Coronal reduced field-of-view diffusion-weighted imaging improves image quality and T staging of upper tract urothelial carcinoma compared with conventional axial DWI: a retrospective study.

PURPOSE: To evaluate whether coronal reduced field-of-view (r-FOV) diffusion-weighted imaging (DWI) improves image quality and local T staging of upper tract urothelial carcinoma (UTUC) compared with conventional axial full field-of-view (f-FOV) DWI. METHODS: In this retrospective single-center study, 60 patients with pathologically confirmed UTUC who underwent preoperative magnetic resonance urography (MRU) were included. The imaging protocol comprised both axial f-FOV DWI and coronal r-FOV DWI acquired on a 3.0-T scanner. Two radiologists independently assessed subjective image quality (sha

Beyond the stone: nephrolithiasis as a metabolic sentinel in upper tract urothelial carcinoma and papillary renal cell carcinoma: toward a risk-stratified management framework.

CONTEXT: Nephrolithiasis is increasingly recognized as a recurrent disorder that often coexists with broader metabolic vulnerability. Observational studies have also raised the possibility that stone disease may identify patients with a higher subsequent risk of upper tract urothelial carcinoma (UTUC) and, less consistently, renal cell carcinoma (RCC), particularly papillary RCC (pRCC). OBJECTIVE: To critically review the epidemiologic and mechanistic literature linking nephrolithiasis to UTUC and RCC, distinguish association from causation, and outline a clinically practical risk-stratified

[PSMA-PET/CT SCAN WAS USEFUL IN DIFFERENTIATING PRIMARY URETERAL CANCER FROM METASTATIC ONE IN A PROSTATE CANCER WITH LEFT HYDRONEPHROSIS: A CASE REPORT].

A 70 year-old male patient with a history of prostate adenocarcinoma developed left hydronephrosis 11 years after receiving heavy particle therapy. Computed tomography (CT) revealed possible left primary ureteral cancer. However, this lesion was positive on prostate-specific membrane antigen positron emission (PSMA-PET) /CT, suggesting metastasis from prostate cancer. After three months of androgen deprivation therapy (ADT), the lesion became negative on PSMA-PET/CT. Six months after initiation of ADT, the left hydronephrosis improved. These findings suggest that PSMA-PET/CT help differentiate

Multidisciplinary perioperative management of robot-assisted renal surgery: practical considerations for partial nephrectomy and nephroureterectomy.

Robot-assisted surgery is established across urology, but its safety depends on more than console performance. Restricted access to the patient, limited tactile feedback, external arm collisions, phase transitions, and communication failures create risks that must be managed collectively. This narrative review summarizes multidisciplinary perioperative management for robot-assisted partial nephrectomy (RAPN) and robot-assisted nephroureterectomy (RANU) and introduces a proposed practice-based framework. The framework was developed by integrating multidisciplinary educational sessions and case-

Guidewire-assisted en bloc resection of periorifice non-muscle-invasive bladder cancer: a retrospective case series.

BACKGROUND: En bloc resection of bladder tumor provides improved specimen quality compared with conventional transurethral resection of bladder tumor; however, periorifice tumors remain technically challenging due to the risk of ureteral injury. We reviewed the perioperative and short-term outcomes of patients who underwent guidewire-assisted en bloc resection for periorifice non-muscle-invasive bladder cancer. METHODS: In this retrospective case series, we included patients with non-muscle-invasive bladder cancer located within 1 cm of the ureteral orifice who underwent guidewire-assisted en

Near-infrared fluorescence imaging for ureteral and sentinel lymph node mapping in gynecological minimally invasive surgery: a review of fluorophores and domestic laparoscopic systems.

As laparoscopic techniques continue to advance, intraoperative tissue visualization has become increasingly important. In gynecological oncology, sentinel lymph node (SLN) mapping remains a critical procedure for assessing lymph node metastasis and guiding lymphadenectomy. Meanwhile, iatrogenic ureteral injury, although rare, can have severe consequences. Thus, achieving rapid, reliable, and cost-effective intraoperative visualization of both SLNs and the ureters during laparoscopy poses a significant clinical challenge. Near-infrared fluorescence (NIRF) imaging has emerged as a promising solu

[A CASE OF MALIGNANCY ARISING IN A NEOBLADDER MORE THAN 50 YEARS AFTER RECONSTRUCTION].

A 75-year-old woman who had undergone right nephrectomy, total cystectomy, and orthotopic neobladder reconstruction using a free intestinal segment in 196X (precise diagnosis and surgical details unknown) presented with discomfort during urine storage and pain during urination. She was referred to our department in October 202Y, and urine cytology was positive. Magnetic resonance imaging revealed direct anastomosis of the free intestinal segment to the bladder neck, with a nodular tumor in the lumen. Cystoscopy revealed a nodular tumor with an abnormal surface vasculature inside the neobladder

Consolidative Surgery for Advanced Upper Urinary Tract Urothelial Carcinoma After Enfortumab Vedotin and/or Immune Checkpoint Inhibitors.

Upper urinary tract urothelial carcinoma (UTUC) is a rare malignancy with a poor prognosis in advanced disease. With the advent of novel systemic therapies, including enfortumab vedotin (EV) and immune checkpoint inhibitors (ICIs), deep and durable responses can now be achieved, potentially enabling consolidative surgery with curative intent. However, evidence supporting this approach in advanced UTUC remains scarce. We report a case series of five patients with advanced UTUC who underwent consolidative surgery after EV and/or ICI therapy. All patients achieved sufficient disease control with

Geriatric 8 score predicts surgical risk and oncologic outcomes in older patients with upper tract urothelial carcinoma undergoing radical nephroureterectomy.

BACKGROUND: Accurate preoperative risk stratification remains challenging in older patients with upper tract urothelial carcinoma (UTUC) undergoing radical nephroureterectomy (RNU). This study evaluated whether preoperative Geriatric 8 (G8) score is associated with perioperative and oncologic outcomes. METHODS: We retrospectively analyzed 338 patients aged ≥ 65 years who underwent RNU between 2013 and 2022. Patients were classified as G8-low (score ≤ 14) or G8-high (score > 14). Multivariable logistic and Cox regression analyses assessed the association between frailty, postoperative complica

The effect of ureteral stenting on the risk of metachronous upper tract urothelial carcinoma in patients with bladder tumors involving the ureteral orifice.

BACKGROUND: Ureteral stenting in patients with bladder cancer involving the ureteral orifice may increase the rate of metachronous upper tract urothelial carcinoma (UTUC). Nevertheless, evidence on the matter is scarce and often contradictory. MATERIALS AND METHODS: We performed an observational study comparing ureteral stenting versus no stenting with respect to the risk of developing metachronous UTUC in patients who underwent transurethral resection of bladder tumors (TURBT) involving the ureteral orifice between January 2015 and December 2024. We performed time-to-event analyses to estima

National Patterns and Predictors of Perioperative Intravesical Chemotherapy Use Following Nephroureterectomy for Upper Tract Urothelial Carcinoma.

INTRODUCTION: Intravesical chemotherapy (IC) administered perioperatively after nephroureterectomy (NUx) for upper tract urothelial carcinoma reduces intravesical recurrence based on 2 randomized trials and is endorsed by contemporary practice guidelines. However, real-world adoption of this intervention remains poorly defined. We evaluated national utilization trends and identified predictors of perioperative IC use after NUx. METHODS: Using the National Cancer Database, we identified patients with nonmetastatic upper tract urothelial carcinoma who underwent NUx between 2013 and 2022. Periop

Imaging in Upper Tract Urothelial Carcinoma: Diagnostic Accuracy of Current Modalities for Local Tumor Staging.

PURPOSE OF REVIEW: To summarize current imaging modalities for upper tract urothelial carcinoma (UTUC) and evaluate their diagnostic performance for tumor staging and risk stratification. RECENT FINDINGS: CT urography remains the diagnostic standard with excellent accuracy for tumor detection, though its reliability for local staging is limited. MR urography shows promise for assessing muscle invasion but is operator dependent. FDG-PET demonstrates value in nodal and metastatic staging rather than primary tumor evaluation. Retrograde pyelography maintains a role in tumor diagnosis in circumst
