Robotic Partial Cystectomy for Urachal Adenocarcinoma: Surgical Technique with Cys-toscopic Correlation.

INTRODUCTION AND OBJECTIVE: Urachal adenocarcinoma is a rare malignancy that typically arises from the bladder dome and is often diagnosed incidentally or during evaluation for hematuria (1, 2). Surgical excision remains the cornerstone of treatment (3, 4). This video demonstrates the robotic technique for partial cystectomy with en bloc urachal resection, combined with synchronized intraoperative cystoscopic imaging-an approach that has proven feasible and oncologically safe in selected cases (5-7). MATERIALS AND METHODS: A 74-year-old hypertensive male presented with self-limited gross hema

Urachal adenocarcinoma: Case report, literature review, and therapeutic algorithm proposal.

Urachal adenocarcinoma is an extremely rare malignancy, accounting for less than 1% of all bladder tumors. Diagnosis is often delayed due to nonspecific symptoms and low clinical suspicion. We report the case of a 52-year-old male with no relevant medical history who presented with lower urinary tract symptoms. Cystoscopy revealed a cystic lesion at the bladder dome. Transurethral resection confirmed mucinous urachal adenocarcinoma. CT imaging showed no locoregional or distant spread, and tumor markers were within normal limits. The patient underwent laparoscopic partial cystectomy with en blo

Research progress in the diagnosis and treatment of pseudomyxoma peritonei.

Pseudomyxoma Peritonei (PMP) is a rare and complex clinical syndrome characterized by the extensive implantation and metastasis of tumor cells from different tissues within the abdominal cavity, accompanied by the continuous secretion of large amounts of jelly-like mucin. PMP primarily originates from appendiceal mucinous tumors, and in rare cases, can also arise from organs or tissues such as the ovary, colon, gallbladder, pancreas, urachus, small intestine, stomach, intestinal duplications, and teratomas. PMP has diverse clinical manifestations and lacks specificity, making diagnosis difficu

Uncommon Encounter in Urachus: A Rare Case of Xanthogranulomatous Urachitis With Review of Literature.

Urachus is a fibrous remnant from embryologic development derived from the allantois, which is involved in waste elimination in the fetus. Typically, it extends from the dome of the bladder to the umbilicus. Xanthogranulomatous urachitis, or xanthogranulomatous inflammation of the urachus, is a highly unusual pathological entity characterized by large lipid-laden macrophages, with only a few cases reported worldwide. In this case, a 48-year-old male patient presented with complaints of persistent abdominal pain and watery discharge from the umbilicus for 1 month. Following en bloc resection of

A case of prune belly syndrome with patent urachus: Pediatric surgical aspects of a rare case report.

Prune belly syndrome (PBS), or Eagle-Barrett syndrome, is a rare congenital disorder defined by a triad of abdominal wall hypoplasia, urinary tract anomalies, and bilateral cryptorchidism. It is associated with high perinatal morbidity and mortality. A male infant born at 37 weeks via vaginal delivery presented with abdominal distension, thin wrinkled abdominal skin, urine discharge from the umbilicus, and bilateral cryptorchidism. Imaging revealed severe bilateral hydronephrosis and a patent urachus, confirming Prune Belly Syndrome. The urachus was surgically closed without complications, and

Follicular lymphoma of the urachus.

This case report describes an elderly man who presented with mild lower urinary tract symptoms without haematuria. During his workup, an incidental finding of a urachal mass was diagnosed by CT. He underwent robotic partial cystectomy with en bloc urachal excision achieving clear frozen section margins. Histology unexpectedly revealed follicular lymphoma (FL) infiltrating the urachal remnant. This is a unique case of FL of the urachus. The treatment for limited stage disease is either radiotherapy or active monitoring. The 18F-fluorodeoxyglucose positron emission tomography with CT (18F-FDG PE

Near infrared ray-guided partial cystectomy using da Vinci Firefly® technology and intraoperative cystoscopy for urachal cyst, suspected of urachal tumor.

PURPOSE: Urachal cyst is a type of congenital anomaly that may undergo malignant transformation. Partial cystectomy with en-bloc resection of the urachus is appropriate treatment for urachal remnants. We performed near infrared ray-guided surgery (NIRGS) for the accurate localization of tumor margins using the Firefly® technology of the da Vinci surgical system and intraoperative cystoscopy. In the normal visible light mode, we could not recognize the cystoscopic light. However, after changing to the Firefly® mode, the near infrared rays transmitted through the bladder wall were visible as gre

Case Report: Urachal inflammatory myofibroblastic tumor with bladder invasion.

BACKGROUND: Urachal inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal neoplasm characterized by non-specific clinical and imaging features, which renders it susceptible to clinical misdiagnosis. Complete surgical resection is the preferred treatment method. CASE PRESENTATION: We herein report a case of urachal IMT in a 12-year-old male child who presented with dysuria for 4 days and abnormal urine color for 2 days. Preoperative ultrasonography and contrast-enhanced computed tomography (CT) both suggested an infected urachal cyst. Laparoscopic resection of the urachal mass was ini

Coexistant Meckel's diverticulum and patent urachus: a case report.

INTRODUCTION AND CLINICAL IMPORTANCE: Concurrent remnants of the omphalomesenteric duct and urachus are exceptionally found. Diagnosis of Meckel's diverticulum is mainly incidental, but a patent urachus is usually clinically possible. CASE PRESENTATION: We admitted a 48-hour-old female patient for an eviscerated Meckel's diverticulum with a history of urine smelling around the umbilicus. Preoperative ultrasound did not identify an associated urachal anomaly. During emergency surgical exploration, a patent urachus was identified. Both remnants were resected, with the Meckel's diverticulum pres

Urachal Mucinous Cystic Tumour of Low Malignant Potential With Incidental Clinical Presentation: A Case Report With Clinicopathologic Features and Review of the Literature.

Urachal mucinous cystic tumour of low malignant potential (MCTLMP) is a rare cystic epithelial neoplasm of urachal origin, with fewer than 50 cases described as such in the English-language scientific literature. This case report describes an instance of urachal MCTLMP discovered incidentally in a 59-year-old female patient following the performance of imaging studies for an unrelated condition: Management consisted of surgical resection of the tumour and partial cystectomy, with no complications and no recurrence after 10 months of follow-up. Sections of the partial cystectomy specimen demons

Ontogenetic Immaturity of Urachal Structures and Its Clinical Implications for Conservative Management in Children.

Abnormalities of the urachus detected in infancy and early childhood are often interpreted as persistent congenital abnormalities requiring surgical correction. However, growing clinical experience suggests that a proportion of these findings may reflect ontogenetic immaturity of the urachus rather than fixed pathological conditions. As a transient embryological structure, the urachus undergoes postnatal remodeling and involution, which may explain the spontaneous regression of urachal anomalies observed during follow-up. This paper proposes ontogenetic immaturity of the urachus as a biologica

Recurrent epithelioid trophoblastic tumor with complete response after surgery, chemotherapy, and immunotherapy: A case report and literature review.

BACKGROUND: Epithelioid trophoblastic tumor (ETT) is the rarest form of gestational trophoblastic neoplasm, accounting for 1.39-2% of gestational trophoblastic diseases. Its clinical behavior and pathogenesis remain poorly understood, and no standardized treatment guidelines currently exist, with surgery considered the most important therapeutic option. Emerging modalities, including immune checkpoint inhibitors, are under investigation. CASE PRESENTATION: We report a case of recurrent ETT achieving complete response after combined surgery, chemotherapy and immunotherapy. Notably, the primary

Colo-urachal fistula with infected urachus; a rare fate of urachus in adults.

Colourachal abnormalities are conditions that occur due to defective obliteration of urachus into the median umbilical ligament during the developmental period. The case of a middle-aged male who presented multiple times with recurrent abdominal pain, vomiting, and intermittent bloody loose stools is presented. On final admission, he developed signs of acute abdomen. Despite multiple visits, his workup remained inconclusive. Explorative laparotomy was performed which showed Colo-urachal fistula with infected urachus from which air was released upon incision and pus drained. The patient underwe

Umbilical vein marsupialization in foals-short literature review.

Umbilical remnant infections are common in neonatal foals and may involve one or more umbilical structures. Diagnosis relies on thorough clinical examination, ultrasonography, and hematologic evaluation. Standard care includes broad-spectrum antibiotics, with surgical resection if conservative therapy fails. Omphalophlebitis may extend to the liver, prompting consideration of umbilical vein marsupialization. Surgery is performed in dorsal recumbency with a fusiform incision around the umbilicus. The umbilical arteries and urachus are ligated and transected, with partial cystectomy when indicat
