Idiopathic Intracranial Hypertension, Basic Concepts and Emerging Molecular, Genetic, and Immunological Therapies.

BACKGROUND: Idiopathic intracranial hypertension (IIH) is a chronic disorder characterized by elevated intracranial pressure (ICP) without an identifiable intracranial mass lesion or abnormal cerebrospinal fluid (CSF) composition. Traditionally viewed as a disorder of impaired CSF absorption or venous outflow obstruction, IIH predominantly affects women of reproductive age with obesity and remains a major cause of preventable visual loss. Emerging evidence, however, suggests that IIH is driven by a broader biologic framework involving dysregulated CSF secretion, metabolic and endocrine dysfunc

Improved visualization and segmentation of the choroid plexus using double inversion recovery MRI.

BACKGROUND: Choroid plexus (CP) volume is an emerging magnetic resonance imaging (MRI) biomarker in various disorders of the central nervous system (CNS). However, clinical translation is hindered by methodological heterogeneity and inconsistent anatomical coverage. Double inversion recovery (DIR) - a sequence providing dual-tissue suppression - is a promising candidate to improve CP segmentation. METHODS: The dataset included 93 scans across healthy subjects and individuals with multiple sclerosis (MS), divided into a training set ( n = 63), an internal test set ( n = 20), and an external te

Case Report: Faricimab in the treatment of subretinal fluid secondary to choroidal osteoma without definite evidence of CNV.

BACKGROUND: Choroidal osteoma is a rare benign osseous tumor that most commonly occurs in young women, typically located in the choroid near the optic disc or in the macular region. Secondary choroidal neovascularization (CNV) or subretinal fluid (SRF) is the main causes of visual impairment in affected patients. CASE REPORT: This article reports a 36-year-old woman who presented with decreased vision in her right eye. Multimodal imaging confirmed a choroidal osteoma in the right eye complicated by SRF in the macular area. After intravitreal injections of ranibizumab, SRF decreased substantia

Interocular symmetry of ocular rigidity and related ocular parameters in isometropia.

PURPOSE: Driven by scleral stiffness, ocular rigidity (OR) is emerging as a critical factor in refractive development. Although a common assumption is that OR is symmetric in a healthy individual's eyes, empirical data remains scarce. This study directly tests this assumption by quantifying intereye OR differences in healthy, isometropic subjects. METHODS: Isometropia was defined as intereye differences in spherical equivalent refraction (SER) of <0.50 diopters (D) and axial length (AL) of <0.15 mm. OR was measured using a validated method based on video-rate optical coherence tomography imag

Update on imaging biomarkers in uveitis.

PURPOSE OF REVIEW: To evaluate which imaging biomarkers in uveitis have approached clinical utility, which remain incompletely validated, and how consensus frameworks and artificial intelligence are reshaping the path toward standardized, decision-useful assessment. RECENT FINDINGS: Quantitative optical coherence tomography (OCT) and optical coherence tomography angiography (OCTA) metrics have gained disease-specific precision. Anterior segment OCT particle-size analysis differentiates inflammatory cells from pigment with high discriminative performance. OCTA vessel density tracks chronic mic

MRI-derived choroid plexus volume and longitudinal cognitive decline in Parkinson's disease.

BACKGROUND: Magnetic resonance imaging (MRI)-derived choroid plexus (CP) volume has been associated with cognition in Parkinson's disease, but its close relationship with ventricular anatomy complicates interpretation. METHODS: We analyzed Parkinson's Progression Markers Initiative (PPMI) data. Baseline FreeSurfer-derived CP and structural volumes were examined using linear models, and post-baseline Montreal Cognitive Assessment (MoCA) trajectories were assessed using mixed-effects models with participant-specific correlated random intercepts and time slopes. Models accounted for demographic

Early preventive effects of a Cathepsin S inhibitor on inflammation in a rat model of endotoxin-induced uveitis.

OBJECTIVES: Uveitis, an inflammation of the uveal tissue, can cause permanent vision loss. A previous study revealed elevated levels of cathepsin S in the tear proteomic analyses of patients with juvenile idiopathic arthritis-associated uveitis. This study investigates the effects of a cathepsin S inhibitor on inflammation in a rat model of uveitis. METHODS: Forty female rats were divided into five groups: Group I (healthy control), Group II (vehicle group), Group III (Z-FL-COCHO-treated group), Group IV (EIU group), and Group V (lipopolysaccharide [LPS] and Z-FL-COCHO-treated group). Uveitis

Time-, Location-, and Layer-Specific Chorioretinal Biomarkers in a Translational Large-Animal Porcine Model of Chorioretinal Ischemia.

PURPOSE: Conditions associated with complete or partial ocular ischemia, such as central retinal artery occlusion (CRAO), are vision-threatening emergencies. Therapeutic interventions are highly time-dependent, yet the exact onset of ischemia is often uncertain. Optical coherence tomography (OCT) has emerged as a potential tool for objective staging; however, quantitative, layer-resolved OCT data within the earliest phase of retinal ischemia in clinically relevant large-animal models are lacking. METHODS: Ten eyes from 10 pigs were included. Physiological retinal perfusion was confirmed using

Glymphatic dysfunction as a potential mechanistic link to cognitive ımpairment in Behçet's disease: a hypothesis.

Behçet's disease (BD) is a chronic multisystem inflammatory disease in which cognitive impairment is increasingly recognized but remains incompletely understood. Although cognitive dysfunction is more common in patients with Neuro-Behçet's disease (NBD), it has also been reported in patients without clinically apparent neurological involvement, suggesting that mechanisms beyond focal inflammatory lesions may contribute to cognitive impairment. The glymphatic system is a brain-wide perivascular clearance pathway that maintains cerebral homeostasis by facilitating the exchange of cerebrospinal a

Transcriptomic Characterization of Terminal Complement Complex-bound Cells in Human Choroid Using Single-Cell RNA Sequencing.

Age-related macular degeneration (AMD) is among the leading causes of blindness worldwide. Early AMD is characterized by dysfunction in the choroid, including early dropout of endothelial cells and increased deposition of the complement cascade's membrane attack complex (MAC) in the choriocapillaris. In this study, we used a single-cell RNA sequencing-based approach with barcoded antibodies to measure abundance of the MAC and other surface proteins at single cell resolution on RPE/choroid samples from four aged human donor eyes. We included antibodies to detect the MAC, CD34, CD45, and complem
