0M0S0 Stage IB. Giant torsed seminoma can mimic app

Simultaneous laparoscopic orchiopexy and high ligation for concurrent inguinal hernia and nonpalpable undescended testis in young infants.

PURPOSE: Inguinal hernia (IH) and undescended testis (UDT) frequently coexist in infants, creating a dilemma regarding optimal surgical timing. Although early IH repair is recommended to prevent incarceration, orchiopexy is typically deferred to allow for potential spontaneous testicular descent. However, spontaneous descent occurs predominantly within the first few months of life and is unlikely after 6 months, particularly in cases of nonpalpable UDT. Despite these considerations, evidence guiding the management of infants with concurrent IH and nonpalpable UDT, especially during early infan

Catamenial Hematuria in Mixed Gonadal Dysgenesis (45,X/46,XY): A Case Report of a Rare Presentation.

Mixed gonadal dysgenesis (MGD) is a rare disorder of sex development (DSD) associated with 45,X/46,XY mosaicism and variable phenotypic presentation. Mullerian remnants may persist but are typically non-functional. We report a 16-year-old phenotypic male with hypospadias and a unilateral undescended testis with prior resection of a streak gonad containing fallopian tube structures, who presented with cyclical hematuria and lower abdominal pain. Imaging suggested a Müllerian duct cyst. Diagnostic laparoscopy and cystoscopy revealed a cystic structure communicating with the posterior urethra, in

Short-Term Outcomes After Orchiopexy for Undescended Testes at two Public Hospitals in Ethiopia: A Prospective Cohort Study.

BACKGROUND: Undescended testis (UDT) is the most common congenital genital anomaly in male children. Although orchiopexy is recommended within the first year of life to optimize testicular function and reduce the risk of malignancy, delayed surgical correction remains common in low-resource settings. Evidence on short-term postoperative outcomes in such contexts is limited. This study assessed the incidence and predictors of short-term complications following orchiopexy for UDT. METHODS: A prospective cohort study was conducted at two Ethiopian public hospitals from April 2023 to October 2024

Assessment of factors associated with delayed diagnosis of cryptorchidism and orchidopexy.

BACKGROUND: Undescended testis (UDT) is a congenital anomaly. Early orchidopexy preserves fertility and reduces the risk of malignancy. However, delays in diagnosis and surgeries remain common in Saudi Arabia. OBJECTIVES: Our study aims to determine the hospital-based proportion of UDT at our facility and factors associated with delayed orchidopexy in patients with a diagnosis or suspicion of UDT. This hospital-based study does not estimate population-level incidence or prevalence. METHODOLOGY: This study included children (0-14 years) with cryptorchidism (2018-2023). SPSS v25 analyzed data

Intra-abdominal Testicular Torsion: A Case Report.

The torsion of an ectopic testis is a rare urological condition encountered only infrequently in clinical practice, and its symptoms may be misleading and interpreted as signs of other common conditions. We report an unusual case of a 20-year-old male patient who sought emergency medical care for acute abdominal pain. On examination, an empty left hemiscrotum was found, and an immediate (CT) scan of the abdomen showed torsion of an intra-abdominal testis. The patient underwent urgent surgical exploration, during which the ectopic testis was found to be viable and was successfully preserved and

Timing of orchiopexy in children with congenital undescended testis in Latin America: A multicenter retrospective study.

BACKGROUND: Congenital undescended testis (CUDT) is the most common urogenital anomaly in boys and is associated with impaired fertility potential and an increased risk of testicular malignancy later in life. International guidelines recommend orchiopexy between 6 and 18 months of age; however, regional data from Latin America remain scarce. OBJECTIVE: To evaluate the timing of orchiopexy in children with CUDT in Latin America. STUDY DESIGN: Multicenter retrospective observational study including patients younger than 18 years with CUDT who underwent orchiopexy between January 1, 2023, and D

Laparoscopic management of intra-abdominal undescended testes: Diagnostic limitations, technique variability, and risk factors for postoperative testicular atrophy.

BACKGROUND: Intra-abdominal undescended testes (IAT) are the most challenging form of cryptorchidism, with high risks of impaired fertility and postoperative atrophy. Laparoscopy is the standard approach, but preoperative imaging is unreliable, intraoperative volume assessment is often difficult, and technique selection varies. Comparative outcome data and the prognostic value of real-world diagnostic limitations remain limited. OBJECTIVE: To identify diagnostic and anatomical predictors of testicular atrophy after laparoscopic management of IAT and compare outcomes among one-stage vessel-pre

Can MRI with diffusion-weighted imaging reliably detect cryptorchidism? A systematic review and diagnostic accuracy meta-analysis.

PURPOSE: Non-palpable undescended testes require accurate localisation, but the role of magnetic resonance imaging (MRI) combined with diffusion-weighted imaging (DWI) remains uncertain. We evaluated the diagnostic accuracy of MRI-DWI for detecting clinically non-palpable undescended testes. METHODS: This PROSPERO (CRD420261411862) registered review searched PubMed, Embase, Web of Science, and CENTRAL to June 6, 2026. Eligible diagnostic-accuracy studies assessed MRI-DWI against diagnostic laparoscopy or operative exploration. Two reviewers extracted 2 × 2 data, and assessed QUADAS-2 risk of

Clinical features and imaging diagnostic challenges of fetus-in-fetu associated with abdominal cryptorchidism: a case report.

BACKGROUND: Fetus-in-fetu (FIF) is a rare congenital anomaly in which a malformed monozygotic twin is incorporated within its host twin. FIF is usually retroperitoneal, whereas association with an intra-abdominal undescended testis is exceptionally uncommon. Because imaging may show a heterogeneous mass containing soft tissue, fat and calcified elements, FIF can be difficult to distinguish preoperatively from mature teratoma, especially when cryptorchidism is also present. CASE PRESENTATION: A 3-day-old male neonate was admitted because of jaundice and a palpable right abdominal mass. Physica
