[A Case of Pancreatic Body and Tail Cancer Detected because of a Sister Mary Joseph's Nodule].

The patient is a 67-year-old female who visited a local clinic with a primary complaint of redness and swelling at the umbilicus. The patient was scheduled for surgical excision because of a suspected umbilical abscess caused by a remnant urachus. During this period, weight loss and elevated tumor marker levels were noted, and further investigation led to the diagnosis of pancreatic body and tail cancer with a Sister Mary Joseph's nodule. Systemic chemotherapy was administered, and initially, both the tumor size and tumor marker levels decreased. However, the tumor subsequently increased in si

Fish bone migration into urachus mimicking urachal malignancy: A rare case report.

Foreign bodies can find their way into and around the bladder usually via urethra by self insertion, migration from surrounding structures and by penetrating trauma. The clinical presentation being varied, symptoms nonspecific and history often concealed makes diagnosis quite difficult. Fish bone ingestion has been reported to caused bowl perforation and rarely migrate to other surrounding structures. We present a case of 39 year old male who presented to us with LUTS and lower abdominal mass. Evaluation revealed a mass extending from the dome of urinary bladder to umbilicus. Imaging modalitie

Musculo-Invasive Urachal Transitional Cell Carcinoma (TCC): Is It Really Sinister and Different?

Urachal remnants are found in a third of adults, commonly at the dome. They can be found anywhere along the bladder midline, including the anterior and posterior walls. As a single entity, urachal carcinomas are rare, comprising 0.01% of all adult cancers and 0.35% of all bladder cancers. The commonest urachal carcinoma is adenocarcinoma, but 5% of urachal growths are urothelial. Whenever a case of urachal carcinoma is discussed, it is presumed to be an adenocarcinoma, but people forget the possibility of a transitional cell carcinoma (TCC) occurring in the urachus. Also, there is confusion ab

Urachal adenocarcinoma: a case report and literature review.

Urachal carcinoma is a rare malignant epithelial neoplasm with an estimated incidence of one in 5 million people. Due to the rarity of these cases, no universally accepted staging system or standardized surgical and oncological treatment protocols currently exist. En bloc surgical resection of the umbilicus, urachal ligament, and involved portion of the bladder remains the preferred treatment. The addition of adjuvant chemotherapy may be considered in the setting of metastatic disease. Here, we present a case of urachal adenocarcinoma in a 67-year-old male who initially presented with purulent

Diagnostic Challenges and Management of Urachal Malformations in an Infant.

BACKGROUND: The urachus is a vestigial remnant of the allantois which naturally degenerates after birth. Failure of closure of the urachus can result in urachal malformations, with the most common being a urachal cyst. Diagnosing a urachal cyst is challenging due to its diverse presentations, often leading to misdiagnosis. Although they are often asymptomatic, urachal cysts can be significant because they can lead to complications. CASE REPORT: We report a late preterm male infant born at 36 weeks + 5 days, in whom an antenatal anomaly scan revealed a unilocular intra-abdominal cyst adjacent

Agreement between sonographic and pathoanatomic classifications of pediatric urachal remnants.

AIM: Ultrasound is the favorite method for follow-up of urachal remnants when conservative management is chosen instead of surgical resection. However, the success of conservative management depends on the type of urachal anomaly, and the agreement between sonographic and pathoanatomic diagnoses has not yet been assessed. MATERIAL AND METHODS: We retrospectively included all minors treated for a urachal anomaly at two German pediatric surgical centers between January 2008 and December 2020. Ultrasound examinations were conducted by specialized pediatric radiologists and compared with pathoana

Imaging of Urachal Adenocarcinoma: A Case Report.

Adenocarcinoma of the urachus is a rare but highly aggressive malignancy that arises from the urachal remnant. Due to its non-specific symptoms and potential to mimic more common and benign conditions, accurate and early diagnosis through imaging is crucial. This report will discuss the role of imaging in urachal adenocarcinoma regarding the characteristics and diagnosis of this tumour, with a focus on CECT, MRI, and ultrasound for detection, staging, and treatment planning. Since urachal adenocarcinoma is a rare and complex disease, optimal results can be achieved only with a combined approac

Comparative Analysis of Signet Ring and Non-Signet Ring Urachal Adenocarcinomas: A National Cancer Database Study.

INTRODUCTION: Primary urachal adenocarcinomas are rare and histologically diverse. Signet ring cell carcinoma (SRCC), a poorly cohesive and aggressive variant, is associated with poor prognosis compared to non-signet ring urachal adenocarcinoma (UA). This study aims to compare the clinicopathologic features, treatment patterns, and survival outcomes between SRCC and UA. METHODS: We retrospectively analyzed data from the National Cancer Database (2004-2020), including patients aged ≥ 18 with SRCC or non-signet ring UA located at the bladder dome. Demographics, staging, treatment modalities, an

Urachal Rhabdomyosarcoma in Children: A Rare Malignancy with Review of Literature.

Urachus is a tubular remnant connecting the bladder's apex to the umbilicus, where urine initially drains in the fetus during embryonic development. We report a rare case of a 7-year-old boy presenting with an asymptomatic lower abdominal lump. Physical examination revealed a nontender hypogastric mass. Contrast-enhanced computed tomography scan was suggestive of a large abdominopelvic mass abutting the anterior abdominal wall. Ultrasound-guided percutaneous biopsy followed by a histological analysis with immunohistochemistry showed embryonal rhabdomyosarcoma (RMS). Further metastatic workup r

The importance of daily life activities in managing obesity in patients with type 2 diabetes mellitus: a study using physical activity records.

BACKGROUND: Human immunodeficiency virus (HIV) prevalence among men who have sex with men (MSM) is thought to be high in Jamaica. The objective of this study was to estimate HIV prevalence and identify risk factors in order to improve prevention approaches. METHODS: With the help of influential MSM, an experienced research nurse approached MSM in four parishes to participate in a cross-sectional survey in 2007. Men who have sex with men were interviewed and blood taken for HIV and syphilis tests, and urine taken for gonorrhoea, chlamydia and trichomonas testing using transcription-mediated am

Urachal actinomycosis mimicking a urachal adenocarcinoma: case report and systematic review.

Actinomycosis is a chronic bacterial infection caused by Actinomyces israelii, a Gram-positive anaerobic bacterium typically found in the oral cavity and gastrointestinal tract. This infection can present in various clinical and radiological forms, affecting cervicofacial, thoracic, abdominal, and pelvic regions, often resembling malignancies. This extremely rare disease should be considered in the differential diagnosis of abdominopelvic masses. Accurate identification and appropriate management are challenging and depend on the clinician's suspicion. Most patients undergo surgery, and are tr

Gender bias in machine learning: insights from official labour statistics and textual analysis.

Streptococcus pneumoniae infection may result in asymptomatic carriage, mucosal or invasive disease. We hypothesize that self-limiting or fatal disease outcome follows infection with S. pneumoniae differential activation of the host immune response. BALB/c and C57BL/6 mice were inoculated intranasally with S. pneumoniae serotype 3 strain WU2 and serotype 14 strain DW14 and mortality, bacterial load, pathological changes in the lungs and cytokines mRNA levels in the spleen were analysed. No differences between the C57BL/6 and the BALB/c inbred mice were observed except for the severity of their

Coexisting Remnants of the Omphalomesenteric Duct and Urachus in an Infant.

INTRODUCTION: Congenital anomalies of the umbilicus, including remnants of the omphalomesenteric duct and urachus, result from the incomplete regression of fetal structures around the 10th week of gestation. The coexistence of these anomalies in a single patient is exceptionally uncommon. This report presents the case of a neonate with an umbilical nodule and periumbilical cyst, subsequently identified as coexisting remnants of the omphalomesenteric duct and urachus. CASE PRESENTATION: This study reports the case of a 17-day-old female infant who presented with a small moist umbilical nodule

Concurrent Congenital Umbilicobiliary Fistula and Vesicourachal Diverticula in a Dog.

Congenital umbilicobiliary fistula is a rare disease reported in humans and dogs. A 2-month-old, intact, male French Bulldog presented with a greenish-yellow discharge dripping from the umbilicus since birth. Complete blood count and serum biochemistry were within normal limits, but serum alkaline phosphatase activity was mildly elevated. A positive contrast cystogram was performed to rule out a patent urachus and confirmed the presence of a vesicourachal diverticula, a type of urachal anomalies. An abdominal ultrasound and computed tomography fistulogram demonstrated a communication between t

Phase I Study of mTORC1/2 Inhibitor Sapanisertib (CB-228/TAK-228) in Combination with Metformin in Patients with mTOR/AKT/PI3K Pathway Alterations and Advanced Solid Malignancies.

BACKGROUND: Sapanisertib (CB-228/TAK-228) is a potent, selective ATP-competitive, dual inhibitor of mTORC1/2. Metformin is thought to inhibit the mTOR pathway through upstream activation of 5'-AMP-activated protein kinase (AMPK) suggesting combination therapy may enhance antitumor activity of sapanisertib. We report preliminary safety, tolerability, and efficacy from the dose-escalation study of sapanisertib in combination with metformin in patients with advanced solid tumors. METHODS: Patients with advanced metastatic solid tumors resistant or refractory to standard treatment, with and witho

A large urachal cyst presenting with lower urinary tract symptoms and a falsely elevated post-void residual on bladder scan.

The urachus is an embryologic remnant that connects the bladder to the umbilicus and typically obliterates during gestation. Urachal cysts are one of the most common congenital urachal anomalies. Urachal cysts may be found incidentally on imaging or present as a palpable mass with abdominal pain and fever. We present a case of a young male with a urachal cyst who presented with lower urinary tract symptoms and a falsely elevated post void residual (PVR) volume on bladder scan. This case illustrates an atypical presentation of a large urachal cyst and highlights the limitations of bladder scann
