Hem-o-Lok® clip migration after robotic prostatectomy: PRISMA-guided review with two illustrative cases.

Hem-o-Lok® polymer clips are widely used during robot-assisted radical prostatectomy. Although generally safe, delayed intravesical migration is an under-recognized source of lower urinary tract symptoms, haematuria and bladder stone formation. We report two illustrative cases - one with large bladder calculus formed around an eroded clip and another with spontaneous urethral passage - then synthesize the contemporary literature using a PRISMA-guided search. Across > 100 reported patients, time to presentation ranges from 1 to 252 months. The commonest presentations are LUTS, haematuria and re

State of the art imaging for endometriosis and adenomyosis.

Endometriosis is a chronic oestrogen-dependent condition characterised by the presence of endometrial-like tissue outside the uterus. Although benign, the associated chronic inflammation and fibrosis can result in substantial symptomatic burden, including chronic pelvic pain, menorrhagia, dysmenorrhoea and dyspareunia. Adenomyosis, which commonly co-exists with endometriosis, is defined by endometrial glands within the myometrium, causing chronic inflammation and fibrosis. Symptoms overlap with endometriosis, including heavy menstrual bleeding, dysmenorrhoea and chronic pelvic pain. Historical

Multifactorial explanatory model of self-reported sleep disturbance in people with rheumatoid arthritis: a structural equation approach.

OBJECTIVES: Sleep disturbance is a common and disabling symptom among patients with rheumatoid arthritis (RA). This study aimed to test a multifactorial explanatory model of cross-sectional associations among factors related to self-reported sleep disturbance in RA. METHODS: This was an ancillary analysis of an observational, cross-sectional, single-centre study including patients with RA. Sleep disturbance, disease activity, perceived disease impact, depressive and anxiety symptoms, and personality traits were assessed using validated questionnaires. Structural equation modelling (SEM) was u

Straining to Pass Urine in Men with Lower Urinary Tract Symptoms.

Straining to void is widely regarded as a clinical indicator of bladder outlet obstruction (BOO) in men with lower urinary tract symptoms (LUTS). However, the physiological significance of this behaviour remains poorly understood. This review examines the role of straining during the voiding cycle and evaluates current evidence regarding its relationship with BOO. Straining represents a voluntary increase in intra-abdominal pressure that may occur at different stages of micturition. Studies comparing patient-reported straining with urodynamic findings demonstrate poor correlation, and the prev

A sound approach: scoping review of adult lower urinary tract foreign body recognition and management.

INTRODUCTION: Insertion of foreign bodies into the lower urinary tract by adults is an uncommon but clinically important presentation encountered by urologists and emergency providers across a range of motivations. Retained or migrated objects can lead to serious complications including infection, calculus formation, and urethral trauma, often necessitating careful and complex management. METHODS: We conducted a scoping review following the PRISMA extension for Scoping Reviews. Because only published, de-identified case material was used, institutional review board approval was not required.

Simultaneous surgery for NMIBC and BOO: evaluating surgical safety to oncological benefit.

BACKGROUND: Current evidence demonstrates that simultaneous surgical management of bladder cancer (BCa) and bladder outlet obstruction (BOO) is oncologically safe, despite well-known concerns. This systematic review and meta-analysis primarily evaluates oncological outcomes of concurrent transurethral resection of bladder tumor (TURBT) and BOO surgery compared to TURBT alone or staged treatments; perioperative safety is descriptively summarized. METHODS: A systematic search of PubMed, Web of Science, and the Cochrane Central Register of Controlled Trials was conducted from January 2000 to Mar

Orgasm-associated urinary incontinence (climacturia) after radical prostatectomy: current concepts in pathophysiology and management.

INTRODUCTION: Radical prostatectomy (RP) for prostate cancer commonly results in sexual dysfunction and stress urinary incontinence, including climacturia-orgasm-associated urinary incontinence. Prevalence estimates range widely (20%-93%, averaging closer to 30%), and the condition is likely underreported despite its substantial psychosocial impact, including embarrassment, relationship strain, and avoidance of sexual activity. This review summarizes its pathophysiology, prevalence, and management following RP. METHODS: A narrative review of the literature was performed, synthesizing data on

Is Temporary Incontinent Vesicostomy Associated With Fewer Early Urinary Outflow-Related Complications After Pediatric Ileocystoplasty?

AIMS: Pediatric augmentation ileocystoplasty carries a risk of urinary outflow-related complications (ORC), including anastomotic leakage, obstruction of the catheterizable stoma, and febrile urinary tract infection. We evaluated whether temporary incontinent vesicostomy (TIV) reduces early ORC and emergency readmissions, and whether it affects 24-month function. METHODS: We retrospectively analyzed 168 children who underwent perimesenteric ileal augmentation with a catheterizable continent stoma between 2000 and 2023: TIV (n = 133) versus no TIV (n = 35). The primary endpoint was ORC within

Making cancer support services sustainable in value-based care.

Bulking agents are used in the treatment of urinary stress incontinence associated with intrinsic sphincter deficiency. Bovine collagen remains the gold standard despite problems such as allergic reaction and absorption of the injected material. Several new products are being evaluated or used clinically to try and overcome problems linked with bovine collagen. The aim of this paper is to review the bulking agents available and to put in prospect use of these products with the recent progress made in the treatment of urinary stress incontinence. The gold standard treatment for intrinsic sphinc

Postvoided Residual/Bladder Emptying: A Mini Review.

Postvoid residual (PVR) urine volume is widely used in the evaluation of patients with lower urinary tract symptoms (LUTS) and benign prostatic obstruction (BPO), yet its clinical role remains debated. This mini review critically appraises current evidence on the definition, pathophysiology, measurement, and clinical utility of PVR. PVR reflects impaired bladder emptying and is influenced by multiple mechanisms, including bladder outlet obstruction, detrusor underactivity, and altered sensory function. However, its diagnostic accuracy for identifying specific conditions is limited by high inte
